RESUMO

THE AIM OF THE STUDY: Optimization of methods for treating dental discoloration using whitening systems based on hydrogen peroxide by laboratory studying their chemical properties. MATERIAL AND METHODS: The effectiveness of the bleaching system was assessed by quantitatively assessing the color change of model samples of hydroxyapatite (HAP) in the CIE L*a*b* system on a Spectron-M color analyzer after exposure to coloring food media. Safety assessment was carried out on samples made from teeth removed according to indications. RESULTS: The results of laboratory tests allow us to conclude that the Chairside Light Whitening System professional teeth whitening system fläsh. (WHITEsmile GmbH, Germany) has high whitening efficiency with a whitening degree of ~84%, which significantly exceeds the norm of GOST R 702.3.004-2021. the microhardness of the enamel of samples exposed to the whitening gel for 15 minutes changes extremely little. After exposure of the studied samples to the Chairside Light Whitening System fläsh whitening system for 45 minutes, an increase in digital microhardness indicators is observed, which indicates the dynamics of processes demineralizing the hard tooth structure. CONCLUSIONS: Chairside Light Whitening System fläsh professional teeth whitening system has high whitening efficiency with minimal impact on the teeth structure and high safety.

BACKGROUND: Dentinal fluid is very close in its physical and mechanical properties and composition to blood plasma, which makes it a potentially aggressive biological environment for modern adhesive systems. An in-depth study of the physiological processes of the functioning of tooth dentin remains relevant in order to solve problems associated with its artificial restoration. PURPOSE OF THE STUDY: Study using computer simulation speed of movement and pressure distribution of dentinal fluid in the dentinal tubule of the tooth to assess the possibilities of their regulation. MATERIAL AND METHODS: To model the distribution of flow velocity and pressure of dentinal fluid in the dentinal tubule, the finite element method (Fluent ANSYS computer program) was used. RESULTS: Immediately behind the spherical tip of the odontoblast, there is a rapid increase in the hydraulic diameter of the flow section of the dentinal tubule, and, accordingly, a decrease in capillary pressure, while the tip of the odontoblast creates a large local hydraulic resistance. The resulting distribution of pressure drop in the damaged dentinal tubule is consistent with the fact that fluid movement is due, to a greater extent, to the capillary effect rather than the inlet pressure into the dentinal tubule. CONCLUSION: By changing the length of the odontoblast process, it is possible to influence the parameters of the hydrodynamics of dentinal fluid in the space of the dentinal tubule.

The article is devoted to the creation and development history of the Dental Diseases Prevention department in National Medical Research Center of Dentistry and Maxillofacial Surgery, Moscow, Russia. The main scientific and clinical achievements of the department's staff are presented in chronological order. The main directions of the scientific and clinical activities of the department are currently: the study of various factors affecting dental morbidity; the development of regionally-oriented programs for the prevention of dental diseases; organizational and methodological guidance and monitoring of primary prevention activities in organized groups of children and adolescents; the implementation of a set of measures for the primary prevention of dental diseases among adolescents and adult population applying to the polyclinic; introduction into practice of new methods and means of prevention and treatment of dental diseases; testing of new methods and means of prevention corresponding to the profile of the department, providing dental care to adolescents and adults; raising the level of theoretical knowledge and training practical skills in the prevention of dental diseases of medical personnel of healthcare institutions; promotion of sanitary and hygienic knowledge among the population.

OBJECTIVE: To study the dynamics of changes in the adhesion between the composite and dentin of the tooth when using different volumes of a single-component adhesive system in case of contamination of it with human blood plasma. MATERIAL AND METHODS: To achieve this goal, the following were used: extracted human teeth, XP Bond one-component adhesive systems (DENTSPLY, Germany), Esthet X HD micrometric restoration material (DENTSPLY, Germany), and centrifuged blood plasma. The strength of the adhesive bond between the composite material and the hard tissues of the tooth was studied using a shear test machine Zwick Roell Z 010 («Zwick¼, Germany). RESULTS: The use of a single-component adhesive system in an amount of 17.7 mg (1 drop from a dispenser) for treating open dentin makes it more resistant to contamination compared to using the same adhesive, but in an amount of 6.6 mg (the amount of adhesive that adsorbs a medium-sized dental take). A decrease in the adhesion force between the composite material and tooth hard tissues from 1.5 to 17.7% occurs when a single-component adhesive system weighing 17.7 mg of blood plasma simulating a dentinal fluid weighing from 0.2 to 2.0 mg enters. The ingestion of the same amount of blood plasma in a single-component adhesive system weighing 6.6 mg leads to a decrease in its adhesion from 4.3 to 43%.

OBJECTIVE: To assess the degree of influence of human blood plasma simulating dentinal fluid on the composite resin adhesion to dental hard tissues. MATERIAL AND METHODS: The strength of the adhesive bond between the composite material and dental hard tissues was studied using a shear test machine Zwick Roell Z 010 («Zwick¼, Germany) on extracted human teeth. XP Bond one-component adhesive system and XENOV self-etching adhesive system (DENTSPLY, Germany), high-definition micrometric restoration material Esthet X HD (DENTSPLY, Germany), as well as centrifuged blood plasma were used. RESULTS: Adhesion strength decreased by 26-78% when blood plasma weighing from 0.2 to 2.0 mg mixed up the self-etching system (weight 6.6 mg). A significant decrease in the adhesion force occurred when plasma with a mass of 0.7 mg or more mixed up with the monomer (a decrease in adhesion by 19.1%). A critical decrease in the adhesion force (43% or more) occurred with the mixture of blood plasma weighing 2.0 mg or more.

OBJECTIVE: To study the hydrodynamics of the dentinal fluid along the dentinal tubule in the event of its opening as a result of mechanical and acid treatment of the dental crown dentin. Material and methods: A licensed version of the ANSYS / FLOTRAN computer program was used, which is a software tool for solving various problems of hydrodynamics, including laminar and turbulent flow of an incompressible or compressible fluid. RESULTS: The results of computer simulation showed that 1.5 ms after drying of the dentin tubule (length 6 mm) it is refilled with dentin fluid, which then begins to accumulate on the treated surface of the dental hard tissues. Reducing the length of the dentinal tubule as a result of the cavity preparation leads to an increase in the speed and mass of pulp fluid released through it.

RESUMO

The article presents the results of spectrophotometric tooth enamel scanning for differential diagnosis of focal enamel demineralization and fluorosis. Research was conducted in vivo on teeth affected by these diseases. VITA EasyShade spectrophotometer measurements were made on the affected area and on the visually healthy part of enamel. The lightness appeared as the only one differential significant optical characteristics of tooth enamel. Lightness metrics were higher in the case of initial caries than on the healthy part of enamel when these metrics were lower in the case of fluorosis than on the healthy part of enamel.

RESUMO

The article describes focal enamel demineralization - the most common disease of teeth hard tissues. Local and general factors of this pathological process advent and development were described. The main tools of focal enamel demineralization diagnostics and treatment were observed. The main role of enamel mesoporous structure in the remineralization therapy possibility was underlined. There are still a lot of questions in focal enamel demineralization diagnostics after publications analysis because of the existing methods subjectivity. The question of indications for different remineralization tools applications optimization depending on the focal enamel demineralization degree is still open.

RESUMO

The article presents the results of spectrophotometric tooth enamel scanning. Research was conducted in vitro on extracted teeth. Numeric color parameters measurements were made by VITA EasyShade spectrophotometer after the 37% phosphoric acid enamel conditioning, that was made for an artificial tooth enamel demineralization reconstruction. Then transverse section of this enamel part was evaluated by scanning electronic microscopy. The scale of enamel demineralization depth in mm/microns and its' lightness parameter compliance was created.

RESUMO

The article describes a structural element of natural teeth - mantle dentin. It has been shown that the presence of this element in the structure of a natural tooth largely ensures its strength under the influence of repeated loads in a functional oral environment and arrests crack growth at the enamel/dentine interface. This later effect is explained by the influence of a thin layer of mantle dentine, which has physical and mechanical characteristics different from that of the main dentin.

RESUMO

The paper describes the elaborated method of computer assisted manufacturing of plastic templates for optimal topography of endodontic access in molars according to CT and laser surface scanning data. The method was tested on extracted teeth, its use in clinical practice will greatly reduce the errors rate and minimize the hard tissues loss by endodontic assess creation.
