RESUMO

OBJECTIVE: The main objective is to determine the functional, clinical, and radiological outcome of patients with low-grade spondylolisthesis undergoing single- or double-level transforaminal lumbar interbody fusion (TLIF). MATERIALS AND METHODS: This quasi-interventional study was conducted during a 2-year period from 2016 to 2018 in Shiraz, Southern Iran. We included all the adult (≥18 years) patients with low-grade spondylolisthesis (Meyerding Grade I and II) who underwent single- or double-level TLIF in our center. The spinopelvic parameters including pelvic incidence (PI), pelvic tilt (PT), sacral slope (SS), lumbar lordosis (LL), and segmental LL (SLL) were measured. The pain intensity and disability were measured utilizing the visual analog scale (VAS) for back and leg pain and Oswestry Disability Index (ODI), respectively, after 1 year. RESULTS: Overall, we included a total number of 50 patients with mean age of 54.1 ± 10.48 years. After the surgery, the PI (P = 0.432), PT (P = 0.782), and SS (P = 0.466) were not found to be statistically changed from the baseline. However, we found that single- or double-level TLIF was associated with increased LL (P < 0.001) and SLL (P < 0.001). Regarding the clinical outcome measures, both back (P = 0.001) and leg (P < 0.001) VAS improved after the surgery significantly. In addition, we found that improved leg VAS was positively correlated with improved ODI (r = 0. 634; P < 0.001). CONCLUSION: Single- or double-level TLIF is associated with increased global and SLL along with improved leg and back pain and disability in patients with low-grade spondylolisthesis. Interestingly, improved leg pain is correlated to improved disability in these patients.

BACKGROUND: Determining the expression profile and target genes of microRNA (miRNA) would assist in determining the pathophysiologic pathways in intervertebral disk degeneration (IDD). The aim of this study was to determine the expression profile of miRNA in degenerated intervertebral disks compared with normal healthy intervertebral disks. METHODS: We conducted a meta-analysis of 3 available miRNA expression datasets to identify a panel of co-deregulated miRNA genes and overlapping biological processes in IDD. Degenerated intervertebral disks were compared with normal healthy disks. We selected 35 miRNA features common to all 3 platforms. Then, we calculated differential expression P values from our unpaired data using metaMA package in R statistical software according to the moderated t test method (Limma). Based on the P values (where the threshold was <0.05), a list of differentially expressed miRNAs was identified. RESULTS: After normalization and selection of common miRNA features across all 3 platforms, we found a total of 5 differentially expressed miRNAs, among which miR-574-3p, miR-199a-5p, and miR-483-5p were not identified in any individual studies. Our results revealed that miR-199a-5p, miR-574-3p, miR-551a, and miR-640 are commonly upregulated in IDDs compared with control disks, whereas miR-483 is commonly downregulated. Pathway analysis of identified dysregulated miRNAs indicated the involvement of extracellular matrix-receptor interaction, adherens junction, and transforming growth factor-beta signaling pathway in the pathogenesis of IDDs. Moreover, the network of predicted targets for these miRNAs identified most affected target genes as ERBB4 and CLTC. CONCLUSIONS: We found that the identified miRNAs through meta-analysis are candidate predictive markers for IDDs through different pathways.

Atlantoaxial rotatory subluxation (AARS) is rarely occurred in adults with trauma as the most common cause. In type A and B it is usually managed with close reduction and external brace; however, in nonresponsive cases, surgical interventions might be needed. Our patient is a 21-year-old man with neck pain and torticollis after a car turn- over. There was C1-C2 rotatory subluxation with left side locked facet and C1 rotation about 40 degrees relative to C2 on computed tomography without evident of ligamentous injury in magnetic resonance imaging (MRI). However, during the first 48 hours, two tries of close reduction using Gardner cervical traction under fluoroscopy were failed. Thus, the patient underwent open reduction of the subluxation and atlantoaxial fixation (Harm's technique) with subsequent relief of pain and torticollis. This a rare case of traumatic AARS type A with unilateral locked facet joint in an adult patient which needed surgical manipulation for reduction. The management of the AARS in adults should be individualized in each patient.

Cervical spine chance fracture (flexion-distraction) is a rare type of fracture, and the surgical management remains a dilemma to the neurosurgeons as there is a paucity of evidence accordingly. We herein report a traumatic chance fracture of C7 fractures and its surgical management. A 33-year-old male was transferred to our center after being hit as pedestrian and was diagnosed to have a chance fracture of C7 and bilateral C7 root injury. He underwent open reduction of the deformity by posterior-only fixation of C5 and C6 with a lateral mass screw and T1 and T2 with pedicular screws. The neurological examination improved postoperatively, and the patients were neurologically intact after 6 months of treatment. Traumatic cervical spine chance fracture is extremely rare and can be managed surgically through posterior fixation and open reduction of the deformity. The best choice of treatment is limited by the paucity of the evidence and should be individualized.

RESUMO

Transient osteoporosis during pregnancy is a rare, self-limiting disease. We report on a 36-year-old woman who had bilateral subcapital femoral neck fractures during the 6th month of pregnancy. The diagnosis was made 4 days after delivery, because radiography was declined by the patient for fear of radiation. Fixation was not feasible owing to bone resorption, and 2-stage bipolar hemiarthroplasty was therefore performed. Magnetic resonance imaging is the best non-invasive investigative tool for pregnant women with hip pain. Early detection can prevent complications and resorting to major surgeries.
