Background: Colostomy is one of the common surgical procedures performed in pediatric surgical practice. The aim of this study was to retrospectively review our experience with colostomy and closure (reversal) in children. Patients and Methods: A retrospective review of the data of all children aged 15 years and below who had colostomy and colostomy closure in the past 5 years. Results: Of the 67 children who had colostomy 42 (62.7%) boys and 25 (37.3%) girls, with an age range between 13 months and 8 years. Fifty-six (83.6%) of the children were <2 years. Anorectal malformation 53 (79.1%) was the common indication. Divided colostomy was performed in 62 (92.5%) patients and loop colostomy was performed in 5 (7.5%) patients. All the patients had intraperitoneal colostomy closure. A complication rate of 26.4% was seen. Duration of hospital stay ranged between 4 and 10 days. No mortality was recorded. Conclusion: Colostomy reversal is a safe procedure but morbidity may ensure and can easily manage.
Résumé Contexte: La colostomie est l'une des interventions chirurgicales courantes pratiquées en chirurgie pédiatrique. Le but de cette étude était de revoir rétrospectivement notre expérience en matière de colostomie et de fermeture (inversion) chez les enfants. Méthode: Une revue rétrospective des données de tous les enfants âgés de 15 ans et moins ayant subi une colostomie et une fermeture de colostomie au cours des 5 dernières années. Résultats: Sur les 67 enfants ayant subi une colostomie, 42 (62,7 %) garçons et 25 (37,3 %) filles, avec une tranche d'âge comprise entre 13 mois et 8 ans. Cinquante-six (83,6 %) des enfants avaient moins de 2 ans. La malformation ano-rectale 53 (79,1 %) était l'indication fréquente. Une colostomie divisée a été réalisée chez 62 (92,5 %) patients et une colostomie en anse réalisée chez 5 (7,5 %) patients. Tous les patients ont eu une colostomie intrapéritonéale fermée. Un taux de complications de 26,4 % a été observé. La durée du séjour à l'hôpital variait entre 4 et 10 jours. Aucune mortalité enregistrée. Conclusion: l'inversion de la colostomie est une procédure sûre mais la morbidité peut être assurée et peut être facilement gérée.

Internet of medical things (IoMT) has made it possible to collect applications and medical devices to improve healthcare information technology. Since the advent of the pandemic of coronavirus (COVID-19) in 2019, public health information has become more sensitive than ever. Moreover, different news items incorporated have resulted in differing public perceptions of COVID-19, especially on the social media platform and infrastructure. In addition, the unprecedented virality and changing nature of COVID-19 makes call centres to be likely overstressed, which is due to a lack of authentic and unregulated public media information. Furthermore, the lack of data privacy has restricted the sharing of COVID-19 information among health institutions. To resolve the above-mentioned limitations, this paper is proposing a privacy infrastructure based on federated learning and blockchain. The proposed infrastructure has the potentials to enhance the trust and authenticity of public media to disseminate COVID-19 information. Also, the proposed infrastructure can effectively provide a shared model while preserving the privacy of data owners. Furthermore, information security and privacy analyses show that the proposed infrastructure is robust against information security-related attacks.

In this paper, we address the problems of fraud and anomalies in the Bitcoin network. These are common problems in e-banking and online transactions. However, as the financial sector evolves, so do the methods for fraud and anomalies. Moreover, blockchain technology is being introduced as the most secure method integrated into finance. However, along with these advanced technologies, many frauds are also increasing every year. Therefore, we propose a secure fraud detection model based on machine learning and blockchain. There are two machine learning algorithms-XGboost and random forest (RF)-used for transaction classification. The machine learning techniques train the dataset based on the fraudulent and integrated transaction patterns and predict the new incoming transactions. The blockchain technology is integrated with machine learning algorithms to detect fraudulent transactions in the Bitcoin network. In the proposed model, XGboost and random forest (RF) algorithms are used to classify transactions and predict transaction patterns. We also calculate the precision and AUC of the models to measure the accuracy. A security analysis of the proposed smart contract is also performed to show the robustness of our system. In addition, an attacker model is also proposed to protect the proposed system from attacks and vulnerabilities.

In this paper, a secure energy trading mechanism based on blockchain technology is proposed. The proposed model deals with energy trading problems such as insecure energy trading and inefficient charging mechanisms for electric vehicles (EVs) in a vehicular energy network (VEN). EVs face two major problems: finding an optimal charging station and calculating the exact amount of energy required to reach the selected charging station. Moreover, in traditional trading approaches, centralized parties are involved in energy trading, which leads to various issues such as increased computational cost, increased computational delay, data tempering and a single point of failure. Furthermore, EVs face various energy challenges, such as imbalanced load supply and fluctuations in voltage level. Therefore, a demand-response (DR) pricing strategy enables EV users to flatten load curves and efficiently adjust electricity usage. In this work, communication between EVs and aggregators is efficiently performed through blockchain. Moreover, a branching concept is involved in the proposed system, which divides EV data into two different branches: a Fraud Chain (F-chain) and an Integrity Chain (I-chain). The proposed branching mechanism helps solve the storage problem and reduces computational time. Moreover, an attacker model is designed to check the robustness of the proposed system against double-spending and replay attacks. Security analysis of the proposed smart contract is also given in this paper. Simulation results show that the proposed work efficiently reduces the charging cost and time in a VEN.

The exponential growth of intelligent vehicles(IVs) development has resulted in a complex network. As the number of IVs in a network increases, so does the number of connections. As a result, a great deal of data is generated. This complexity leads to insecure communication, traffic congestion, security, and privacy issues in vehicular networks (VNs). In addition, detecting malicious IVs, data integration, and data validation are major issues in VNs that affect network performance. A blockchain-based model for secure communication and malicious IV detection is proposed to address the above issues. In addition, this system also addresses data integration and transaction validation using an encryption scheme for secure communication. A multi-chain concept separates the legitimate and malicious data into two chains: the Integrity chain (I-chain) and Fraud chain (F-chain). This multi-chain mechanism solves the storage problem and reduces the computing power. The integration of blockchain in the proposed model provides privacy, network security, transparency, and immutability. To address the storage issue, the InterPlanetary File System (IPFS) is integrated with Certificate Authority (CA). A reputation mechanism is introduced to detect malicious IVs in the network based on ratings. This reputation mechanism is also used to prevent Sybil attack. The evaluation of the proposed work is based on the cost of smart contracts and computation time. Furthermore, two attacker models are presented to prevent the selfish mining attack and the Sybil attack. Finally, a security analysis of the proposed smart contracts with their security vulnerabilities is also presented.

Trypanosoma brucei is one of the most pathogenic species of the genus Trypanosoma, and T. brucei brucei is one of the subspecies that is of great economic concern to animals. A large range of labouratory animals are commonly used in Trypanosoma studies. This study is aimed at exploring the possibility of using guinea fowls as experimental models for future studies and preservation of T. b. brucei. In achieving our aim, we studied the infectivity and pathogenicity of T. b. brucei in guinea fowls in relation to rabbits. The level of parasitaemia, mean body weight, mean temperature, haematological and histopathological parameters were accessed. Ten each of rabbits (Oryctolagus cuniculus) (control model) and guinea fowls (Numidia meleagris) (study model) (5 in the uninfected groups and 5 in the infected groups) were used for this study. The infected rabbits were inoculated intraperitoneally, while the infected guinea fowls were inoculated through the wing veins. Both animals were inoculated with 0.20 ml of T. b. brucei-infected blood estimated to be 1× 106 parasites/ ml. The infected rabbits and guinea fowls were screened daily for the presence of T. b. brucei using the haematocrit centrifugation technique (HCT). The mean weight, mean temperature and haematological parameters were accessed weekly, while the histopathological parameters were accessed at the end of the study. Trypanosoma b. brucei was detected in the blood of infected rabbits about 8 days post-infection, while there was no parasitaemia in the infected guinea fowls. The haemoflagellate exerted a significant (p < 0.05) effect on the mean body weight, mean temperature and haematological parameters of rabbits compared to guinea fowls. The pathological effects of T. b. brucei infection was seen in the liver and kidney of infected rabbits, and in the spleen of infected guinea fowls. There appears to be no successful multiplication and proliferation of T. b. brucei in the guinea fowls, making it not to be a suitable animal model for experimental studies and preservation of T. b. brucei.

This study aims at investigating the protective effects of flavonoid fractions of diosmin and hesperidin in mitigating sub-chronic lead acetate-induced biochemical, oxidative stress, and histopathological alterations in adult male Wistar rats. Forty animals were randomly assigned into five groups, each consisting of eight animals. Group I animals was treated with deionised water only, group II, IV, and V were administered lead acetate 90 mg/Kg body weight (1/20th of the LD50), groups III, and IV was administered Daflon (100 mg/Kg), while group V was administered Daflon (200 mg/Kg), 30 min prior treatment with lead acetate. All treatments lasted for 42 days. Blood lead levels, electrolyte parameters, zinc protoporphyrin (ZPP) levels, activities of antioxidant enzymes, and histopathology of vital organs, were evaluated following standard practice. Sub-chronic lead acetate exposure induced a decrease in levels of serum electrolytes, and activities of antioxidant enzymes, while blood lead levels, ZPP, and malondialdehyde levels were increased. Lead exposure also instigated marked variation in histopathology of vital organs. Conversely, co-treatment with graded doses of daflon improved the levels of blood lead, electrolytes, ZPP, activities of antioxidant enzymes, and histopathology of vital organs. Data obtained from the current study indicate that rats exposed to sub-chronic doses of lead acetate show increased blood lead levels, electrolyte imbalance, alongside impairment in ZPP levels, activities of antioxidant enzymes, and histopathology, while pretreatment using daflon mitigated the ensued perturbations. This, therefore, suggests that consumption of foods enriched with flavonoid fractions of diosmin and hesperidin may be beneficial for individuals inhabiting lead-polluted environments.

OBJECTIVES: This study investigated serum sialic acids for a predictive and diagnostic biomarker of diabetes mellitus (DM) in dogs and its prognostic value with ethanolic extract of Anogeissus leiocarpus. DESIGN: Four groups of 3 dogs were used; non-diabetic controls (ND), diabetic-untreated (DU), diabetic insulin-treated (DI) and diabetic extract-treated (DE). Free serum sialic acids (FSSA) and erythrocyte surface sialic acids (ESSA) were assayed in all groups, pre-and post-induction of hyperglycaemia and results were presented as means ± standard error of means (SEM) and subjected to ANOVA using Tukey's post-hoc tests with GraphPad Prism® statistical package. RESULTS: FSSA increased in DU and plateaued at third week (61.8 ± 0.41 µg/ml), (P < 0.002) with additional 38.2 µg/ml (62%) generated, coinciding with hyperglycaemia. FSSA of DI increased but declined to 22.3 ± 1.55 µg/ml. Extract of Anogeissus leiocarpus effectively modulated FSSA in DE as increased value declined to 21.4 ± 0.78 µg/ml. Pre-induction DU ESSA (8.27 ± 0.11 µg/ml) significantly (P < 0.002) decreased by third week (2.33 ± 1.49 µg/ml), coinciding with hyperglycaemia. Strong negative correlation coefficient (r = -0.92) occurred between DU's FSSA and ESSA and ND (P < 0.03). Sialic acid expression in dog's insulin dependent diabetes mellitus (IDDM) is 18% lower than normal. Extract of A. leiocarpus restored ESSA completely. ESSA cleaved in DU, 5.94 µg/ml (72%), could not account for the extra FSSA (32.26 µg/ml); liver and kidneys are contributors. CONCLUSION: FSSA predicts canine DM.

Equine piroplasmosis, an economically important disease of equids caused by the hemoprotozoan parasites Theileria equi, T. haneyi, and Babesia caballi, has a worldwide distribution. These parasites are transmitted by ixodid ticks. To improve the detection of horses in Nigeria exposed to piroplasm parasites, 72 horses with variable clinical signs of piroplasmosis were sampled from Northwest and Northcentral Nigeria and tested by nPCR and cELISA. Blood and serum samples were collected from each horse via jugular venesection. Individually, nPCR or cELISA failed to identify all horses exposed to piroplasms. A combination of species-specific nPCR and the OIE-approved T. equi and B. caballi cELISAs enhanced the detection of horses exposed to parasites. The results also demonstrated horses showing abnormal hematology were positive for only T. equi, except for one sample that was coinfected with T. equi and T. haneyi. We also identified ticks collected from some of the horses, with Rhipicephalus evertsi evertsi being the most prevalent. This study shows that a larger proportion of horses in the sample set were exposed to T. equi than B. caballi or T. haneyi. Additionally, ticks that have been previously reported as potential vectors for these parasites were found to have infested sampled horses. Further studies are needed to investigate which tick species are competent vectors for Theileria spp. and Babesia caballi in Nigeria.

BACKGROUND: The folkloric claim that Cnidoscolus aconitifolius (Chaya) could ameliorate anaemia requires scientific revalidation in anaemic models. OBJECTIVE: The aim of this study is to investigate the anti-anaemic effect of Cnidoscolus aconitifolius on cyclophosphamide-induced anaemia in rats. METHODS: The leaves of Cnidoscolus aconitifolius were collected, dried and extracted with ethanol. Twenty five Wistar Albino rats weighing 120-180 kg were used. Anaemia was induced in groups 2, 3, 4 and 5 with a single injection of cyclophosphamide, while group 1 served as a negative control without anemia and was treated with 0.5 mL of normal saline, Group 2 rats were treated with 100 mg/kg body weight of Cnidoscolus aconitifolius. Group 3 rats were treated with 500 mg/kg body weight of Cnidoscolus aconitifolius. Group 4 served as the positive control rats and was treated with 0.5 mL of normal saline. Group 5 which served as the standard control rats and were treated with 5 mg/kg body weight of standard drug Chemiron. The treated lasted for two weeks during which blood samples were collected from each rat for haematological analysis. RESULTS: The result shows that hematological parameters were elevated in groups 2, 3 and 5 rats treated with 100 mg/kg and 500 mg/kg body weight of ethanol extract of Cnidoscolus aconitifolius and chemiron respectively. CONCLUSIONS: Ethanol leaf extract of Cnidoscolus aconitifolius at a graded dose of 500 mg/kg body weight had higher ameliorative effect on the haematological parameters of cyclophosphamide-induced anemia in rats.

One hundred each, of Muturu and Bunaji cattle were screened, using polymerase chain reaction (PCR), for trypanosomes in Makurdi and Gboko Local Government Areas of Benue State, Nigeria. Erythrocyte surface sialic acid (ESSA) and free serum sialic acid (FSSA) concentrations were determined and compared in both breeds with the aim of providing baseline data for research and diagnostic purposes. Five per cent (5%) and 23% of the Muturu and Bunaji cattle, respectively, were positive for trypanosomes. The result at p=0.005 was significantly different, with p value of 0.0002 and odd ratio of 0.1762. The Trypanosoma species circulating in Benue State, as detected in the two breeds of cattle, were Trypanosoma congolense, T. vivax, T. brucei and T. evansi. This study, therefore, reports for the first time a natural infection of cattle with T. evansi and the use of a novel PCR in the diagnosis of trypanosome infections in cattle in Benue State, Nigeria. The determination of the ESSA and FSSA concentrations in Muturu cattle in Nigeria is also reported for the first time. The Muturu cattle have a significantly higher ESSA than the Bunaji cattle, this may be responsible for their relative trypanotolerance.

The study was designed to determine the effect of Trypanosoma evansi infection on some pregnancy biomarkers of Yankasa ewes (YE). Twenty pregnant YE were assigned into 3 groups (A, B and C) comprising 7 ewes each in groups A and B, while group C comprise 6 YE. Groups A and B were each inoculated with blood containing approximately 1.0 × 10(6) of T. evansi through the jugular vein on days 59 and 110 of pregnancy, representing second and third trimesters, respectively, while group C served as the uninfected control. Progesterone (P4) and pregnancy specific protein-B (PSPB) of YE in group A were significantly (p < 0.05) high at weeks 4 and 12 post infection (pi) respectively, while there was no significant (p > 0.05) difference in P4 and PSPB of YE in groups B. Estrone sulfate (E1S) significantly (p < 0.05) decrease for YE in group A at weeks 2 and 11 pi. However, it was not significantly (p > 0.05) different in group B. Cortisol concentration of YE in group A was significantly (p < 0.05) decreased at week 12 pi. Conversely, the cortisol concentration of YE in group B significantly (p < 0.05) increased at week 3 pi. There was no significant (p > 0.05) association among the pregnancy biomarkers of YE in groups A and B throughout the study, except between progesterone and cortisol in group B, which were significantly associated (r = 0.77, p < 0.05). It was therefore concluded that T. evansi infection affects pregnancy biomarkers more at mid pregnancy than at late pregnancy.

Conjoined twins are rare and are classified as symmetrical or asymmetrical, in which a member, the host (autosite), is near normal and bears the parasite, which is incomplete, smaller, and fully dependent for growth on it. This form of conjoined twins is referred to as heteropagus and when attached to the epigastrium of the autosite is called epigastric heteropagus. Only 44 cases of epigastric heteropagus twins have been previously reported in the world literature. We hereby report the successful separation of a pair of heteropagus twins.

Although lipoma is a common benign mesenchymal tumor, its occurrence in the omentum is a rare finding. We report an unusual case of omental lipoma in a 13-year-old adolescent girl. The mass was completely excised and weighed 12.3 kg. The patient is alive and well with no evidence of recurrence at 4 months of follow-up.
