Key Clinical Message: Clinicians should carefully consider generalized lymphadenopathy, particularly post viral infections, as one of the possible systemic lupus erythematous (SLE) first signs regarding unusual joint involvements such as sacroiliitis. Late diagnosis of this autoimmune inflammatory disease, could lead to irreversible morbidity and higher mortality. Abstract: Lymphadenopathy could represent various etiologies, including infections, malignancies, and rheumatologic diseases. SLE is known as the great mimicker which could be presented with different first manifestations. We report a 42-year-old woman in the acute phase of Epstein-Barr infection, admitted with polyarticular peripheral arthritis, sacroiliitis, and generalized lymphadenopathy. She had no similar history or taken unpasteurized dairy. Nodes were soft, mobile, and tender without skin change on top. During the process, she was diagnosed with SLE and discharged with prednisolone 30 mg/day and hydroxychloroquine 400 mg/day. After 2 weeks of follow-up, all lymphadenopathy and symptoms were diminished. This case underscores the thousand faces innate of SLE. Clinical awareness would lead to an accurate diagnosis and early intervention.

BACKGROUND: Diffuse midline gliomas (DMGs) encompass a set of tumors, and those tumors with H3K27M mutation carry a poor prognosis. In recent years, machine learning (ML)-based radiomics have shown promising results in predicting gene mutation status non-invasively. Therefore, this study aims to comprehensively evaluate the diagnostic performance of ML-based magnetic resonance imaging (MRI) radiomics in predicting H3K27M mutation status in DMG patients. METHODS: A systematic search was conducted using relevant keywords in PubMed/Medline, Scopus, Embase, and Web of Science from inception to May 2023. Original studies evaluating the diagnostic performance of ML models in predicting H3K27M mutation status in DMGs were enrolled. Quality assessment of the enrolled studies was conducted using QUADAS-2. Data were analyzed using STATA version 17.0 to calculate pooled sensitivity, specificity, positive (PLR) and negative likelihood ratio (NLR), diagnostic score, and diagnostic odds ratio (DOR). RESULTS: A total of 13 studies, including 12 retrospectives and one both retrospective and prospective study, enrolled 1510 (male=777) DMG patients. Six studies underwent meta-analysis which showed a pooled sensitivity, specificity, PLR, NLR, diagnostic score, and DOR of 0.91 (95% CI 0.77-0.97), 0.81 (95% CI 0.73-0.88), 4.86 (95% CI 3.25-7.24), 0.11 (95% CI 0.04-0.29), 3.75 (95% CI 2.62-4.88), and 42.61 (95% CI 13.77-131.87), respectively. CONCLUSION: Non-invasive prediction of H3K27M mutation status in patients with DMGs using MRI radiomics is a promising tool with good diagnostic performance. However, the pooled metrics had a wide confidence interval, which required further studies to enhance ML algorithms' accuracy and facilitate their integration into daily clinical practice.

BACKGROUND: This review compare different Vocal Tract Discomfort (VTD) versions. This comparison is based on their validity and reliability parameters in the translation and adaptation process. We aimed to prepare numerical evidence to prove the validity of this easy screening tool. VTD is able to perform an accurate diagnosis of voice discomforts, particularly in primary stages. METHODS: Articles were selected from databases including Google Scholar, PubMed, Science Direct and Scopus. Our relevant papers were gathered by searching the phrase: VTD in titles, abstracts, and keys. Studies not followed an adaptive procedure were excluded. Based on the selection criteria, out of 23 collected articles, eight were studied in this review. RESULTS: Standard psychometric protocol steps were followed in all selected articles and simultaneously high reliability and validity were reported in their translation procedure. Such analogous results may confirm the efficacy of this research tool. CONCLUSION: This review affirms VTD, perceptual patient-based scale, as a valuable evaluation tool to investigate the occurrence of voice disorders. Based on its structure and performance, VTD can work as a quick and precise source for predicting vocal discomforts. Moreover, this capability can help professional therapists to plan more efficient treatment procedures. The other important advantage of VTD is its diagnostic and prognostic capacity to inform patients about their current and future conditions so that they would be motivated to follow treatment procedures more consistently.
