RESUMO

PURPOSE OF REVIEW: In patients with systemic lupus erythematosus (SLE), cardiovascular involvement is common and a major cause of morbidity and mortality. There have been few recent updates regarding the cardiac involvement in this clinical entity. The purpose of the review is to provide an update on the role of echocardiography in the management of these patients. RECENT FINDINGS: Echocardiography remains the imaging modality of choice and should be considered even in asymptomatic patients with SLE to detect cardiac abnormalities which are frequently not clinically apparent. Transesophageal echocardiography has higher sensitivity and specificity in identifying valvular lesions, and should be utilized in high risk patients when transthoracic echocardiography is negative. New advances such as speckle tracking echocardiography has shown promise in the detection of occult myocardial dysfunction, but more studies are needed to have a proper perspective of its role in SLE patients. SLE has protean cardiac manifestations. The most common involvement is pericarditis. Complicated pericarditis such as tamponade and constriction are rare but should be considered if the symptoms do not subside with treatment. Valvular involvement can take several forms. Libman-Sacks endocarditis is the most common form and is more prevalent in patients with high disease activity and with the presence of antisphopholipid antibodies. Myocardial involvement portends poor prognosis and should be sought and treated promptly to prevent morbidity and mortality.

RESUMO

BACKGROUND: Cholangiocarcinoma has poor prognosis and short term-survival. Here, we report the case of a patient with unusually prolonged survival. CASE PRESENTATION: Our patient was a 56-year-old Arab man with a 6-month history of obstructive jaundice. A computed tomography scan of his abdomen revealed a mass at the confluence of the hepatic ducts with suspected malignant strictures on endoscopy. A positive tissue diagnosis was achieved more than 18 months after commencement of his symptoms. He remained functional throughout this period despite recurrent episodes of cholangitis. CONCLUSIONS: Cholangiocarcinoma is a presumably fatal disease, especially because patients tend to present late with unresectable disease. Many patient-related and disease-related factors may alter survival.

RESUMO

OBJECTIVES: This study was conducted to determine the prevalence of smoking, knowledge about the ill effects of smoking on health, and the influence of family members' smoking habits among Saudi female students. METHODS: This is a type of cross-sectional study. A sample of 1,070 female students was selected by a nonrandom and convenient sampling method from five colleges (Medicine, Business and Administration, Computer Sciences, Education, and Languages and Translation) of King Saud University, Riyadh, Saudi Arabia. A self-administrated questionnaire was used to determine the personal, social, and educational characteristics of the respondents. In addition, questions about their smoking types, status, duration of smoking, knowledge about the ill effects of smoking, daily cigarette consumption, and reasons for quitting smoking were included. RESULTS: The students' response rate was 85%. The prevalence of current smoking was 4.3% and 5.6% for cigarettes and water-pipes, respectively, whereas 3.9% of the participants were ex-smokers. The prevalence of current smoking was highest in the College of Business and Administration (10.81%) and lowest in the College of Medicine (0.86%). The majority (77%) of the smokers' parents (current and ex-smokers) were also smokers. More than half (54%) of the smokers started their smoking habit for entertainment, and 44.4% of the participants did not know that smoking causes serious health problems. The most common factors for quitting smoking were health concerns (54%), religious beliefs (29%), and parent's advice (17%). CONCLUSION: The study concludes that the prevalence of smoking varies in different subject streams and that family and friends have a great influence on individuals starting or stopping smoking. Extensive health education programs are needed to educate young women on the health hazards of smoking and help stop them from smoking.

RESUMO

Osteosarcoma OS of the talus is extremely rare, and few cases have been reported in the literature. We present a case of a 33-year-old male with painful swelling of his left ankle joint. He underwent several radiological diagnostic modalities that showed osteolytic lesion in the posterior aspect of the left talus associated with new bone formation projecting from the posterior-medial aspect of that bone. His chest x-ray showed multiple rounded lung metastases, some of them showed calcifications. Open biopsy was performed. The histopathology confirmed the diagnosis of osteoblastic OS of the talus.
