RESUMO

Altogether 125 patients with mandibular fractures of different localization were treated. Implantation of ultrahigh dispersed hydroxyapatite was included in the treatment schemes, both surgical and conservative, of 65 of these patients. Such implantations were found easy to perform, well tolerated by the patients, and causing no side effects. The incidence of complications in this group was by 6.78% lower than in controls treated by the traditional methods alone and by 7.16% lower than in all patients with mandibular fractures treated over a year. The mean disability period was reduced by 4.8 days when this preparation was used.

RESUMO

Hydroxyapatite paste with lincomycin (linco-HAP) was for the time used for the treatment of such a prevalent disease as periodontitis. A follow-up of 380 patients demonstrated a positive therapeutic effect of this complex expressed in reduction of the depth of periodontal pouches and inflammation arrest, thus appreciably shortening the treatment duration.

Pyoinflammatory processes run an uncommonly malignant course in diabetics. Healing of postoperative wounds in diabetics is caused by impairement of all types of metabolism and by development of endogenous intoxication which depresses the protein-generating function of the liver. The plasma sorption technique used at the clinic of Department of Maxillofacial Surgery and Dentistry, Russian State Medical University, helps more effectively control endogenous intoxication, this, in its turn, promoting activation of cleansing and healing of postoperative wounds. Basing on the clinical and laboratory findings, the authors demonstrated that plasma sorption used in multiple-modality treatment of diabetics with pyoinflammatory processes accelerates healing of purulent wounds by 5-6 days.

RESUMO

Protegentin is a formulation in the form of an ointment for local application. Its high efficacy was demonstrated in the treatment of wounds in the 1st phase of the process, extended decubitus, trophic ulcers, wounds of various genesis not healing for prolonged periods, urogenital infections, abscesses, maxillofacial phlegmon and abscessing parodontosis. The ointment showed good draining, proteolytic and bactericidal effects. The tolerance was good. The positive results were observed in 97 per cent of the cases.

RESUMO

Studies of a relationship between the severity of the developing secondary tissue hypoxia and reduction of the neutrophil phagocytic activity in patients with combined maxillofacial and craniocerebral injuries of varying severity have prompted the authors to include ultraviolet irradiation of the blood and essential therapy in the complex of therapeutic measures aimed at prevention of pyoinflammatory complications with due consideration for the severity of secondary tissue hypoxia detected in the patients on admission to hospital.

RESUMO

Study of the relationship between the time course of secondary tissue hypoxia development and changes in the neutrophilic phagocytic activity, which was carried out in 65 patients with combined maxillofacial and craniocerebral injury, has revealed a detrimental effect of hypoxia on antibacterial defense factors of the body, this effect being conductive to development of pyoinflammatory complications; this should be borne in mind when planning treatment strategy.

RESUMO

RMBF was comparatively assessed in patients with lower jaw fractures under orthopedic therapy of the osteal suture. To this end, the 99m pertechnetate was injected bilaterally in masseter muscles. The data suggest that double jaw fixation had an adverse effect on RMBF, especially ipsilateral to the fracture. Nevertheless, the mineral metabolism intensity in the callus is higher under fixation as related to the cases surgically sutured.

RESUMO

An analysis of causes of bleedings in 19 patients with malignant tumors of the head and neck has been made. The combined treatment was performed for the III-IV degree tumorous process. As a rule, the bleeding was of erosive character due to ingrowth of the tumor into the walls of major vessels. Alterations of the vascular wall after radiation therapy make the arrest of bleeding in the wound impossible. So, the most reliable method is ligation of the vessels at length.

RESUMO

Under analysis were observations of 34 patients with obliterating diseases of the lower extremity vessels. All of them were treated by autotransfusion of ultraviolet irradiated blood. It was noted that the patients' state improved, pains subsided and the ulcers were epithelized. Control examinations have revealed better blood filling of the tissues and increased oxygen saturation of blood.

An analysis of outcomes of the surgical treatment of patients with malignant tumors of the hairy part of head has shown expediency of using pedicle flaps from neighbouring and remote parts. When planning the procedures the esthetic effect of the operation should be taken into consideration because of the necessity to restore the hair of the scalp.

The authors analyze the experience with the reinfusion of the autoblood subjected to ultraviolet irradiation to 25 patients with inflammatory diseases of the maxillofacial area. The positive curative effect of this method of hemotherapy is demonstrated.

A serological study of Leishmania clones from experimentally and naturally infected sand flies was undertaken. The cloning was done by means of Fonbrun's micromanipulator. Serological studies were conducted according to Adler's method in Safjanova's modification. Among the clones of Phlebotomus papatasi experimentally infected with Leishmania tropica major and L. gymnodactyli four were found to be serologically identical with L. tropica major, and two had close antigene relationships with both initial strains of Leishmania. No clones identical with L. gymnodactyli were found. By means of serological comparison to museum strains of Leishmania heterogeneity of the strains isolated from naturally infected sand flies and great variety of antigenic properties of clones obtained from them have been shown. Thus, among clones of Ph. caucasicus four are identical with L. donovani, one is closely related to Leptomonas pessoai and one displays no antigenic relationships to any standard strain of Leishmania. Two clones from naturally infected Ph. papatasi are identical with L. tropica major, one displays close antigenic relationships with L. tropica major and L. donovani and one could not be identified. The question is raised on the necessity to study possible mechanisms of genetic information exchange to explain the appearance of clones with mixed properties.
