Background Musculoskeletal pain (MSP) is widely recognized as a prevalent public health issue that affects individuals of various genders and age groups. The aim of this study is to assess the influence of musculoskeletal pain on the quality of life (QoL) of adult individuals living in Saudi Arabia. Method This is a cross-sectional study using an online-administered questionnaire that was distributed via online platforms in Saudi Arabia for the duration between January and August 2023. Three questionnaire instruments were used in this study. The 36-item Short Form Health Survey questionnaire (SF-36), The 5-level EQ-5D version (EQ-5D-5L), and Roland-Morris Disability Questionnaire. Binary logistic regression analysis was used to identify predictors of better QoL and having mild to moderate disability. Results A total of 8359 participants were involved in this study. The most commonly reported sites of musculoskeletal pain were the lower back, neck, and shoulder, accounting for 36.8% (n= 3072), 30.5% (n= 2549), and 30.1% (n= 2514), respectively. The mean pain score for the study participants was 4.3 (SD: 2.3), which indicates mild degree of pain. The median EQ-5D-5L index value for the study participants was 0.827 (0.756-1.00), which demonstrates a high quality of life. The mean SF-36 score for the study participants was 63.11 (17.4), which demonstrates moderate quality of life. The median Roland-Morris Disability score for the study participants was 1.00 (0.00-7.00), which demonstrates a low level of pain-related disability. Male gender, younger age (30-39 years), having higher education attainment, having higher monthly income (more than SAR 20000), and having lower BMI (less than 25.8 kg/cm2) were predictors of better QoL (p<0.05). Conclusion Musculoskeletal pain is a multifactorial condition influenced by structural, physical, psychological, social, lifestyle, and comorbid health elements. It ranges from acute to chronic pain and, despite rarely being fatal, has a significant impact on QoL. Musculoskeletal discomfort varies in terms of intensity, affected regions, and demographic and lifestyle factors. This study sheds light on the multifaceted nature of MSP, its impact on QoL, and the significance of early intervention and individualized management strategies to improve the QoL of those affected.

Spinal accessory nerve palsy (SANP) is rare and is commonly presented following iatrogenic injury. Their diagnosis is often missed on initial presentation. Injury following blunt trauma is rare, with few cases reported in literature describing blunt-associated SANP and their treatment and recovery. We present and discuss a case of SANP following an aggressive soft tissue adjustment by an uncertified individual that has been responsive to nonsurgical measures over 18 months. We also reviewed the related literature on similar cases that were presented as result of direct pressure on the nerve from soft tissue manipulation or heavy lifting and their outcome following treatment. Chiropractic is generally a safe complimentary medicine and must only be practiced by trained personnel. We found that blunt-caused SANP injuries should initially be treated conservatively as they are likely to respond and recover unlike when presented following invasive trauma accordingly to what we found in literature.

Objective: The goal of this study was to see if there was a link between individuals who had a sleeve gastrectomy and their knee discomfort, as well as what factors could influence knee pain after a sleeve gastrectomy. Materials and Methods: The general population of Saudi Arabia was studied in this cross-sectional survey, which took place from August 2021 to February 2022. Only people who had had a sleeve gastrectomy and were experiencing knee discomfort were included in the research. Results: The results revealed that bariatric surgery had improved knee pain status in most of the participants (76%) while (12.1%) complained that the pain worsened after the surgery. The results also showed that nearly half of our participants (49.8%) that complain of knee pain have lost more than 35 kg after bariatric surgery, which illustrates the impact of a number of kilograms (kg) lost after bariatric surgery. Only (12.8%) of the participants considered physiotherapy as an intervention for their knee pain. Also, the results showed that severe pain was considerably more prevalent in the older age group and those with concomitant chronic disease. Conclusions: Our data revealed that a small percentage of participants had worsening knee discomfort, which might be connected to inactivity following sleeve gastrectomy surgery. As a result, we urge that further clinical studies be conducted to learn more about the exacerbation of knee pain after a gastrectomy.

OBJECTIVES: To establish the prevalence of Neuropsychiatric Systemic Lupus Erythematosus (NPSLE) symptoms in Saudi Arabia and the variables that are linked to it. METHODS: This cross-sectional study was conducted in June 2021 among SLE patients in Saudi Arabia. The Saudi Rheumatism Association exploited social media platforms to provide a self-administered online questionnaire to SLE patients. All data analyses were performed using the Statistical Packages for Social Sciences (SPSS) version 26. RESULTS: Two hundred and eight SLE patients participated in the study (females 91.3% vs. males 8.7%). In addition, 13.5% of patients had a family history of SLE, and 26% had SLE for one to 3 years. The most common symptoms of NPSLE were alteration or loss of sensation (53.4%), followed by fear (52.4%), and headache (48.1%). The prevalence of patients with NPSLE was 40%. In a multivariate regression model, fear, altered sensations, cerebrovascular illness, sleep disruption, and diminished interest in routine activities were identified as independent risk variables for NPSLE CONCLUSION: Nearly half of SLE patients demonstrated NP manifestations, with significant symptoms including fear, alteration of sensation, cerebrovascular disease, sleep disturbance, and reduced interest in normal activities. To detect the pathophysiology of NPSLE, it is necessary to understand the relationship between neuropsychiatric morbidity and other relevant rheumatic disorders in the SLE population.

RESUMO

A few cases reported simultaneous ipsilateral distal and proximal forearm fractures. This case report highlights a rare mechanism of injury that occurred due to extended forearm undergoing forced hyper-supination without the presence of ligamentous injury and the use of flexible fixation. A 49-year-old male truck driver presented to the emergency department as a motor vehicle accident (MVA) patient. A radiographic examination revealed a right forearm fracture with proximal and distal bone fractures. The distal radius was stabilized with three Kirschner wires (K-wires), and the radial neck fracture was stabilized with a single intramedullary K-wire. The olecranon and distal ulna fractures were also fixed with two intramedullary wires. On the eighth week after surgery, all K-wires were removed, and the fiberglass splint was reapplied for another two weeks. After the cast was removed, physical rehabilitation began. During the fourth month, a follow-up radiograph revealed complete healing and full wrist range of motion (ROM) with good hand grip.

OBJECTIVES: To assess psychiatrists' knowledge and perception regarding telepsychiatry and evaluate their willingness to adopt telepsychiatry clinical practice in Saudi Arabia. METHODS: A cross-sectional study was conducted among psychiatrists working in Saudi Arabia from November 2020 through May 2021. A self-administered questionnaire comprising socio-demographic data, factors related to knowledge, perception, willingness, barriers, and the effectiveness of telepsychiatry, was distributed via. online platform. Data were tabulated and cleaned in MS Excel, and all statistical analyses were performed using SPSS v26. RESULTS: There were 328 psychiatrists enrolled in the group with an average age of 25-35 years (48.8%). The group comprised mainly Saudis (83.5%); male participants outnumbered females (70.4% to 29.6%). Overall, the psychiatrists' telepsychiatry knowledge level was poor (51.8%), while (48.2%) of the respondents showed good knowledge. However, nearly all respondents exhibited good perception (80.8%), with only 19.2% classified as poor. In addition, older individuals, consultants, clinicians with 11-15 years of experience, clinicians interacting with patients via email, and those who frequently received patient questions regarding online communication indicated increased knowledge. CONCLUSION: Although perception was positive regarding telepsychiatry, psychiatrists' knowledge on the subject was deemed insufficient. Psychiatrists' knowledge depended on their age, position, years of experience, frequent interaction with patients through an online platform, and clients that provided their online contact details.
