Introduction: This study aimed to investigate whether hyperbaric oxygen treatment (HBOT) could ameliorate ischaemia-reperfusion injury in a rat model of ovarian torsion-detorsion. Methods: Twenty-seven rats were divided among four groups: surgical sham rats (S) (n = 6) underwent identical anaesthesia and surgical incisions to other groups (n = 7 per group) but with no ovary intervention; torsion rats (T) underwent laparotomy, ovarian torsion, relaparotomy and sacrifice after three hours; torsion and detorsion rats (T/DT) underwent laparotomy, ovarian torsion (three hours), relaparotomy and detorsion, and sacrifice after one week; torsion, detorsion, hyperbaric oxygen rats (T/DT/HBOT) underwent laparotomy, ovarian torsion, relaparotomy and detorsion, and sacrifice after one week during which HBOT was provided 21 times (100% oxygen at 600 kPa for 50 min). In all groups blood collection for markers of oxidative stress or related responses, and ovary collection for histology were performed after sacrifice. Results: When the T/DT, and T/DT/HBOT groups were compared, 8-hydroxy-2'-deoxyguanosine (a marker of oxidative damage to DNA) and malondialdehyde (a product of lipid peroxidation) levels were lower in the T/DT/HBOT group. Anti-Mullerian hormone levels were higher in the T/DT/HBOT group compared to the T/DT group. In addition, oedema, vascular occlusion, neutrophilic infiltration and follicular cell damage were less in the T/DT/HBOT group than in the T/DT group. Conclusions: When biochemical and histopathological findings were evaluated together, HBOT appeared reduce ovarian ischaemia / reperfusion injury in this rat model of ovarian torsion-detorsion.

BACKGROUND: To compare dentofacial effects of distalization with miniscrew-supported pendulum and infrazygomatic crest miniscrews. METHODS: The study included 36 patients whose lateral cephalometric radiographs were obtained before (T0) and after (T1) distalization. Patients were divided into two groups according to the treatment methods: 1) distalization with miniscrew-supported pendulum (MSP) (n = 19; 14 girls and 5 boys; mean age 16.9 ± 1.46 years) and 2) distalization with infrazygomatic crest (IZC) miniscrews (n = 17; 8 girls and 9 boys; mean age 17.0 ± 1.68 years). Dental, skeletal and soft tissue measurements were performed on lateral cephalograms taken from individuals. RESULTS: A significant molar distalization was achieved both in the MSP group (3.52 ± 0.76 mm at 8.71 ± 2.02 months) and the IZC group (3.5 ± 0.74 mm at 9.7 ± 2.5 months) (P < 0.001 for both). Premolar distalization was significantly lower in the MSP group (1.73 ± 1.09 mm) than in the IZC group (2.81 ± 0.79 mm) (P < 0.01). Significant molar tipping was observed in both groups (P < 0.001), while no significant difference was found between the groups (P > 0.05). Retrusion of maxillary incisors were significantly higher in the IZC group (2.75 mm) than in the MSP group (0.98 mm) (P < 0.01). A significant difference was found between the two groups with regard to the changes in overjet and overbite (P < 0.05). CONCLUSIONS: Both distalization methods were found to be effective in achieving a Class I molar relationship in patients with Class II malocclusion.

PURPOSE: Staphylococcus epidermidis is the most common causative microorganism of ventriculoperitoneal shunt infections. This study aimed to compare linezolid and vancomycin treatments and to examine the effect of these antibiotics alone and combined with hyperbaric oxygen therapy on the amount of bacterial colonies in the experimental S. epidermidis shunt infection model. METHODS: A shunt catheter was placed in the cisterna magna of 49 adult male Wistar albino rats. The rats were randomly divided into seven groups, as follows: sterile control, infected control, vancomycin, linezolid, hyperbaric oxygen, vancomycin + hyperbaric oxygen, linezolid + hyperbaric oxygen. In all groups except the sterile control group, 0.2 ml 107 CFU/mL S. epidermidis was inoculated to the cisterna magna. Parenteral vancomycin was administered 40 mg/kg/day to the vancomycin groups, and 50 mg/kg/day of enteral linezolid to the linezolid groups. Hyperbaric oxygen groups were given 100% oxygen at a pressure of 2.4 ATA for 50 min a day. One day after the last treatment, colony quantities in the shunt catheters and CSF were analyzed. RESULTS: The number of CSF colonies in the linezolid group was significantly lower than in the vancomycin group (p < 0.05). The number of CSF colonies in the linezolid + HBO group was significantly lower than in the vancomycin + HBO group (p < 0.05). CONCLUSIONS: Linezolid treatment was found to be more effective than vancomycin in ventriculoperitoneal shunt infection caused by S. epidermidis. There was no statistical difference among other treatment groups. Hyperbaric oxygen therapy is shown to contribute to the sterilization of cultures.

STATEMENT OF PROBLEM: Subtractive and additive computer-aided design and computer-aided manufacturing (CAD-CAM) systems have been used in the fabrication of cobalt-chromium (Co-Cr) long-span restorations. However, the accuracy of fit of multiunit frameworks is unclear. PURPOSE: The purpose of this in vitro study was to compare the marginal and internal fit of implant-supported, cement-retained 3-unit, 4-unit, and 5-unit Co-Cr metal frameworks fabricated by the lost wax (LW), CAD-CAM milling, and selective laser melting (SLM) techniques. MATERIAL AND METHODS: A total of 90 Co-Cr metal frameworks were fabricated for 3-unit, 4-unit, or 5-unit implant-supported cement-retained restorations on stock abutments with 3 different manufacturing technique subgroups (LW, CAD-CAM milling, and SLM). The silicone replica technique was used to evaluate the marginal and internal discrepancy values. By using a light microscope at ×45 magnification and a digital measurement program, the thickness of the silicone layer was measured at 16 reference points on each abutment for a total of 3360 measurements. The effect of manufacturing techniques and number of units (groups) on discrepancy values was evaluated using a full factorial ANOVA model. Group and manufacturing technique effects were analyzed separately by 1-way ANOVA in case of significant interactions. Pairwise comparisons were evaluated using the Tukey post hoc test (α=.05). RESULTS: The mean marginal discrepancy of 3-unit frameworks showed no statistically significant differences in the LW (35 µm) and SLM (25 µm) techniques; however, the frameworks manufactured by CAD-CAM milling (68 µm) had the highest marginal discrepancy values (P<.001). The mean marginal discrepancy values were 40 µm (LW), 33 µm (CAD-CAM milling), and 25 µm (SLM) for 4-unit frameworks, and no significant differences were found among the manufacturing techniques. For 5-unit frameworks, CAD-CAM milling techniques had the widest mean marginal discrepancy values (85 µm), and copings manufactured by the LW technique had the lowest mean marginal discrepancy values (36 µm). For all manufacturing techniques, axial discrepancy values were not affected with respect to the unit number (P=.526). The highest internal discrepancy values were measured at the occlusal area in all groups. CONCLUSIONS: CAD-CAM milling had the poorest marginal fit values for 5-unit frameworks, whereas the LW technique demonstrated the best results. Unit number had no significant influence on the marginal and internal fit of the LW manufactured frameworks.

OBJECTIVES: The aim of this prospective clinical study was to evaluate the clinical outcomes of dental implants placed in the mandibular molar sites and immediately functionally restored compared with conventionally loaded controls in an in-patient study. MATERIAL AND METHODS: Twenty-four dental implants were placed in 12 patients who had first molar loss bilaterally in the mandibular area. One site of the patient was determined as immediately loaded (IL) and the other side was conventionally loaded (CL). Resonance frequency analyses for implant stability measurements, radiographic examinations for marginal bone levels and peri-implant evaluations were performed during the clinical follow-up appointments within 12 months. RESULTS: During the 12-month follow-up period, only one implant was lost in the IL group. The mean implant stability quotient values were 74.18+/-5.72 and 75.18+/-3.51 for Groups IL and CL at surgery, respectively, and the corresponding values were 75.36+/-5.88 and 75.64+/-4.84 at 1-year recall, respectively. The difference was not statistically significant between the two groups during the 12-month study period (P>0.05). When peri-implant parameters were evaluated, excellent peri-implant health was demonstrated during the 1-year observation period and all implants showed less than 1 mm of marginal bone resorption during the first year. CONCLUSIONS: In the present study, immediate functionally loading did not negatively affect implant stability, marginal bone levels and peri-implant health when compared with conventional loading of single-tooth implants.

RESUMO

This report describes the clinical evaluation of a patient with a mandibular gunshot defect and the prosthodontic treatment with an implant-supported screw-retained fixed partial denture. Treatment included cemented crowns for 2 maxillary implants and an implant-supported screw-retained fixed partial denture supported by 8 mandibular implants. Despite the implant malposition limitations, the esthetic and functional demands of the patient were fullfilled.

RESUMO

Deviation of the mandible toward the resected side is dependent upon the amount of soft and hard tissue resected, the method of surgical site closure, the degree of impaired tongue function, the presence and condition of teeth, the loss of proprioceptive sense of occlusion, and the timing of prosthodontic therapy. Prosthodontic therapy combined with physical therapy may be beneficial for reducing and minimizing mandibular deviation. This clinical report describes the fabrication of cast metal guidance flange prostheses. The supporting flanges assist in controlling mandibular deviation and coordinate masticatory movements for a patient following a segmental mandibulectomy.

RESUMO

OBJECTIVE: Prosthetic treatment for speech disorders attributable to surgically acquired soft palate defects are introduced. CASE: A patient who underwent soft palate resection for cancer is presented. The resected portion of the soft palate was confined to the posterior segments. A prosthesis with a speech bulb was adapted to the patient. CONCLUSION: Excellent restoration of speech and improvement of velopharyngeal function was achieved following placement of the special prosthesis.
