RESUMO

UNLABELLED: Myometrial invasion is one of the most important prognostic factors in endometrial cancer. MATERIAL AND METHOD: We have studied a cohort of 62 patients with endometrial cancer who underwent surgery in 4-th Gynecology Clinic of "Cuza Voda" Hospital, Iasi between 1997-2008. Myometrial invasion was determined intraoperatory by gross visual inspection and frozen section exam and by histopathological exam after surgery. RESULTS: We have investigated the relationship between myometrial invasion and other prognostic factors: histological type, grading and lymph node metastasis. In 36 cases the invasion was absent or minimal, and only in a cases the myometrum was completely invaded.

RESUMO

UNLABELLED: Total abdominal hysterectomy(TAH) and bilateral salpingo-oophorectomy (BSO) have been the hallmarks of therapy for endometrial cancer. However, in the last years the significance of surgical staging is increasing; pelvic lymph node dissection is an important part of surgical staging. MATERIAL AND METHOD: We have studied a cohort of 76 patients with endometrial cancer who underwent surgery in the 4th Gynecology Clinic of "Cuza Voda" Hospital, Iasi, between 1997-2008. Our purpose was to determinate the risk factors for lymph node invasion and the value of pelvic lymphadenectomy regarding both prognosis and therapeutic significance. RESULTS: From the 76 cases, 39 underwent selective pelvine lymphadenectomy (51.31%). 48.86% of cases are in the first stage of neoplasia. Invazia cervicala este mai des intâlnita in cazul tumorilor slab diferentiate, voluminoase, cu invazie miometriala profunda. 9.21% of cases had endocervical glands invasion and 18.42% presented glandular and stromal invasion. CONCLUSION: Lymphadenectomy is beneficial for prognosis and in some cases for therapy.

RESUMO

HELLP syndrome consists in serious liver alterations during pregnancy induced hypertension, that causes hemolysis, elevated liver enzymes and low platelet count. It is frequently accompanied by evidence of severe kidney and brain involvement. Our paper aims to stress the importance of an early accurate diagnosis and an adequate management by presenting an illustrative case.

RESUMO

Subtotal hysterectomy may represent, in specific conditions, a justified alternative to total hysterectomy. Some of the reasons may be vaginal stenosis, preservation of the menstrual function, conservation of the erogene cervical zone. The main disadvantage consists in developing cancer on the cervical stump. In order to reduce the risk of developing cervical carcinoma, total hysterectomy remains the procedure of choice for most women. Our study presents 16 cases with carcinoma of the cervical stump after subtotal hysterectomy from the lot of 313 cases with cervical neoplasms operated on at the Division of Gynecologic Oncology of the 4th Clinic of Obstetrics and Gynecology in Iasi. The main conclusion of this study is illustrated by the firm conviction of the authors that subtotal hysterectomy is an unjustified surgical procedure in women after 40 years old.
