[The development and introduction of diagnostic and treatment methods for neurovascular diseases of the upper extremity]. / Razrabotka i vnedrenie metodov diagnostiki i lecheniia neirovaskuliarnykh zabolevanii verkhnei konechnosti.

The diagnosis complex, permitting to reveal the disease cause and the character of the affect faulty and quickly, was elaborated, based on the experience of treatment analysis of more than 600 patients with the upper extremities neurovascular diseases--thoracic inlet syndrome, Raynaud disease and syndrome. Methods of conservative therapy are introduced, indications for operative treatment are substantiated. Application of new methods of restoration-reconstructive interventions permitted to raise the treatment quality, particularly in seriously ill patients with complicated forms of upper extremities neurovascular diseases.

[Stages in the establishment and the developmental outlook of clinical ultrasonic diagnosis]. / Etapy stanovleniia i perspektivy razvitiia ul'trazvukovoi diagnostiki v klinike.

The ultrasonic semiotics of different diseases of hepar, pancreas, gallbladder, colon, postoperative complications was elaborated and complicated. New and complicated technological procedures for the complex ultrasonic investigation in before, intra- and postoperative periods were proposed. Minor traumatic transcutaneous intervention procedures under the ultrasonic control for the abdominal cavity and retroperitoneal space abscess, cysts of various localization were used for the first time in domestic practice. The treatment procedures applied ranged from puncture with subtle aspirating needle to one- or two-staged drainage performance. Clinical effectiveness constituted 85.2%.

[Ways to improve the results of the treatment of acute obstruction of the aorta and major arteries of the lower extremities in middle-aged and elderly patients]. / Puti uluchsheniia rezul'tatov lecheniia ostroi neprokhodimosti aorty i magistral'nykh arterii konechnostei u bol'nykh pozhilogo i starcheskogo vozrasta.

The authors treated acute arterial obstruction in 190 patients. A complex of diagnostic and therapeutic measures, including the establishment of indications for operative treatment, development of tactics, use of ++physico-technical methods (ultrasound study, rheography, electrocardiography, ++roentgeno-contrast angiography, ultra-violet blood irradiation, electromyostimulation) was developed. A positive result of treatment was obtained in 78.9% of patients, incidence of an amputation performance was 13.7% lethality--12.6%.

[Clinical aspects of using extremely high frequency electromagnetic radiation in the complex treatment of occlusive vascular diseases of the lower extremities]. / Klinicheskie aspekty primeneniia élektromagnitnogo izlucheniia kraine vysokoi chastoty v kompleksnom lechenii obliteriruiushchikh zabolevanii sosudov nizhnikh konechnostei.

A method of local influencing with the extremely high frequency electromagnetic irradiation on the points of acupuncture was used in the complex treatment of 44 patients with obliterative diseases of the vessels of the lower extremities at the age of 34-75 years. Obliterative atherosclerosis was diagnosed in 15 of them, obliterative endarteritis --in 29. A high effectiveness of the method even in late stages of vascular pathology was noted. No complications were observed.

[Changes in the lymphatic system of the lower limbs in varices and post-thrombophlebitic syndrome]. / Izmeneniia limfaticheskoi sistemy nizhnikh konechnostei pri varikoznoi bolezni i posttromboflebiticheskom sindrome.

The complex study of the venous and lymphatic system in 151 patient with different forms of the impairement in venous outflow was carried out. The direct dependence of the state of the lymphatic system of the lower extremities on the state of hemodynamics was noted.

[Diagnosis and surgical treatment results in neurovascular thoracic outlet compression syndrome]. / Diagnostika i rezul'taty khirurgicheskogo lecheniia neirososudistogo kompressionnogo sindroma grudnogo vykhoda.

A detailed study of 44 patients with the neuro-vascular compression syndrome of thoracic exit has established the diagnostic value of clinical and special methods of diagnosing the disease. The most frequent symptoms were pain in the anterior scalene muscle (ASM) during palpation (96%) and paresthesia in fingers (93%). Rheography, arteriography and phlebography of the upper extremities combined with the positional tests should be resorted to. In case of marked neurological symptoms the data on the nerve block of the ASM are informative. Surgical treatment was undertaken in 35 patients: resection of the I rib in 14 cases (in 4 resection combined with upper thoracic sympathectomy), resection of the ASM in 12, removal of the accessory cervical rib with scalenotomy in 5, decompressive vascular reconstructive operations in 10 patients with vascular complications. Good results were obtained in 32 patients. It is stressed that detailed examinations of patients with vaso-neurological symptoms in the upper extremity are important for timely diagnosis and surgery before the complications supervene.
