BACKGROUND: This study aimed to evaluate atherosclerosis as comorbidity by measuring the carotid (bulb and common carotid artery) Carotid intima-media thickness in COPD-diagnosed patients and to evaluate the relationship of atherosclerosis with the prevalence of COPD, hypoxemia and hypercapnia. METHODS: This study was conducted out between January 2019-December 2019 consisting of a total of 140 participants (70 COPD-diagnosed patients-70 healthy individuals). The COPD-diagnosed patients have been planned according to the selection and diagnosis criteria as per the GOLD 2019 guide. It is planned to evaluate as per prospective matching case-control study of the carotid thickness, radial gas analysis, spirometric and demographic characteristics of COPD diagnosed patients and healthy individuals. RESULTS: The average Carotid intima-media thickness in COPD patients was 0.8746±0.161 (p<0.05), and the thickness of the carotid bulb was 1.04±0.150 (p<0.05). In the control group, the average CCA intima-media thickness was 0.6650±0.139 (p<0.05), and the thickness of the carotid bulb was 0.8250±0.15(p<0.05) For the carotid thickness that has increased in COPD diagnosed patients a significant relationship is determined between hypoxemia (p<0.05) and hypercapnia(p<0.05). A significant relationship determined between CIMT and severity of COPD (p<0.05) The CIMT was high in COPD patients with hypoxemia and hypercapnia(p<0.05). CONCLUSION: Significant difference was determined between the severity (grades) of COPD (mild, moderate, severe, very severe) in carotid thickness. Also, CIMT was found to be high in patients who is in the early phases of the prevalence of COPD. In COPD-diagnosed patients, it was determined that severity of COPD, hypoxemia, hypercapnia and age were determining factors of atherosclerosis.

Background Anthracosis may cause a positron emission tomography/computed tomography (PET/CT) false positivity in mediastinal and hilar lymph nodes. We aimed to evaluate the radiological features and the maximum standardized uptake values (SUVmax) of the mediastinal lymph nodes with anthracosis or squamous cell lung cancer metastasized. Methodology Patients diagnosed with anthracosis or squamous cell lung cancer with endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) between January 1, 2015, and November 15, 2020, in a tertiary hospital were enrolled. The squamous cell subtype of lung cancer was selected due to its association with tobacco use, biomass, and air pollution. Anthracosis may occur due to the same etiologic reasons. Results A total of 190 patients met the study enrollment criteria, of which 86 were diagnosed with anthracosis and 33 with squamous cell lung cancer lymph metastasis. Median values for short axis, long axis, SUVmax, shape features, and presence of calcification were found significantly different between the groups. In receiver operating characteristic (ROC) analysis, the SUVmax cut-off value was calculated as 6.61. With this cutoff value, the negative predictive value (NPV) was 92.5% and the positive predictive value (PPV) was 54% for differentiating anthracosis and malignant lymph nodes metastasis. Conclusions We conclude that the evaluation of the shape and metabolic activities of the anthracotic lymph nodes detected by PET/CT together with EBUS-TBNA granted a more accurate staging of the patients and more cancer patients will benefit from surgical treatment.

Pertussis is a vaccine-preventable disease that is transmitted from infected to susceptible individuals by respiratory route. Bordetella pertussis infection may occur at any age as neither vaccine nor natural infection induced immunity lasts life-long. This study was planned to demonstrate the serological evidence of infection among adults, to raise awareness among clinicians and to provide data for the development of strategies to protect vulnerable infants. A total of 538 patients (345 female, 193 male) ages between 18-87 years who had a complain of prolonged cough for more than two weeks were included in the study. Anti-pertussis toxin (PT) IgG and anti-filamentous hemagglutinin (FH) IgG levels from single serum samples were measured by an in-house ELISA test which was standardized and shown to be efficient previously. Anti-PT IgG antibody levels of ≥ 100 EU/ml were considered as acute/recent infection with B.pertussis. In our study, 9.7% (52/538) of the patients had high levels of anti-PT IgG (≥ 100 EU/ml) and among those patients 43 (43/52; 82.7%) also had high (≥ 100 EU/ml) anti-FHA IgG levels. There were no statistically significant differences in terms of age, gender, education level, DPT (diphtheria-pertussis-tetanus) vaccination history, smoking history or average daily cigarette consumption (p> 0.05) between the cases with high antibody levels (n= 52). When the symptoms and the presence of cases with high antibody levels were evaluated, it was detected that no one parameter was significantly different from others, except that 24.1% of the cases with inspiratory whooping had high anti-PT levels. There was also no statistically significant difference between high anti-PT levels ≥ 100 EU/ml and the patients with risk factors [smoking (21/200; 10.5%), presence of disease that cause chronic cough and/or drug usage (19/171; %11.1), and whole factors which cause chronic cough (32/306; %10.5)] and without risk factors (p= 0.581; p= 0.357; p= 0.249, respectively). The distribution of anti-PT IgG geometric mean titer (GMT) according to the age groups, was as follows; 32.41 in 18-30 years; 36.28 in 31-50 years; 36.82 in 51-70 years and 31.15 in ≥ 71 years. Our results indicated that B.pertussis infections are also present among adult population with a frequency not to be underestimated (9.7%) and the results also emphasized that since typical whooping cough symptoms may not be seen in adults, pertussis infection should be considered as a differential diagnosis in adults with prolonged cough, even if there are some other underlying factors of cough. The data obtained from this study was also considered to be helpful in the development of adult vaccination policies for the protection of infants who have not completed the vaccination schedule yet.

BACKGROUND AND AIMS: In lung cancer, many factors have prognostic significance, including thrombocytosis, which is frequently observed. Associations between vascular events, which are the outcomes of paraneoplastic symptoms, and mortality and morbidity has been evaluated in many studies. The aim of the present study was to evaluate the relationship between thrombocytosis and vascular events. MATERIALS AND METHODS: In total, 281 patients, who were histopathologically diagnosed with lung cancer between March 2007 and August 2009, were evaluated retrospectively. Data analysis was performed using the Statistical Package for the Social Sciences (SPSS) software (ver. 11.5 for Windows). Analysis of the distribution of constant variance for normality was assessed using the Shapiro-Wilk test. Nominal variables were evaluated using Pearson's chi-squared or Fisher's exact chi-squared tests. Significant correlations between continuous variables were investigated using Spearman's correlation test. RESULTS: Of the 281 patients, 234 (83.3%) were males and 47 (16.7%) were females, with a median age of 60.6 (31-83 years). Histopathologically, 40 (14.2%) were diagnosed with small-cell lung cancer and 241 (85.8%) with non-small cell lung cancer. In total, 17 (6.04%) vascular events were identified: 11 (64.7%) deep vein thromboses, three (17.6%) pulmonary thromboembolisms, one (5.9%) cerebral arterial thrombosis, and one (5.9%) vena cava superior thrombosis. Thrombocytosis was not determined during thrombosis, but during subsequent visits. CONCLUSIONS: Thrombocytosis is frequently observed in patients with lung cancer. Further prospective studies are required to evaluate the need for prophylactic anticoagulants in these patients. The association between vascular events and survival, the next step of the present study, will be evaluated prospectively.

A field experiment was conducted during summer season of 2005 at the Research Station (altitude 180 m above sea level, 41 degrees 21' N and 36 degrees 15'E) Faculty of Agriculture, Ondokuz Mayis University, Samsun, Turkey. Experiment consisted of three irrigation levels and a non-irrigation level. Drip irrigation treatments consisted of three soil water deficits in the 90 cm soil profile depth was replenished to field capacity. Irrigation treatments were A: no irrigation, B: irrigation at 50% of available soil water capacity, C: irrigation at 30% of available soil water capacity, D: irrigation at 15% of available soil water capacity. The average seasonal water use values ranged from 257.14 to 285.71 mm in corn treatments. Irrigation frequencies (intervals) significantly affected corn crop yields. The average corn grain yields varied from 7.98 to 29.16 t ha(-1). The treatment D was recorded significantly higher corn grain yield 29.16 t ha(-1) compared to B (21.59 t ha(-1)); C (19.15 t ha(-1)) and A (7.98 t ha(-1)), respectively. According to research results, the maximum corn grain yield was obtained when the corn plants were irrigated at 15% of available soil water capacity to field capacity.

Twenty seven years old woman was admitted to our hospital with dyspnea, severe hemoptysis and iron deficiency anemia. The chest X-ray showed bilateral interstitial markings with homogenous infiltration at right costodiafragmatic sinus. The patient was investigated for all alveolar hemorrhagic syndromes. The diagnosis of idiopathic pulmonary haemosiderosis (IPH) was made by open lung biopsy. IPH usually presents in infancy or within the first decade of life and is unknown aetiology. It is most common between ages 1-17 and exceedingly rare in adults. Clinical presentation of IPH varies from an insidious onset with anemia, cough, dyspnea to a fulminant onset with recurrent acute hemoptysis. Histological confirmation with open lung biopsy is often necessary for definite diagnosis.

The prevalence and features of nutritional status in patients with chronic obstructive pulmonary disease (COPD) have been studied extensively in stable conditions, but are poorly defined in the presence of an acute exacerbation. The aim of this study is to evaluate the nutritional status of COPD patients with acute exacerbation and possible relationship between nutritional parameters and pulmonary functions. The study group consisted of 53 COPD patients acutely admitted to the hospital for standardized medical treatment. The nutritional status of patients was assessed by anthropometric measurements, biochemical analysis, and immunologic testing. The patients were divided into two groups as having severe (FEV1 < 50%) and mild to moderate (FEV1 ? 50%) COPD and weight loss greater than 5% for the comparison of the study parameters. Ideal body weight (IBW%) was found as 104.42 +/- 4.30 in severe COPD, where as it was 115.31 +/- 7.28 in mild to moderate COPD group (p= 0.07). There was no relationship demonstrated between IBW% and FEV1. IBW% was correlated with DLCO for the total study population (r= 0.353, p= 0.035). Weight loss greater than 5% of body weight (BW) was observed in 54% of patients. Comparison of the patient's actual weight to their usual weight revealed statistically significant weight loss (p< 0.01). Mean values of serum albumin, transferrin were found in normal range. Delayed type hypersensitivity skin test revealed normal immune status. When the study parameters were compared, no any statistically significant differences in parameters related to nutritional status were detected, between severe and mild to moderate COPD groups. As a statistically significant weight loss was found between the actual and usual weights of the patients, monitoring of nutritional parameters and eventual dietetic treatment should also be included in the goals of the medical treatment of patients with COPD in acute exacerbation.

BACKGROUND: The use of nasal intermittent positive pressure ventilation (NIPPV) would be expected to ameliorate dyspnea, ventilatory capacity and exercise tolerance durability in individuals with hypercapnic respiratory failure secondary to restrictive thoracic disease. OBJECTIVES: The purpose of this study was to determine the short-term effect of NIPPV on respiratory muscle endurance, exercise capacity and respiratory functions in patients with chronic respiratory failure due to restrictive thoracic disease. METHODS: Twelve patients with chronic ventilatory failure due to restrictive thoracic disease underwent nasal bilevel positive airway pressure (BiPAP) ventilation for 2 h a day during 15 consecutive days. The effects were assessed by spirometry, arterial blood gas analysis, 6-min walking test, sensation of dyspnea according to the American Thoracic Society dyspnea scoring scales (ATS) and surface electromyogram of the diaphragm (EMGdi) before and after the study (on day 15). RESULTS: Nasal BiPAP reduced the ATS dyspnea score from 2.5 +/- 0.9 to 1.6 +/- 0.4 (p < 0.01). Distances walked in 6 min increased from 320.66 +/- 93.56 to 382.41 +/- 121.20 m (p < 0.05). Comparison of baseline with levels after nasal BiPAP ventilation showed a statistically significant improvement in PaCO(2) (p < 0.05). Forced vital capacity increased from 35 to 50% of the predicted value (p < 0.01). There were no statistically significant reductions in the amplitude of EMGdi after the therapy. CONCLUSION: These results indicate that NIPPV delivered via nasal BiPAP improves respiratory functions, exercise capacity, and reduces dyspnea in the short term in patients with chronic respiratory failure due to restrictive thoracic disease. Whether such short-term improvements can be sustained merits further study.
