The authors believe that children from tuberculosis risk groups, i.e. those with primary positive or hypersensitive response to tuberculin and living in the infection foci, should be examined in specialized hospitals. Community pediatricians and phthisiotherapists should pay more attention to carrying out closer control over chemoprophylaxis, treatment of chronic infection and concomitant diseases in risk-group children.

The paper analyzes the functional and morphofunctional status of the thyroid in 33 patients with sarcoidosis in relation to the duration and severity of the disease. The levels of thyroid-stimulating hormone and thyroxin-binding globulin have been found to increase greatly in patients with recurrent sarcoidosis, while the levels of T3 and T4 largely decrease early in the disease. An attempt was made to associate the increased serum prolactin levels with the presence of central neurological symptomatology in patients with the involvement of brain hypothalamohypophyseal regions in the granulomatous process of the brain, which requires early glucocorticoidal therapy.

An outbreak of pneumocytosis in a children's tuberculosis hospital was analyzed. The infection was characterized by few signs and favourable progress. Antibiotic therapy failed. To eliminate the outbreak of pneumocystosis in the hospital it was necessary to detect all the children with pneumocystosis and carriers of pneumocysts among the patients and medical staff, to use furazolidone for etiotropic treatment of the patients with pneumocystosis and to perform one-stage sanation of the carriers with antiparasitic agents.

There are cases of isolated affection of the bronchial tree by tuberculosis leading to a separate form of tuberculosis. However, bronchial tuberculosis is rarely diagnosed in practice that can be accounted for by difficulties to differentiate it from other diseases. Specific affection of the small bronchi is noted more commonly. Three types of tuberculous lesions of the bronchial tree are singled out on the basis of an x-ray picture.

Altogether 43 patients with a primary diagnosis of sarcoidosis of the respiratory organs were investigated to reveal the presence and spreading of dysfunction of external respiratory indices and indices of capillary blood supply in patients with sarcoidosis. Of them sarcoidosis was confirmed clinically in 36. Of the other 7 patients 5 had tuberculosis of various forms, one--lung cancer, and one--chronic obstructive bronchitis. All the patients were subjected to radionuclide investigation with 133Xe, 99mTc-microspheres, and 67Ga-citrate. The results obtained permit objective assessment of the presence and a degree of disorders of external respiration and capillary blood supply and the determination of disease activity on the basis of 67Ga-citrate accumulation that is important for the evaluation of therapeutic efficacy. The results were assessed with relation to a degree of sarcoidosis and the type of a pathological process.

Sclerotic changes are brought about rather early in case of sarcoidosis. Therefore, it is extremely important for the clinicians to assess a functional condition of the sarcoidosis patients' lungs, to locate the lesions and their extent and to have an idea about the process activity. To a certain degree, the answers to all these questions can be found by means of radionuclide methods using 133Xe, 99mTc and 67Ga. A complex of radioisotope investigations makes it possible to produce a proper assessment of external respiration and capillary blood flow in patients with pulmonary sarcoidosis, including the extent and activity of the process itself.

A method for estimating efficiency of advanced medical training is described. It was used at 9 courses in advanced medical training of phthisiologists. The method provided estimation of the students knowledge in all the subjects of the program and early correction of the teaching procedure if necessary for improving the results.

Cases of pneumocystic pneumonia were recorded among children with tuberculosis of the respiratory organs. Epidemiological and clinicoroentgenological characteristics of pneumocystic pneumonia in 15 children at the age of 3 to 15 years with tuberculosis of the respiratory organs are presented. Peculiar clinical manifestations of pneumocystic pneumonia in the children of the preschool and younger school ages were studied. Special laboratory methods and in particular the immunofluorescence test (IFT) made it possible to detect the pathogen in the contents of the respiratory tracts, as well as immunological shifts in the children.
