RESUMO

Approximately 3% of all joint dislocations involve the hip joint, and only 8-10% of these will be anterior. Traumatic anterior open dislocation of the hip is rare in children and prone to be associated with injuries, extensive soft tissue damage, and avascular necrosis of the femoral head. We present a case of a 13-year-old boy who had an open anterior dislocation of the hip with ipsilateral avulsion of the greater trochanter after a tractor wheel crush in an agricultural accident. Additional lesions included a diaphyseal closed fracture of the contralateral femur. We report this case because of the rarity and seriousness of this injury due to its progressive complications and difficulties related to its management.

INTRODUCTION: Septic elbow arthritis is a rare disease, especially in children. The aim of this study was to report the functional outcome of our pediatric population with both demographic and microbiological features. METHODS: We retrospectively reviewed all our cases of pediatric septic elbow arthritis between 2007 and 2017. Demographic, microbiological, biological, and radiological features were analyzed. Functional outcome was evaluated according to the Morrey Elbow Performance Score (MEPS). RESULTS: In total, 14 cases were reviewed. The mean age was 9 years. Microbiological findings revealed Staphylococcus aureus in 10 cases. The mean white blood cells count in synovial fluid was 101,306 cells per mm 3. The skin was the most frequently reported entry portal. The mean MEPS score was 86.13 points (excellent in eight cases, good in three cases, fair in two cases, and poor in two cases). Stiffness was observed in three cases. No growth disturbance was reported. DISCUSSION: Septic elbow arthritis in children is rare. Biological inflammatory markers are sensitive but not specific. White blood cell count in the synovial fluid is specific. S. aureus is the most frequently cultured agent. Treatment is based on surgical debridement, antibiotics, and elbow immobilization. Concomitant osteomyelitis is frequent and should be diagnosed and treated simultaneously. Functional outcomes are usually satisfactory in contrast to osteomyelitis. CONCLUSION: Pediatric septic elbow arthritis is rare. It may be primary or secondary to trauma and surgery of the elbow. Demographic, biological, and microbial features are similar to those reported for osteomyelitis; however, functional outcomes are better with a negligible rate of growth disturbance.

RESUMO

Congenital constriction of the limbs is usually due to amniotic band syndrome, which often causes damage to the skin and soft tissues. We report an unusual case in which a neonate had recent fractures of both leg bones with an amniotic band encircling the limb. Non-operative treatment was successful. Challenges to the management of neonatal limb constriction include the absence of a consensus about the best treatment and the high frequency of damage to vessels and nerves.

Anaesthetists often stand in the front line to manage postpartum neurological deficits, although epidural analgesia is rarely responsible for these complications. An epidural analgesia was performed to relieve pain during spontaneous labor in a 34-year-old parturient. An emergency C-section was subsequently required due to fetal heart rate abnormalities. Twelve hours after catheter removal, the parturient developed a severe right leg motor and sensory neurological deficit, predominant on L5 and S1 roots and diagnosed by a neurologist as a central nerve root injury. Lumbar MRI identified a non-compressive epidural bleeding in front of the L5 vertebral body. Epidural bleeding after labor epidural analgesia is a rare complication that may jeopardize the functional prognosis. It may be difficult in some cases to differentiate an upper plexus injury due to labor and delivery from a central epidural analgesia-related nerve root lesion. Fetal head compression at the pelvic brim may induce neurological deficits in several well-differentiated nervous territories, thus mimicking an anaesthetic-induced perimedullar radiculopathy.

INTRODUCTION: Tracheal rupture is one of the most serious post-intubation complication. However, it is widely underestimated. CLINICAL CASE: An 86-year-old patient with a history of pancreas adenocarcinoma treated with gemcitabin was admitted in intensive care unit for an acute respiratory failure with no identified etiology. The worsening of her respiratory status required invasive mechanical ventilation. One laryngoscopy, performed by a trained operator, found a Cormack 1. Intubation was realized without stylet and the cuff inflated with a syringe. Hemodynamic instability, impaired gas exchange and an extensive subcutaneous emphysema occurred immediately. A CT-scan showed a supracarinal tracheal rupture. COMMENT: The etiological analysis of this case identifies several causes of pars membranosa fragility, such as female sex, age greater than 50 years and the short stature. The emergency intubation and the cuff inflated by a syringe were the risk factors of tracheal rupture in this patient. CONCLUSION: Special care should be paid to this complication, early diagnosis has probably a prognostic value. Training operators in the use of stylets and monitoring cuff pressure are required.

PURPOSE OF THE STUDY: We report a retrospective series of 83 patients (86 knees) who underwent reconstruction surgery for chronic anterior knee laxity. The purpose of this study was to analyze mid-term results and assess prognosis factors. MATERIAL AND METHODS: All patients underwent artrhoscopic reconstruction of the anterior cruciate ligament using a central one-third patellar tendon graft. Full follow-up data were available for 51 patients (52 knees) and partial data for 24 others (25 knees). Eight patients (9 knees) were lost to follow-up. The IKDC criteria were used to analyze outcome at a mean 6 years post-surgery. RESULTS: Graft failure was observed in 5 knees and a graft tears after a new sprain was seen at 3 years. The patients were satisfied or very satisfied in 88.5% of the cases. The Trillat-Lachman test revealed a persistent dampened brake in 5 knees and a frank click in 4 (7.7%). Complete movement was recovered in all knees excepting 2 exhibiting persistent flexion. Residual laxity (active Lachman test) was 5 mm in 81.5% of the cases, between 6 and 10 mm in 17%, and greater than 10 mm in only 1 case (2.5%). IKDC scores were A=25%, B=50%, C=21% and D=4%. Forty-two patients (61%) returned to their sports activities and 9 of the 12 high-level athletes resumed competition at the same level as preoperatively. Arthroscopy enabled a well- or very well-positioned femoral tunnel in 88% of the cases, conditioning final IKDC outcome (p<0.02). There was a correlation between the meniscal status and residual laxity. DISCUSSION: This study demonstrated a high proportion (21%) of patients with an incomplete repair (21% IKDC class C) with a residual laxity greater than 5 mm and a late hard brake. Simple patellar tendon graft provided insufficient repair of the anterior cruciate ligament. The subjective outcome was better than the objective outcome since 88.5% of the patients were satisfied or very satisfied (patients seen at last follow-up or contacted by telephone). Good objective outcome was correlated with good femoral position of the transplant and preservation of the meniscus. Failures were explained by poor position of the transplant, long-standing laxity, and renewal of sports activities too early. CONCLUSION: Arthroscopic repair of the anterior cruciate ligament is a reliable procedure, but as failures are observed, indications should take into consideration the type of laxity and the status of the meniscus. For unique anterior laxity, the central one-third patellar tendon graft gives good results. For advanced anterior laxity, augmentation with an extra-articular lateral tenodesis would be necessary.
