Introduction Jet aircraft pilots are exposed to huge pressure variation during flight, which affect physiological functions as systems, such as the respiratory system. Objectives The objective of the present investigation was to evaluate inflammatory changes of paranasal sinuses of jet aircraft pilots before and after a jet aircraft training program, using multislice computed tomography (CT), in comparison with a group of nonairborne individuals with the same age, sex, and physical health conditions. A second objective of the present study was to assess the association between the ostiomeatal complex obstruction and its anatomical variations. Methods The study group consisted of 15 jet aircraft pilots participating in the training program. The control group consisted of 41 nonairborne young adults. The 15 fighter pilots were evaluated before initiating the training program and after their final approval for the presence of inflammatory paranasal sinus disease. The ostiomeatal complex anatomical variations and obstructions were analyzed in pilots after the training program. Results Jet aircraft pilots presented higher incidence of mucosal thickening in maxillary sinus and anterior ethmoid cells than controls. Prominent ethmoidal bulla showed significant association with obstruction of the osteomeatal complex. Conclusions Jet aircraft pilots present increased inflammatory disease when compared with nonairborne individuals. The presence of a prominent ethmoidal bulla is associated with ostiomeatal complex obstruction.

Sex estimation is an important procedure in forensic anthropology for human identification. The development of new technologies, such as three-dimensional computed tomography (CT), has provided excellent alternatives for this purpose. This study examined and compared a morphological method for sex estimation using two different approaches - direct measurement of physical structures and tomographic analysis using 3D images. A total of 111 skulls from the Museum of Human Anatomy at the University of São Paulo (MAH-USP) were used, (60 males and 51 females). All specimens were scanned by Philips Brilliance 64 CT scanner equipment and their corresponding images were reconstructed in three-dimensional (3D) models. The morphological characteristics of the skulls were analyzed by an observer who was blinded to the sex of the specimens. Five cranial structures were analyzed: external occipital crest, mastoid process, supraorbital margin, glabella, and mental eminence. The structures were scored 1 to 5 according to Buikstra and Ubelaker and validated by Walker. The success rates of the sex estimates obtained through direct measurement of the dry skulls ranged from 67.4% to 70.4% as compared to 60.2% to 68.1% for CT reconstruction. When analyzed separately, the maximum accuracy of the method was 68.33% in males and 88.24% in females in the physical analysis of structures. The glabella and mastoid process were the most effective structures to estimate sex through both techniques, respectively. Our results show that 3D CT images can be accurately used in the morphological analysis for sex estimation, representing a viable alternative in forensic anthropology.

Purpose: This study evaluated maxillary sinus volume changes in military jet aircraft pilot candidates before and after the training program, in comparison with a control group, considering the effects of pressurization, altitude, and total flight hours, through multislice computed tomography. Materials and Methods: Fifteen fighter pilots were evaluated before initiating the training program and after the final approval. The control group consisted of 41 young adults who had not flown during their military career. The volumes of each maxillary sinus were measured individually before and at the end of the training program. Results: When comparing the initial and final volumes in the pilots, a statistically significant increase was observed both in the left and right maxillary sinuses. When evaluating the average total volume of the maxillary sinuses (i.e., the average volume of the right and left maxillary sinuses together), a significant increase in the volume of the maxillary sinuses was observed in the pilot group when compared to the control group. Conclusion: The maxillary sinus volumes in aircraft pilot candidates increased after the 8-month training program. This may be explained by changes in the gravitational force, the expansion of gas, and positive pressure from oxygen masks. This unprecedented investigation among pilots might lead to other investigations considering paranasal sinus alterations in this singular population.

As doenças neurometabólicas hereditárias representam um desafio diagnóstico e precisam de classificações eficientes para sua compreensão. São um grupo de doenças raras, mas atingem pelo menos um a cada mil nascimentos. Algumas têm possibilidade terapêutica. Os quadros clínico e radiológico são variáveis e, às vezes, superponíveis, a depender da fase da doença. Por isso, observam-se na literatura vários tipos de classificações, algumas de difícil aplicação prática. O objetivo deste estudo é ilustrar a classificação das doenças neurometabólicas hereditárias baseada exclusivamente em achados radiológicos. Trata-se de um estudo retrospectivo a partir de exames de imagem do sistema nervoso central, particularmente de crianças, realizados em uma rede de hospitais. Todos os casos foram estudados por tomografia computadorizada multidetectores e/ou ressonância magnética, tendo sido avaliados por dois neurorradiologistas. Consideraram-se os casos com diagnóstico definitivo. A classificação propõe 10 subgrupos, de acordo com os achados radiológicos. Todos os casos estudados apresentaram, pelo menos, um desses achados. Na maioria dos casos observou-se mais de um achado, o que aumentou a especificidade e restringiu os diagnósticos diferenciais. Após avaliar os dados de literatura e os deste estudo, demonstra-se que é possível classificar as doenças neurometabólicas hereditárias pelo aspecto radiológico, o que favorece um diagnóstico definitivo.

Abstract Inherited neurometabolic disorders represent a diagnostic challenge, and an efficient classification system is needed in order to improve the understanding of these diseases. Although they constitute a group of rare diseases, they have a collective incidence of at least one case per 1,000 live births. Some inherited neurometabolic disorders are treatable. The clinical and radiological presentations are variable and sometimes overlap, depending on the stage of the disease. Therefore, a number of classification systems have been devised, some of which are difficult to apply in practice. The aim of this study was to illustrate a classification system for inherited neurometabolic disorders, based exclusively on radiological findings. This was a retrospective study of imaging examinations of the central nervous system, particularly of children, performed in a network of hospitals. All of the cases were studied by multidetector computed tomography, magnetic resonance imaging, or both, the images having been obtained by two neuroradiologists. We included only cases in which a definitive diagnosis was made. The classification system separates the relevant radiological findings into 10 categories. All of the cases studied presented at least one of those findings. In most of the cases, more than one finding was observed, which increased specificity and narrowed the differential diagnosis. Data from the literature and from this study demonstrate that it is possible to classify inherited neurometabolic disorders by their radiological aspects, which favors a definitive diagnosis.

Intracranial calcifications, which are common in the daily routine of radiologists, can have a physiological or pathological origin. Determining the cause of intracranial calcifications can represent a challenge. The anatomical location, distribution, dimensions and morphology of such calcifications are important findings, which, in conjunction with the clinical history and age group, can facilitate the differential diagnosis. The aim of this pictorial essay is to demonstrate the different types of intracranial calcifications and their origins. The images evaluated were those stored in picture archiving and communication systems. All of the cases included were studied by computed tomography, magnetic resonance imaging, or both. We identified, classified, and described 64 types of intracranial calcifications.


Calcificações intracranianas fazem parte da rotina do médico radiologista, as quais podem ter origem fisiológica ou patológica. A diferenciação entre as calcificações pode representar um desafio. Localização anatômica, distribuição, dimensões e morfologia das calcificações são importantes achados que, associados a história clínica e faixa etária, podem ajudar no diagnostico diferencial. O objetivo deste estudo é demonstrar os diferentes tipos de calcificações intracranianas e suas origens. Foram avaliadas as imagens armazenadas no sistema de comunicação e arquivamento de imagens das unidades. Todos os casos inclusos foram estudados por tomografia computadorizada e/ou ressonância magnética. Foram encontrados 64 tipos de calcificações intracranianas, que foram classificadas e descritas.

Obtaining accurate and reproducible estimates of internal shell volume is a vital requirement for studies into the ecology of a range of shell-occupying organisms, including hermit crabs. Shell internal volume is usually estimated by filling the shell cavity with water or sand, however, there has been no systematic assessment of the reliability of these methods and moreover no comparison with modern alternatives, e.g., computed tomography (CT). This study undertakes the first assessment of the measurement reproducibility of three contrasting approaches across a spectrum of shell architectures and sizes. While our results suggested a certain level of variability inherent for all methods, we conclude that a single measure using sand/water is likely to be sufficient for the majority of studies. However, care must be taken as precision may decline with increasing shell size and structural complexity. CT provided less variation between repeat measures but volume estimates were consistently lower compared to sand/water and will need methodological improvements before it can be used as an alternative. CT indicated volume may be also underestimated using sand/water due to the presence of air spaces visible in filled shells scanned by CT. Lastly, we encourage authors to clearly describe how volume estimates were obtained.

New research on assessing neuropsychiatric manifestations of Alzheimer´s Disease (AD) involves grouping neuropsychiatric symptoms into syndromes. Yet this approach is limited by high inter-subject variability in neuropsychiatric symptoms and a relatively low degree of concordance across studies attempting to cluster neuropsychiatric symptoms into syndromes. An alternative strategy that involves dichotomizing AD subjects into those with few versus multiple neuropsychiatric symptoms is both consonant with real-world clinical practice and can contribute to understanding neurobiological underpinnings of neuropsychiatric symptoms in AD patients. The aim of this study was to address whether the number of neuropsychiatric symptoms (i.e., presence of few [≤2] versus multiple [≥3] symptoms) in AD would be associated with degree of significant gray matter (GM) volume loss. Of particular interest was volume loss in brain regions involved in memory, emotional processing and salience brain networks, including the prefrontal, lateral temporal and parietal cortices, anterior cingulate gyrus, temporo-limbic structures and insula. We recruited 19 AD patients and 13 healthy controls, which underwent an MRI and neuropsychiatric assessment. Regional brain volumes were determined using voxel-based morphometry and other advanced imaging processing methods. Our results indicated the presence of different patterns of GM atrophy in the two AD subgroups relative to healthy controls. AD patients with multiple neuropsychiatric manifestations showed more evident GM atrophy in the left superior temporal gyrus and insula as compared with healthy controls. In contrast, AD subjects with few neuropsychiatric symptoms displayed more GM atrophy in prefrontal regions, as well as in the dorsal anterior cingulate ad post-central gyri, as compared with healthy controls. Our findings suggest that the presence of multiple neuropsychiatric symptoms is more related to the degree of atrophy in specific brain networks rather than dependent on the global severity of widespread neurodegenerative brain changes.

Neuroendocrine tumours are epithelial neoplasms with predominant neuroendocrine differentiation. The nasal cavity and paranasal sinuses are rare locations for neuroendocrine carcinomas, and only a few related papers have been published in the literature to date. Here we present the case of a 64-year-old male with neuroendocrine carcinoma of the sphenoid sinus, along with the main MRI findings.

OBJECTIVE: To identify the distribution and frequency of computed tomography (CT) findings in patients with nosocomial rapidly growing mycobacterial (RGM) infection after laparoscopic surgery. METHOD: A descriptive retrospective study in patients with RGM infection after laparoscopic surgery who underwent CT imaging prior to initiation of therapy. The images were analyzed by two radiologists in consensus, who evaluated the skin/subcutaneous tissues, the abdominal wall, and intraperitoneal region separately. The patterns of involvement were tabulated as: densification, collections, nodules (≥1.0 cm), small nodules (<1.0 cm), pseudocavitated nodules, and small pseudocavitated nodules. RESULTS: Twenty-six patients met the established criteria. The subcutaneous findings were: densification (88.5%), small nodules (61.5%), small pseudocavitated nodules (23.1 %), nodules (38.5%), pseudocavitated nodules (15.4%), and collections (26.9%). The findings in the abdominal wall were: densification (61.5%), pseudocavitated nodules (3.8%), and collections (15.4%). The intraperitoneal findings were: densification (46.1%), small nodules (42.3%), nodules (15.4%), and collections (11.5%). CONCLUSION: Subcutaneous CT findings in descending order of frequency were: densification, small nodules, nodules, small pseudocavitated nodules, pseudocavitated nodules, and collections. The musculo-fascial plane CT findings were: densification, collections, and pseudocavitated nodules. The intraperitoneal CT findings were: densification, small nodules, nodules, and collections. KEY POINTS: • Rapidly growing mycobacterial infection may occur following laparoscopy. • Post-laparoscopy mycobacterial infection CT findings are densification, collection, and nodules. • Rapidly growing mycobacterial infection following laparoscopy may involve the peritoneal cavity. • Post-laparoscopy rapidly growing mycobacterial intraperitoneal infection is not associated with ascites or lymphadenopathy.

Fundamento: Há poucos dados sobre a definição de parâmetros simples e robustos para predizer artefato de imagem em tomografia computadorizada (TC) cardíaca. Objetivos: Avaliar o valor da simples medida da espessura do tecido subcutâneo (espessura pele-esterno) como preditor de artefato de imagem em TC cardíaca. Métodos: O estudo avaliou 86 pacientes submetidos a angiotomografia computadorizada cardíaca (ATCC) com sincronização prospectiva com ECG e avaliação de escore de cálcio coronário com 120 kV e 150 mA. A qualidade da imagem foi medida objetivamente pelo artefato de imagem na aorta em ATCC, sendo 'artefato baixo' definido como aquele < 30 UH. Os diâmetros torácicos anteroposterior e laterolateral, o artefato de imagem na aorta e a espessura pele-esterno foram medidos como preditores de artefato em ATCC. A associação de preditores e artefato de imagem foi avaliada usando-se correlação de Pearson. Resultados: A dose média de radiação foi 3,5 ± 1,5 mSv. O artefato de imagem médio na ATCC foi de 36,3 ± 8,5 UH, sendo o artefato de imagem médiona fase sem contraste do exame de 17,7 ± 4,4 UH. Todos os preditores foram independentemente associados com artefato em ATCC. Os melhores preditores foram espessura pele-esterno, com correlação de 0,70 (p < 0,001), e artefato de imagem na fases em contraste,com correlação de 0,73 (p < 0,001). Ao avaliar a habilidade de predizer artefato de imagem baixo, as áreas sob a curva ROC para o artefato de imagem na fases em contraste e para a espessura pele-esterno foram 0,837e 0,864, respectivamente. Conclusão: Tanto espessura pele-esterno quanto artefato de escore de cálcio são preditores simples e precisos de artefato de imagem em ATCC. OBJECTIVES: To evaluate the value of a simple measure of subcutaneous tissue as a predictor of image noise in cardiac CT. METHODS: 86 patients underwent prospective ECG-gated coronary computed tomographic angiography (CTA) and coronary calcium scoring (CAC) with 120 kV and 150 mA. The image quality was objectively measured by the image noise in the aorta in the cardiac CTA, and low noise was defined as noise < 30 HU. The chest anteroposterior diameter and lateral width, the image noise in the aorta and the skin-sternum (SS) thickness were measured as predictors of cardiac CTA noise. The association of the predictors and image noise was performed by using Pearson correlation. RESULTS: The mean radiation dose was 3.5 ± 1.5 mSv. The relationship between structural changes in grey matter and treatment response in patients with late-life depression remains an intriguing area of research. This magnetic resonance imaging (MRI) study compares the baseline grey matter volume of elderly people with and without major depression (according to the DSM-IV-TR criteria) and assesses its association with antidepressant treatment response. Brain MRI scans were processed using statistical parametric mapping and voxel-based morphometry. The sample consisted of 30 patients with depression and 22 healthy controls. We found a significant volumetric reduction in the orbitofrontal cortex bilaterally in patients in comparison with controls. According to their remission status after antidepressant treatment, patients were classified as remitted or not remitted. Compared with controls, remitted patients showed a volumetric reduction in the orbitofrontal cortex bilaterally and in another cluster in the right middle temporal pole. Non-remitted patients showed an even greater volumetric reduction in the orbitofrontal cortex bilaterally compared with controls. To investigate predictive factors of remission after antidepressant treatment, we used a logistic regression. Both baseline Mini Mental State Examination score and baseline left superior lateral orbitofrontal cortex volume (standardized to the total grey matter volume) were associated with remission status. Our findings support the use of regional brain atrophy as a potential biomarker for depression. In addition, baseline cognitive impairment and regional grey matter abnormalities predict antidepressant response in patients with late-life depression.

Sudden hearing loss (SHL) is an ENT emergency defined as sensorineural hearing loss (SNHL) > 30 dB HL affecting at least 3 consecutive tonal frequencies, showing a sudden onset, and occurring within 3 days. In cases of SHL, a detailed investigation should be performed in order to determine the etiology and provide the best treatment. Otoacoustic emission (OAE) analysis, electronystagmography (ENG), bithermal caloric test (BCT), and vestibular evoked myogenic potential (VEMP) assessments may be used in addition to a number of auxiliary methods to determine the topographic diagnosis. OBJECTIVE: To evaluate the contribution of OAE analysis, BCT, VEMP assessment, and magnetic resonance imaging (MRI) to the topographic diagnosis of SHL. METHOD: Cross-sectional and retrospective studies of 21 patients with SHL, as defined above, were performed. The patients underwent the following exams: audiometry, tympanometry, OAE analysis, BCT, VEMP assessment, and MRI. Sex, affected side, degree of hearing loss, and cochleovestibular test results were described and correlated with MRI findings. Student's t-test was used for analysis of qualitative variables (p < 0.05). RESULTS: The mean age of the 21 patients assessed was 52.5 ± 15.3 years; 13 (61.9%) were women and 8 (38.1%) were men. Most (55%) had severe hearing loss. MRI changes were found in 20% of the cases. When the audiovestibular test results were added to the MRI findings, the topographic SHL diagnosis rate increased from 20% to 45%. CONCLUSION: Only combined analysis via several examinations provides a precise topographic diagnosis. Isolated data do not provide sufficient evidence to establish the extent of involvement and, hence, a possible etiology...

INTRODUCTION: Neuroimaging has been widely used in studies to investigate depression in the elderly because it is a noninvasive technique, and it allows the detection of structural and functional brain alterations. Fractional anisotropy (FA) and mean diffusivity (MD) are neuroimaging indexes of the microstructural integrity of white matter, which are measured using diffusion tensor imaging (DTI). The aim of this study was to investigate differences in FA or MD in the entire brain without a previously determined region of interest (ROI) between depressed and non-depressed elderly patients. METHOD: Brain magnetic resonance imaging scans were obtained from 47 depressed elderly patients, diagnosed according to DSM-IV criteria, and 36 healthy elderly patients as controls. Voxelwise statistical analysis of FA data was performed using tract-based spatial statistics (TBSS). RESULTS: After controlling for age, no significant differences among FA and MD parameters were observed in the depressed elderly patients. No significant correlations were found between cognitive performance and FA or MD parameters. CONCLUSION: There were no significant differences among FA or MD values between mildly or moderately depressed and non-depressed elderly patients when the brain was analyzed without a previously determined ROI.

RESUMO

Tumores miofibroblásticos inflamatórios raramente acometem vias urinárias ou crianças, comumente mimetizando neoplasias malignas nos exames de imagem. Foram descritos apenas 35 casos desses tumores na bexiga de crianças, segundo a literatura recente. Os autores apresentam o caso de uma criança com um tumor miofibroblástico vesical que evoluiu favoravelmente após ressecção cirúrgica completa.


Inflammatory myofibroblastic tumors rarely affect the urinary tract or children, and frequently mimic malignancy on imaging studies. According to the recent literature, only 35 cases of such bladder tumors in children have been reported. The authors present the case of a child with a bladder myofibroblastic tumor with favorable progression following complete surgical resection.

The objective was to determine the effects of carbohydrate (CHO) supplementation on exercise-induced hormone responses and post-training intramyocellular lipid stores (IMCL). Twenty-four elite male athletes (28.0±1.2 years) were randomized to receive CHO (maltodextrin solution) or zero energy placebo solution (control group). The high-intensity running protocol consisted of 10×800 m at 100% of the best 3000-m speed (Vm3 km) and 2×1000 m maximal bouts in the morning and a submaximal 10-km continuous easy running in the afternoon of day 9. IMCL concentrations were assessed by (1)H-MRS before (-day 9) and after training (day 9) in soleus (SO) and tibialis anterior (TA) muscles. Blood hormones were also measured before, during, and post-exercise. The percent change (Δ%) in TA-IMCL was higher in the CHO group (47.9±24.5 IMCL/Cr) than in the control group (-1.7±13.1, respectively) (P=.04). Insulin concentrations were higher in the CHO group post-intermittent running compared to control (P=.02). Circulating levels of free fatty acids and GH were lower in the CHO group (P>.01). The decline in performance in the 2nd 1000-m bout was also attenuated in this group compared to control (P<.001 and P=.0035, respectively). The hormonal milieu (higher insulin and lower GH levels) in the CHO group, together with unchanged free fatty acid levels, probably contributed to the increased IMCL stores. This greater energy storage capacity may have improved post-exercise recovery and thus prevented performance deterioration.
