RESUMO

BACKGROUND: The shortage of nurses has been a global human resources problem. A good professional growth environment is essential to developing potential nursing students and attracting nurses to join, and it has great significance in reducing nurse turnover. However, nurses' comprehensive perceptions of professional growth have not yet been examined. METHODS: A cluster sampling method was used to conduct a professional growth questionnaire survey on young nursing talents from a large Chinese public tertiary A hospital in March 2022. RESULTS: The score of professional growth among 243 young nursing talents was 57.92 ± 9.607, with a scoring rate of 77.23%. The scores for dimensions of professional growth, from lowest to highest, were rehabilitation growth, promotion speed, professional goal progress, and professional ability development. Attitudes towards participating in training, service as the quality manager or clinical teacher, self-efficacy, professional title, work-family support, education, and organizational commitment of young nursing talents were significantly associated with professional growth. CONCLUSION: The professional growth of young nursing talents was at a moderate level and needed to be strengthened. Nursing leaders and managers are expected to develop management practices to enhance young nursing talents' professional growth in combination with the related factors.

RESUMO

Objective: In intensive care units (ICU), frequent false alarms from medical equipment can cause alarm fatigue among nurses, which might lead to delayed or missed responses and increased risk of adverse patient events. This review was conducted to evaluate the effectiveness of intelligent management interventions to reduce false alarms in ICU. Method: Following the framework of Whitmore and Knafl, the reviewers systematically searched six databases: PubMed, EMBASE, CINAHL, OVID, Cochrane Library, and Scopus, and studies included intelligent management of clinical alarms published in the English or Chinese language from the inception of each database to December 2022 were retrieved. The researchers used the PICOS framework to formulate the search strategy, developed keywords, screened literature, and assessed the studies' quality using the Joanna Briggs Institute-Meta-Analysis of Statistics, Assessment, and Review Instrument (JBI-MAStARI). The review was preregistered on PROSPERO (CRD42023411552). Results: Seven studies met the inclusion criteria. The results showed that different interventions for intelligent management of alarms were beneficial in reducing the number of false alarms, the duration of alarms, the response time to important alarms for nurses, and the alarm fatigue levels among nurses. Positive results were found in practice after the application of the novel alarm management approaches. Conclusion: Intelligent management intervention may be an effective way to reduce false alarms. The application of systems or tools for the intelligent management of clinical alarms is urgent in hospitals. To ensure more effective patient monitoring and less distress for nurses, more alarm management approaches combined with artificial intelligence will be needed in the future to enable accurate identification of critical alarms, ensure nurses are responding accurately to alarms, and make a real difference to alarm-ridden healthcare environments.

RESUMO

Objective: This study aimed to investigate the level and influencing factors of nurses' antimicrobial stewardship (AMS) engagement in China based on the capability, opportunity, motivation, and behavior (COM-B) theory, providing valuable insights for developing effective strategies to improve nursing quality in AMS. Methods: This cross-sectional study was conducted in 17 tertiary hospitals in Hunan, China, from November 2021 to January 2022. A total of 4,514 nurses were selected. The Nurse AMS Engagement Questionnaire (NAEQ), developed using the COM-B theory, was used for evaluation. The questionnaire included capability (14 items), opportunity (7 items), motivation (6 items), and behavior (12 items) four dimensions, 39 items. Results: The total NAEQ score was 155.08 ± 27.12, indicating a moderate level. The score of the capability, opportunity, motivation, and behavior dimensions were 52.33 ± 13.48, 28.64 ± 5.76, 24.57 ± 4.57 and 49.53 ± 8.83, respectively. Significant differences in nurses' AMS engagement were based on professional titles, whether working as a part-time infection control nurse, whether knowing the AMS teams and the defined daily doses of antibiotics, department type, the deployment of clinical pharmacists, and frequency of antimicrobial training and physician-nurse joint rounds (P < 0.05). Nurses with junior titles had higher scores on the NAEQ than nurses with intermediate titles (P < 0.05). Nurses who worked as part-time infection control nurses, knew the AMS team, and the defined daily doses of antibiotics had higher NAEQ scores than those who didn't (P < 0.01). Nurses working in the ICU and infectious disease department had lower NAEQ scores than those in other departments, such as the ear, nose, and throat (ENT) department (P < 0.01). Nurses who had clinical pharmacists deployed in their department had higher NAEQ scores than those without or unclear deployment (P < 0.01). Furthermore, nurses who received more frequent antimicrobial training and participated in physician-nurse joint rounds had higher NAEQ scores (P < 0.01). Conclusion: Multiple strategies, including enhanced education and training and improved multidisciplinary communication and collaboration, are expected to improve nurse AMS engagement. It is important to give more attention to nurses with intermediate professional titles, less experience, and those working in specific departments.

RESUMO

Background: Spontaneous intracerebral hemorrhage (SICH) is associated with high mortality and disability. Accurately predicting adverse prognostic risks of SICH is helpful in developing risk stratification and precision medicine strategies for this phenomenon. Methods: We analyzed 413 patients with SICH admitted to Hefei Second People's Hospital as a training cohort, considering 74 patients from the First Affiliated Hospital of Anhui Medical University for external validation. Univariate and multivariate logistic regression analyses were used to select risk factors for 90-day functional outcomes, and a nomogram was developed to predict their incidence in patients. Discrimination, fitting performance, and clinical utility of the resulting nomogram were evaluated through receiver operating characteristic (ROC) curves, accuracy, sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), calibration plots, and decision curves analysis (DCA), respectively. Results: Of the 413 patients, 180 had a poor prognosis. Univariate analysis showed significant variance of age, systolic pressure, intraventricular hemorrhage (IVH), Glasgow Coma Scale (GCS) scores, National Institute of Health Stroke Scale (NIHSS) scores, and hematoma volume between the groups (p < 0.05). Logistic multivariate regression analysis showed that age, IVH, NIHSS, and hematoma volume were associated with unfavorable outcomes. Based on the results, a nomogram model was developed with an area under the ROC curve of 0.91 (95% CI; 0.88-0.94) and 0.89 (95% CI; 0.80-0.95) in the training and validation sets, respectively. In the validation set, the accuracy, sensitivity, specificity, PPV, and NPV of the model were 0.851, 0.923, 0.812, 0.727, and 0.951, respectively. The calibration plot demonstrates the goodness of fit between the nomogram predictions and actual observations. Finally, DCA indicated significant clinical adaptability. Conclusion: We developed and validated a short-term prognostic nomogram model for patients with SICH including NIHSS scores, age, hematoma volume, and IVH. This model has valuable potential in predicting the prognosis of patients with SICH.

RESUMO

High-voltage aqueous rechargeable energy storage devices with safety and high specific energy are hopeful candidates for the future energy storage system. However, the electrochemical stability window of aqueous electrolytes is a great challenge. Herein, inspired by density functional theory (DFT), polyethylene glycol (PEG) can interact strongly with water molecules, effectively reconstructing the hydrogen bond network. In addition, N, N-dimethylformamide (DMF) can coordinate with Zn2+ , assisting in the rapid desolvation of Zn2+ and stable plating/stripping process. Remarkably, by introducing PEG400 and DMF as co-solvents into the electrolyte, a wide electrochemical window of 4.27 V can be achieved. The shift in spectra indicate the transformation in the number and strength of hydrogen bonds, verifying the reconstruction of hydrogen bond network, which can largely inhibit the activity of water molecule, according well with the molecular dynamics simulations (MD) and online electrochemical mass spectroscopy (OEMS). Based on this electrolyte, symmetric Zn cells survived up to 5000 h at 1 mA cm-2 , and high voltage aqueous zinc ion supercapacitors assembled with Zn anode and activated carbon cathode achieved 800 cycles at 0.1 A g-1 . This work provides a feasible approach for constructing high-voltage alkali metal ion supercapacitors through reconstruction strategy of hydrogen bond network.

RESUMO

OBJECTIVES: To compare ankle proprioception between professional adolescent table tennis players at national and regional levels and age-matched non-athletes, and, in a nominally upper-limb sport, to explore the relationships between single- and dual-task ankle proprioception, years of training and sport-specific performance. DESIGN: Cross-sectional observational study. METHODS: Fifty-five participants (29 professional adolescent table tennis players and 26 non-athletic peers) volunteered. Ankle proprioception was first assessed using the active movement extent discrimination apparatus (AMEDA-single) for all; yet only the players were then re-assessed while executing a secondary ball-hitting task (AMEDA-dual). The mean Area Under the Receiver Operating Characteristic Curve was calculated as the proprioceptive score, and years of training and hitting rate were recorded. RESULTS: National-level players had significantly better ankle proprioception as shown by higher AMEDA-single scores than the other groups (all p < 0.05). Ankle proprioceptive performance was significantly impaired while ball-hitting (F1,28 = 58.89, p ≤ 0.001, ηp2 = 0.69). National-level players outperformed the regional-level significantly on the AMEDA-dual task (F1,27 = 21.4, p ≤ 0.001, ηp2 = 0.44). Further, ankle proprioceptive performance was related to expertise, in that both AMEDA-single and AMEDA-dual proprioceptive scores were correlated with years of training and ball-hitting rate (r from 0.40 to 0.54, all p < 0.05). CONCLUSIONS: Ankle proprioception is a promising measure that may be used to identify different ability levels among adolescent table tennis players. Superior ankle proprioception may arise from rigorous training and contribute to stroke accuracy. Dual-task proprioceptive assessment suggests how elite table tennis players perform differently from lower-ranked players in complex and changeable sports circumstances.

RESUMO

Consensus is lacking regarding whether high-intensity interval training (HIIT) effectively improves VO2max (VO2peak) in elite athletes (Athlete must be involved in regular competition at the national level). This meta-analysis compared the effects of HIIT and conventional training methods (continuous training, repeated-sprint training, high volume low-intensity training, high-intensity continuous running, sprint-interval training, moderate-intensity continuous training)on VO2max in elite athletes. Nine studies were included, comprising 176 elite athletes (80 female). Compared to that with conventional training, VO2max was significantly increased after HIIT (overall: 0.58 [0.30, 0.87], I2 = 0.49, P = 0.03; males: 0.41 [0.06, 0.76], I2 = 0%, P = 0.89). VO2max had positive training effects when the HIIT recovery period had an interval time ≥2 min (0.44 [0.03, 0.84], I2 = 0%, P = 0.99) and recovery phase intensity ≤40% (0.38 [0.05, 0.71], I2 = 0%, P = 0.96). Thus, HIIT shows superiority over conventional training methods in improving VO2max, promoting aerobic capacity, in elite athletes.

RESUMO

Prolificacy is a crucial characteristic of livestock, particularly for species such as sheep that have many births. The objectives of this study were as follows: (1) to investigate the genetic diversity of the 13 new and 7 known variants in the BMPRIB, GDF9, BMP15, LEPR, and B4GALNT2 genes in Ujimqin (UM), the F1 population of Dorper × Ujimqin crossbred (DPU), the F1 population of Suffolk × Ujimqin crossbred (SFKU), Sonid sheep (SN), Tan sheep (Tan), Hu sheep (Hu), and Small-tailed Han sheep (STH) sheep breeds/populations; (2) to perform an association analysis of the above 20 variants with litter size in 325 UM, 304 DPU, and 66 SFKU sheep populations; (3) to compare the frequencies of the litter-size-related alleles of these 20 variants among 8 sheep breeds/populations (the above seven sheep breeds + Mongolia sheep breed). With the use of the Sequenom MassARRAY®SNP assay technology, these 20 mutations were genotyped. The association analysis results showed that the c.746A>G (FecB) mutation in BMPR1B was significantly associated with the litter size of UM and DPU, the c.994A>G (FecGA) in GDF9 was significantly associated with the litter size of SFKU, and the c.31_33CTTinsdel (B1) in BMP15 was significantly associated with the litter size of UM. Our findings might provide valuable genetic markers for expanding sheep litter sizes.

RESUMO

Transition metal selenides anodes with fast reaction kinetics and high theoretical specific capacity are expected to solve mismatched kinetics between cathode and anode in Li-ion capacitors. However, transition metal selenides face great challenges in the dissolution and shuttle problem of lithium selenides, which is the same as Li-Se batteries. Herein, inspired by the density functional theory calculations, heterogeneous can enhance the adsorption of Li2 Se relative to single component selenide electrodes, thus inhibiting the dissolution and shuttle effect of Li2 Se. A heterostructure material (denoted as CoSe2 /SnSe) with the ability to evolve continuously (CoSe2 /SnSe→Co/Sn→Co/Li13 Sn5 ) is successfully designed by employing CoSnO3 -MOF as a precursor. Impressively, CoSe2 /SnSe heterostructure material delivers the ultrahigh reversible specific capacity of 510 mAh g-1 after 1000 cycles at the high current density of 4 A g-1 . In situ XRD reveals the continuous evolution of the interface based on the transformation and alloying reactions during the charging and discharging process. Visualizations of in situ disassembly experiments demonstrate that the continuously evolving interface inhibits the shuttle of Li2 Se. This research proposes an innovative approach to inhibit the dissolution and shuttling of discharge intermediates (Li2 Se) of metal selenides, which is expected to be applied to metal sulfides or Li-Se and Li-S energy storage systems.

RESUMO

The rapid expansion of the study of chiral inorganic structures has led to the extension of the functional boundaries of inorganic materials. Nature-inspired self-assembled chiral inorganic structures exhibit diverse morphologies due to their high assembly efficiency and controlled assembly process, and they exhibit superior inherent properties such as mechanical properties, chiral optical activity, and chiral fluorescence. Although chiral self-assembled inorganic structures are becoming more mature in chiral catalysis and chiral optical regulation, biomedical research is still in its infancy. In this paper, various forms of chiral self-assembled inorganic structures are summarized, which provides a structural starting point for various applications of chiral self-assembly inorganic structures in biomedical fields. Based on the few existing research statuses and mechanism discussions on the chiral self-assembled materials-mediated regulation of cell behavior, molecular probes, and tumor therapy, this paper provides guidance for future chiral self-assembled structures to solve the same or similar medical problems. In the field of chiral photonics, chiral self-assembled structures exhibit a chirality-induced selection effect, while selectivity is exhibited by chiral isomers in the medical field. It is worth considering whether there is some correspondence or juxtaposition between these phenomena. Future chiral self-assembled structures in medicine will focus on the precise treatment of tumors, induction of soft and hard tissue regeneration, explanation of the biochemical mechanisms and processes of its medical effects, and improvement of related theories.

RESUMO

This paper investigated the impact of eye-closed and weighted training (EWMT) on the stroke effect of adolescent table tennis players. Forty-eight adolescent table tennis players were randomly selected from the China Table Tennis College and were divided into two groups as 1) the experimental group (EG, n = 24) in which they engaged in multi-ball exercise with eye-closed and weighted swing for 10 weeks, and 2) the control group (CG, n = 24) in which they received a normal training without eye-closed and weighted swing intervention. The stroke effect was assessed by three outcome measures: accuracy, stability, and ball speed. Results showed that 1) both the traditional training method and EWMT can improve the stroke effect of adolescent table tennis players. 2) In terms of accuracy, the number of stroke in the corner area was significantly different between EG and CG after the experiment (p = 0.022, p < 0.001, respectively). 3) In terms of stroke stability, there was a significant difference in the number of net ball strokes between EG and CG after the experiment (p = 0.014). 4) In terms of ball speed, there was no significant difference between EG and CG after the experiment (p = 0.871). 5) After EWMT, the stroke stability of backspin had more significant improvement than that of topspin. Thus, compared with the traditional training method, the EWMT method can improve the stroke effect of adolescent table tennis players in terms of accuracy and stability more significantly; the EWMT method can improve the stroke effect of backspin more significantly than that of topspin in terms of stability.

RESUMO

We demonstrate a temperature-insensitive waveguide sensor based on a silicon-on-insulator platform. The sensor consists of a ring resonator and a Mach-Zehnder interferometer (MZI). A free spectral range of the sensing ring is designed to be slightly different from that of the MZI; hence, the Vernier effect can be employed to improve sensitivity. By optimizing structural parameters of the MZI, the envelope peak position of a cascaded transmission spectrum can be immune to the temperature variation, and only dependent on analyte change in the sensing area. The experimental results show that bulk refractive index (RI) sensitivity of the proposed sensor is 3552 nm/RI unit, while its temperature sensitivity is less than 4 pm/K, which is two orders of magnitude smaller than the conventional cascaded sensor structure without temperature compensation. The proposed temperature-insensitive waveguide sensor does not need polymer cladding or extra thermal stabilization, making it more robust in practical applications.
