Cardiomyopathy is a major cause of death in Chagas disease and early detection of cardiac involvement is essential. Myocardial strain is a reliable technique for assessment of subtle left ventricular (LV) contractility alterations. This study assessed LV global longitudinal strain (GLS) in a large Chagas disease population living in remote areas. Between 2015 and 2016, Chagas disease patients were selected in the northern of the Minas Gerais state. All patients underwent T. cruzi antibodies tests and those who had positive tests were included. A resting 12-lead ECG was recorded and classified using the Minnesota Code criteria. Echocardiography was performed at public health primary care units and speckle-tracking strain was analyzed offline. LV dysfunction was defined as ejection fraction (LVEF < 50%) and reduced GLS was defined as < 16% (absolute value). A total of 1387 patients were included, mean age of 60.0 ± 12.5 years, 68% were women, and 14% had LV dysfunction. Among patients with normal LVEF, 59% had impaired LV GLS. Overall, patients with impaired GLS were older, had more comorbidities and ECG abnormalities than those with normal GLS. The independent factors associated with reduced GLS were ST-T abnormalities (OR 1.954; 95% CI 1.027-3.718), QRS duration (OR 1.009; 95% CI 1.002-1.016), LVEF (OR 0.947; 95% CI 0.923-0.972), and E/e' ratio (OR 1.059; 95% CI 1.009-1.112). In a cohort of Chagas disease from endemic areas, impaired LV GLS was detected in a significant proportion of patients, despite normal ECG and preserved LVEF. The main determinants of reduced LV GLS were ST-T abnormalities, QRS duration, LVEF and E/e' ratio, adjusting for demographical and clinical data.

Introdução: a insuficiência cardíaca (IC) é uma condição que determina grande sofrimento aos pacientes. Entretanto, a prática de cuidados paliativos (CP) no seu manejo ainda é bastante incipiente. Objetivos: analisar a distribuição territorial dos serviços de CP no Brasil e discutir o papel destes no manejo de pacientes com IC. Metodologia: estudo exploratório documental, com informações demográficas e epidemiológicasobtidas do DATASUS e do IBGE. A relação dos serviços de CP foi obtida da Associação Brasileira de Cuidados Paliativos (ABCP), da Academia Nacional de Cuidados Paliativos (ANCP) e do Ministério da Saúde (MS). A revisão bibliográfica foi realizada nas bases PubMed, LILACS e Scielo, usando os descritores insuficiência cardíaca, cuidados paliativos,qualidade de vida e assistência terminal. Resultados: entre 1996 e 2013 houve mais de 500 mil óbitos por IC no país. Em 2014, os custos hospitalares para tratamento de IC foram superiores a R$ 315 milhões. Embora os CP sejam eficazes no manejo da IC, há baixa disponibilidade desses serviços, que se concentram principalmente no Sudeste, vinculados a instituições oncológicas. Conclusão: a IC é uma síndrome clínica complexa que exige abordagem interdisciplinar. A atenção holística provida pelos CPs mostra-se eficaz na redução de internações por doença descompensada, na incidência de depressão e na melhoria da qualidade de vida do paciente.


Introduction: Heart failure (HF) is a condition that results in great suffering to patients. However, the practice of palliative care in their management is still incipient. Objective: The aim of this study was to analyze the territorial distribution of palliative care services in Brazil and discuss their role in the treatment of patients with HF. Methods: This is a documental exploratory study, with information obtained from DATASUS, IBGE, the Brazilian Association of Palliative Care (ABCP) and the National Palliative Care Academy (ANCP). A literature review was conducted on PubMed, LILACS and Scielo, using the descriptors Heart Failure, Palliative Care, Quality of Life and Terminal Care. Results: Between 1996 and 2013 there were over than 500.000 deaths due to HF in Brazil. In 2014, hospital costs for treating HF were higher than R$ 315 million. Despite the evidence of the effectiveness of palliative care in heart failure?s management, there is low availability of these services, which is mainly concentrated in the southeast of the country, linked to oncological institutions. Conclusion: The HF is a clinical syndrome that requires an interdisciplinary approach. The holistic care provided by palliative care is effective in reducinghospitalizations for complications of the HF, the incidence of depression and in improving the patient?s quality of life.

RESUMO

We describe the case of a 41-year-old man with congenital heart disease and infective endocarditis (IE), who presented multiple vegetations attached to the pulmonary, mitral, and aortic valves. Three valve replacements were performed, but the patient developed an abscess at the mitral-aortic intervalvular fibrosa and died due to sepsis. We briefly discuss the indications for surgery in IE, emphasizing its role in the treatment of uncontrolled infection.

Paciente do sexo masculino, 41 anos, portador de cardiopatia congênita apresentando-se com endocardite infecciosa (EI) e vegetações nas valvas pulmonar, aórtica e mitral. Três trocas valvares foram realizadas, mas o paciente evoluiu com recidiva da infecção, desenvolvendo abscesso na região da fibrosa intervalvar mitro-aórtica progredindo para sépsis e óbito. Nesse relato, discutimos brevemente as indicações para a cirurgia na EI, destacando sua indicação no tratamento da infecção não controlada.

RESUMO

Over the last 20 years a great advance has been observed in many aspects of medicine, and the advent of novel imaging techniques is certainly amongst the most important. In schistosomiasis these new methods caused a revolution in the definition of the clinical forms of the disease and in the evaluation of its complications, such as, liver fibrosis, pulmonary hypertension and neuroschistosomiasis, as never before. Herein we present an overview of the image methods used to diagnose schistosomiasis mansoni nowadays.

PURPOSE: The 6-minute walk test (6MWT) is a simple and reliable method routinely used in the evaluation of many conditions including cardiac diseases. However, it is rarely performed in patients with pacemakers (PMs), and the applicability and safety in this group is not established. The purpose of this study was to verify the applicability and safety of the 6MWT in the evaluation of functional capacity of patients with PMs, considering the ergometry test (ET) as the criterion standard. METHODS: Twenty-four patients with rate-responsive PMs underwent a treadmill ET, using the Chronotropic Assessment Exercise Protocol, and a 6MWT, performed twice with a rest interval of 20 minutes. Functional capacity, heart rate, and blood pressure in each test were recorded. RESULTS: The distance covered during the 6MWT significantly correlated with the oxygen uptake estimated from the treadmill ET (r = 0.709, P < .001). Peak heart rates and systolic blood pressure levels reached during the 2 tests were also significantly correlated. No adverse events or malignant arrhythmias were observed during the 6MWT. The distance walked during the 6MWT accurately predicted the patient performance on the ET (area under the receiver operating characteristic curve = 0.921). The ability to walk more than 538 m in the 6MWT predicted, with a sensitivity of 90% and a specificity of 79%, an estimated oxygen uptake above 10 metabolic equivalents (METs) on the ET. CONCLUSIONS: The 6MWT can be considered safe to assess the submaximal functional capacity and can be used as an alternative test to evaluate the results of interventions in patients with permanent cardiac PMs.
