RESUMO

Mitochondrial dysfunction was reported to be involved in the development of lung diseases including chronic obstructive pulmonary disease (COPD). However, molecular regulation underlying metabolic disorders in the airway epithelia exposed to air pollution remains unclear. In the present study, lung bronchial epithelial BEAS-2B and alveolar epithelial A549 cells were treated with diesel exhaust particles (DEPs), the primary representative of ambient particle matter. This treatment elicited cell death accompanied by induction of lipid reactive oxygen species (ROS) production and ferroptosis. Lipidomics analyses revealed that DEPs increased glycerophospholipid contents. Accordingly, DEPs upregulated expression of the electron transport chain (ETC) complex and induced mitochondrial ROS production. Mechanistically, DEP exposure downregulated the Hippo transducer transcriptional co-activator with PDZ-binding motif (TAZ), which was further identified to be crucial for the ferroptosis-associated antioxidant system, including glutathione peroxidase 4 (GPX4), the glutamate-cysteine ligase catalytic subunit (GCLC), and glutathione-disulfide reductase (GSR). Moreover, immunohistochemistry confirmed downregulation of GPX4 and upregulation of lipid peroxidation in the bronchial epithelium of COPD patients and Sprague-Dawley rats exposed to air pollution. Finally, proteomics analyses confirmed alterations of ETC-related proteins in bronchoalveolar lavage from COPD patients compared to healthy subjects. Together, our study discovered that involvement of mitochondrial redox dysregulation plays a vital role in pulmonary epithelial cell destruction after exposure to air pollution.

RESUMO

Colonic hamartomatous polyps are clinically benign tumors. Colonic hamartomas are polypoid lesions that are rare in adults and most commonly encountered in infants and children. We report an unusual case of giant colonic hamartomatous polyps that were found incidentally during a medical workup for acute lower gastrointestinal bleeding in a 26-year-old woman. We present the color Doppler ultrasound, computed tomography scan, and endoscopic pattern of colonic hamartomatous polyps.

Surgical smoke is a common chemical hazard produced from the use of electrocautery, laser, or ultrasonic scalpels during surgery. It has been proved harmful to medical personnel. Thus, it is important to monitor surgical smoke concentrations in the operating room. In the past decade, many researches regarding surgical smoke were discussed in different professional healthcare fields, but few showed the correlation between surgical smoke and otolaryngology surgery. In this study, the concentrations of particulate matter and formaldehyde were measured during thirty cases of several types of otolaryngology surgery in a regional research hospital in Taiwan. The concentrations of 0.3 µm and 0.5 µm particulate matter raised rapidly in the main knife range at the beginning of the electrocautery knife used, and then decreased by half after 5-10 min of use. The concentrations of formaldehyde were ranged from 1 to 2 ppm during the surgery, which is higher than the permissible exposure limit. While many medical staffs are working in the operating room and are exposed to the smoke hazard, effective strategies for collecting and eliminating the smoke should be taken in all medical facilities.

The evolution of the inhomogeneous distribution of the transformation stress (σs) and strain fields with an increasing number of cycles in two differently orientated grains is investigated for the first time using a combined technique of digital image correlation and data-driven identification. The theoretical transformation strains (εT) of these two grains with crystal orientations [5 3 26]ß and [6 5 11]ß along the loading direction are 10.1% and 7.1%, respectively. The grain with lower εT has a higher σs initially and a faster decrease in σs compared with the grain with higher εT. The results show that the grains with higher σs might trigger more dislocations during the martensite transformation, and thus result in greater residual strain and a larger decrease in σs during subsequent cycles. Grain boundary kinking in bicrystal induces an additional decrease in transformation stress. We conclude that a grain with crystal orientation that has high transformation strain and low transformation stress (with respect to loading direction) will exhibit stable transformation stress, and thus lead to higher functional performance in Cu-based shape memory alloys.

Surface condition and corrosion resistance are major concerns when metallic materials are going to be utilized for applications. In this study, FeCoNiCr medium-entropy alloy (MEA) is first treated with a nitrogen atmospheric-pressure plasma jet (APPJ) and then coated with octadecyltrichlorosilane (OTS) for the surface modification. The hydrophobicity of the FeCoNiCr MEA was effectively improved by OTS-coating treatment, APPJ treatment, or the combination of both treatments (OTS-coated APPJ-treated), which increased the water contact angle from 54.49° of the bare MEA to 70.56°, 93.94°, and 88.42°, respectively. Potentiodynamic polarization and electrochemical impedance spectroscopy tests demonstrate that the APPJ-treated FeCoNiCr MEA exhibits the best anti-corrosion properties. X-ray photoelectron spectroscopy reveals that APPJ treatment at 700 °C oxidizes all the alloying elements in the FeCoNiCr MEA, which demonstrates that a short APPJ treatment of two-minute is effective in forming a metal oxide layer on the surface to improve the corrosion resistance of FeCoNiCr MEA. These results provide a convenient and rapid method for improving surface properties of FeCoNiCr MEA.

WHAT IS KNOWN ON THE SUBJECT?: Internalized stigma in people diagnosed with mental illness has several negative outcomes; however, it remains unclear in an Indonesian context. The human rights of people diagnosed with mental illness in Indonesia have been routinely violated because of the existing stigma against mental illness and deficiencies in the country's mental healthcare services. WHAT THE PAPER ADDS TO EXISTING KNOWLEDGE?: To the best of our knowledge, this is the first study to examine the level of internalized stigma and related factors among Indonesians diagnosed with schizophrenia. Almost one-third of the subjects had moderate-to-severe levels of internalized stigma, among which discrimination was the strongest type. Specifically, younger and unemployed people with more psychotic symptoms had higher levels of internalized stigma. WHAT ARE THE IMPLICATIONS FOR PRACTICE?: The provision of adequate pharmacological and psychosocial treatments to help people manage their psychotic symptoms is extremely important to ameliorate stigma. Vocational rehabilitation and employment support for unemployed people may potentially reduce their internalized stigma and promote recovery. ABSTRACT: Introduction The human rights of people diagnosed with mental illness in Indonesia have been routinely violated because of societal stigma associated with mental illness and deficiencies in the country's mental healthcare services. Knowledge about internalized stigma among people diagnosed with schizophrenia in lower middle-income Muslim countries is also scarce. Aim To identify the level of internalized stigma and related factors among people diagnosed with schizophrenia in Indonesia. Method A cross-sectional survey was conducted with a purposive sample of 300 people diagnosed with schizophrenia from an urban Indonesian psychiatric hospital. Results Almost one-third of the subjects had moderate-to-severe levels of internalized stigma, among which discrimination was the strongest type. Younger age, unemployment status and having psychotic symptoms significantly increased the risk of internalized stigma when analysed in a multivariable ordinal logistic regression. Discussion The present study firstly identified the high levels of internalized stigma among Indonesian individuals diagnosed with schizophrenia. Implications for practice Mental health nurses are suggested to provide early anti-stigma illness management and family psychoeducation interventions to people diagnosed with schizophrenia and their family caregivers, helping them to develop an optimistic understanding about the concept of schizophrenia and mitigating the negative consequences of public and internalized stigma.

The staff in hospitals is the high-risk group affected by indoor air pollution for operational reasons. However, there is a lack of relevant information and research on surgical smoke in Taiwan. This study analyses the substance of the smoke and develops relative strategies to prevent harm from the working place. This study aims to evaluate the surgical smoke generated during laparotomy and laparoscopic operation. The relationship between smoke and the risk of health-care workers was also discussed in order to set suitable solutions for smoke collection and emission. The study measured surgical smoke of 30 cases in operating rooms in three different hospitals, smoke was collected at different timings and different areas during the operation using a low-flow sampler. The concentration and accumulation number of size 0.3 µm suspended particles were the highest among all size of particles in different detection areas and timing. The concentration number of 0.3 µm and 0.5 µm particles reached the maximum after 10 min of electrosurgical treatment; however, the concentration number of 5 µm particles began to decrease after 5-15 min of the treatment. Besides, the cumulative particle numbers of 0.3 µm and 0.5 µm in laparoscopic operation were higher than those of laparotomy after 10 min of the treatment. This study suggests that surgical smoke prevention should use smoke evacuator; health-care workers should also wear a highly efficient tight seal-fit mask in the operating room to avoid damage to their health and cause chronic diseases.Implications: Indoor air pollution in medical institutions is a topic of less concern, and the health of healthcare workers is threatened by surgical smoke that generated during operations. This study measured and analyzed the harmful substances in the surgical smoke in the operating rooms of hospitals, and put forward the corresponding suggestions. In April 2018, legislators in Taiwan also convened a seminar on "Respiratory protection for operating room personnel in medical institutions". The seminar applied the findings of this study and adopted the recommendations to improve related equipment and strategies in future plans.

BACKGROUND: Autism spectrum disorder (ASD) is a lifelong developmental disability characterized by deficits in social communication and social interaction across multiple contexts. Existing literature on social relationships and well-being among adolescents with ASD in Asian countries is scant. AIMS: This study compared the perceptions of adolescents with ASD with those of their neurotypical peers toward their friendship quality, activity participation, and emotional well-being, and examined the relationships between friendship quality, activity participation, and emotional well-being. METHODS: The study participants-101 adolescents with ASD and 101 neurotypical peers, aged 10-19 years-completed the following self-administered questionnaires: the Friendship Quality Questionnaire, the Child and Adolescent Scale of Participation, the Beck Anxiety Inventory, and the short-form UCLA loneliness scale. RESULTS: Adolescents with ASD reported lower friendship quality, lower school participation, and higher levels of anxiety and loneliness compared to their neurotypical peers. Loneliness correlated negatively with friendship quality and school participation and positively with anxiety. Adolescents with ASD experienced increased levels of anxiety when low friendship quality was accompanied by greater loneliness. CONCLUSIONS AND SIGNIFICANCE: These findings reveal that friendship quality, school participation, and loneliness have a considerable effect on the emotional well-being of adolescents with ASD, thus indicating the need for therapeutic interventions that address interpersonal relationships and emotional well-being.

Superelasticity in shape memory alloys is an important feature for actuators and medical devices. However, the function of the devices is typically limited by mechanical bandwidth and fatigue, which are dominated by the microstructures. Thus, in order to correlate the mechanical response and the microstructures, the microstructural evolution in NiTi single crystals under the compression, tensile, and shearing tests is simulated by molecular dynamics (MD) in the current study. Then, the martensite variant identification method, which identifies the crystal variants/phases of each lattice based on the transformation matrix, is used to post-process the MD results. The results with the detailed information of variants and phases reveal many features that have good agreement with those reported in the literature, such as X-interfaces and the transitional orthorhombic phase between the austenite and monoclinic phases. A new twin structure consisting of diamond and wedge-shaped patterns is also discovered. The macroscopic behavior, such as stress-strain curves and the total energy profile, is linked with the distribution of dislocation and twin patterns. The results suggest that the loading cases of shear and compression allow a low critical strain for the onset of martensitic transformation and a better superelasticity behavior. Therefore, the two loading cases are suitable to apply to the NiTi actuators. The current work is expected to provide insight into the mechanical responses and design guideline for NiTi shape memory alloy actuators.

Happiness, an important factor in maintaining health, not only enhances the abilities of self-control, self-regulation, and coping but also promotes mental health. Mindfulness therapy has been increasingly used in recent years. Therefore, the purpose of the present article is to introduce the concepts of mindfulness and to describe the relationship between mindfulness and happiness. Further, we provide brief introductions to mindfulness-based stress reduction and mindfulness cognitive therapy as well as present the current evidence related to the effects of mindfulness programs and therapies in clinical patient care. The information in the present article may be referenced and used by nurses in patient care and may be referenced by health professionals to promote their own mental health in order to maintain optimal fitness for providing high-quality patient care.
