There is conflicting evidence in select mouse models and humans that suggest angiotensin-converting enzyme 2 expression is increased due to treatment with angiotensin converting enzyme inhibitors and angiotensin receptor blockers (ACEI/ARBs). Given the wide range of conditions that these medications treat, further evaluation is necessary to determine safety in the context of COVID-19. We sought to determine the association between use of ACEI/ARBs and COVID-19 severity in patients with essential hypertension. We included 714 patients with essential hypertension diagnosed with COVID-19 and admitted to University of Iowa Healthcare from March 1, 2020 to June 29, 2021. Severity of COVID-19 infection was assessed based on mortality, length of stay in hospital, intensive care unit admission, and use of supplemental oxygen, invasive ventilation, and vasopressors. Multivariable logistic and linear regression analyses were used for binary and continuous outcomes, respectively. Prior exposure to ACEI/ARBs before admission was significantly associated with lower mortality (OR: 0.454, p = .015), shorter length of stay in hospital (p < .001), and decreased adjusted odds of intensive care admission (OR: 0.719; p < .042). The present results suggest that patients with essential hypertension hospitalized with COVID-19 who had a prescription for ACEI/ARBs prior to admission exhibited less severe COVID-19 and lower in-hospital mortality.

BACKGROUND: Angiotensin receptor blockers (ARB) and angiotensin-converting enzyme inhibitors (ACEI) increase the expression of ACE2, which is a receptor for entry of SARS-CoV-2 into cells. Though evidence suggests that ARB/ACEI are safe among the general population with COVID-19, their safety in patients with overweight/obesity-related hypertension deserves further evaluation. OBJECTIVE: We assessed the association between ARB/ACEI use and COVID-19 severity in patients with overweight/obesity-related hypertension. METHODS: This study included 439 adult patients with overweight/obesity (body mass index ≥ 25 kg/m2) and hypertension, diagnosed with COVID-19 and admitted to University of Iowa Hospitals and Clinic from March 1 to December 7, 2020. Mortality and severity of COVID-19 were evaluated based on length of stay in hospital, intensive care unit admission, use of supplemental oxygen, mechanical ventilation, and vasopressors. Multivariable logistic regression was used to examine the associations of ARB/ACEI use with mortality and other markers of COVID-19 severity, with a two-sided alpha set at 0.05. RESULTS: Exposure to ARB (n = 91) and ACEI (n = 149) before hospitalization was significantly associated with lower mortality (odds ratio [OR] = 0.362, 95% confidence interval [CI] 0.149 to 0.880, p = 0.025) and a shorter length of stay (95% CI -0.217 to -0.025, p = 0.015). Additionally, patients using ARB/ACEI showed a non-significant trend toward lower intensive care unit admission (OR = 0.727, 95% CI 0.485 to 1.090, p = 0.123), use of supplemental oxygen (OR = 0.929, 95% CI 0.608 to 1.421, p = 0.734), mechanical ventilation (OR = 0.728, 95% CI 0.457 to 1.161, p = 0.182), and vasopressors (OR = 0.677, 95% CI 0.430 to 1.067, p = 0.093). CONCLUSION: Results suggest that hospitalized patients with COVID-19 and overweight/obesity-related hypertension who were prescribed ARB/ACEI before admission to the hospital exhibit lower mortality and less severe COVID-19 than those who were not taking ARB/ACEI. The results also suggest that exposure to ARB/ACEI may protect patients with overweight/obesity-related hypertension from severe COVID-19 and death.

OBJETIVO: Avaliamos a associação entre o uso de IECA/BRA e a gravidade da COVID-19 em pacientes com hipertensão relacionada ao sobrepeso/obesidade. Type 2 diabetes is often complicated with hypertension, such that 73.6 % of patients ≥ 18 years with type 2 diabetes also have hypertension. With increasing age, the risk of developing these conditions rises significantly. Co-existence of type 2 diabetes and hypertension suggests that there are overlapping genetic, physiologic, and environmental factors at play. Research has shown several overlying metabolic abnormalities including sympathetic nervous system overactivation, insulin resistance, inflammatory and endothelial dysfunction, deleterious expression of microRNAs, and obstructive sleep apnea. Though diet and physical activity are the mainstays of treatment early in the disease course, they are generally not a viable long-term strategy. Treatment with novel hypoglycemic and anti-hypertensive medications can greatly assist in preventing long-term complications of these chronic conditions. Recognition of the common causes and consequences of type 2 diabetes and comorbid hypertension allows for a robust response towards being proactive regarding prevention and treatment of this unique patient population. Thus, the purpose of this current review is four-part: to (1) review the latest discoveries into the mechanisms of hypertension in the setting of type 2 diabetes, (2) discuss the latest treatments of hypertension with a brief discussion about novel hypoglycemic agents in patients with type 2 diabetes, (3) summarize blood pressure targets and (4) to address evidence gaps in treating patients with diabetes and resistant hypertension, and elderly individuals with both hypertension and diabetes.

This scoping review synthesizes the existing research on the use of very low-calorie diets (VLCDs) in subjects with nonalcoholic fatty liver disease (NAFLD) and end-stage liver disease (ESLD). 19 studies were included, of which 5 were clinical trials, 11 were cohort studies, 1 was a case-control study, and 2 were case series totaling 968 subjects. About 17 studies were focused on patients with NAFLD while the two case series described in patients with ESLD on the transplant list or post-liver transplant. Six studies included subjects managed with VLCDs prior bariatric surgery. Most studies were short term and demonstrated acute improvement of diverse liver biomarkers including liver function tests, indices of hepatosteatosis and reduction in liver size. Adherence rates in these studies were between 69% and 93%. Eight studies did not report any adverse events and four subjects were reported to have discontinued VLCD due to adverse effects in two different studies. Aggregated adverse events were mild. Treatments based on VLCD in subjects with NAFLD seem to be safe and tolerable but can result in mild adverse effects. The findings of this scoping review suggest that the use of VLCD in patients with obesity complicated with NAFLD and potentially in ESLD appear to be effective to induce weight loss and to acutely reduce hepatosteatosis.

The changes in near-surface soil freeze-thaw cycles (FTCs) are crucial to understanding the related hydrological and biological processes in terrestrial ecosystems under a changing climate. However, long-term dynamics of soil FTCs at the hemisphere scale and the underlying mechanisms are not well understood. In this study, the spatiotemporal patterns and main driving factors of soil FTCs across the Northern Hemisphere (NH) during 1979-2017 were analyzed using multisource data fusion and attribution approaches. Our results showed that the duration and the annual mean area of frozen soil in the NH decreased significantly at rates of 0.13 ± 0.04 days/year and 4.9 × 104 km2/year, respectively, over the past 40 years. These were mainly because the date of frozen soil onset was significantly delayed by 0.1 ± 0.02 days/year, while the end of freezing and onset of thawing were substantially advanced by 0.21 ± 0.02 and 0.15 ± 0.03 days/year, respectively. Moreover, the interannual FTC changes were more drastic in Eurasia than in North America, especially at mid-latitudes (30°-45° N) and in Arctic regions (>75° N). More importantly, our results highlighted that near-surface air temperature (T a ) and snowpack are the main driving factors of the spatiotemporal variations in soil FTCs. Furthermore, our results suggested that the long-term dynamics of soil FTCs at the hemisphere scale should be considered in terrestrial biosphere models to reduce uncertainties in future simulations.

The alternative stable state (ASS) theory provides a plausible framework to explain the spatial distribution of biomes and their dynamics. Existing studies to test the alternative biome states (ABSs) mainly focused on tree-dominated biomes. It is still uncertain whether ABSs are present in a wide range of terrestrial biomes. This study was to examine the ABSs in the terrestrial vegetated areas of Africa and the maintaining factors. The potential landscapes were reconstructed separately for forest, grassland and shrubland using the MODIS Vegetation Continuous Field data along the gradients of temperature, precipitation and aridity index (AI, the ratio of precipitation to potential evapotranspiration). The differences of soil organic carbon density (SOCD), fire count and grazing intensity were compared to test the feedback hypothesis to maintain the ABSs. The results showed that AI performed well in detecting the ABSs at the continental scale of Africa. Forest (at the wetter end) and shrubland (at the drier end) were well separated along the AI axis. Forest had three stable states (i.e. closed forest, woody savanna, and savanna) and shrubland had two stable states (i.e. closed shrubland and open shrubland). Grassland had two stable states (i.e. dense grassland and sparse grassland) distributing in a large AI range. The stable states that shared a specific AI range were regarded as the ABSs. Climate aridity greatly determined the distribution of the ABSs but the positive feedbacks between vegetation and SOCD, fire count, and livestock density played potential roles in driving the shifts between the ABSs. Our study indicated that the ABSs commonly existed in varied biomes (both tree-dominated and non-tree-dominated) in the African continent, which provided an enlarged picture of the ABSs of the terrestrial biomes. The findings contribute to a deeper understanding of large scale vegetation patterns and their dynamics and facilitate to macro management of the terrestrial ecosystems in facing the possible regime shifts of the biomes.

The ability to identify a specific type of leukemia using minimally invasive biopsies holds great promise to improve the diagnosis, treatment selection, and prognosis prediction of patients. Using genome-wide methylation profiling and machine learning methods, we investigated the utility of CpG methylation status to differentiate blood from patients with acute lymphocytic leukemia (ALL) or acute myelogenous leukemia (AML) from normal blood. We established a CpG methylation panel that can distinguish ALL and AML blood from normal blood as well as ALL blood from AML blood with high sensitivity and specificity. We then developed a methylation-based survival classifier with 23 CpGs for ALL and 20 CpGs for AML that could successfully divide patients into high-risk and low-risk groups, with significant differences in clinical outcome in each leukemia type. Together, these findings demonstrate that methylation profiles can be highly sensitive and specific in the accurate diagnosis of ALL and AML, with implications for the prediction of prognosis and treatment selection.
