RESUMO

Choosing between screw-retained and cement-retained restorations in the esthetic zone may have a significant implication on the restorative contour. This study analyzes the effect of facial-palatal implant positioning on the facial emergence angle of implant restorations in the maxillary anterior region. 133 maxillary anterior implant cases were captured with intraoral scans and used to create digital prosthetic designs. The facial emergence angle of the restoration and the depth of the implant were determined using images at the mid-facial cross-section. Simple logistic and linear regression models were used to analyze the interrelation between the emergence angle, depth, and retention methods. The average facial emergence angle for all restorations in this study was 34.4 degrees. A significant difference was found in emergence angle between screw-retained and cement-retained groups, with an average emergence angle of 37.3 degrees and 27.9 degrees, respectively. There is no difference in the mean depth between those two groups. There is a strong negative linear relationship between depth and emergence angle for the screw-retained group. Overall, implant positioning in the anterior maxilla has a significant influence on emergence angle. Facial emergence angle is significantly greater for implants in screw-retained position. This may be partially alleviated by deeper implant placement.

Fourth-year away rotations are well recognized as an important modifiable variable that has been shown to increase a student's opportunity to match into orthopaedic surgery. The purpose of this article was to determine whether allopathic (MD) and osteopathic (DO) medical students have equal opportunities for away rotations in terms of (1) eligibility and (2) fees associated with rotations after the single accreditation merger. Methods: A cross-sectional study was performed during the 2021 application cycle (April to November) by reviewing all nonmilitary, Accreditation Council for Graduate Medical Education-accredited orthopaedic surgery programs (n = 194). Each program's website, affiliated school of medicine's website, and visiting student applications service portal were searched. Eligibility criteria for an away rotation and associated fees were recorded. Results: Of the 194 programs, 18 (9.3%) of programs were found to have publicly published eligibility for away rotations that prohibited students based on applicant degree. Five (2.6%) programs/medical schools had fees that were larger for DO medical students compared with MD medical students ranging for $50 to $5,000. No programs/medical schools had larger fees for MD medical students compared with DO medical students. Conclusions: Although DO and MD degrees are equivalent degrees for licensing and credentialing and graduate medical education has transitioned to a single accrediting body, there remained discord in the opportunities for fourth-year away rotations between osteopathic and allopathic medical students. This study highlights the necessity for greater reform, consistency, and transparency among orthopaedic surgery residency programs and their affiliated institutions.

Purpose: To use fantasy football points as a simple measure alongside on-field statistics to compare performance in National Football League (NFL) offensive skill position players before and after anterior cruciate ligament (ACL) reconstruction. Methods: A retrospective review of all NFL quarterbacks (QB), running backs (RB), wide receivers (WR), and tight ends (TE) who sustained an isolated, unilateral ACL injury from 1988 to 2017 was conducted. Data were collected from public data sources, team releases, NFL injury reports, press releases, and other Internet resources. For each player, a matched control with similar demographics was identified. Their in-game performance post-ACL reconstruction was analyzed using fantasy football points as an outcome measure. Results: A total of 13 QBs, 30 RBs, and 29 WRs who underwent ACL reconstruction from 1988 to 2017 and who met inclusion criteria were retrospectively identified and reviewed. Of the 13 quarterbacks included in the study, there was no statistically significant difference in fantasy football points between the pre- and post-ACL reconstruction groups, as well as post-ACL and matched control groups. There was a statistically significant decrease in career fantasy football performance of running backs post-ACL reconstruction compared with matched control groups (129.6 vs 553.6; P < .0001). There was also a statistically significant decrease in per game fantasy football points post-ACL reconstruction (4.4 vs 11.2; P < .0001). Lastly, WRs also demonstrated a decrease in career fantasy football performance post-ACL reconstruction compared with matched controls (145.3 vs 460.9; P = .002). In addition, they also had a decrease in per game fantasy football performance (5.0 vs 7.7; P = .042). Conclusions: Quarterbacks did not have a statistically significant decrease in performance following ACL reconstruction based on fantasy football performance. Conversely, both running backs and wide receivers had decreased per game and career performance post-ACL reconstruction based on their fantasy football statistics. Furthermore, RBs had the largest decline in production each season over a 3-year period following ACLR compared to QBs and WRs, respectively. Level of Evidence: Level III, case-control study.

PURPOSE: To identify the time to return to play (RTP) and evaluate the performance level in wide receivers in the National Football League following anterior cruciate ligament (ACL) reconstruction. METHODS: A total of 29 wide receivers in the National Football League who underwent ACL reconstruction between 2013 and 2017 who met inclusion criteria were retrospectively identified and reviewed. For each player, a matched control with similar demographics was identified to compare various in-game performance measurements and seasons played. RESULTS: Of the wide receivers that met the inclusion criteria, 9 of 29 (31%) did not RTP in a regular season game following ACL reconstruction. For players who did RTP, 20 of 29 (69%), the average time was 10.9 months (331.4 ± 41.6 days). When we compared the tear group with the matched control cohort, players with ACL tears ended their careers on an average of 1.9 seasons earlier (2.2 vs 4.1 seasons, P < .001) and also played less than half the number of games (25.5 vs 56.6 games, P = .001), respectively. Those that RTP also saw decreased performance statistics in targets (353.6 vs 125.2 P < .001), receptions (208.0 vs 74.4, P = .001), receiving yards (2691.0 vs 987.9, P = .001), and touchdowns (17.4 vs 6.2, P = .002). CONCLUSIONS: Sixty-nine percent of wide receivers who underwent ACL reconstruction were able to RTP at an average of 10.9 months, or 331.4 days. Despite the majority of players being able to RTP, there was a significant decrease in both statistical performance and career duration. LEVEL OF EVIDENCE: Level III, case-control study.

OBJECTIVE: This study provides an assessment of heat stress in indoor rice vermicelli manufacturing factories. METHODS: Worker interviews and heat stress assessments were conducted in food manufacturing factories in Singapore. The Wet Bulb Globe Temperature (WBGT) and Heat Stress Index (HSI) were used as heat stress indicators. RESULTS: The highest WBGT and HSI levels recorded in the rice vermicelli manufacturing factories were 36.68°C and 3777 in the drying and steaming process respectively. These levels were above the recommended permissible HSI and WBGT action limit for heat exposure and considered to be high risk. CONCLUSION: Workers in indoor rice vermicelli manufacturing factories can be exposed to heat stress, and the current measures in place may not be sufficient to protect workers against heat stress injuries. Preventive measures such as engineering controls and heat acclimatization programs are important.

INTRODUCTION: In Singapore, the notification of workplace accidents and occupational diseases is a legal requirement under the Workplace Safety and Health (Incident Reporting) Regulations. This review is to identify back injury cases with physical work stresses and propose recommendations for preventive measures. MATERIALS AND METHODS: Cases involving back injuries notified to the Ministry of Manpower (MOM) from 1 January 2011 to 31 December 2012 were reviewed. Using predefined criteria, cases with physical work stresses were identified and the associated variables analysed. RESULTS: A total of 1124 notifications involving back injuries were received from 2011 to 2012; 579 cases (52%) were identified to have physical work stresses, of which 447 cases (77%) were males and 375 (64%) of them were between the ages of 21 to 40 years. The commonest physical work stresses were carrying (35%), awkward postures (22%) and lifting (20%). Majority of the cases came from the following sectors: construction (14%), accommodation and food services (12%), transport and storage (12%), manufacturing (10%) and wholesale and retail trade (10%). CONCLUSION: We identified the main physical work stresses in the back injury cases as carrying, awkward postures and lifting. The common industries involved were construction, accommodation and food services, and transport and storage sectors. This is useful for programmes aimed at reducing physical work stresses which can lead to improved work and health outcomes for our workers.
