RESUMO

Mayer-Rokitansky-Küster-Hauser syndrome (MRKH) is a congenital disorder syndrome characterized by failure of the uterine and vaginal organs to develop normally. The prevalence of MRKH is estimated to occur in about 1 in 5000 of female live births. A 25-year-old female patient comes to a general obstetric and gynecological polyclinic with complaints of not being able to menstruate at all since birth. There is a history of vaginal discharge but it is neither viscous nor had an odor. On ultrasound examination, the structure of the uterus and ovaries was not found in a normal place. On follow up MRI examination showed agenesis of the uterus and proximal two-third of the vagina accompanied by abnormal positioning of both ovaries, thus supporting to atypical form of MRKHS. The patient is not given drug therapy, but she was planned to transplant the uterine organs. This case report suggests MRKH syndrome can be characterized by ectopic ovaries and a uterus that is not fully developed and can also be accompanied by agenesis of the vaginal organs. Pelvic ultrasound is the main modality chosen to be performed in patients with symptoms of primary amenorrhea. When pelvic organs cannot be visualized properly, it will be performed MRI examination. MRI examination is known to have sensitivity and specificity up to 100% in diagnosing MRKH syndrome. This case report describes a 25-year-old woman with primary amenorrhea with MRKH syndrome. MRI is a sensitive and specific examination to confirm the diagnosis.

The management of hypervascularity tumor in infants and children is challenging due to its hemorrhage risk which can be life threatening. A proper and precise approach should be performed in preventing massive bleeding. A one-month-old male infant came with hypovolemic shock due to intratumor hemorrhage on the right chest that presented at birth and grew progressively with a size of 20 × 15 × 7 cm. After stabilization, Magnetic resonance imaging and Doppler ultrasonography of the tumor showed a solid lesion with an increase in vascular flow. He underwent two rounds of digital subtraction angiography (DSA) and trans arterial embolization (TAE) on the internal mammary, superior thoracic, and right thoracoacromial arteries. On the sixth day after second embolization and ensure significantly reduced vascularity using the ultrasound Doppler modality, he performed tumor removal surgery and skin grafted closure. There were no post-embolization or post-operative complications. Histopathological examination with immunohistochemistry staining on the tumor tissue indicated as Kaposiform hemangioendothelioma. Fluoroscopic technique of DSA and TAE should be considered prior to the tumor removal surgery has been proven to be safe and effective options in progressive large mass with high vascular flow management.

Diabetes mellitus is a chronic hyperglycemic condition that can affect the body's immune response to SARS-CoV-2 This study aimed to determine the relationship between diabetes mellitus and lung severity in COVID-19 patients. METHODS: A cross-sectional design was conducted at Hasan Sadikin General Hospital during the January-May 2021 period. Data were based on medical records of patients aged 18 years and over with COVID-19. The chi-square test was performed to assess the relationship between diabetes mellitus and lung severity based on the BRIXIA score. RESULTS: This study included 538 subjects, mostly aged <60 years (71.9%) and female (60.2%). A total of 125 subjects had abnormal blood glucose levels with an average HbA1c of 9.00 ± 1.77% in patients with diabetes mellitus and a median HbA1c of 5.85% (4.5-6.4%) in patients with reactive hyperglycemia. Lung abnormalities were found in 357 subjects (66.4%). The results of the BRIXIA score to assess lung severity found as many as 77 subjects (14.3%) had a score of 11-18 with 14 people with diabetes mellitus, five people with reactive hyperglycemia. In the population aged ≥60 years, as many as 32 people had a score of 11-18 with three people with diabetes mellitus, two with reactive hyperglycemia and 27 with normal blood glucose. A significant relationship was found between diabetes mellitus and lung severity (p = 0.024). CONCLUSION: There is a significant relationship between diabetes mellitus and lung severity in COVID-19 patients aged ≥60 years.

Tenosynovial Giant Cell Tumors (TSGCT) are the second most frequent benign soft tissue tumor in the hand and wrist after ganglion cysts. The tumor arises from joint synovia and tendon sheaths. It is divided into 2 categories: localized and diffuse type. The well-circumscribed localized type usually responds nicely to single excision surgery. The diffuse type, on the other hand, has a proclivity for becoming an aggressive lesion. We reported a case of a 56-year-old woman who was experiencing shortness of breath as a result of lung metastases. A TSGCT, localized type, was found after radiology, and histology examination of the re-excised surgical lesion was done. This rare case demonstrates the possibility of lung metastases from tenosynovial giant cell tumor, localized type.
