INTRODUCTION: Pediatric and young adult brain tumors (PYBT) account for a large share of cancer-related morbidity and mortality among children in the United States, but their etiology is not well understood. Previous research suggests the Appalachian region of Kentucky has high rates of PYBT. This study explored PYBT incidence over 25 years in Kentucky to identify geographic and temporal trends and generate hypotheses for future research. METHODS: The Kentucky Cancer Registry contributed data on all PYBT diagnosed among those aged 0-29 during years 1995-2019. Age- and sex-adjusted spatio-temporal scan statistics-one for each type of PYBT, and one for all types-comprised the primary analysis. These results were mapped along with environmental and occupational data. RESULTS: Findings indicated that north-central Kentucky and the Appalachian region experienced higher rates of some PYBT. High rates of astrocytomas were clustered in a north-south strip of central Kentucky toward the end of the study period, while high rates of other specified types of intracranial and intraspinal neoplasms were significantly clustered in eastern Kentucky. The area where these clusters overlapped, in north-central Kentucky, had significantly higher rates of PYBT generally. DISCUSSION: This study demonstrates north-central Kentucky and the Appalachian region experienced higher PYBT risk than the rest of the state. These regions are home to some of Kentucky's signature industries, which should be examined in further research. Future population-based and individual-level studies of genetic factors are needed to explore how the occupations of parents, as well as prenatal and childhood exposures to pesticides and air pollutants, impact PYBT incidence.

RESUMO

A new decomposition algorithm based on QR factorization is introduced for processing and comparing irregularly shaped stress and deformation datasets found in structural analysis. The algorithm improves the comparison of two-dimensional data fields from the surface of components where data is missing from the field of view due to obstructed measurement systems or component geometry that results in areas where no data is present. The technique enables the comparison of these irregularly shaped datasets without the need for interpolation or warping of the data necessary in some other decomposition techniques, for example, Chebyshev or Zernike decomposition. This ensures comparisons are only made between the available data in each dataset and thus similarity metrics are not biased by missing data. The decomposition and comparison technique has been applied during an impact experiment, a modal analysis, and a fatigue study, with the stress and displacement data obtained from finite-element analysis, digital image correlation and thermoelastic stress analysis. The results demonstrate that the technique can be used to process data from a range of sources and suggests the technique has the potential for use in a wide variety of applications.

Population-based ecological and cross-sectional studies have observed high risk for several cancers in areas of Central Appalachia where mountaintop removal coal mines operate. Case-control studies could provide stronger evidence of such relationships, but misclassification of exposure is likely when based on current residence, since individuals could have inhabited several residences with varying environmental exposures over many years. To address this, we used residential histories for individuals enrolled in a previous case-control study of lung cancer to assess residential proximity to mountaintop removal coal mining over a 30-year period, using both survey data and proprietary data from LexisNexis, Inc. Supplementing the survey data with LexisNexis data improved precision and completeness of geographic coordinates. Final logistic regression models revealed higher odds of high exposure among cases. These findings suggest that living in close proximity to mountaintop removal coal mining sites could increase risk for lung cancer, after adjusting for other relevant factors.

A novel methodology is introduced for quantifying the severity of damage created during testing in composite components. The method uses digital image correlation combined with image processing techniques to monitor the rate at which the strain field changes during mechanical tests. The methodology is demonstrated using two distinct experimental datasets, a ceramic matrix composite specimen loaded in tension at high temperature and nine polymer matrix composite specimens containing fibre-waviness defects loaded in bending. The changes in the strain field owing to damage creation are shown to be a more effective indicator that the specimen has reached its proportional limit than using load-extension diagrams. The technique also introduces a new approach to using experimental data for creating maps indicating the spatio-temporal distribution of damage in a component. These maps indicate where damage occurs in a component, and provide information about its morphology and its time of occurrence. This presentation format is both easier and faster to interpret than the raw data which, for some tests, can consist of tens of thousands of images. This methodology has the potential to reduce the time taken to interpret large material test datasets while increasing the amount of knowledge that can be extracted from each test.

Thermoelastic stress analysis using arrays of small, low-cost detectors has the potential to be used in structural health monitoring. However, evaluation of the collected data is challenging using traditional methods, due to the lower resolution of these sensors, and the complex loading conditions experienced. An alternative method has been developed, using image decomposition to generate feature vectors which characterize the uncalibrated map of the magnitude of the thermoelastic effect. Thermal data have been collected using a state-of-the-art photovoltaic effect detector and lower cost, lower thermal resolution microbolometer detectors, during crack propagation induced by both constant amplitude and frequency loading, and by idealized flight cycles. The Euclidean distance calculated between the feature vectors of the initial and current state can be used to indicate the presence of damage. Cracks of the order of 1 mm in length can be detected and tracked, with an increase in the rate of change of the Euclidean distance indicating the onset of critical crack propagation. The differential feature vector method therefore represents a substantial advance in technology for monitoring the initiation and propagation of cracks in structures, both in structural testing and in-service using low-cost sensors.

RESUMO

A new method has been developed for creating localized in-plane fibre waviness in composite coupons and used to create a large batch of specimens. This method could be used by manufacturers to experimentally explore the effect of fibre waviness on composite structures both directly and indirectly to develop and validate computational models. The specimens were assessed using ultrasound, digital image correlation and a novel inspection technique capable of measuring residual strain fields. To explore how the defect affects the performance of composite structures, the specimens were then loaded to failure. Predictions of remnant strength were made using a simple ultrasound damage metric and a new residual strain-based damage metric. The predictions made using residual strain measurements were found to be substantially more effective at characterizing ultimate strength than ultrasound measurements. This suggests that residual strains have a significant effect on the failure of laminates containing fibre waviness and that these strains could be incorporated into computational models to improve their ability to simulate the defect.

We present systematic experiments of the rupture and dewetting of thin films of a nonvolatile polar liquid on partially wetting substrates due to a moving slot jet, which impinges at normal incidence. The relative motion was provided by a custom-built spin coater with a bidirectionally accessible axis of rotation that enabled us to measure film thickness profiles in situ as a function of substrate velocity using dual-wavelength interference microscopy. On partially wetting polymeric substrates, dry spots form in liquid films with a residual thickness well below 1 µm. We measured the density of dry spots as well as the density and size distribution of the residual droplets as a function of film thickness. In a certain parameter range, the droplet distributions exhibit pronounced anisotropy due to the effect of long-range shear stresses on the dewetting rim instability. We find robust power-law scaling relations over a large range of film thicknesses and a striking similarity to literature data obtained with ultrathin polymer melt layers on silicon substrates.

We study the deformation of thin liquid films subjected to impinging air-jets that are moving with respect to the substrate. The height profile and shape of the deformed liquid film is evaluated experimentally and numerically for different jet Reynolds numbers and translation speeds, for different liquids and substrate materials. Experiments and numerical results are in good agreement. On partially wetting substrates film rupture occurs. We imaged the appearance of dry spots and emergence of droplet patterns by high-speed, dual-wavelength interference microscopy. We systematically evaluated the resulting average droplet size and droplet density as a function of the experimental conditions. We show that within experimental accuracy the distribution of dry spots is dependent only on the residual film thickness and is not directly influenced by the shear stress and pressure gradients of the air-jet, nor by the speed of the substrate.

Thin liquid films on partially wetting substrates are subjected to laminar axisymmetric air-jets impinging at normal incidence. We measured the time at which film rupture occurs and dewetting commences as a function of diameter and Reynolds number of the air-jet. We developed numerical models for the air flow as well as the height evolution of the thin liquid film. The experimental results were compared with numerical simulations based on the lubrication approximation and a phenomenological expression for the disjoining pressure. We achieved quantitative agreement for the rupture times. We found that the film thickness profiles were highly sensitive to the presence of minute quantities of surface-active contaminants.

OBJECTIVE: The purpose of this study was to assess the association between self-perceived mental health status and mammography screening in Kentucky. METHODS: Using a cross-sectional design, we examined survey data from the 2002 Kentucky Behavioral Risk Factor Surveillance System (BRFSS) for women aged > or =40. Mental health status was measured by the reported number of days that mental health was not good; the number of days feeling sad, blue, or depressed; and the number of days feeling worried, tense, or anxious. The outcome was mammography within the last 2 years. Three logistic regression analyses were performed, one with each of the mental health status questions as the predictor variable. Analyses controlled for age, race, marital status, education, income, and health insurance status. RESULTS: The numbers of poor mental health days, depressed days, and anxious days were found to be significant or near-significant predictors of recent mammography. Odds ratios (ORs) comparing women reporting 30 poor mental health days, depressed days, or anxious days with similar women reporting zero days were estimated to be 1.68 (95% confidence interval [CI] 1.08-2.63), 1.49 (0.93-2.40), and 1.46 (0.96-2.23), respectively. CONCLUSIONS: Self-reported poor mental health, depression, and anxiety may be associated with nonreceipt of regular mammography screening. How mental health symptoms and self-reported poor mental health status contribute to decreased mammography screening should be explored.

RESUMO

We present a new and simple method to produce superhydrophobic surfaces with ultralow hysteresis. The method involves surface modification of SU-8 using an excimer laser treatment. The modified surface is coated with a hydrophobic plasma-polymerized hexafluoropropene layer. The advancing and receding water contact angles were measured to be approximately 165 degrees . The achieved water contact angle hysteresis was below the measurement limit. This low hysteresis can be ascribed to nanoscale debris generated during the excimer laser process.
