ABSTRACT: CD30-positive primary cutaneous lymphoproliferative disorders (CD30 + PCLPD) are a heterogeneous group of cutaneous T-cell lymphoma (CTCL) that includes lymphomatoid papulosis (LyP) and primary cutaneous anaplastic large cell lymphoma. They exist as a clinical and pathological spectrum, which display significant overlap and variability. The diagnosis is made based on correlation between clinical and histopathologic findings. LyP with 6p25.3 rearrangement subtype represents <5% of LyP cases and is defined by DUSP22-IRF4 rearrangement on 6p25.3 locus. The reported cases express the alpha/beta T-cell receptor and follow an indolent clinical behavior typical of LyP. The same rearrangement is detected in 28% of anaplastic large cell lymphoma. We hereby present an extraordinary case of CD30 + PCLPD with DUSP22-IRF4 rearrangement and novel expression of gamma/delta T-cell immunophenotype in a young patient. Although the gamma/delta T-cell immunophenotype has been described in many other T-cell lymphomas, this is the first reported association with CD30 + PCLPD with DUSP22-IRF4 rearrangement.

RESUMO

Verrucous psoriasis (VP) is a rare histologic variant of psoriasis. To date, fewer than 40 cases have been reported, with pediatric cases representing an exceedingly small fraction. Treatment options are limited, and the disease is often refractory to conventional methods. Herein, we report a case of VP in an adolescent male which previously failed biologic and systemic therapy with adalimumab and acitretin, respectively, ultimately responding with great success to ixekizumab in combination with topical corticosteroids. Palmoplantar pustulosis (PPP) is a chronic, debilitating autoimmune skin disease characterized by the presence of recurrent neutrophilic pustules on acral surfaces. Although similarities exist, PPP is currently regarded as a distinct pathologic entity from palmoplantar psoriasis.1.

RESUMO

Necrotizing fasciitis (NF) is a progressive inflammatory infection of the fascia that is often aggressive and advances insidiously. NF can be preceded by traumatic injury or surgical intervention or may occur spontaneously. Here, we describe a patient who presented with monomicrobial subacute necrotizing fasciitis due to Serratia marcescens. Dermatology was consulted at the local hospital for suspected cellulitis with no resolution using appropriate antibiotics. Our patient failed to show improvement with antibiotic treatment and experienced an above-the-knee amputation. We have also included a review of the current literature on subacute necrotizing fasciitis and S. marcescens. A case review of 25 patients resulted in rates of 52.1% for overall mortality, 72% for mortality among patients treated with only antibiotics, and 40% for mortality among patients treated with surgical debridement. There needs to be a high suspicion for necrotizing fasciitis even in patients presenting with cellulitis without systemic symptoms. If S. marcescens is isolated, more aggressive treatment may be warranted. More cases of subacute necrotizing fasciitis and S. marcescens need to be reported in the literature to better understand early diagnosis and best management practices.

Here we present the crystal structure, experimental and theoretical characterization of a Au24(SAdm)16 nanomolecule. The composition was verified by X-ray crystallography and mass spectrometry, and its optical and electronic properties were investigated via experiments and first-principles calculations. Most importantly, the focus of this work is to demonstrate how the use of bulky thiolate ligands, such as adamantanethiol, versus the commonly studied phenylethanethiolate ligands leads to a great structural flexibility, where the metal core changes its shape from five-fold to crystalline-like motifs and can adapt to the formation of Au(24±1)(SAdm)16, namely, Au23(SAdm)16, Au24(SAdm)16, and Au25(SAdm)16. The basis for the construction of a thermodynamic phase diagram of Au nanomolecules in terms of ligands and solvent features is also outlined.

RESUMO

Au137(SR)56, a nanomolecule with a precise number of metal atoms and ligands, was synthesized. The composition was confirmed by MALDI and ESI mass spectrometry using three unique ligands (-SCH2CH2Ph, -SC6H13, and -SC4H9) and nano-alloys with Ag and Pd. The electrocatalytic properties were tested for CO2 reduction.

Au30S(S-t-Bu)18 cluster, related closely to the recently isolated "green gold" compound Au30(S-t-Bu)18, has been structurally solved via single-crystal XRD and analyzed by density functional theory calculations. The molecular protecting layer shows a combination of monomeric (RS-Au-SR) and trimeric (RS-Au-SR-Au-SR-Au-SR) gold-thiolate units, bridging thiolates, and a single sulfur (sulfide) in a novel µ3-coordinating position. The chiral gold core has a geometrical component that is identical to the core of the recently reported Au28(SPh-t-Bu)20. Both enantiomers of Au30S(S-t-Bu)18 are found in the crystal unit cell. The calculated CD spectrum bears a close resemblance to that of Au28(SPh-t-Bu)20. This is the first time when two structurally characterized thiol-stabilized gold clusters are found to have such closely related metal core structures and the results may increase understanding of the formation of gold clusters when stabilized by bulky thiolates.
