RESUMO
The people who are treated in the community setting will often have multiple comorbidities. Systemic medical conditions can have a negative impact on oral health. In addition, the medications used to treat systemic conditions may also themselves cause oral symptoms. As a large proportion of patients treated by the community nursing team will be elderly, this paper will focus on common geriatric conditions that can display oral or dental symptoms. The effects of medications will be discussed and linked to oral complaints patients may express. The primary aims are to give a broad overview of the oral effects of ageing, of illness and of polypharmacy and advise on how these can be best managed by the nursing team.

Oral disease can have a significant impact on the health and wellbeing of the housebound patient. The aetiology of oral conditions such as dental caries and periodontal disease have been well investigated and there is a solid evidence base in how to best prevent their progress. The Department of Health document Delivering better oral health: an evidence-based toolkit for prevention is a valuable resource that outlines the current best preventative evidence in the form of practical advice for clinicians and patients. This article aims to distil and present this advice for the benefit of community nurses. It will identify areas of particular importance for people with additional needs, particularly the elderly and infirm. Outlining how to best tailor preventative advice and treatment for this patient group.

Oral disease is common the housebound population ( Tomson et al, 2015 ). There can be a significant impact on the wellbeing of a housebound individuals ( Locker et al, 2000 ), who tend to be elderly, with poorer systemic health ( Qiu et al, 2010 ) and barriers to accessing care ( Freeman, 1999 ). Access to dental care can be limited (Fox, 2010) and prevention of oral disease is therefore paramount. Community nurses are in an excellent position to promote good oral care to the housebound patient and to support family in carers in preventing oral disease. This initial article in a series aims to provide information on common oral conditions with the next focusing on prevention and the role of nursing team.
