RESUMO

GOAL OF THE STUDY: To make the comparative analysis of the changes of main parameters of haemodynamic during caesarean section with spinal anaesthesia (SA) in patients with uncomplicated pregnancy and pregnancy with hypertensive disorders. MATERIALS AND METHODS: The study included 103 pregnant women undergoing caesarean section under SA. The comparative analysis of the main haemodynamic parameters was performed in patients into three groups: an uncomplicated pregnancy (n = 35); pregnancy with chronic hypertension (n = 33) and pregnancy complicated by severe preeclampsia (n = 35). Heart rate (HR), mean blood pressure (BP), cardiac index (CI) and systemic vascular resistance (SVR) were measured by noninvasive bioimpedans technology. Data were recorded at 5 stages of the study. RESULTS AND DISCUSSION: The data of study demonstrated that haemodynamic stability better if SA is used during caesarean section in women with normal pregnancy. In the group of pregnant women with severe preeclampsia observed the most significant hemodynamic changes: BP and SVR decreased significantly greater at all stages of the operation, whereas the increase SI on the 3rd stage was higher in comparison with the control group. CONCLUSIONS: There were significant differences changes in the main hemodynamic parameters between groups at all stages of the study. Results of this survey suggest that hemodynamic response to anaesthesia depends on the initial state of the circulatory system.

RESUMO

The article deals with a multicenter study that demonstrates the possibility and feasibility of noninvasive ventilation in patients with skeletal trauma complicated wiith fat embolism syndrome. The authors found additional criteria for the severity of the condition of patients with trauma. Important criteria for the choose a type of ventilation (non-invasive and invasive) is the lack of consciousness, desynchronization of a patient with ventilator and the need for a specialized regimes or miorelaxation to synchronize with the respirator.

RESUMO

The chronic stress in albino rats caused by exposure to the whole-body vibration induced the significant changes in the functional-metabolic status of the blood cells. It involved the phagocytosis level and the lysosomal cationic proteins in the neutrophils, oxidative and hydrolytic processes in the neutrophils and lymphocytes. All the determined intracellular parameters revealed the differentiated response to stress as well as to the additive combined administration of the antioxidants (glycine and alpha-tocopherol acetate).

RESEARCH OBJECTIVE: To study influence of various anaesthesia methods on key central haemodynamics parameters in patients with heavy preeclampsia during surgical delivery. MATERIALS AND METHODS: Prospective randomized comparative research was carried out. 60 pregnant women were included in research with a heavy preeclampsia after Cesarean section. Patients were randomized (a method of envelopes) in 2 groups (30 patients each): group with spinal anesthesia and general anaesthesia. RESULTS: In spinal anaesthesia group there was more favorable haemodynamic profile at all investigation phases: moderate decrease in blood pressure, stroke volume increase and peripheral vascular resistance decrease were observed. General sevoflurane anaesthesia was accompanied by tachycardia, arterial hypertension, stroke volume decrease at a prenatal stage and stabilization of indicators only at the following investigation phases. pinal anaesthesia during surgery delivery in heavy preeclampsia patients is more preferable from the point of view of haemodynamic stability.
