BACKGROUND: Mechanical behavior of the skin can be evaluated by different non-invasive methods. In this study, we applied a new measurement device based on indentometry to determine the skin mechanical properties in healthy individuals and in patients with systemic sclerosis (SSc). MATERIAL AND METHODS: Three studies were performed. Study 1 included 100 healthy individuals (46 male and 54 female) divided into four age groups with mean ages of 21.5, 28.9, 51.2, and 69.3 years, respectively. Test sites were located on the center of the forehead and the middle of both volar forearms. Study 2 included 16 healthy individuals (two males and 14 females). Test sites were on both volar forearms. Measurements were made before and after the application of Vaseline and emulsion with 12% urea. Study 3 included 20 patients (one male and 19 females) with SSc and 60 age-matched healthy individuals (23 males and 37 females). Test sites were on the center of the forehead and the middle of both volar forearms. Skin stiffness was measured with skin Indentometer IDM 800 (Courage + Khazaka, Cologne, Germany) equipped with two probes with pin diameters of 3 and 5 mm, respectively. The stiffer the skin, the less deep the displacement by the indenter. The smaller the diameter, the deeper the pin will go into the skin when using the same force. In addition, the Corneometer CM 820 (Courage + Khazaka) was used to determine epidermal water content in study 2. RESULTS: Indentometric (IDM) values of healthy subjects measured with both probes were lower on the forehead compared to volar forearms. There was no significant difference between both forearms. In all age groups, the IDM values on the male forearms were lower than on the female forearms whereas there was no significant difference on the forehead. In both sex and on all test locations a significant positive correlation between age and IDM values measured with both probes was observed. There was a significant positive correlation between IDM values measured with both probes. The application of moisturizers induced significant changes in epidermal water content whereas the IDM values remained unchanged. At both the forehead and volar forearms, the IDM values in patients with SSc were significantly lower compared to the healthy control skin. CONCLUSION: The non-invasive indentometric method used can successfully distinguish the changes in normal skin mechanical properties related to age, sex, and anatomical location, as well as in patients with SSc. The method is not appropriate to study the changes related to epidermal hydration.

BACKGROUND: Hospitalized and severely ill coronavirus disease 2019 (COVID-19) patients necessitate prophylactic or therapeutic anticoagulation to minimize the risk of thrombosis at different sites. Life-threatening bleeding complications include spontaneous iliopsoas hematoma, peritoneal bleeding, and extra-abdominal manifestations such as intracranial hemorrhage. CASE SUMMARY: Bleeding in the abdominal wall results in less severe complications than seen with iliopsoas hematoma or peritoneal bleeding. In our case series of 9 patients, we present retroperitoneal and abdominal bleeding complications following anticoagulation in hospitalized COVID-19 patients with severe acute respiratory syndrome coronavirus 2 pneumonia. Contrast-enhanced computed tomography (CE-CT) is the best imaging modality for assessing hematoma secondary to anticoagulation and determines the therapeutic approach, whether interventional, surgical, or conservative management. CONCLUSION: We present the role of CE-CT for rapid and precise localization of the bleeding site and prognostic counseling. Finally, we provide a brief review of the literature.

RESUMO

We report the case of a 67-year-old woman who developed onychomadesis on 9 of her fingers 2 months after recovering from COVID-19, with subsequent full nail regrowth after 4 months. The development of onychomadesis in COVID-19 is probably related to inhibition of nail proliferation due to fever, direct viral damage, or an inflammatory process associated with endothelial damage and obliterative microangiopathy in the nail matrix area. Clinicians should be aware of nail changes and actively seek them out in patients with COVID-19.

We describe a rare case of asynchronous bilateral carcinoma of the mammary glands in a 66-year-old man. The patient was admitted to the Department of Dermatology and Venereology due to exacerbation of chronic eczema. During the examination, a 3×2-cm tumor with retraction of the mammilla was found in the right mammary gland. Mastectomy and regional lymphatic dissection were performed. Histological examination showed invasive ductal carcinoma, ER (+) 70%, PR (-), HER2 (-). Treatment included chemotherapy, radiotherapy, and hormone therapy. Three years later, the patient reported a lump in the left breast. The presence of a tumor formation was confirmed by mammography and ultrasound examination. A radical mastectomy with regional lymphatic dissection was performed. Histological examination showed invasive ductal carcinoma of the mammary gland, ER (3+) 80%, PP (2+) 60%, HER2 (+++), and Ki67 (+) 80%. Treatment with chemotherapy and radiation therapy was carried out. Five years after diagnosis of the second carcinoma, the patient is in a good general condition. Regardless of its rarity, the described case should draw doctors' attention to this pathology. Assessment of risk factors and periodic breast examination in men would allow early diagnosis, timely treatment, and better prognosis of the disease.

INTRODUCTION: Diseased skin in localized scleroderma (LS) and plaque psoriasis (PPs) is characterized by induration that can be evaluated by non-invasive bioengineering methods. In this study, we applied a new measurement device based on indentometry to determine the changes of skin mechanical properties in patients with LS and PPs. MATERIAL AND METHODS: A total of 30 sclerodermatous plaques in 12 patients with LS and 46 psoriatic plaques in 19 patients with PPs were measured with Indentometer IDM 800 (Courage + Khazaka, Cologne, Germany). The device measures the penetration depth of the probe indenter (pin) into the skin in mm. We used two probes with pin diameters 3 and 5 mm, respectively. The stiffer the skin, the less deep is the displacement by the indenter. The smaller the diameter, the deeper the pin will go into the skin when using the same force. The measurements were made on diseased skin and in adjacent normal skin served as control. In addition, the psoriatic plaques were measured before and after treatment. RESULTS: The sclerodermatous skin and the psoriatic skin showed lower Indentometer values compared to adjacent normal skin as measured with both probes. This suggests that diseased skin is stiffer than healthy skin. The values of psoriatic plaques increased after treatment applied that correlates with the clinical improvement. The Indentometric readings significantly negatively correlated with clinical scores of skin induration. There was a significant correlation between the measurements with probe 3 mm and probe 5 mm. CONCLUSION: The non-invasive method used is appropriate for objective and quantitative determination of the degree of skin induration in LS and PPs as well as for assessment of the disease evaluation and treatment efficacy.

INTRODUCTION: Finger skin thermometry is one of the most commonly used methods for evaluating the response of the digital vessels to cold stimulation. The aim of this study was to evaluate the applicability of a novel finger skin temperature device for performing cold-stimulation test (CST) in subjects with primary and secondary Raynaud's phenomenon (RP). METHODOLOGY: A total of 155 consecutive subjects were studied. They were divided into three groups: 73 patients with primary RP (8 males, 65 females, mean age 38.5 ± 16.2), 42 patients with secondary RP (4 males, 38 females, mean age 49.6 ± 13.1, connected with lupus erythematosus and systemic scleroderma), and 40 healthy controls (5 males, 35 females, mean age 38.8 ± 16.6). Standardized CST consisting of exposure of both hands to water with a temperature of 10℃ for 5 minutes was performed. Changes in skin temperature of both wrists and 2-5 fingers were measured using a novel finger temperature device (Courage & Khazaka). Measurements were made before and 5, 10, 15, 20, 25, and 30 minutes after cold stimulation. The time of recovery for baseline temperature of all fingers below 15 minutes was considered normal. RESULTS: The CST was normal in 6 (8.2%) of the patients with primary RP, in 7 (16.6%) of the patients with secondary RP, and in 28 (70%) of the healthy control subjects. The time of complete recovery of baseline temperature with respect to the first finger and for all 2-5 fingers in the three groups was as follows: 24.8 and 28.5 minutes (primary RP), 21.7 and 26.8 minutes (secondary RP), and 11.1 and 15.2 minutes (healthy subjects). Furthermore, the microcirculation was seriously disturbed (rewarming time >31 minutes of all 2-5 digits of both hands) in 54.1% (n = 79), 34.5% (n = 29) and 5% (n = 4) in the same study groups. DISCUSSION: Our results suggested that skin microcirculation is more disturbed in patients with primary RP than in patients with secondary RP. In support of this unexpected finding were the results reported by Ruaro B. et al (2019). They investigated the blood perfusion (BP) by laser speckle contrast analysis (LASCA) at different skin areas of hands and found that it was significantly lower in primary RP than in secondary RP related to systemic sclerosis. CONCLUSION: The new finger temperature device used could be considered useful for performing cold-stimulation test in patients with Raynaud's phenomenon.

BACKGROUND: Dermoscopy is a widely used technique, recommended in clinical practice guidelines worldwide for the early diagnosis of skin cancers. Intra-European disparities are reported for early detection and prognosis of skin cancers, however, no information exists about regional variation in patterns of dermoscopy use across Europe. OBJECTIVE: To evaluate the regional differences in patterns of dermoscopy use and training among European dermatologists. MATERIALS & METHODS: An online survey of European-registered dermatologists regarding dermoscopy training, practice and attitudes was established. Answers from Eastern (EE) versus Western European (WE) countries were compared and their correlation with their respective countries' gross domestic product/capita (GDPc) and total and government health expenditure/capita (THEc and GHEc) was analysed. RESULTS: We received 4,049 responses from 14 WE countries and 3,431 from 18 EE countries. A higher proportion of WE respondents reported dermoscopy use (98% vs. 77%, p<0.001) and training during residency (43% vs. 32%) or anytime (96.5% vs. 87.6%) (p<0.001) compared to EE respondents. The main obstacles in dermoscopy use were poor access to dermoscopy equipment in EE and a lack of confidence in one's skills in WE. GDPc, THEc and GHEc correlated with rate of dermoscopy use and dermoscopy training during residency (Spearman rho: 0.5-0.7, p<0.05), and inversely with availability of dermoscopy equipment. CONCLUSION: The rates and patterns of dermoscopy use vary significantly between Western and Eastern Europe, on a background of economic inequality. Regionally adapted interventions to increase access to dermoscopy equipment and training might enhance the use of this technique towards improving the early detection of skin cancers.

BACKGROUND: Intra-abdominal hypertension is known as a factor affecting cerebral haemodynamics. Sustainably elevated abdominal pressure may disturb the balance of intracranial/blood pressure ratio, eventually developing perfusion pressure to drop. AIM: The aim of this study is to investigate the influence of artificially elevated intra-abdominal pressure upon brain pial vessels condition and contractile reactivity of isolated rat arteria carotis communis and vena jugularis to norepinephrine and serotonin. MATERIALS AND METHODS: The abdominal pressure of rats anaesthetized with xylazine 10 mg/kg and ketamine 100 mg/kg was increased up to 25 mm Hg by insufflation of air through venflon cannula and maintained for period of 1 to 3 hours. Craniotomy of left parietal area was carried out by micro drill. Open scull and cranial window techniques were applied. Outer diameters of superficial pial vessels were measured by USB digital microcamera (magnification up to 400x). Contractile reactivity of smooth muscle preparations from arteria carotis communis and vena jugularis of euthanized abdominal-hypertensive (AH) rats was registered isometrically. RESULTS: Increased smooth muscle reactivity of a. carotis communis from AH rats to serotonin (10-8-10-4 mol/l) but not to norepinephrine compared to controls was registered. The changes tended to be higher in long lasting (3 hours) exposure of AH rats. Increase in outer diameter of pial vessels during maintenance of abdominal hypertension in both open scull and cranial window techniques was found. CONCLUSIONS: The increased intra-abdominal pressure causes dilatation of small superficial cerebral blood vessels and increases the smooth muscle reactivity of isolated arteria carotis communis to 5-HT.

BACKGROUND: Bier spots are asymptomatic white anemic macules with irregular shape located on the upper limbs and less often on the lower limbs and trunkus. They are observed predominantly in youg males and may be idiopathic or associated with other conditions. OBJECTIVE: We report two cases with Bier spots in which videodermatoscopy and nailfold capillaroscopy were performed. RESULTS: Patient 1 was a 34-year-old otherwise healthy male who presented with white spots on his upper limbs for 10 years. Dermoscopy revealed marked narrowing of skin capillaries within the borders of white macules whereas capillaroscopy pattern was normal. Patient 2 was a 20-year-old female with symptoms of Raynaud's phenomenon who presented with white spots on the volar aspects of her hands for 2 years. The capillaroscopy showed abnormal findings with presence of homogenous enlarged capillaries. CONCLUSION: Bier spots do not require any treatment. The associated conditions found should be monitored.

BACKGROUND: Psoriasis vulgaris (PsV) is a chronic skin condition that has a major impact on health-related quality of life (HRQOL). AIM: To determine the individual burden of PsV on HRQOL using willingness to pay (WTP) instrument. MATERIALS AND METHODS: Fifty-one consecutive PsV patients were asked to evaluate their overall health and psoriasis affected health by visual analogue scale (VAS), and interviewed on 8 domains (physical, emotional, sleep, work, social, self-care, intimacy, and concentration) of HRQOL and WTP for a hypothetical cure in each domain. Two additional questions proposing 6 alternatives for therapy were also asked. The analysis is performed with descriptive and frequency statistics, Mann-Whitney and Kruskal-Wallis tests. RESULTS: The domains ranked highly were: physical comfort (90%), social comfort (77%), emotional health (75%) and work (53%). The following tendencies concerning WTP for top four impacted domains were found: the median WTP were the highest in the top impacted domains; the younger patients were willing to pay more than the older ones; the highest median WTP amounts appear in the lowest income group; the highest median WTP is associated with smaller psoriasis affected health VAS scores. The largest proportion and number of patients (37.3%, n=19) stated preferences for the systemic therapy. The second preferred choice was the thalassotherapy (29.4%, n=15). CONCLUSIONS: The utility and reliability of the instrument based on the assessment of WTP stated preferences for 8 domains of HRQOL for evaluation the individual burden of psoriasis were strongly supported.

Kindler syndrome (KS), the fourth major type of hereditary epidermolysis bullosa (HEB), is a rare, autosomal recessive disorder characterized by skin fragility and blistering at birth followed by development of marked photosensitivity and progressive poikilodermatous skin changes in later years. We reported here the case of a 54-year-old woman, who fulfills the diagnostic criteria of KS type of HEB, putting accent on the nailfold capillaroscopic changes. Using videocapillaroscopy we observed pronounced alterations in finger nail capillaries including reduction in capillary density, features of neoangiogenesis (architectural derangement, elongated loops, extremely tortuous, bushy or branching capillaries, thin, branching and interconnected capillaries), enlarged and giant capillaries. We consider the changes observed as an adaptive mechanism that compensate the loss of capillaries due to chronic periungual trauma. Further studies with larger number of patients are needed to confirm the significance of capillaroscopy findings for patients with HEB.

We present a 60-year-old male patient who, three months after a holiday in Southern Greece, found a small 'pimple' on his back, which gradually got as big as a small walnut, the central part becoming ulcerated and scabby. Dermatological examination found an erythematous-to-livid nodular lesion on the right shoulder; it was 16 mm in diameter with central ulceration, covered with brownish crust which discharged pus-like secretion upon pressure. Microscope examination of Romanowsky-Giemsa stained lesion material detected amastigote forms of Leishmania tropica. The culture investigation and serological tests for leishmaniasis were negative. Dermoscopy of the lesion found the following features: erythema, hyperkeratosis, central ulceration covered with brownish crust, "yellow tears-like" structures and "white starburst-like" patterns, and various vascular structures (including dotted vessels, comma-shaped vessels, hairpin- and glomerular-like vessels). The patient was diagnosed with cutaneous leishmaniasis and underwent four cryotherapy sessions every other week with excellent therapeutic results - complete resolution of infiltrate with subsequent gentle hypopigmented scarring. In conclusion, dermoscopy is an easily accessible non-invasive method which can be useful for the diagnosis of cutaneous leishmaniasis. Cryotherapy is the treatment of choice for single skin lesions.

In this study the complex interrelationship between physical factors, job stress, lifestyle and genetic factors on symptoms of work-related musculoskeletal disorders of the upper limbs is demonstrated by a case report and discussion of the literature. A 58 year old woman with long lasting complaints of the upper limbs with increasing intensity and duration, generalisation, combined with skin thickness, Raynaud's phenomenon, joint disorders, arterial and pulmonary hypertension, metabolic lipid dysfunctions is presented. Occupational history proves continuous duration of service at a job with occupational physical static load with numerous repetitive monotonous systematic motions of fingers and hands as a weaver of Persian rugs followed by work at an automated loom and variable labour activities. Though the complaints dated since the time she was a manual weaver, the manifestations of generalized joint degenerative changes, system sclerosis with Raynaud's phenomenon with similar upper extremities signs and symptoms discount upper limbs musculoskeletal disorder as caused only or mainly by occupational risk factors. The main principles and criteria for occupational diagnosis of musculoskeletal upper limb disorders and legislative requirements for their reglamentation are discussed.

Fungal infections of the skin are a common condition, usually easy to diagnose and treat. When the infection is clinically mimicking another cutaneous disorder or when the clinical presentation is modified by the use of inappropriate treatment, it is referred to as tinea atypica or tinea incognito.We report a series of nine cases of patients with tinea atypica, imitating and diagnosed initially as different skin diseases. Two patients were defined as pyoderma in the facial and pubic regions (caused respectively by Trichophyton mentagrophytes var. mentagrophytes and Microsporum canis) and one as herpes zoster ophthalmicus (caused by Trichophyton rubrum). Six additional patients were initially misdiagnosed: (1) Plaque-like formation of the skin misdiagnosed as an impetiginized eczema (with isolated agent Trichophyton verrucosum). (2) A rare form of skin infection of the hand caused by T. rubrum, imitating clinically cutaneous infection with tuberculum mulgentium. (3) Rosacea-like dermatitis with an isolated agent Fusarium. (4) A patient with the typical clinical symptoms of seborrheic dermatitis of the face (and with isolated T. rubrum as a causative agent). (5) Another patient presented with a widespread folliculitis by Trichophyton mentagrophytes. (6) In a patient with bullous pemphigoid and immunosuppression pemphigoid-like eruptions were caused by Malassezia pachydermatis and T. rubrum. The diagnosis in the presented cases was based on direct microscopic examination with KOH and a culture on Sabouraud agar.After the diagnosis of tinea, treatment with topical and systemic antifungal agents was administrated, followed by complete clinical remissions in all cases.The clinical manifestations of tinea atypica can mimic a large number of other dermatoses, which often leads to misdiagnosing, and as a consequence--to serious difficulties in the management of clinical symptoms and in offering appropriate therapy.

INTRODUCTION: Diagnosis of systemic sclerosis (SSc) at very early stage could allow starting an appropriate therapy and improving the patient outcome. Skin involvement is often the first non-Raynaud's phenomenon (RP) symptom. Its uncovering may play an important role for the initial diagnosis. OBJECTIVE: To introduce a simplified method for non-invasive evaluation of skin mechanical properties in patients with clinically evident or suspected SSc. MATERIAL AND METHODS: A total of 94 patients and 162 healthy subjects were studied. According to clinical and nailfold videocapillaroscopy findings the patients were divided into four groups: 20 with edematous phase of SSc (group 1), 28 with indurative phase of SSc (group 2), 26 with suspected secondary RP (group 3), and 20 with primary RP (group 4). Mechanical properties of the volar forearm skin were evaluated using a non-invasive suction device (Cutometer) equipped with 2-mm diameter probe. The skin mechanical parameters analyzed were distensibility (Uf), elasticity (Ua/Uf) and viscoelasticity (Uv/Ue). RESULTS: Skin distensibility was reduced and skin viscoelasticity increased in group 1-3 compared to age matched healthy controls. There were no significant changes in skin elasticity. Mechanical parameters in group 4 were normal. Comparison of individual patient's values with population 95% confidence intervals of the mean showed increased skin viscoelasticity in group 1 (100%), group 2 (93%), and group 3 (81%), whereas the incidence in group 4 was 10%. CONCLUSIONS: Noninvasive method applied is appropriate for objective and quantitative evaluation of sclerodermatous skin. In combination with nailfold videocapillaroscopy it could be predictive in pre-scleroderma patients. The increased skin viscoelasticity parameter could be proposed as the possible new fifth sign for the very early diagnosis of SSc.
