Ketone ingestion can alter metabolism but effects on exercise performance are unclear, particularly with regard to the impact on intermittent-intensity exercise and team-sport performance. Nine professional male rugby union players each completed two trials in a double-blind, randomized, crossover design. Participants ingested either 90 ± 9 g carbohydrate (CHO; 9% solution) or an energy matched solution containing 20 ± 2 g CHO (3% solution) and 590 mg/kg body mass ß-hydroxybutyrate monoester (CHO + BHB-ME) before and during a simulated rugby union-specific match-play protocol, including repeated high-intensity, sprint and power-based performance tests. Mean time to complete the sustained high-intensity performance tests was reduced by 0.33 ± 0.41 s (2.1%) with CHO + BHB-ME (15.53 ± 0.52 s) compared with CHO (15.86 ± 0.80 s) placebo (p = .04). Mean time to complete the sprint and power-based performance tests were not different between trials. CHO + BHB-ME resulted in blood BHB concentrations that remained >2 mmol/L during exercise (p < .001). Serum lactate and glycerol concentrations were lower after CHO + BHB-ME than CHO (p < .05). Coingestion of a BHB-ME with CHO can alter fuel metabolism (attenuate circulating lactate and glycerol concentrations) and may improve high-intensity running performance during a simulated rugby match-play protocol, without improving shorter duration sprint and power-based efforts.

In sport, testosterone has been positioned as a substrate for motivation with both directional and time dependencies. However, evidence is scarce when considering the complexities of competitive sport and no work has explicitly modeled these dependencies. To address these gaps, we investigated the bidirectional and time-dependent interrelationships between testosterone and training motivation in an elite rugby environment. Thirty-six male athletes were monitored across training weeks before and after eight international rugby matches. Pre-breakfast measures of salivary testosterone and training motivation (1-10 rating) were taken on training, competition, and recovery days (up to 40 tests). Using a continuous-time (CT) model, within-person estimates of autoregressive effects (persistence) and cross-lagged effects (relationships) were derived. A stronger, more persistent temporal association was identified for testosterone than for motivation. Cross-lagged effects verified that training motivation was positively related to testosterone at latter time points (p < 0.001). Discrete-time analyses revealed a non-linear association; increasing in strength from a zero-time lag to peak after 2.83 days (standardized effect = 0.25), before dissipation over longer lagged intervals. The testosterone relationship with ensuing training motivation was also positive, but non-significant. Match effects also appeared (p < 0.001) with a predicted decline in training motivation, but a rise in testosterone, at match onset. In summary, a positive association emerged between within-person fluctuations in self-appraised motivation to train and testosterone concentration in an elite rugby environment. The lagged, non-linear nature of this relationship and match predictions on both outcomes support, and extend, theoretical models linking testosterone and competitive behaviors.

OBJECTIVES: Research has linked physiological (e.g., hormonal, affective, fatigue) outcomes to performance indicators in rugby competition, but no work has integrated and contextualised these factors within a test-match environment. We addressed this gap by monitoring 29 athletes from a training squad across eight international rugby matches. DESIGN: Longitudinal observational study. METHODS: Pre-match (8-9am) measures of salivary testosterone and cortisol concentrations, sleep duration, pulse rate, muscle soreness, stress, mood, and motivation were taken. Contextual factors were playing time, internal training load (ITL), test-match experience, opponent ranking, and crowd size. Performance was indexed by coach and player ratings of performance (CRP, PRP) and quantitative metrics; offloads, turnovers, runs with ball in hand (RWB), tackles, passes, and defenders beaten (DFB). RESULTS: Morning cortisol, sleep and mood were positively related to CRP and PRP (standardised coefficient estimates from 0.17 to 0.22). Cortisol, sleep, stress, mood and motivation were associated with one (or more) of turnovers, RWB, tackles, passes and DFB (incidence rate ratio [IRR] from 0.74 to 1.40). Playing time was positively related to all quantitative performance indicators (IRR from 1.01 to 1.04) with ITL, opponent ranking, and crowd size predicting selected outputs (IRR from 0.89 to 1.15). The explanatory models varied (conditional R2=0.15-0.83) but were generally stronger with both physiological and contextual inputs. CONCLUSIONS: Multiple physiological and contextual factors appear to contribute to player performance in international rugby competition. Measurement of these factors may guide training and management practices, a potential practical consequence but also advancing understanding from marker to causal link.

The use of physical tests to profile physical capabilities, and provide training direction to athletes is common practice. Likewise, in professional team sports, notational analysis codes the key contributions of each player during competition. Limited studies have however investigated relationships between physical capabilities and key performance indicators (KPIs) of rugby union match-play. Elite professional players, categorised as forwards (n = 15) or backs (n = 14), from an international rugby union squad (n = 29) undertook assessments of isometric mid-thigh pull (IMTP), bilateral and unilateral countermovement jumps (CMJ) and drop jumps (DJ; from 40 and 20 cm, respectively), and assessment of acceleration (10 m), a 5 m weighted sled drive, and a Yo-Yo intermittent recovery test level 1 (Yo-Yo IRTL1). Game statistics of the same players from 92 matches (~23 matches per player) during the 2014-15 season were analysed for effort and performance-based metrics. For forwards, Yo-Yo IRTL1 correlated significantly with; number of tackles made (r = 0.717), first three players at a ruck in both attack (r = 0.568) and defence (r = 0.581), number of effective rucks (r = 0.630), total possessions (r = 0.522), passes made (r = 0.651), percentage of carries over the gainline (r = 0.610), effective ruck success (r = 0.600), tackle success (r = 0.540), and the number of turnovers made (r = 0.518). Drop jump performance in forwards was associated with; the number of clean breaks (r = 0.558), dominant collisions (r = 0.589), and offloads (r = 0.594). For backs, the sled-drive test correlated with; number of carries (r = -0.751), first three players at an attacking ruck (r = -0.613), effective attacking rucks (r = -0.584), number of dominant collisions (r = -0.792) and offloads (r = -0.814). Likewise, for backs, IMTP peak force was related to; the number of possessions (r = 0.793), passes made (r = 0.792), effective attacking ruck percentage (r = 0.628), and the number of offloads (r = 0.621) whilst relative peak force correlated with; the percentage of carries over the gainline (r = 0.533), percent tackle success (r = 0.603) and effective attacking ruck percentage (r = 0.584). Regression analyses highlighted that only a small number of variables (i.e., carries, tackles, attacking and defensive first three at ruck) returned practically achievable changes (<20%) in physical qualities. In spite of this, and while leaving scope identification of further physical and/or performance predictors, greater strength, power and intermittent running performance were positively related to match-derived KPIs during competition. This may provide a basis for better integrating the strategies used by physical and technical performance-focused coaching staff to improve key performance indicators, and thus match performance, of rugby union players.

The assessment of competitive movement demands in team sports has traditionally relied upon global positioning system (GPS) analyses presented as fixed-time epochs (e.g., 5-40 min). More recently, presenting game data as a rolling average has become prevalent due to concerns over a loss of sampling resolution associated with the windowing of data over fixed periods. Accordingly, this study compared rolling average (ROLL) and fixed-time (FIXED) epochs for quantifying the peak movement demands of international rugby union match-play as a function of playing position. Elite players from three different squads (n = 119) were monitored using 10 Hz GPS during 36 matches played in the 2014-2017 seasons. Players categorised broadly as forwards and backs, and then by positional sub-group (FR: front row, SR: second row, BR: back row, HB: half back, MF: midfield, B3: back three) were monitored during match-play for peak values of high-speed running (>5 m·s-1; HSR) and relative distance covered (m·min-1) over 60-300 s using two types of sample-epoch (ROLL, FIXED). Irrespective of the method used, as the epoch length increased, values for the intensity of running actions decreased (e.g., For the backs using the ROLL method, distance covered decreased from 177.4 ± 20.6 m·min-1 in the 60 s epoch to 107.5 ± 13.3 m·min-1 for the 300 s epoch). For the team as a whole, and irrespective of position, estimates of fixed effects indicated significant between-method differences across all time-points for both relative distance covered and HSR. Movement demands were underestimated consistently by FIXED versus ROLL with differences being most pronounced using 60 s epochs (95% CI HSR: -6.05 to -4.70 m·min-1, 95% CI distance: -18.45 to -16.43 m·min-1). For all HSR time epochs except one, all backs groups increased more (p < 0.01) from FIXED to ROLL than the forward groups. Linear mixed modelling of ROLL data highlighted that for HSR (except 60 s epoch), SR was the only group not significantly different to FR. For relative distance covered all other position groups were greater than the FR (p < 0.05). The FIXED method underestimated both relative distance (~11%) and HSR values (up to ~20%) compared to the ROLL method. These differences were exaggerated for the HSR variable in the backs position who covered the greatest HSR distance; highlighting important consideration for those implementing the FIXED method of analysis. The data provides coaches with a worst-case scenario reference on the running demands required for periods of 60-300 s in length. This information offers novel insight into game demands and can be used to inform the design of training games to increase specificity of preparation for the most demanding phases of matches.

OBJECTIVES: To investigate the effect of oral contraceptive (OC) use on salivary testosterone (sal-T) concentrations and performance-related statistics in international field hockey matches. DESIGN: A cohort observational study with repeated measures. METHODS: Twenty-three elite female athletes were monitored across four international field hockey matches over a nine-day period. Salivary T was assessed 45min before each match and several match performance statistics were collated; load (i.e. ratings of perceived exertion×playing time), video-derived positive actions (PA) and negative actions (NA), plus coach and player ratings of performance. The sal-T and match performance profiles of OC (n=7) and Non-OC (n=16) players were compared and predictive relationships tested. RESULTS: Pre-match sal-T concentrations were 35% higher in the Non-OC than the OC group (p=0.001), representing a large effect size (ES) difference of 0.96. The OC and Non-OC groups did not differ on any performance statistic (p≥0.348) with ES differences from -0.22 to 0.11. Salivary T was positively related to the number of PA during match play (p=0.017). Additional linkage between sal-T and NA emerged, but with opposing slopes (p=0.008) in the OC (B=-1.783, p=0.030) and Non-OC (B=0.692, p=0.127) groups. CONCLUSIONS: OC usage by elite women athletes was accompanied by lower sal-T concentrations, but the performance outputs of the OC and Non-OC groups were similar. This suggests that the T differences had no impact on match performance. On an individual (population-averaged) level, sal-T was associated with PA and NA during these matches, though the response curves predicting NA differed for OC and Non-OC athletes.

OBJECTIVES: Evidence suggests that stress-induced changes in testosterone and cortisol are related to future competitive behaviours and team-sport outcomes. Therefore, we examined whether salivary testosterone and cortisol reactivity to a mid-week stress test can discriminate a match outcome in international rugby union competition. DESIGN: Single group, quasi-experimental design with repeated measures. METHOD: Thirty-three male rugby players completed a standardised stress test three or four days before seven international matches. Stress testing involved seven minutes of shuttle runs (2×20m), dispersed across one-minute stages with increasing speeds. Salivary testosterone and cortisol were measured in the morning, along with delta changes from morning to pre-test (Morn-PreΔ) and pre-test to post-test (Pre-PostΔ). Data were compared across wins (n=3) and losses (n=4). RESULTS: The Morn-PreΔ in cortisol increased before winning and decreased prior to losing (p<0.001), with a large effect size difference (d=1.6, 90% CI 1.3-1.9). Testosterone decreased significantly across the same period, irrespective of the match outcome. The Morn-PreΔ in testosterone and cortisol, plus the Pre-PostΔ in testosterone, all predicted a match outcome (p≤0.01). The final model showed good diagnostic accuracy (72%) with cortisol as the main contributor. CONCLUSIONS: The salivary testosterone and cortisol responses to mid-week testing showed an ability to discriminate a rugby match outcome over a limited number of games. The Morn-PreΔ in cortisol was the strongest diagnostic biomarker. This model may provide a unique format to assess team readiness or recovery between competitions, especially with the emergence of rapid hormonal testing.

This study compared the movement demands of elite international Under-20 age grade (U20s) and senior international rugby union players during competitive tournament match play. Forty elite professional players from an U20 and 27 elite professional senior players from international performance squads were monitored using 10Hz global positioning systems (GPS) during 15 (U20s) and 8 (senior) international tournament matches during the 2014 and 2015 seasons. Data on distances, velocities, accelerations, decelerations, high metabolic load (HML) distance and efforts, and number of sprints were derived. Data files from players who played over 60 min (n = 258) were separated firstly into Forwards and Backs, and more specifically into six positional groups; FR-Front Row (prop & hooker), SR-Second Row, BR-Back Row (Flankers & No.8), HB-Half Backs (scrum half & outside half), MF-Midfield (centres), B3 -Back Three (wings & full back) for match analysis. Linear mixed models revealed significant differences between U20 and senior teams in both the forwards and backs. In the forwards the seniors covered greater HML distance (736.4 ± 280.3 vs 701.3 ± 198.7m, p = 0.01) and severe decelerations (2.38 ± 2.2 vs 2.28 ± 1.65, p = 0.05) compared to the U20s, but performed less relative HSR (3.1 ± 1.6 vs 3.2 ± 1.5, p < 0.01), moderate (19.4 ± 10.5 vs 23.6 ± 10.5, p = 0.01) and high accelerations (2.2 ± 1.9 vs 4.3 ± 2.7, p < 0.01) and sprint•min-1 (0.11 ± 0.06 vs 0.11 ± 0.05, p < 0.01). Senior backs covered a greater relative distance (73.3 ± 8.1 vs 69.1 ± 7.6 m•min-1, p < 0.01), greater High Metabolic Load (HML) distance (1138.0 ± 233.5 vs 1060.4 ± 218.1m, p < 0.01), HML efforts (112.7 ± 22.2 vs 98.8 ± 21.7, p < 0.01) and heavy decelerations (9.9 ± 4.3 vs 9.5 ± 4.4, p = 0.04) than the U20s backs. However, the U20s backs performed more relative HSR (7.3 ± 2.1 vs 7.2 ± 2.1, p <0.01) and sprint•min-1 (0.26 ± 0.07 vs 0.25 ± 0.07, p < 0.01). Further investigation highlighted differences between the 6 positional groups of the teams. The positional groups that differed the most on the variables measured were the FR and MF groups, with the U20s FR having higher outputs on HSR, moderate & high accelerations, moderate, high & severe decelerations, HML distance, HML efforts, and sprints•min-1. For the MF group the senior players produced greater values for relative distance covered, HSR, moderate decelerations, HML distance and sprint•min-1. The BR position group was most similar with the only differences seen on heavy accelerations (U20s higher) and moderate decelerations (seniors higher). Findings demonstrate that U20s internationals appear to be an adequate 'stepping stone' for preparing players for movement characteristics found senior International rugby, however, the current study highlight for the first time that certain positional groups may require more time to be able to match the movement demands required at a higher playing level than others. Conditioning staff must also bear in mind that the U20s players whilst maintaining or improving match movement capabilities may require to gain substantial mass in some positions to match their senior counterparts.

Ketosis, the metabolic response to energy crisis, is a mechanism to sustain life by altering oxidative fuel selection. Often overlooked for its metabolic potential, ketosis is poorly understood outside of starvation or diabetic crisis. Thus, we studied the biochemical advantages of ketosis in humans using a ketone ester-based form of nutrition without the unwanted milieu of endogenous ketone body production by caloric or carbohydrate restriction. In five separate studies of 39 high-performance athletes, we show how this unique metabolic state improves physical endurance by altering fuel competition for oxidative respiration. Ketosis decreased muscle glycolysis and plasma lactate concentrations, while providing an alternative substrate for oxidative phosphorylation. Ketosis increased intramuscular triacylglycerol oxidation during exercise, even in the presence of normal muscle glycogen, co-ingested carbohydrate and elevated insulin. These findings may hold clues to greater human potential and a better understanding of fuel metabolism in health and disease.

The purpose of this study was to quantify movement demands of elite international age grade (U20) rugby union players during competitive tournament match play. Forty elite professional players from an U20 international performance squad were monitored using 10 Hz global positioning systems (GPS) during 15 international tournament matches during the 2014/15 and 2015/16 seasons. Data on distances, velocities, accelerations, decelerations, high metabolic load (HML) distance and efforts, and number of sprints were derived. Data files from players who played over 60 min (n = 161) were separated firstly into Forwards and Backs, and more specifically into six positional groups; FR--Front Row (prop & hooker), SR--Second Row, BR--Back Row (Flankers & No.8), HB--Half Backs (scrum half & outside half), MF--Midfield (centres), B3--Back Three (wings & full back) for match analysis. Analysis revealed significant differences between forwards and backs positions. Backs scored higher on all variables measured with the exception of number of moderate accelerations, decelerations (no difference). The centres covered the greatest total distance with the front row covering the least (6.51 ± 0.71 vs 4.97 ± 0.75 km, p < 0.001). The front row also covered the least high speed running (HSR) distance compared to the back three (211.6 ± 112.7 vs 728.4 ± 150.2 m, p < 0.001) who covered the most HSR distance, affirming that backs cover greater distances but forwards have greater contact loads. These findings highlight for the first time differences in the movement characteristics of elite age grade rugby union players specific to positional roles.

OBJECTIVES: Many International Rugby Board (IRB) sevens competitions require that two tournament stages are played over consecutive weekends, but the impact this has on player physical performance and recovery is lacking. We examined the influence of two consecutive tournaments on neuromuscular function (NMF) and muscle damage in rugby sevens players. DESIGN: Ten elite international rugby sevens players completed this observational study over 2 tournaments, separated by 5 days, during the IRB sevens series. METHODS: On the morning of day 1 and 2, of both tournament 1 (T1) and 2 (T2), players performed countermovement jumps (CMJ; jump height [JH]) and capillary blood samples (creatine kinase [CK]) were collected. After the last match of each day, further capillary samples were collected. Additional, CMJ were performed 12 and 60 h post-T1. RESULTS: Player JH decreased from day 1 to day 2 during T1 (mean ± SD; -6.0 ± 5.4%; P=0.016), was reduced at 12 (-26.1 ± 5.0%; P<0.001) and 60 h post-T1 (-7.1 ± 4.8%; P=0.003) and remained lower, at am day 1 of T2 (-8.0 ± 6.0%; P=0.007), when compared with day 1 of T1. Player JH was lower on day 1 and 2 of T2, compared with T1 (P<0.05). CK concentrations were greater than baseline at all time points during each tournament (P<0.001); no between tournament differences in CK responses existed (P=0.302). CONCLUSIONS: A single sevens tournament reduces NMF such that players are not fully recovered by the start of the second competition stage, however CK returns to baseline in-between and shows the same pattern across two consecutive tournaments.

Measurement of steroid hormones in saliva is increasingly common in elite sport settings. However, this environment may enforce handling and storage practices that introduce error in measurement of hormone concentrations. We assessed the influence of storage temperature and duration on reproducibility of salivary steroid levels. Nine healthy adults provided morning and afternoon saliva samples on two separate occasions. Each sample was divided into identical saliva aliquots which were stored long-term (i.e. 28 and 84 days) at - 80°C or - 20°C (testing day 1), and short-term (i.e. 1, 3, 7 and 14 days) at 4°C or 20°C (testing day 2). Samples were analyzed for cortisol, testosterone and estradiol using ELISA. In non-freezer conditions, there was a decrease from baseline to 7 days in testosterone (- 26 ± 15%) and estradiol (- 58 ± 17%) but not cortisol concentrations (p < 0.001). This decrease was larger in samples stored at room temperature than in the refrigerator (p ≤ 0.01). There were small but significant changes in measured concentrations of all hormones after 28 and/or 84 days of storage in freezer conditions (p ≤ 0.01), but these were generally within 12% of baseline concentrations, and may be partly explained by inter-assay variability. Whole saliva samples to be analyzed for cortisol, testosterone and estradiol should be frozen at - 20°C or below within 24 h of collection, and analyzed within 28 days. Storage of samples for measurement of testosterone and estradiol at temperatures above - 20°C can introduce large error variance to measured concentrations.

Dihydrotestosterone (DHT) exerts both functional and signaling effects extending beyond the effects of testosterone in rodent skeletal muscle. As a primer for investigating the role of DHT in human skeletal muscle function, this study aimed to determine whether circulating DHT is acutely elevated in men following a bout of repeat sprint exercise and to establish the importance of training status and sprint performance to this response. Fourteen healthy active young men (Vo2max 61.0 ± 8.1 ml·kg body mass(-1)·min(-1)) performed a bout of repeat sprint cycle exercise at a target workload based on an incremental work-rate maximum (10 × 30 s at 150% Wmax with 90-s recovery). Venous blood samples were collected preexercise and 5 and 60 min after exercise. Five minutes after exercise, there were significant elevations in total testosterone (TT; P < 0.001), free testosterone (FT; P < 0.001), and DHT (P = 0.004), which returned to baseline after 1 h. Changes in DHT with exercise (5 min postexercise - preexercise) correlated significantly with changes in TT (r = 0.870; P < 0.001) and FT (r = 0.914; P < 0.001). Sprinting cadence correlated with changes in FT (r = 0.697; P = 0.006), DHT (r = 0.625; P = 0.017), and TT (r = 0.603; P = 0.022), and habitual training volume correlated with the change in TT (r = 0.569, P = 0.034). In conclusion, our data demonstrate that DHT is acutely elevated following sprint cycle exercise and that this response is influenced by cycling cadence. The importance of DHT in the context of exercise training and sports performance remains to be determined.

UNLABELLED: Rugby preseason training involves high-volume strength and conditioning training, necessitating effective management of the recovery-stress state to avoid overtraining and maximize adaptive gains. PURPOSE: Compression garments and an electrostimulation device have been proposed to improve recovery by increasing venous blood flow. These devices were assessed using salivary testosterone and cortisol, plasma creatine kinase, and player questionnaires to determine sleep quality, energy level, mood, and enthusiasm. METHODS: Twenty-five professional rugby players were assigned to 1 of 2 treatments (compression garment or a concurrent combination of electrostimulation and compression) in a crossover design over 2 × 2-wk training blocks. RESULTS: Substantial benefits were observed in self-assessed energy levels (effect size [ES] 0.86), and enthusiasm (ES 0.80) as a result of the combined treatment when compared with compression-garment use. The combination treatment had no discernable effect on salivary hormones, with no treatment effect observed. The electrostimulation device did tend to accelerate the return of creatine kinase to baseline levels after 2 preseason rugby games when compared with the compression-garment intervention (ES 0.61; P = .08). CONCLUSIONS: Electrostimulation elicited psychometric and physiological benefits reflective of an improved recovery-stress state in professional male rugby players when combined with a lower-body compression garment.

PURPOSE: To investigate how different warm-ups influenced subsequent sled-pull sprint performance in Olympic-level bob-skeleton athletes as part of their preparation for the 2010 Winter Olympics. METHODS: Three female and 3 male athletes performed 5 different randomized warm-ups of differing intensities, durations, and timing relative to subsequent testing, each 2 days apart, all repeated twice. After warm-ups, testing on a sled-pull sprint over 20 m, 3 repeats 3 min apart, took place. RESULTS: Performance testing showed improvement (P < .001, ES > 1.2) with both increasing intensity of warm-up and closeness of completion to testing, with 20-m sled sprinting being 0.1-0.25 s faster in higher-intensity protocols performed near testing In addition, supplementing the warm-ups by wearing of a light survival coat resulted in further performance improvement (P = .000, ES 1.8). CONCLUSIONS: Changing timing and intensity of warm-up and using an ancillary passive heat-retention device improved sprint performance in Olympic-level bob-skeleton athletes. Subsequent adoption of these on the competitive circuit was associated with a seasonal improvement in push times and was ultimately implemented in the 2010 Winter Olympics.

Repeated cycles of vascular occlusion followed by reperfusion initiate a protective mechanism that acts to mitigate future cell injury. Such ischemic episodes are known to improve vasodilation, oxygen utilization, muscle function, and have been demonstrated to enhance exercise performance. Thus, the use of occlusion cuffs represents a novel intervention that may improve subsequent exercise performance. Fourteen participants performed an exercise protocol that involved lower-body strength and power tests followed by repeated sprints. Occlusion cuffs were then applied unilaterally (2 × 3-min per leg) with a pressure of either 220 (intervention) or 15 mm Hg (control). Participants immediately repeated the exercise protocol, and then again 24 h later. The intervention elicited delayed beneficial effects (24 h post-intervention) in the countermovement jump test with concentric (effect size (ES) = 0.36) and eccentric (ES = 0.26) velocity recovering more rapidly compared with the control. There were also small beneficial effects on 10- and 40-m sprint times. In the squat jump test there were delayed beneficial effects of occlusion on eccentric power (ES = 1.38), acceleration (ES = 1.24), and an immediate positive effect on jump height (ES = 0.61). Thus, specific beneficial effects on recovery of power production and sprint performance were observed both immediately and 24 h after intermittent unilateral occlusion was applied to each leg.

INTRODUCTION: This study aimed to determine whether caffeine ingestion would increase the workload voluntarily chosen by athletes in a limited-sleep state. METHODS: In a double-blind, crossover study, 16 professional rugby players ingested either a placebo or 4 mg/kg caffeine 1 hr before exercise. Athletes classified themselves into nondeprived (8 hr+) or sleep-deprived states (6 hr or less). Exercise comprised 4 sets of bench press, squats, and bent rows at 85% 1-repetition maximum. Athletes were asked to perform as many repetitions on each set as possible without failure. Saliva was collected before administration of placebo or caffeine and again before and immediately after exercise and assayed for testosterone and cortisol. RESULTS: Sleep deprivation produced a very large decrease in total load (p = 1.98 × 10(-7)). Caffeine ingestion in the nondeprived state resulted in a moderate increase in total load, with a larger effect in the sleep-deprived state, resulting in total load similar to those observed in the nondeprived placebo condition. Eight of the 16 athletes were identified as caffeine responders. Baseline testosterone was higher (p < .05) and cortisol trended lower in non-sleep-deprived athletes. Changes in hormones from predose to preexercise correlated to individual workload responses to caffeine. Testosterone response to exercise increased with caffeine compared with placebo, as did cortisol response. CONCLUSIONS: Caffeine increased voluntary workload in professional athletes, even more so under conditions of self-reported limited sleep. Caffeine may prove worthwhile when athletes are tired, especially in those identified as responders.

Jet lag has potentially serious deleterious effects on performance in athletes following transmeridian travel, where time zones are crossed eastwards or westwards; as such, travel causes specific effects related to desynchronization of the athlete's internal body clock or circadian clock. Athletes are particularly sensitive to the effects of jet lag, as many intrinsic aspects of sporting performance show a circadian rhythm, and optimum competitive results require all aspects of the athlete's mind and body to be working in tandem at their peak efficiency. International competition often requires transmeridian travel, and competition timings cannot be adjusted to suit individual athletes. It is therefore in the interest of the individual athlete and team to understand the effects of jet lag and the potential adaptation strategies that can be adopted. In this review, we describe the underlying genetic and physiological mechanisms controlling the circadian clock and its inherent ability to adapt to external conditions on a daily basis. We then examine the fundamentals of the various adaptation stimuli, such as light, chronobiotics (e.g. melatonin), exercise, and diet and meal timing, with particular emphasis on their suitability as strategies for competing athletes on the international circuit. These stimuli can be artificially manipulated to produce phase shifts in the circadian rhythm to promote adaptation in the optimum direction, but care must be taken to apply them at the correct time and dose, as the effects produced on the circadian rhythm follow a phase-response curve, with pronounced shifts in direction at different times. Light is the strongest realigning stimulus and careful timing of light exposure and avoidance can promote adjustment. Chronobiotics such as melatonin can also be used to realign the circadian clock but, as well as timing and dosage issues, there are also concerns as to its legal status in different countries and with the World Anti-Doping Agency. Experimental data concerning the effects of food intake and exercise timing on jet lag is limited to date in humans, and more research is required before firm guidelines can be stated. All these stimuli can also be used in pre-flight adaptation strategies to promote adjustment in the required direction, and implementation of these is described. In addition, the effects of individual variability at the behavioural and genetic levels are also discussed, along with the current limitations in assessment of these factors, and we then put forward three case studies, as examples of practical applications of these strategies, focusing on adaptations to travel involving competition in the Rugby Sevens World Cup and the 2016 Summer Olympics in Rio de Janeiro, Brazil. Finally, we provide a list of practice points for optimal adaptation of athletes to jet lag.

The objective of this study was to determine if salivary free testosterone can predict an athlete's performance during back squats and sprints over time and the influence baseline strength on this relationship. Ten weight-trained male athletes were divided into 2 groups based on their 1 repetition maximum (1RM) squats, good squatters (1RM > 2.0 × body weight, n = 5) and average squatters (1RM < 1.9 × body weight, n = 5). The good squatters were stronger than the average squatters (p < 0.05). Each subject was assessed for squat 1RM and 10-m sprint times on 10 separate occasions over a 40-day period. A saliva sample was collected before testing and assayed for free testosterone and cortisol. The pooled testosterone correlations were strong and significant in the good squatters (r = 0.92 for squats, r = -0.87 for sprints, p < 0.01), but not significant for the average squatters (r = 0.35 for squats, r = -0.18 for sprints). Cortisol showed no significant correlations with 1RM squat and 10-m sprint performance, and no differences were identified between the 2 squatting groups. In summary, these results suggest that free testosterone is a strong individual predictor of squat and sprinting performance in individuals with relatively high strength levels but a poor predictor in less strong individuals. This information can assist coaches, trainers, and performance scientists working with stronger weight-trained athletes, for example, the preworkout measurement of free testosterone could indicate likely training outcomes or a readiness to train at a certain intensity level, especially if real-time measurements are made. Our results also highlight the need to separate group and individual hormonal data during the repeated testing of athletes with variable strength levels.

BACKGROUND: We investigated the effects of sleep deprivation with or without acute supplementation of caffeine or creatine on the execution of a repeated rugby passing skill. METHOD: Ten elite rugby players completed 10 trials on a simple rugby passing skill test (20 repeats per trial), following a period of familiarisation. The players had between 7-9 h sleep on 5 of these trials and between 3-5 h sleep (deprivation) on the other 5. At a time of 1.5 h before each trial, they undertook administration of either: placebo tablets, 50 or 100 mg/kg creatine, 1 or 5 mg/kg caffeine. Saliva was collected before each trial and assayed for salivary free cortisol and testosterone. RESULTS: Sleep deprivation with placebo application resulted in a significant fall in skill performance accuracy on both the dominant and non-dominant passing sides (p < 0.001). No fall in skill performance was seen with caffeine doses of 1 or 5 mg/kg, and the two doses were not significantly different in effect. Similarly, no deficit was seen with creatine administration at 50 or 100 mg/kg and the performance effects were not significantly different. Salivary testosterone was not affected by sleep deprivation, but trended higher with the 100 mg/kg creatine dose, compared to the placebo treatment (p = 0.067). Salivary cortisol was elevated (p = 0.001) with the 5 mg/kg dose of caffeine (vs. placebo). CONCLUSION: Acute sleep deprivation affects performance of a simple repeat skill in elite athletes and this was ameliorated by a single dose of either caffeine or creatine. Acute creatine use may help to alleviate decrements in skill performance in situations of sleep deprivation, such as transmeridian travel, and caffeine at low doses appears as efficacious as higher doses, at alleviating sleep deprivation deficits in athletes with a history of low caffeine use. Both options are without the side effects of higher dose caffeine use.
