RESUMO

This paper shows it possible to use geoinformational systems for mapping and analyzing medical statistical data on those with malignant laryngeal neoplasms. The fact that the distribution of the patients can be shown on the map of a town in relation to the location of their houses (if they have an address) allows differentiation of the ecological living conditions of these patients, which in turn makes it possible to put not only high morbidity areas on the map of a town, but also to consider the characteristics of the environment in these areas.

RESUMO

The report is concerned with an analysis of lung cancer morbidity (1979-1986) in the population of the Norilsk industrial region versus age. In the 50-59 and 60-69 years age groups, it was found to be 2.1-2.5 times the national and Krasnoyarsk regional levels. Carcinogenic hazards involved in processing ores of different metals contribute to such morbidity.

RESUMO

Data on 148 cases of cancer of the gallbladder and extrahepatic bile ducts were studied. Jaundice proved the cardinal symptom. No clear-cut clinical picture of the disease was identified. Diagnostic procedure should start with ultrasonography. Cholangiectasia and the enlarged pancreatic head make the case for fiber bronchoscopy and hypotonic duodenography. Cancer-negative patients should further undergo transcutaneous transhepatic cholangiography and, if proving still negative, retrograde cholangiopancreatography. Resection of bile ducts with simultaneous lymphadenectomy is considered radical. The authors suggest a surgical procedure for cancer of the gallbladder which includes resection of the liver, hepatico-choledoctomy and cholecystectomy with formation of cholangio-jejuno-anastomosis using disposable transhepatic drains. Recanalization of bile ducts by transhepatic drain is considered optimal for palliation. Survival depends upon extent of surgery and level of bile duct obstruction.

RESUMO

Errors in the diagnosis and treatment of pancreatic cysts have been studied at various stages of rendering medical aid. In a polyclinic the diagnostic errors are due to inadequate skill of physicians, insufficient study of medical records, inadequate examination and ungrounded refusal to consult a surgeon. The most informative methods of examination are X-ray of the stomach, duodenography in the state of hypotonia, pneumoperitoneography in combination with retropneumoperitoneography and ultrasound examination. It is emphasized that the most common complication of cysts of the pancreas are suppuration of its contents and malignant transformation of its walls. The operation of choice in this condition is pancreatico-jejunostomy with an isolated jejunum according to the Roux-en-Y technique. External drainage of the cyst is indicated only in suppuration. Pancreatogastrostomy is considered an emergency operation.

RESUMO

Tumors were studied in 38 patients, tissues located far away from tumor being used as control. Breast and stomach tumors revealed relatively high levels of glutathione reductase, glutathione-S-transferase (I) and, particularly, gamma-glutamyl transferase (II). The said changes were not observed in colorectal tumors. Gamma-glutamyl transferase (II) may be used as a marker for breast and stomach malignancies in man but it is not a universal and absolutely specific marker for all tumors. The activity of glutathione peroxidase (III) either did not change or decreased. The activating effect of cAMP on III and I remained generally unchanged in all tumors studied.

RESUMO

The authors elaborated an operation--enterocystoplasty combined with direct and transcutaneous electric stimulation of the artificial urinary bladder in its sclerosis (contracted urinary bladder) in patients with affection of the spinal cord. The results of 21 operations with follow-up periods of 9 years are discussed. Three patients died. The results in the remaining patients were good: the act of urination was restored, the urge to urinate reappeared, and the ability to hold the urine was restored. The restoration of the act of urination was conducive to preservation of the renal function and made it possible to change the patients to a vertical position and develop the habit of walking.
