RESUMO

The objective of the present work was to substantiate rational pharmacotherapy allowing not only to affect the pathogen but also to restore the initial state of the compromised immune system of the patient. This paper is focused on the markers of the immunodeficiency state in the patients presenting with acute and chronic bacterial rhinosinusitis taking into consideration the nature of the pathogen. A rationale is presented for the management of this pathology with the use of the natural topical immunostimulator IRS-19 that can be recommended for the introduction into combined therapy of acute and chronic rhinosinusitis for the achievement of well-apparent and stable results.

RESUMO

We studied correlations between clinical characteristics of ailing children and their immunohistochemical parameters. Clinical status was assessed by the degree of adenoid vegetations, duration of the disease, the presence of chronic tonsillitis or its combination with adenoiditis. We used the following immunohistochemical parameters: the level of CD3+ in histological slices and the presence or absence of IgA-producing cells. Correlation was found between the presence or absence of IgA-producing cells and the degree of adenoid vegetations as well as the presence only of chronic tonsillitis or chronic tonsillitis plus adenoiditis. We found also important but statistically insignificant differences: in a group of patients with the disease duration up to 2 years CD3+ cells were absent, while in children with the disease duration more than 5 years these cells were present. Thus, immunohistochemical parameters are rather informative in interpretation of a clinical picture in ailing children.

RESUMO

The addition of nebulizer therapy with antiedematous, steroid and mucolytic drugs to antibacterial therapy in patients with different forms of acute laryngitis was assessed. Basing on the main symptoms of the disease, three variants of nebulizer therapy were proposed. As shown by the results of treatment of 48 patients with catarrhal laryngitis, 19 patients with laryngeal angina and 27 patients with phlegmonous laryngitis, nebulizer therapy is highly effective in different forms of laryngeal diseases including purulent process with stenosis.

Patients of young age predominated in the group of 113 patients (69 males, 44 females) with orbital rhinogenic inflammation observed by the authors. 57.5% of the cases arose due to acute sinusitis, 42.5%-due to exacerbation of chronic sinusitis. The inflammation focus developed more frequently in the frontal sinuses and ethmoid labyrinth. Periostitis was diagnosed in 17 patients, subperiosteal orbital abscess in 9 patients. Inflammation of the orbital soft tissues presented as eyelid and orbital fat edema in 66 patients, as retrobulbar abscess in 11 and phlegmon in 10 patients. The treatment policy combined intensive antiinflammatory therapy and surgery if indicated. The treatment aimed at both elimination of the inflammatory focus in the sinus and drainage of the inflammatory focus in the sinus and drainage of the secondary suppuration focus in the orbit. A 100% cure was achieved, though residual phenomena such as limited movements of the eyeball, partial ptosis, reduced visual acuity, loss of vision (3, 3, 2 and 1 patients, respectively) remained.
