RESUMO

Search and development of new physiotherapeutic technologies of regenerative medicine for the treatment of patients with different diseases is an urgent task of modern medicine. OBJECTIVE: To analyze scientific data on the effectiveness of the peripheral magnetic stimulation (PMS) application in patients with different diseases. MATERIAL AND METHODS: An analysis of publications in databases of electronic resources (PEDro, PubMed, Embase, eLibrary, Cochrane Library) over the past 20 years was carried out, the results of PMS application in patients with different nosologies were presented. RESULTS: The majority of the presented articles confirm the clinical effectiveness of PMS application mainly in patients with diseases of the nervous system, spine and genitourinary system. CONCLUSION: Further research to confirm the effectiveness of the therapeutic impact of magnetic stimulation in patients with other nosologies is needed.

RESUMO

Previous research experience on cardiac rehabilitation programs as a part of general health care system has shown that they are an important part of the management of cardiovascular patients. Improving quality of life, reducing the severity of risk factors, increasing physical performance, slowing disease progression, decrease in morbidity and mortality indicate the clinical efficacy of cardiac rehabilitation and make it an integral part of therapeutic interventions. Heart rehabilitation is a 1st class recommendation in the majority of modern cardiovascular guidelines around the world.

Currently important task of modern medicine is the development of personalized comprehensive physiotherapy technologies for the treatment of patients with chronic prostatitis (CP). OBJECTIVE: To analyze scientific data about efficacy of physiotherapy methods for the treatment of CP. MATERIAL AND METHODS: The number of publications equal 55 about evaluation of therapeutic effects of instrumental physiotherapy in patients with CP have been conducted. The search was done in electronic databases (PEDro, PubMed, EMBASE, eLIBRARY), systematic reviews databases (Cochrane library) for the last 20 years by keywords in Russian and English: «Ñ…ронический простатит¼ (chronic prostatitis), «ÑÐ»ÐµÐºÑ‚ротерапия¼ (electrotherapy), «Ð¼Ð°Ð³Ð½Ð¸Ñ‚отерапия¼ (magnetotherapy), «Ð»Ð°Ð·ÐµÑ€Ð½Ð°Ñ терапия¼ (laser therapy), «ÑƒÐ´Ð°Ñ€Ð½Ð¾-волновая терапия¼ (shock wave therapy), «ÑƒÐ»ÑŒÑ‚развук¼ (ultrasound). RESULTS: The clinical effects and suggested action mechanisms of instrumental physiotherapy methods in patients with CP were presented. CONCLUSION: Based on the results of the randomized placebo-controlled trials, presented in the review, it can be concluded, that certain physiotherapeutic interventions, such as transcutaneous electrical nerve stimulation, extracorporal shock wave therapy, ultrasound, impulsive magnetotherapy lead to decrease of prostatitis symptoms.

Postthrombophlebitic syndrome (PTPS) develops in 20-50% of patients who have had deep vein thrombosis (DVT). Patients with chronic venous insufficiency (CVI) of class C4-C5 according to the CEAP clinical classification, which developed as a result of DVT of the lower extremities, including those who underwent endovascular treatment (iliac vein stenting), are subject to staged medical rehabilitation. In this regard, the development of personalized complex technologies for the sanatorium treatment of patients with PTPS is an important medical and social problem. PURPOSE OF THE STUDY: Study of clinical efficacy and identification of the mechanisms of action of a new complex of spa treatment of patients with PTPS of the lower extremities using supravenous laser radiation, low-frequency magnetotherapy, dry-air carbon dioxide baths and structured therapeutic exercises in the gym. MATERIAL AND METHODS: 60 patients with PTPS of the lower extremities (CVI C4-C5 according to CEAP) were under observation. All patients were randomly divided into 2 groups: first group (main group) included 30 patients who received a treatment, including procedures for supravascular laser blood irradiation, pulsed magnetotherapy and dry-air carbon dioxide baths, as well as structured therapeutic exercises in gym under the supervision of an exercise therapy instructor; second group (control group) included 30 patients who received standard elastic compression (compression class 2-3) while taking lymphovenotonics (a combination of diosmin and hesperidin) and therapeutic exercises in the gym. RESULTS: Against the background of the course of treatment in patients of the main group, to a greater extent than in the control group, a decrease in the clinical symptoms of the disease was noted: a more pronounced regression of edema, a decrease in heaviness in the legs, as evidenced by the data of anthropometric studies and questionnaires on the CIVIQ-2 scale. Positive dynamics in the microcirculation system (MC) was established, which was confirmed by the data of laser Doppler flowmetry. In patients with spastic-congestive type of MC, a decrease in the initially increased myogenic and neurogenic tone of arterioles was registered. There was a decreasing of stagnation in the venular link. In patients with hyperemic-congestive type of MC, the initially reduced tone of arterioles increased, which contributed to the improvement of blood flow in the capillaries. There was also a decrease in congestion in the venular link of the microvasculature. CONCLUSION: A new effective complex method for the rehabilitation of patients with PTPS has been developed, including laser exposure according to the general method, pulsed magnetotherapy and dry-air carbon dioxide baths, which have a multifocal effect on different links in the pathogenesis of PTPS.

Rehabilitation of patients after total knee arthroplasty is still a challenge for modern medicine. At the same time, there are few publications in the medical literature on rehabilitation programs for patients after total knee arthroplasty. Available scientific studies have proven the effectiveness of low-intensity laser therapy and a pulsed low-frequency electrostatic field (PLFEF) in tissue repair by modulating the inflammatory process and relieving pain. OBJECTIVE: Scientific substantiation of the feasibility of the combined use of low-intensity laser radiation (LILR) and PLFEF in patients after total knee arthroplasty. MATERIAL AND METHODS: 90 patients aged from 55 to 80 years after total knee arthroplasty were examined. All patients were randomly assigned to three groups. Patients of the 1st group underwent therapeutic exercises with an instructor and low-intensity laser exposure; in the 2nd group, therapeutic exercises with an instructor, low-intensity laser therapy, and PLFEF were performed without a time interval; patients of the 3rd group received only therapeutic exercises with an instructor. RESULTS: After the course of treatment, a positive dynamics of the state of microcirculation was noted in all patients, which correlated with a significant regression of the pain syndrome and an improvement in the performance of the 10-meter walk test with external support on crutches. However, a more significant decrease in pain intensity after the first procedures and at the end of the course of treatment was found in the group of patients who received complex therapy, which included LILR, PLFEF, and therapeutic exercises. CONCLUSION: Thus, based on the data of this study, it is possible to recommend the combined use of LILR and PLFEF for the treatment of patients after total knee arthroplasty.

Physical activity is one of the main components of the rehabilitation of patients with cardiovascular disease (CVD). As shown by practice and the results of evidence-based studies, the beneficial effects of physical activity on disease outcomes in a number of cardiac nosologies are comparable to drug treatment. This gives the doctor another tool to influence the unfavorable epidemiological situation in developed countries with the spread of diseases of the cardiovascular system and CVD mortality. Reliable positive results of cardiorehabilitation (CR) were obtained using various methods. The goal of CR is to restore the optimal physiological, psychological and professional status, reduce the risk of CVD and mortality. In most current CVD guidelines worldwide, cardiac rehabilitation is a Class I recommendation. The molecular mechanisms described in the review, initiated by physical activity, underlie the multifactorial effect of the latter on the function of the cardiovascular system and the course of cardiac diseases. Physical exercise is an important component of the therapeutic management of patients with CVD, which is supported by the results of a meta-analysis of 63 studies associated with various forms of aerobic exercise of varying intensity (from 50 to 95% VO2) for 1 to 47 months, which showed that CR based on physical exercise improves cardiorespiratory endurance. Knowledge of the molecular basis of the influence of physical activity makes it possible to use biochemical markers to assess the effectiveness of rehabilitation programs.

One of the current health care problems is a menopausal syndrome (MS), a set of symptoms characterized by specific metabolic, neurological, autonomous, and psychosomatic changes. Many authors believe that one of the main methods of prevention and treatment of menopausal disorders is hormone replacement therapy, but there are many contraindications. Therefore, there is increasing interest in the development of non-medicinal therapy methods, in particular, physiotherapeutic treatment. OBJECTIVE: To study the effect of total cryotherapy alone and in combination with transcranial magnet therapy on the microcirculation state and psychological status in patients with MS. MATERIAL AND METHODS: The study included 62 female patients (mean age was 51±1.9 years). Thirty-five (56%) patients had mild menopausal syndrome and 27 (44%) moderate severity. All patients were divided into two clinically and functionally comparable groups by simple randomization: Group 1: 30 patients who received total cryotherapy (TCT); Group 2: 32 patients who received complex treatment including TCT and transcranial magnet therapy. The blood lipid spectrum was studied in all patients before and after the treatment course. The psychological examination was carried out using the well-being/activity/mood test (WAM). The microcirculation state was assessed by laser Doppler flowmetry (LDF). RESULTS: The majority of the patients had abnormalities in the lipid spectrum, decreased psychological adaptation, and worsened microcirculation indices (according to LDF data). After a treatment course, improvement in the lipid spectrum, LDF, and WAM test scores according to all scales was more pronounced in group 2 patients receiving complex therapy. CONCLUSION: The combined effect of total cryotherapy and transcranial magnet therapy had a more pronounced positive impact on the lipid spectrum, psychoemotional state, and microcirculatory processes in patients with menopausal syndrome compared with the total cryotherapy alone. It can be assumed that this complex will be effectively used in practical healthcare.

There are convincing data on the association of psycho-emotional disorders with the degree of bone mineral density (BMD) loss and the risk of fractures on the basis of osteoporosis (OP) but the nature of the causal relationship has not yet been clearly established. The study of this issue is important to substantiate the need and nature of psychological correction within the framework of comprehensive rehabilitation programs in patients with osteoporotic fractures. OBJECTIVE: Study of the severity of symptoms of depression and anxiety in patients who have suffered compression fractures of the vertebrae (VF) of osteoporotic genesis who entered the II stage of medical rehabilitation, as well as the contribution to the development of psycho-emotional age disorders, the severity of OP, pain syndrome and the use of orthotics. MATERIAL AND METHODS: The study sample consisted of 120 women 50 to 80 years old with an established diagnosis of OP based on the results of bone densitometry. The main group included 60 patients with OP complicated by at least one VF. The control group included 60 patients with OP without a history of osteoporotic fractures comparable in age, body mass index and BMD in the spine with the main group. The complex of examination included the collection of complaints, anamnesis of OP, previous fractures, assessment of pain syndrome according to VAS, BMD study and verification of VF by X-ray methods. To assess the level of depression (DL) we used the Tsung depression scale modified by T.I. Balashova, situational (SA) and personal anxiety (PA) - Spielberger-Khanin questionnaire. RESULTS: In the main group in comparison with the control group the proportion of patients without symptoms of depression was lower (66.7 and 88.3%, respectively; p=0.042), as well as the frequency of detection of high degree of SA was higher (85.0 and 73.4%, respectively; p=0.039). In patients with VF it was higher than in the control, DL - 46.0 [42.0; 54.5] (27-70) and 43.0 [38.0; 47.5] (25-65) points, respectively (p=0.0009), as well as the SA degree - 61.5 [54.0; 71.0] (20-75) and 52.5 [43.5; 64.0] (20-68) points, respectively (p=0.0006). Statistically significant direct correlation dependences of DL on age (r=0.317; p=0.00042), the duration of the postmenopausal period (r=0.325; p=0.0003), the number of VFs (g= -0.245; p=0.00013) were established. Moreover, the intensity of pain syndrome (g= -0.234; p=0.00034), as well as feedbacks of this indicator with BMD in the spine (r= -0.342; p=0.00017) and the duration of the use of thoracolumbar orthoses (r = -0.504; p = 0.00016). There were direct dependence of the SA degree on age (r=0.281; p=0.0019) and the intensity of pain syndrome (g=0.258; p=0.0044). Negative correlation of SA with body weight (r= -0.183; p=0.045), BMD in the spine (r= -0.207; p=0.026), duration of orthosis application (r= -0.327; p=0.0095) and the amount of VF in the lumbar spine (g= -0.214; p=0.044) were detected. There was a significant correlation between the degree of PA and BMD in the lumbar vertebrae (r= -0.18; p=0.046) and the intensity of pain syndrome (g=0.137; p=0.039). CONCLUSION: The results obtained indicate the need for psychological correction in the framework of the complex rehabilitation of women who underwent VF based on OP due to increased DL and SA especially in older age groups.

There is a high prevalence of osteoporosis (OP) among patients of the older age undergoing rehabilitation. Therefore, it is obvious that physicians working in the field of physical and rehabilitative medicine should be well oriented in this medical problem. OBJECTIVE: To study the relevance of the OP problem for physicians working in the field of physical and rehabilitation medicine, their awareness of the main methods of diagnosis, treatment and prevention of OP, as well as the frequency of using these methods in daily clinical practice. MATERIAL AND METHODS: A cross-type study was carried out using a questionnaire survey. The study included 157 doctors (male 34, female 123) of 8 medical specialties working in 27 specialized medical institutions on the profile of «medical rehabilitation¼ or «spa treatment¼. The questionnaire for doctors consisted of 21 items of special questions. RESULTS: 90.45% of doctors considered the problem of OP is relevant for their clinical activities, 100% of respondents indicated that the presence of OP significantly affects the rehabilitation prognosis, 95.54% - the effectiveness of medical rehabilitation. According to the respondents the patients with OP cover 30.0% of total patients flow on average. 145 (92.36%) doctors noted that they know the OP risk factors, 155 (98.73%) - methods for OP diagnosing, 108 (68.79%) - methods for OP treatment, 126 (80, 25%) - methods for OP prevention, 74 (47.13%) - FRAX. 55 (35.01%) respondents identified their level of awareness on the OP problem as sufficient for managing patients with this pathology. Diagnostic procedures for OP were recommended by all interviewed endocrinologists (100%), most of traumatologists-orthopedists (72.73%), obstetricians-gynecologists (66.67%) and cardiologists (64.28%), as well as on average 1/2 (50%) neurologists and therapists. OP treatment was mainly performed by endocrinologists (100%), obstetricians-gynecologists (66.67%) and therapists (60%). 32.48% of physicians in the study sample have ever referred their patients to a densitometry and the diagnosis of OP was established in 80.25% of cases on the basis of bone densitometry data. CONCLUSION: The problem of OP is relevant for the clinical practice of doctors working in the field of rehabilitation and balneology medicine; the presence of OP significantly affects the prognosis and effectiveness of medical rehabilitation. The results of the study demonstrated the need to expand the availability of OP diagnostics methods in rehabilitation and balneology organizations, as well as the need to raise awareness of the OP problem among specialists working in this area.

High requirements for macro- and micronutrients of the organism of a pregnant woman living in the Far North are associated with the influence of extreme factors of high latitudes, and the needs of the mother and the growing fetus. Only the products of the unique food culture of the Arctic people - the Nenets, make it possible to meet the emerging needs. The aim of the work was to study the impact of traditional nutrition on the reproductive health of Nenets women living in the Arctic zone of Western Siberia. Material and methods. 619 indigenous inhabitants (Nenets) of the Yamal, Nadym and Taz districts of the Yamal-Nenets Autonomous Okrug at the age of 18-65 years were examined. The influence of the type of nutrition (traditional or imported products) on reproductive health indicators (the number of pregnancies, the number of children, the number of spontaneous abortions, frequency of gestosis, threats of termination of pregnancy, operative deliveries) for the period 2013-2018 was studied by analyzing official statistics and by questioning of the female population in own research. Results. The consumption of traditional products (venison, local fishery products, at least 3 times a week) by Nenets women was more often accompanied by a normal physiological course of pregnancy and childbirth (75.2 versus 64.2%, χ2=8.7; p=0.003). It led to a 1.5-fold decrease in the frequency of complications during pregnancy (gestosis, the threat of abortion) (χ2=5.8; p=0.01) and a 20% decrease in the frequency of delivery by cesarean section (χ2=16.6; p<0.001). As a result, a family whose diet was dominated by traditional products had a statistically significantly larger number of children (per child). Conclusion. The preservation of the consumption of reindeer and river fishery products by indigenous women will contribute to the sufficient intake of macro- and micronutrients, that will make it possible to gestate and give birth to a healthier generation of the Nenets society.

Elimination of vitamin D and calcium deficiencies is of particular importance in older patients undergoing medical rehabilitation after a serious illness, surgery or injury and having a high risk of fractures. Preventing falls and fractures, including during the course of rehabilitation, is an important challenge that can be addressed in these patients, in particular through improved nutrition and vitamin D and calcium supplementation. The aim of the study was to evaluate the effect of long-term intake of a complex dietary supplement with calcium and vitamins D3 and B6 on calcium homeostasis and the frequency of falls in patients with high fracture risk undergoing medical rehabilitation. Material and methods. The study enrolled 109 women and 10 men (mean age 65.5±7.9 years) with high fracture risk who were undergoing medical rehabilitation. After baseline examination, 41 patients have been receiving antiresorptive therapy already comprised group 1, and patients who didn't receive osteoporotic therapy were randomized into groups 2 (n=39) and 3 (control, n=39). Patients in groups 1 and 2 for 12 months were prescribed a dietary supplement containing calcium in a daily dose of 200 mg (in the form of citrate 1000 mg), 600 IU of vitamin D3 and 2 mg of vitamin B6. All patients underwent assessment of bone mineral density (BMD), calculation of absolute 10-year fracture risk according to FRAX, assessment of food calcium intake, etermination of biochemical parameters of calcium-phosphorus metabolism and bone remodeling (total calcium, inorganic phosphorus, alkaline phosphatase activity - by colorimetric method in blood serum; immunoreactive parathyroid hormone (PTH) and osteocalcin - by electrochemiluminescence immunoassay in blood serum; ß-isomer of C-terminal telopeptide of type I collagen (CTx) and 25(OH)D in blood plasma - by immunochemiluminescence analysis), cases of falls and fractures were fixed. Results. Average daily intake of calcium in the studied sample (n=119) was 782.9±243.4 mg, and 67.2% of patients consumed less than 800 mg of calcium daily. Vitamin D deficit was detected in 38.4% of the examined, its insufficiency - in 32.8%. An increase in 25(OH)D concentration was noted in groups 1 and 2 after 6 and 12 months (p<0.01), while in group 3 there was no dynamics of 25(OH)D (p>0.05). Patients in group 1 showed an increase in the level of osteocalcin and total calcium after 6 and 12 months, as well as alkaline phosphatase activity after 6 months (p<0.05). In group 3, there was an increase of PTH levels after 6 (p<0.05) and 12 months (p<0.01), CTx and alkaline phosphatase activity after 12 months (p<0.05). In group 1, there was an increase in BMD in the spine (+4.2%, p=0.024), femoral neck (+3.0%, p=0.041), and total femur (+2.7%, p=0.045), in patients of group 2 - an increase in BMD in the spine (+1.8%, p=0.048). In group 1, there was also a decrease in proportion of patients who fell after 6 months (χ2=4.97, p=0.026) and a decrease in the total number of falls after 12 months (χ2=4.89, p=0.027). Group 2 showed a decrease in the number of patients who fell after 6 and 12 months (χ2=48.58, p=0.0034 at both stages of the study) and the number of falls in general after 6 months (χ2=6.02, p=0.0142). Conclusion. The obtained data allow us to recommend prescription of dietary supplements containing calcium and vitamin D3 as a part of complex rehabilitation of patients with high fracture risk.

New prospects associated with the use of laser radiation of different wavelengths in the ultraviolet (365 nm) and the green emission spectrum (525 nm) have appeared in recent years. The high prevalence of the trophic leg ulcers necessitates the search for more effective methods of conservative treatment. PURPOSE OF THE STUDY: Development and scientific justification for the low-intensity laser radiation of different wavelengths combined use in patients with trophic leg ulcers and chronic venous insufficiency (CVI). MATERIAL AND METHODS: 102 patients with trophic leg ulcers and CVI were examined. They were divided into 3 equal groups using simple randomization. Patients in the control group received only traditional conservative therapy; comparison group - traditional conservative therapy and an intravenous laser blood irradiation (ILBI) (λ - 635 nm); the main group - traditional conservative therapy and different wavelengths laser therapy (LT) course. RESULTS AND CONCLUSION: A more significant improvement in the clinical condition occurred under the influence of the course application of combined different wavelengths laser therapy in patients with trophic venous leg ulcers. Compared to patients who received traditional conservative therapy and in combination with ILBI 635 nm, the speed of reparative regeneration of trophic ulcers increased, the time for cleansing trophic ulcers from devitalized tissues was reduced. Computer capillaroscopy data showed that the positive effect of laser exposure on all parts of the microcirculation underlies its therapeutic efficacy.

RESUMO

THE PURPOSE OF THIS WORK: Is to analyze the current state of the sanatorium-and-spa complex of the Russian Federation and to develop promising ways of its development. MATERIAL AND METHODS: The state analysis was carried out on the basis of the results of field events, including the assessment of the quality of medical care; video conference call results; data from the State Register of the Resort Fund of the Russian Federation; reports on medical rehabilitation for people with disabilities and children with disabilities in medical and sanatorium-and-spa organizations. When assessing, an integral indicator of the current state of the medical industry was used, based on which the total integral indicator of the sanatorium-and-spa complex was calculated. RESULTS: Based on the analysis, a rating of the constituent entities of the Russian Federation was compiled depending on the state of the sanatorium-and-spa treatment, the problems of the industry and ways to solve them were highlighted. The development paths of the sanatorium-and-spa complex of the Russian Federation are substantiated, which are reflected in the Development Strategy of the sanatorium-and-spa complex of the Russian Federation, approved by order of the Government of the Russian Federation dated November 26, 2018 №2581-r. CONCLUSION: The implementation of the proposed measures will restore the world-recognized achievements of domestic balneology and increase the effectiveness and accessibility of sanatorium-and-spa treatment, as well as achieve the targets specified in the Development Strategy of the spa complex of the Russian Federation.

RESUMO

The aim of this review is to analyze the problem of biofilm formation by pathogenic bacteria in food enterprises and the risks associated with this phenomenon. The aspects of the formation of biofilms by pathogenic microorganisms at food industry enterprises have been considered from the point of view of the potential danger of infection and colonization of processing plants by biofilm-forming strains. Biofilms are the preferred form of bacteria existence. It is known that bacteria in a state of biofilm are protected from adverse environmental factors and antibacterial substances, they are often resistant to standard cleaning and disinfection procedures. The formation of biofilms on biotic and abiotic surfaces is a potential hazard, contributing to the constant circulation of pathogens in the conditions of food production and contamination of foods. The problem of biofilm formation is characteristic of all food enterprises.
