Background: Treating hyperthyroidism induced by autonomously functioning thyroid nodules (AFTNs) through radioactive iodine and surgery often results in undesirable hypothyroidism. Radiofrequency ablation (RFA) has arisen as a favorable option. European guidelines recommend RFA for small AFTN in young patients, aiming to restore normal thyroid function and avoid irradiation. The procedure, costing between 500 and 1,000 euros, is conducted in outpatient clinics and takes 15 to 40 minutes. We aimed to describe the clinical outcomes of AFTN patients treated with RFA in Ecuador. Case Description: We included eight patients with toxic thyroid nodules suppressed thyroid-stimulating hormone (TSH), with symptomatic hyperthyroidism. The mean age was 41.63 years [standard deviation (SD): 14.97 years]. The median follow-up time was 8 months. Nodules were solid (37.5%) or predominantly solid (62.5%). The mean volume pre-RFA was 5.27 mL [interquartile range (IQR), 0.70-9.66 mL]. After ablation, the median volumes at 1, 3, and 6 months were [2.25 (SD: 1.67; P<0.12), 1.28 (SD: 1.1; P=0.013), and 1.37 (SD: 1; P=0.23) mL], respectively. The volume reduction (VR) was 45.8%, 75.1%, and 69.7% at 1-, 3-, and 6-month follow-up, respectively. Conclusions: RFA holds promise as a potential therapeutic approach for managing AFTNs. The success and the feasibility of RFA in this series are consistent with other studies as a treatment option in young patients with small AFTN. However, more research is needed to establish comprehensive guidelines and protocols to maximize the benefits of RFA in AFTNs.

BACKGROUND: The global prevalence of thyroid cancer is on the rise. About one-third of newly diagnosed thyroid cancer cases comprise low-risk papillary thyroid cancer (1.5 cm or more minor). While surgical removal remains the prevailing approach for managing low-risk papillary thyroid cancer (LPTC) in patients, other options such as active surveillance (AS), radiofrequency ablation (RFA), microwave ablation (MWA), and laser ablation (LA) are also being considered as viable alternatives. This study evaluated and compared surgical thyroid resection (TSR) versus non-surgical (NS) methods for treating patients with LPTC. METHODS: The study encompassed an analysis of comparisons between surgical thyroid resection (TSR) and alternative approaches, including active surveillance (AS), radiofrequency ablation (RFA), microwave ablation (MWA), or laser ablation (LA). The focus was on patients with biopsy-confirmed low-risk papillary thyroid cancer (LPTC) of less than 1.5 cm without preoperative indications of local or distant metastasis. The primary outcomes assessed were recurrence rates, disease-specific mortality, and quality of life (QoL). Data were collected from prominent databases, including Cochrane Database, Embase, MEDLINE, and Scopus, from inception to June 3rd, 2020. The CLARITY tool was utilized to evaluate bias risk. The analysis involved odds ratios (OR) with 95% confidence intervals (CI) for dichotomous outcomes, as well as mean differences (MD) and standardized mean differences (SMD) for continuous outcomes. The study is registered on PROSPERO under the identifier CRD42021235657. RESULTS: The study incorporated 13 retrospective cohort studies involving 4034 patients. Surgical thyroid resection (TSR), active surveillance (AS), and minimally invasive techniques like radiofrequency ablation (RFA), microwave ablation (MWA), and laser ablation (LA) were performed in varying proportions of cases. The analysis indicated that specific disease mortality rates were comparable among AS, MWA, and TSR groups. The risk of recurrence, evaluated over different follow-up periods, showed no significant differences when comparing AS, RFA, MWA, or LA against TSR. Patients undergoing AS demonstrated better physical health-related quality of life (QoL) than those undergoing TSR. However, no substantial differences were observed in the overall mental health domain of QoL when comparing AS or RFA with TSR. The risk of bias was moderate in nine studies and high in four. CONCLUSION: Low-quality evidence indicates comparable recurrence and disease-specific mortality risks among patients with LPTC who underwent ablation techniques or active surveillance (AS) compared to surgery. Nevertheless, individuals who opted for AS exhibited enhanced physical quality of life (QoL). Subsequent investigations are warranted to validate these findings.

INTRODUCTION: Studies evaluating the rate and histology of appendiceal neoplasms between complicated and uncomplicated appendicitis include a small number of patients. Therefore, we sought a meta-analysis and systematic review comparing the rates and types of appendiceal neoplasm between complicated and uncomplicated appendicitis. METHODS: We included articles published from the time of inception of the datasets to September 30, 2022. The electronic databases included English publications in Ovid MEDLINE In-Process & Other Non-Indexed Citations, Ovid MEDLINE, Ovid EMBASE, and Scopus. RESULTS: A total of 4962 patients with appendicitis enrolled in 4 comparative studies were included. The mean age was 43.55 years (16- 94), and half were male (51%). Based on intra-operative findings, 1394 (38%) had complicated appendicitis, and 3558 (62%) had uncomplicated appendicitis. The overall incidence rate of neoplasm was 1.98%. No significant difference was found in the incidence rate of appendiceal neoplasm between complicated (3.29%) and uncomplicated (1.49%) appendicitis (OR 0.44, 95% CI 0.16- 1.23; p < 0.087; I2 = 54.9%). The most common appendiceal neoplasms were Neuroendocrine Tumors (NET) (49.21%), Nonmucinous Adenocarcinoma (24.24%), Mixed Adeno-Neuroendocrine Tumor (MANEC) (11.40%), Mucinous Adenocarcinoma (4.44%). There was a significant difference between complicated and uncomplicated appendicitis in rates of adenocarcinoma (50% vs. 13%), NET (31% vs. 74%), MANEC (19% vs. 13%) (P < 0.001). CONCLUSION: While there was no significant difference in the overall neoplasm rate between complicated and uncomplicated appendicitis, the NET rate was significantly higher in uncomplicated appendicitis. In comparison, the Adenocarcinoma rate was considerably higher in Complicated appendicitis. These findings emphasize the importance of evaluating risk factors for neoplasm when considering appendectomy in patients with appendicitis.

In Leishmania mexicana, the protease gp63 has been documented as the protein responsible for cyclooxygenase (COX) activity. The present work aimed to obtain a monoclonal antibody capable of recognizing this protein without blocking the COX-like enzymatic activity. The antibody produced by the selected hybridoma was named D12 mAb. The antigen recognized by the D12 mAb was characterized by the determination of COX activity associated with immune complexes in the presence of exogenous arachidonic acid (AA) using the commercial Activity Assay Abcam kit. LSM-SMS analysis validated the identity of the antigen associated with the D12 mAb as the L. mexicana protease gp63. Confocal microscopy assays with the D12 mAb detected, by cross-recognition, similar proteins in other protozoan parasites. COX-like molecules are located in vesicular structures, homogeneously distributed throughout the cytoplasm in amastigotes (intracellular infectious phase) and promastigotes of L. mexicana, and trophozoites of Entamoeba histolytica, Acanthamoeba castellanii, and Naegleria fowleri. However, in Giardia duodenalis trophozoites, the distribution of the COX-like molecule was also in perinuclear areas. In comparison, in Trypanosoma cruzi trypomastigotes, the distribution was mainly observed in the plasma membrane. Structural analyses of COX-2-like antigens revealed continuous and discontinuous epitopes for B cells, which could be relevant in the cross-reaction of D12 mAb with the analyzed parasites. These results indicate that the D12 mAb against the L. mexicana gp63 also recognizes a COX-like molecule in several protozoan parasites, suggesting that this D12 mAb could potentially be used in combined therapies against infectious diseases.

Inorganic pollutants in Colombian cocoa (Theobroma cacao L.) agrosystems cause problems in the production, quality, and exportation of this raw material worldwide. There has been an increased interest in bioprospecting studies of different fungal species focused on the biosorption of heavy metals. Furthermore, fungi constitute a valuable, profitable, ecological, and efficient natural soil resource that could be considered in the integrated management of cadmium mitigation. This study reports a new species of Talaromyces isolated from a cocoa soil sample collected in San Vicente de Chucurí, Colombia. T. santanderensis is featured by Lemon Yellow (R. Pl. IV) mycelium on CYA, mono-to-biverticillade conidiophores, and acerose phialides. T. santanderensis is distinguished from related species by its growth rate on CYAS and powdery textures on MEA, YES and OA, high acid production on CREA and smaller conidia. It is differentiated from T. lentulus by its growth rate on CYA medium at 37 °C without exudate production, its cream (R. PI. XVI) margin on MEA, and dense sporulation on YES and CYA. Phylogenetic analysis was performed using a polyphasic approach, including different phylogenetic analyses of combined and individual ITS, CaM, BenA, and RPB2 gene sequences that indicate that it is new to science and is named Talaromyces santanderensis sp. nov. This new species belongs to the Talaromyces section and is closely related to T. lentulus, T. soli, T. tumuli, and T. pratensis (inside the T. pinophilus species complex) in the inferred phylogeny. Mycelia growth of the fungal strains was subjected to a range of 0-400 mg/kg Cd and incorporated into malt extract agar (MEA) in triplicates. Fungal radial growth was recorded every three days over a 13-day incubation period and In vitro cadmium tolerance tests showed a high tolerance index (0.81) when the mycelium was exposed to 300 mg/kg of Cd. Results suggest that T. santanderensis showed tolerance to Cd concentrations that exceed the permissible limits for contaminated soils, and it is promising for its use in bioremediation strategies to eliminate Cd from highly contaminated agricultural soils.

MATERIALS AND METHODS: This was a cross-sectional study. During 2003, anti-HBc was analyzed at INSP in 19 907 sera, and the related sociodemographic factors were determined. RESULTS: Anti-HBc prevalence was greater among men, but it was also associated to age, residence in a rural area, low socio-economic status, and illiteracy. Clusters of very high anti-HBc prevalence were found in several rural communities where the prevalence of anti-HBc in adults is 3 to 20 times the national average. CONCLUSIONS: Besides a low endemicity of HBV in Mexico, distribution is heterogeneous as was shown in several of the states studied, where there are rural towns with very high prevalence of HBV markers. National serosurveys are useful tools for identifying communities with hepatitis B hyperendemicity, where focused research and control measures are needed.

OBJECTIVE: To perform a hepatitis B, C, and HIV survey in an enhanced sample from two rural communities with the highest anti-HBc prevalence found in ENSA 2000. MATERIALS AND METHODS: This was a cross-sectional study.We studied the communities of La Calera and Cuambio, two adjacent rural towns of the district of Zirándaro, Guerrero, targeted because of the high prevalence of anti-HBc shown in ENSA 2000.We assessed the prevalence of hepatitis B and C, and HIV serological markers and associated factors among inhabitants older than 10 years. RESULTS: A very high prevalence of hepatitis B markers associated with socioeconomic factors was confirmed, while hepatitis C prevalence was low and HIV absent in the two communities addressed. CONCLUSIONS: National serosurveys are an accurate tool for identifying communities with hepatitis B high endemicity where focused research and control measures are needed.

OBJETIVO: Estudiar la distribución de anti-HBc en 10 estados con prevalencia mayor a la media nacional en la ENSA 2000. MATERIAL Y MÉTODOS: Durante 2003 se analizó en el INSP anti-HBc en 19 907 muestras de suero, se determinaron factores sociodemográficos relacionados. RESULTADOS: La prevalencia de anti-HBc fue mayor en hombres. Se asoció con la edad, residencia en áreas rurales, bajo nivel socioeconómico y analfabetismo. Se encontraron agrupamientos de alta prevalencia de anti-HBc en comunidades rurales en las cuales la prevalencia de anti-HBc en adultos está entre 3 y 20 veces por arriba de la media nacional. CONCLUSIONES: Contrastando con la baja endemicidad del VHB en México, su distribución es heterogénea. En varios estados se hallaron localidades rurales con muy alta prevalencia de anti-HBc. Las encuestas de salud permiten identificar comunidades donde la hepatitis B es hiperendémica y en las cuales se requiere enfocar la investigación y tomar medidas de control.

OBJETIVO: Determinar la prevalencia de hepatitis B, C, y VIH en una muestra ampliada de dos de las comunidades rurales en las que se registró la mayor prevalencia de anticuerpos anti-core del virus de la hepatitis (anti-HBc) en la Encuesta Nacional de Salud 2000 (ENSA 2000). MATERIAL Y MÉTODOS: Se realizó un estudio transversal en las comunidades de La Calera y Cuambio, dos poblaciones rurales adyacentes del municipio de Zirándaro, Guerrero, seleccionadas por mostrar muy alta prevalencia de anti-HBc en la ENSA 2000. Se determinó la prevalencia de marcadores séricos de hepatitis B y C, y VIH, así como factores sociodemográficos asociados entre los habitantes mayores de 10 años. RESULTADOS: Se confirmó una muy alta prevalencia de marcadores de hepatitis B, asociada con la edad, baja escolaridad y relaciones con trabajadoras sexuales en Estados Unidos, mientras que la prevalencia de hepatitis C fue baja y no se detectó ningún caso de VIH. CONCLUSIONES: Existen en México comunidades rurales con alta endemicidad de hepatitis B, en las cuales es conveniente profundizar la investigación de determinantes de la transmisión de este virus. Estas comunidades son identificadas con certeza por las encuestas nacionales de salud.

OBJETIVO: Determinar la frecuencia de anticuerpos antitreponémicos en mexicanos adultos de ambos sexos y las características de la población relacionadas con la infección sifilítica. MATERIAL Y MÉTODOS: Se analizaron para anti-Treponema pallidum 12 010 sueros seleccionados de forma aleatoria de la Encuesta Nacional de Salud 2000, con aplicación de un algoritmo conformado por técnicas serológicas treponémicas y no treponémicas, de tamizaje y confirmatorias, y se realizó un análisis estadístico para identificar los factores vinculados con la seropositividad. RESULTADOS: La seroprevalencia de anticuerpos antitreponémicos fue de 3.1 por ciento (IC95 por ciento 2.5-3.7). Las características relacionadas de modo significativo con la exposición a la infección fueron la mayor edad (RM 3.3), sexo masculino (RM 1.3), residencia en la región geográfica sur del país (RM 1.7), vivir en unión libre (RM 2.7) o ser viudo (RM 1.8), inicio de relaciones sexuales a edad temprana (RM 6.3) y, en las mujeres en particular, se identificó el antecedente de mortinatos (RM 1.8) y el informe de falta de uso del condón como método anticonceptivo (RM 14.1). CONCLUSIONES: Los hallazgos del estudio demuestran que la magnitud del antecedente de la infección por el treponema en la población mexicana adulta es considerablemente mayor respecto del que permite calcular la notificación oficial de casos de la enfermedad. Como sucede con otras infecciones de transmisión sexual, aspectos sociodemográficos, de género y las prácticas sexuales no seguras incrementan la transmisión del agente etiológico. En México existe la necesidad de reforzar la vigilancia de la infección sifilítica a través de un diagnóstico oportuno y eficaz de los casos, más su adecuado tratamiento, para coadyuvar al control del padecimiento.


OBJECTIVE: To measure for the first time in a population based study the prevalence of antibodies against Treponema pallidum among the Mexican adult population and correlates of infection. MATERIALS AND METHODS: This study was derived from the Mexican National Health Survey carried out in 2000. Anti-T. pallidum seroprevalence was determined according to an algorithm based on a set of treponemic and non-treponemic serological tests for 12 010 sera randomly selected from participating women and men 20 years and older. A statistical analysis was applied to identify seropositivity associated variables. RESULTS: Anti-T. pallidum seroprevalence was 3.1 percent (CI95 percent 2.5-3.7). Population characteristics significantly related to infection exposure were: older age (OR 3.3), being male (OR 1.3), inhabiting the southern region of the country (OR 1.7), having a regular partner not being married (OR 2.7) or being a widow (OR 1.8), early age at sexual debut (OR 6.3), and for women, having had still births (OR 1.8) and not using the condom as a contraceptive measure (OR 14.1). CONCLUSIONS: Results obtained show that exposure to T. pallidum infection among Mexican adults is at present a true public health problem, unknown until now. Factors associated with seropositivity were those expected considering the epidemiology of sexually transmitted infections. Health authorities must reinforce the surveillance and diagnosis of syphilis in Mexico to help prevent and control the transmission of the etiologic agent.
