RESUMO

Lymphoedema--is a chronic disease, characterized by the volume enhancement of some part of body, more often--of the limbs, as a consequence of tissues oedema, caused by the lymphatic system transport and resorptive function disorder. Modern approaches to lymphoedema treatment foresees combination of conservative and surgical ones, for example, microsurgical, taking into account the oedema character, stage of the disease, the previous treatment efficacy.

RESUMO

Gastric shunting (GS) constitutes one of the most effective surgical methods of treatment of morbid obesity. In USA and European countries GS constitutes standard bariatric intervention, in Ukraine these operations were not performed before. The technique of the most popular GS operations was depicted, own results of their application were presented.

RESUMO

Results of examination and surgical treatment of 77 patients with severe forms of chronic venous and lymphovenous insufficiency of lower extremities were analyzed. The compression syndrome concept and its role in pathogenesis of severe forms of chronic venous and lymphovenous insufficiency of lower extremities were determined. There was elaborated the tactics of complex treatment of patients, application of which had permitted to achieve the trophic ulcers healing in 98% operated patients, previously considered noncurable.

RESUMO

In 85.8% of patients with cholelithiasis (CL) the reflux-gastritis was revealed, of whom in 71.4% caused by Helicobacter pylori. In 80% of patients the positive effect achievement is guaranteed by the early accomplishment of cholecystectomy and an active conduction of rehabilitation measures in early postoperative period.

RESUMO

Complex investigations of hepatoportal circulation and hepatic functional reserve were conducted in 90 patients with liver cirrhosis and the portal hypertension syndrome in the stages of compensation, sub- and decompensation. While the disease grow progressing the indexes of common hepatic blood flow and its components lowers, hyperdynamic state of splenic circulation changes to congested one. Hemodynamic disturbances mentioned are making for hepatic functional reserve worsen.

RESUMO

In the differentiation of A-insulocytes in the human pancreas the following stages were distinguished: nondifferentiated cells of the epithelial anlage (5-6 weeks)--cell of primary duct (5-6 weeks)--duct cell containing lumps of secretory material (7-8 weeks)--cell precursor of A-insulocyte containing immature secretory granules (7-8 weeks)--A-insulocyte containing separate mature granules (9-10 weeks)--mature A-insulocyte (9-10 weeks). All this is accompanied by appearance at the 7-8 week of glucagon in the pancreatic tissue (76.20 +/- 17.28 ng/mg of wet tissue). The content of glucagon rises in 9-10 weeks 8.3 times.

RESUMO

The results of complex examination and treatment of 172 patients with varicocele are presented. Roentgenoendovascular occlusion of the testicular vein was performed in 52 patients with the positive result in 43 of them. The disease recurrence was revealed in control examination 6 mos later in 9 patients. A new microsurgical operation of creating the double spermatico-venous++ anastomosis, which permits to accomplish the more complete correction of venous hemodynamics in the system of the testicular vein, to reduce the traumatism of the operation, to improve the conditions for normalization of spermatogenesis, is suggested. A recurrence of varicocele after its treatment by means of the microsurgical correction occurred in 1 patient. Of the 56 women, whose husbands suffered from infertility, after microsurgical operations performed for varicocele, the pregnancy resulted in 19.

RESUMO

In the experiment on dogs and in patients with duodenal ulcer disease, it was established that the regulation of acid production besides the vagal nerves was mediated by the intragastric gastrin transport with the blood flow. A method of preoperative diagnosis of the intensity of intragastric blood flow permitting to define more accurately the indications for the choice of a method for surgical treatment has been developed. Supplementation of the selective proximal vagotomy with circular mucosectomy at the boundary between the antrum and body of a stomach enhances the effectiveness of operation due to reduction of the antral gastrin influence on the acid-producing zones of the stomach.
