RESUMO

Our objective was to determine the genotype of the dengue virus type 2 (DENV-2) that circulated in the Amazon region of Peru between November 2010 and January 2011. We analyzed eight samples collected during dengue surveillance activities in the cities of Iquitos, Yurimaguas, Trujillo, Tarapoto and Lima between November 2010 and January 2011 that were sent to Insitituto Nacional de Salud. The viruses were isolated in C6/36 HT cell line. Viral RNA was extracted and the serotype (RT - PCR multiplex) and genotype (RT-Nested PCR of the region E/NS1) were determined. Finally, the E/ NS1 amplicons were sequenced and analyzed by phylogeny. The phylogenetic analysis revealed the introduction of a different lineage which entered in Peru by the end of 2010. These isolates found in Iquitos and other cities in Peru are closely related to DENV-2 isolates that circulated in Brazil during 2007 and 2008, associated with severe dengue cases and deaths. In conclusion, we detected the introduction of a different lineage of DENV-2 America / Asia genotype in Peru that could be associated with the presence of more severe cases.

We conducted a clinical description of confirmed dengue cases admitted to the Hospital " César Garayar García" in Iquitos from January 25 to February 5, 2011. According to current WHO classification, major warning signs leading to hospitalization were: abdominal pain (28/28) and persistent vomiting (15/28), the causes of severity were: hypotension (9 / 13), severe bleeding (6 / 13) and plasma extravasation with respiratory distress (4 / 13). The median patient age was 22 years, however, 8 / 13 patients classified as severe dengue were under 16 years. In conclusion, unlike previous epidemics, we found a high involvement of young people and severity in the clinical presentation predominating a high frequency of shock (hypotension) and vascular leakage, which could be related to the circulation of an emerging variant DENV-2 which is more virulent.

RESUMO

We present the case of a full-term female newborn, whose mother died seven days postpartum from multi-organ failure due to severe dengue confirmed by NS1 antigen detection and positive IgM. The newborn did not have any complication, but at the fourth day of life she developed fever, jaundice, signs of plasma leakage, thrombocytopenia, hepatomegaly, ascitis, and others signs of systemic inflammation response syndrome. She fully recovered with supportive treatment. The RT-PCR test of a peripheral blood sample revealed a positive result for the dengue virus serotype 2, confirming the first case of neonatal dengue reported in Peru.

El objetivo del estudio fue determinar el genotipo del virus dengue tipo 2 (DENV-2) que circuló en la región Amazónica de Perú entre noviembre de 2010 y enero de 2011. Se analizaron ocho muestras de pacientes captados durante la vigilancia para dengue en las ciudades de Iquitos, Yurimaguas, Trujillo, Tarapoto y Lima entre noviembre de 2010 y enero de 2011 que fueron remitidas al Instituto Nacional de Salud. Se realizó el aislamiento viral en la línea C6/36 HT y la extracción del ARN viral. Se aplicaron técnicas de biología molecular para establecer el serotipo (RT - PCR múltiple) y genotipo (RT-Nested PCR de la región E/NS1) seguidas de secuenciación y análisis filogenético. El análisis filogenético reveló la introducción de un linaje diferente que ingresó a Perú a finales del 2010. Estos aislamientos encontrados en Iquitos y otras ciudades de Perú están muy relacionados con aislamientos de DENV-2 que circularon en Brasil durante el 2007 y 2008 asociados con casos de dengue grave y muertes. En conclusión se detectó la introducción de un linaje diferente del DENV-2 genotipo América/Asia en Perú que podría estar asociado con la presencia de casos más graves de dengue.

Se realizó una descripción clínica de pacientes con dengue confirmado internados en el Hospital de Apoyo de Iquitos "César Garayar García" desde el 25 de enero al 05 de febrero de 2011. Según la actual clasificación de la OMS, los principales signos de alarma que motivaron hospitalización fueron: dolor abdominal (28/28) y vómitos persistentes (15/28); las causas de gravedad fueron: hipotensión (9/13), sangrado grave (6/13) y extravasación de plasma con dificultad respiratoria (4/13). La mediana de edad de los pacientes fue 22 años, sin embargo, 8/13 pacientes clasificados como dengue grave fueron menores de 16 años. En conclusión, se evidencia una mayor afectación de la población joven y gravedad en la presentación clínica a diferencia de epidemias anteriores, predominando el shock (hipotensión) por extravasación vascular, lo cual podría estar relacionado con la circulación de una variante emergente del DENV-2 con mayor virulencia.

RESUMO

Se presenta el caso de un neonato de sexo femenino, a término, producto de madre fallecida al séptimo día de su puerperio, por falla multiorgánica debido a dengue grave confirmado por detección de antígeno NS1 e IgM. La recién nacida (RN) no tuvo complicaciones, pero a partir del cuarto día de vida desarrolló fiebre, ictericia, manifestaciones de extravasación de plasma, hepatomegalia, ascitis, plaquetopenia y otros signos de síndrome de respuesta inflamatoria sistémica. Su evolución final fue favorable con tratamiento instaurado. Se demuestra con PCR en tiempo real, la presencia del virus dengue serotipo 2 en sangre de la RN, confirmándose el primer caso de dengue neonatal reportado en el Perú.

Conocer la etiología del síndrome febril agudo en pacientes que acudieron a tres establecimientos de saludde la provincia de Jaén entre mayo de 2004 y abril de 2005. Materiales y métodos: Estudio descriptivo prospectivo realizado en tres establecimientos de salud: Hospital General de Jaén, Hospital de Apoyo Bellavista y Centro de Salud Morro Solar. Se incluyeron pacientes entre 5 y 65 años con fiebre de menos de ocho días de evolución y sin foco infeccioso aparente. Inicialmente se les realizó gota gruesa para malaria y frotis sanguíneo para Bartonelosis; de los casos negativos se obtuvo una segunda muestra de sangre para la búsqueda de ELISA IgM y microaglutinación para el diagnóstico de leptospirosis, ELISA IgM para dengue, Mayaro, Oropuche y encefalitis equina venezolana, e inmunofluorescenciaindirecta para Rickettsiosis. Resultados: De 1039 febriles incluidos, se determinó la etiología en 680(65,4 por ciento) casos, malaria por P. falciparum 312 (30,0 por ciento), leptospirosis 115 (11,1 por ciento), dengue 105 (10,1 por ciento), malaria por P.vivax 76 (7,3 por ciento), leptospirosis más dengue 30 (2,9 por ciento), Rickettsiosis 15 (1,4 por ciento), Bartonelosis 17 (1,6 por ciento), leptospirosis másRickettsiosis 7 (0,7 por ciento), y leptospirosis, dengue más Rickettsiosis 3 (0,3 por ciento). Los serovares de Leptospira más frecuentesfueron varilla (35,7 por ciento) y bratislava (32,5 por ciento). Conclusión: La malaria es la principal causa de síndrome febril agudo en Jaén, se destaca la presencia de la leptospirosis como segunda causa, por delante del dengue; es necesario considerar dentro del diagnóstico diferencial Rickettsiosis y Bartonelosis.


Objective: To know the etiology of acute febrile syndrome in patients presenting to three healthcare facilities in Jaen province,between May 2004 and April 2005. Materials and methods: A prospective and descriptive study was performed in three healthcare facilities:Jaen General Hospital,Bellavista Support Hospital and Morro Solar Health Center.Patients between 5 and 65 years were included.They had fever for less than eight days and with no apparent infectious etiology. Initially patients were screened with a thick smear blood test for malaria and a thin smear for Bartonellosis.Patients reported as negative for these test underwent a second blood test which included the following assays:IgM ELISA and microagglutination for leptospirosis,IgM ELISA for dengue fever,Mayaro,Oropuche and Venezuelan equine encephalitis, and indirect immunofluorescence for Rickettsiosis. Results: Of 1039 febrile patients included,a diagnosis was made in 80 65,4%)ases:P.alciparum malaria n 12 30,0%),eptospirosis in 15 11,1%),engue ever in 05 10,1%), P.vivax malaria in 76 (7,3%),leptospirosis and dengue fever in 30 (2,9%),rickettsiosis in 15 (1,4%),and leptospirosis plus dengue plus Rickettsiosis in 3 (0,3%).Most frequent Leptospira serovars were varilla (35,7%)and bratislava (32,5%). Conclusion: Malaria is the main cause of acute febrile syndrome in Jaen.It is also noteworthy the presence of leptospirosis in second place,above dengue fever;it is necessary to include this condition,as well as Bartonellosis and Rickettsiosis in the differential diagnosis.

Objetivos: Identificar mediante trascripción reversa-reacción en cadena de la polimerasa (RT-PCR) y sitios específicos de restricción - reacción en cadena de la polimerasa (RSS-PCR) al agente causal del brote epidémico presentado en el distrito de Comas, Lima en abril del año 2005. Materiales y métodos: veinte muestras de suero colectadas durante el brote de dengue fueron procesados por RT-PCR para determinar el serotipo, esta técnica se realizó en un solo paso. Luego se aplicó la técnica RSS-PCR para la identificación del genotipo circulante y se corroboraron los resultados posteriormente con aislamiento viral y secuenciamiento. Resultados: El análisis del RTPCR del ARN extraído de las muestras presentó un producto amplificado de 290pb que corresponden al dengue serotipo 3 (DEN 3). El análisis de los productos de RSS-PCR del ARN extraído a partir de aislamientos de DEN 3 correspondió al patrón C, incluido en el genotipo III. Los aislamientos de los virus dengue 3 en líneas celulares C6/36, tipificadas por IFI y el secuenciamiento genético confirmaron los resultados obtenidos por las pruebas previamentedescritas. Conclusión: Durante el brote epidémico de dengue clásico en Lima, circuló el genotipo III del virus DEN 3.


Objectives: To identify the causative agent of the outbreak that occurred in Comas, Lima in April 2005 using reverse transcription-polymerase chain reaction (RT-PCR) and specific restriction site-PCR (RSS-PCR). Materials and methods: 20 serum samples collected during the dengue outbreak were assessed using RT-PCR for identifying serotypes, this technique was performed in a single step. Later, the RSS-PCR method was used to identify the circulating serotypes, and the results were corroborated by viral isolation and sequencing. Results: RT-PCR of viral RNA taken from the samples showed a 290-pb amplified product corresponding to dengue virus serotype 3 (DEN 3). RSS-PCR product analysis of RNA from DEN 3 isolates corresponded to pattern C, included in genotype III. Dengue 3 virus isolates in C6/36 cell lines identified using indirect immunofluorescence and gene sequencing confirmed the results obtained using the aforementioned tests. Conclusion: During the classic dengue fever outbreak in Lima, DEN 3 genotype III circulated.
