OBJECTIVES: The aim of our study was to assess the proportion of high-grade histological lesions, according to HPV type, in patients referred for colposcopy involving a positive HPV-HR test and ASC-US cytology. METHODS: This is a retrospective descriptive study of asymptomatic patients aged 25 to 65 with a positive HPV-HR test and ASC-US cytology. Data were collected at the Nord Franche-Comté Hospital from September 2019 to February 2022. The primary outcome was the proportion of high-grade histological lesions in patients with ASC-US cytology combined to HPV 16 or 18 (associated or not with other HPV type), compared with HPV no 16 no 18. RESULTS: Among the 298 patients included, 67% were HPV no 16 no 18, 22% HPV 16 associated or not with others and 11% HPV 18 associated or not with others. We found significantly fewer high-grade lesions in patients with ASC-US cytology and HPV no 16 no 18 than in patients with HPV 16 or 18 (8.5% versus 22.7%, P<0.01). In patients presenting an ASC-US cytology with HPV no 16, no 18: 53% of the biopsies found no histological lesion compared with 30% for the other HPVs (P<0.01). CONCLUSIONS: We have highlighted the reality of colposcopists: a high ratio of normal coloscopy associated with normal or low-grade biopsies, in patients with a positive HPV-HR test no 16, no 18 and ASC-US cytology.

RESUMO

We conducted a retrospective study of pregnant persons hospitalized for severe acute respiratory syndrome coronavirus 2 infection in France. Delta variant infection had a relative risk of 14.33 for intensive care unit admission and 9.56 for high supplemental oxygen support. The Delta variant might cause more severe illness during pregnancy.

INTRODUCTION: The SBra (Smart Bra) project aims to develop an intelligent bra, combining sensors for measuring skin temperature and the electrical impedance of breast tissue, which could be used for breast cancer screening. The objective of this study is to anticipate both the hindrances to usage and acceptability of SBra with respect to the breast cancer screening practices of healthcare professionals and patients, and then to propose ways to modify the shape and functions of the device to facilitate its potential insertion into the healthcare system. METHODS: A qualitative survey was conducted between September 2019 and December 2020, consisting of a series of interviews conducted with hospital and private healthcare professionals (N = 22) working in Burgundy-Franche-Comté and related to breast cancer, and with women aged 38 to 74 years old living in Burgundy-Franche-Comté and Auvergne-Rhône-Alpes (N = 21) who have or have not had breast cancer, and who either practice or refuse screening. RESULTS: If patients say they are ready to use such a device, at most once a year, and subject to its usability, the majority of them prefer an examination in the office, performed by a gynecologist or a general practitioner. Health professionals point out that this option generates institutional (remuneration and cost of the procedure) and organizational needs, which are both material and human. DISCUSSION: The study highlights the need to pluralize the system in order to respond to the multiplicity of local situations.

RESUMO

Schistosomiasis is the second endemic parasitic disease in the world and is a common cause of urogenital infections. Ectopic pregnancies due to tubal obstruction by schistosoma's eggs are usually reported in Africa. Schistosomiasis also affects travelers but infection of the female genital tract is less frequently described. We report an unusual clinical case of two successive ectopic pregnancies with tubal schistosomiasis in a French woman, seven years after a travel to Mali. The first event was discovered after histologic examination of salpingectomy and the second event required a controlateral salpingotomy with an injection of methotrexate, two months later.

The ability to temporarily maintain information in order to successfully perform a task is important in many daily activities. However, the ability to quickly and accurately update existing mental representations in distracting situations is also imperative in many of these same circumstances. In the current studies, individuals varying in working memory capacity (WMC) performed different varieties of go/no-go tasks that have been hypothesized to measure inhibitory ability. The results indicated that low-WMC individuals relative to high-WMC individuals showed worse performance specifically in certain conditions of the conditional go/no-go task. Further analyses showed that increasing the temporal lag/number of intervening items between the previous target and the current lure had a deleterious effect on the performance of the low-WMC group only. The results indicate a relationship between WMC and the ability to selectively update, maintain, and retrieve information, especially in interference-rich conditions.

Knowledgeable and confident teachers are crucial for students with epilepsy. In this study, 91 current teachers of a student with epilepsy (CTs) and 203 teachers in general (TiGs) were surveyed using a new scale. CTs knew more school-relevant epilepsy facts than TiGs, even after controlling for special education background, F(1, 293)=5.75, P=0.017, η(2)=0.02. Both groups of teachers, however, knew less than one-half of the facts (means=10.6 [CTs] 8.7 [TiGs] of 25 items). CTs also expressed greater confidence than TiGs in their ability to meet an array of instructional, safety, and psychosocial requirements, even when between-group differences in teachers' background in special education were controlled, F(1, 293)=34.97, P<0.001, η(2)=0.11. Still, neither group communicated a high absolute level of confidence. As expected, more knowledgeable teachers expressed greater confidence (r=0.43, P<0.001). Results suggest that some facts about epilepsy require additional dissemination to educators.

Flat epithelial atypia (FEA) is recognized as a precursor of breast cancer and its management (surgical excision or intensive follow-up) remains unclear after diagnosis on core needle biopsy (CNB). The aim of this study was to determine the underestimation rate of pure FEA on CNB and clinical, radiological, and pathological factors of underestimation. 4,062 CNBs from 5 breast cancer centers, performed over a 5-year period, were evaluated. A CNB diagnosis of pure FEA was made in 60 cases (1.5%) (the presence of atypical ductal hyperplasia, lobular neoplasia, radial scars, phyllodes tumor, papillary lesions, ductal carcinoma in situ or invasive carcinoma at CNB were exclusion criteria), and subsequent surgical excision was systematically performed. The histological diagnosis was retrospectively reviewed using standardized criteria and the precise terminology of the World Health Organization by two pathologist physicians. At surgical excision, 6 (10%) ductal carcinoma in situ and 2 (3%) invasive carcinoma were diagnosed. The total underestimation rate was 13%. FEA was associated with atypical ductal hyperplasia in 10 (17%) cases and with lobular neoplasia in 2 (3%) at final pathology. Residual FEA was found in 14 (23%) cases. No clinical, radiological or pathological factors were significantly associated with underestimation. Our data highlight the importance of recognizing and diagnosing FEA in core needle biopsies. Thus, the presence of FEA on CNB, even in isolation, warrants follow-up excision.
