Aim: The aim is to evaluate the relationship between C-reactive protein (CRP) to albumin ratio (CAR) and radial artery thrombosis in patients undergoing radial angiography. Patients & methods: We prospectively included 261 consecutive patients undergoing radial angiography, assessing radial artery diameter and thrombosis presence. Results: The CRP values were significantly higher in radial artery thrombosis group compared with group without thrombosis (13.01 vs. 4.33 mg/l, p < 0.001, respectively). Also CAR was statistically significantly different between the group with thrombosis and the group without thrombosis (0.102 vs. 0.349, p < 0.001). Conclusion: Our study is the first to assess CAR in radial thrombus development post-procedure in patients undergoing radial angiography. CAR can be useful in determining radial artery thrombosis after the coronary angiography.

AIM: This study aimed to identify the effects of a web-based birth preparation programme based on the Health Belief Model and supported by motivational interviews on fear of childbirth, self-efficacy and the labour process in primigravida women. METHODS: The study was conducted in a single-blind, randomized controlled design with 73 pregnant women. The Birth Health Belief Scale, the Wijma Delivery Expectation/Experience Questionnaire A-B, the Childbirth Self-efficacy Scale and the Labour Process Assessment forms were used to collect data. Five sessions of motivational interviews were held with the pregnant women in the experimental group, while those in the control group were administered placebo education. RESULTS: Fear of birth was lower, and the birth self-efficacy total score was higher in the experimental group. None of the groups indicated significant differences in terms of their mode of delivery. CONCLUSION: Health Belief Model-based web-based birth preparation programme and motivational interviews were found to reduce the fear of birth, increase self-efficacy and positively affect the birth process in primigravida women.

Purpose: The purpose of this study is to determine the relationship between female university students' breast cancer risk factors and their health beliefs about breast self-examination (BSE). Methods: The sample of this descriptive and correlational study was 389 female students who were determined by stratified sampling method. Data were collected through the Socio-demographic Characteristics and Breast Cancer Risk Factors Determination Form and the Champion's Health Belief Model Scale (CHBMS). Data obtained from the study were analysed using SPSS for Windows 16.0 program. Data analysis was performed using percentage distributions, z test, and Spearman correlation analysis. Statistical significance was accepted p<0.05. Results: While a positive correlation was detected between students' perceived breast cancer risk for themselves and perceived susceptibility, severity, benefit, self-efficacy related to BSE and health motivation mean scores, a negative and significant correlation was detected with perceived barriers to BSE mean score (p<0.05). Conclusion: Results showed that although female university students had some risk factors in terms of breast cancer, they have low levels of perceived risk factors for themselves.

RESUMO

In this clinical vignette, we describe a case of a patient with windsock-like posterior mitral annulus perforation at the site of annular calcification caused by infective endocarditis. Three-dimensional transesophageal images of the perforation resembling an "alien's mouth" are very striking for how 3-dimensional imaging can improve visualization of anatomy of the heart. BACKGROUND: Accessory mitral valve tissue (AMVT) is an extremely rare causes left ventricular outflow tract (LVOT) obstruction and is usually incidentally detected in childhood. It is often associated with other cardiac and vascular congenital malformations. CASE PRESENTATION: In this case, we present a 15-year-old girl was diagnosed with AMVT by transesophageal echocardiography, resulting in LVOT obstruction during systole. Interestingly enough, the patient's accessory mitral valve remained undetected for years until he became symptomatic for wide ASD. Successful closure of the ASD with resection of the AMVT was performed with a transaortic approach. The patient was hemodynamically stable postoperatively. There were no abnormalities in the mitral valves and LVOT. CONCLUSION: It was also unusual to see AMVT with ASD instead of other frequently associated other congenital anomalies. Accessory mitral valve should be considered a rare but important cause of left ventricular outflow tract obstruction in childhood.

RESUMO

Pacemaker-associated superior vena cava (SVC) syndrome is increasingly recognized as a nonmalignant type of SVC syndrome. Computed tomographic (CT) angiography is the first choice of imaging for diagnosis but in some cases there is discrepancy between the clinical findings and CT images. In this report we present the role of contrast study with transesophageal echocardiography in a patient with suspicion of SVC syndrome but inconclusive CT findings.

Objective: To investigate the effects of sexual education provided via WhatsApp on the level of sexual knowledge and sexual myths in midwifery students in Turkey. Methods: Students were randomly divided into WhatsApp education, conventional education, and control groups. Data were collected using a Questionnaire Form, the Sexual Health Knowledge Test, and the Sexual Myth Scale. Results: Post-education and one-month-follow-up the Sexual Health Knowledge Test medians in the control group (24.0 and 27.0 respectively) were significantly lower than those of the WhatsApp (31.0 and 32.5 respectively) and conventional education groups (30.5 and 32.0 respectively) and Sexual Myth Scale medians in the control group (70.0 and 67.0 respectively) were significantly higher than those of the WhatsApp (55.5 and 60.0) and conventional education groups (53.0 and 55.0, respectively) (p < .001). There was no difference between WhatsApp and conventional education groups in terms of the Sexual Health Knowledge Test and Sexual Myth Scale scores (p > 0.05). Conclusion: Sexual education provided by conventional methods and via WhatsApp increased sexual health knowledge and decreased sexual myths.

RESUMO

Long-standing left-to-right shunting across a patent ductus arteriosus (PDA) can result in Eisenmenger syndrome. In this report, we present echocardiographic findings of a 27-year-old female patient with pulmonary hypertension. In diagnostic work-up especially Doppler findings of the pulmonary artery suggested the presence of a reversed PDA as a cause of pulmonary hypertension. The diagnosis was confirmed by contrast study and computed tomography.

Objective: Fragmented QRS (fQRS) detected on a 12-lead electrocardiogram (ECG) has been demonstrated to be a marker of myocardial fibrosis. Intercellular adhesion molecule-1 (ICAM-1) is a protein which plays an important role in fibro-inflammatory processes. In this study, we aimed to investigate the relationship between ICAM-1 levels and the presence of fQRS in hypertensive patients.Methods: Ninety consecutive patients who were diagnosed with hypertension were included in the study. ECG and transthoracic echocardiography were performed to all patients. fQRS was defined as additional R' wave or notching/splitting of S wave in two contiguous ECG leads. Serum ICAM-1 levels were measured using the enzyme-linked immunosorbent assay method. Patients were divided into two groups according to the presence of fQRS.Results: A total of 90 patients (female, 65%; mean age: 54.6 ± 8.5 years) were included in the study. fQRS was detected on ECG recordings of 47 (52.2%) patients. The demographic characteristics were similar between the groups. Left atrial diameter (p = .003), interventricular septal thickness (p = .013), posterior wall thickness (p = .01), left ventricular mass (p = .002), left ventricular mass index (p < .001), left ventricular hypertrophy (p = .001), and ICAM-1 levels (p < .001) were found to be significantly increased in fQRS(+) group. In multivariate analysis, only high ICAM-1 level was observed to be an independent predictor for the presence of fQRS (odds ratio: 1.029; 95%Confidence Interval: 1.013-1.045, p < .001).Conclusion: A significant association exists between serum ICAM-1 levels and the presence of fQRS in hypertensive patients. The presence of fQRS may be used as an indicator of inflammation in hypertensive patients.

RESUMO

Coronary sinus atrial septal defect (ASD) is a rare congenital cardiac anomaly, which might be difficult to diagnose. In this report, we describe a patient with small secundum ASDs and an associated large coronary sinus ASD, which had been missed at initial evaluation. The diagnosis of coronary sinus ASD was established by using transesophageal echocardiography after percutaneous closure of a small secundum ASD at another center. Patient underwent corrective surgery.

BACKGROUND: Epicardial adipose tissue (EAT) is a cardiometabolic risk factor, and its possible relationship with hypertension has been reported previously. Fragmented QRS (fQRS) detected on electrocardiography (ECG) has been demonstrated to be a marker of myocardial fibrosis. In this study, we aimed to investigate the relationship between the thickness of EAT, and presence of fQRS in hypertensive patients. METHODS: Consecutive patients who were diagnosed with hypertension were included in the study. ECG and transthoracic echocardiography (TTE) were performed to all patients. fQRS was defined as additional R' wave or notching/splitting of S wave in two contiguous ECG leads. Thickness of EAT was measured by TTE. RESULTS: This study enrolled 69 hypertensive patients with fQRS on ECG and 45 hypertensive patients without fQRS as the control group. Age (P = .869), and gender distribution (P = .751) were similar in both groups. Left atrial diameter (P = .012), interventricular septal thickness (P < .001), posterior wall thickness (P < .001), left ventricular ejection fraction (P = .009), left ventricular mass (P = .006), left ventricular mass index (P = .014), left ventricular hypertrophy (P = .003), and EAT thickness (P < .001) were found to be significantly increased in patients with fQRS. In multivariate analysis, among these variables only EAT was observed to be an independent predictor of fQRS (odds ratio:3.306 [95% confidence interval, 0.030-0.118], P = .001). CONCLUSION: A significant association exists between the presence of fQRS and EAT thickness in hypertensive patients. The presence of fQRS, just as EAT thickness, may be used as a cardiometabolic risk factor in hypertensive patients.

PURPOSE: The research was conducted to determine the effects of peer education model on sexual myths of Turkish university students. DESIGN AND METHODS: This is an interventional study. Peer educators were asked to perform educational activities for the intervention group. The participants in the control group were not subjected to any interventions. Data collection tools were simultaneously administered to all students in the intervention (n = 646) and control groups ( n = 532). FINDINGS: The Peer Education Program Pertaining to Sexual Myths applied to the interventon group by peer educators is effective in reducing sexual myths and increasing sexual knowledge level. PRACTICE IMPLICATIONS: Peer education model should be integrated into the health education programs by nurses to improve the sexual health and reproductive health of young people.

Objective: This study determined the effect of an educational program based on cognitive behavioral techniques (CBT) provided for pregnant women to address their fear of childbirth. Methods: This study was conducted experimentally using pretest-posttest and a control group. A total of 111 Turkish pregnant women were recruited with 52 in the intervention group and 59 in the control group. The intervention group attended the educational program on coping with childbirth fears based on CBT, which took 3 weeks over six sessions. The Wijma Delivery Expectancy/Experience Questionnaire, version-A (W-DEQ-A) was administered to both groups as the posttest. The researchers monitored the pregnant women's labor and delivery and the State Anxiety Inventory (SAI), Numeric Pain Rating Scale (NPRS) and Birth Process Evaluation Form were completed. Results: The post-education W-DEQ-A score was 63.5 in the control group and 39.4 in the intervention group (p < .001). Their labor pain was lower, the second stage of labor was shorter and birth was more satisfactory for the intervention group than for the control group (p < .05). The CBT-based educational program and pain severity during childbirth were the significant predictors of satisfaction with childbirth (ß = 0.354; ß = -0.324, respectively; p < .05 for all). Conclusions: The CBT-based educational program on coping with childbirth fears reduced pregnant women's fear of childbirth and positively affected their birth experience.

Diabetes mellitus (DM) is associated with more extensive coronary atherosclerosis and more vulnerable plaque phenotypes. However, DM should not be considered a homogeneous and purely binary entity in terms of risk assessment. We evaluated the impact of prediabetic status on coronary atherosclerosis burden in patients with first-time acute coronary syndrome (ACS) who underwent urgent coronary angiography. The patients were divided into DM, prediabetes, and control groups. The 3-vessel disease (TVD) rates and SYNTAX and Gensini scoring systems for defining atherosclerotic burden were compared. The study was conducted in 469 consecutive patients admitted with a diagnosis of ACS. Of these, 250 patients were admitted at the first occurrence of ACS undergoing diagnostic coronary angiography. SYNTAX and Gensini scores and TVD rates were higher in prediabetic patients than in nondiabetic patients ( P = .004, P = .008, and P = .014, respectively), but similar in prediabetic and diabetic patients ( P = .912, P = .773, and P = 1.000, respectively). Coronary atherosclerosis burden is more advanced in prediabetic patients than in nondiabetic patients and is comparable between prediabetic and diabetic patients at first presentation of ACS. Cardiologists should not miss the opportunity to diagnose prediabetes and DM when patients present with an ACS.
