RESUMO

Background: Almost 80% of global tobacco usage is concentrated in low- and-middle-income countries (LMICs) like India. Added to this, there is dearth of dedicated tobacco cessation specialist services in healthcare settings in these regions. Identification of challenges in the delivery of cessation interventions and understanding the experiences of tobacco users will aid in formulating successful quit strategies. Objectives: This qualitative study in India aimed to understand the perspectives of tobacco-using patients in healthcare facilities regarding tobacco use and cessation. Methods: This qualitative study was conducted in urban and rural areas of four study sites, two each in the North and South India. Using purposive sampling, patients who were tobacco users were selected from healthcare facilities. The interviews were transcribed, coded and organised into themes. Analysis was done using NVivo 10 software. Results: A total of 22 in-depth interviews were conducted on participants aged 23 to 80 years who were either current or past tobacco users. A majority of the participants were aware of their increased health risks associated with tobacco consumption and had attempted quitting; however, barriers such as peer influence, formed habit, certain cultural barriers and the addictive nature of nicotine prevented them from successfully quitting. Familial and peer support, the government's role in spreading public awareness, and limiting the sale of tobacco were stated as facilitators for tobacco cessation. Conclusions: The findings of this study point out that despite awareness of the perils of tobacco among smokers, there are various barriers and beliefs related to tobacco use and cessation. These findings would prove advantageous for policy-makers to implement and promote addiction treatment programmes for successful tobacco cessation efforts. In order to optimise strategies, policies must be well informed by ongoing dialogue between the public, service providers and policy-makers.

Salmonella enterica serovar Typhimurium DT160 was the predominant cause of notified human salmonellosis cases in New Zealand from 2000 to 2010, before it was superseded by another S. Typhimurium strain, DT56 variant (DT56v). Whole genome sequencing and phenotypic testing were used to compare 109 DT160 isolates with eight DT56v isolates from New Zealand animal and human sources. Phylogenetic analysis provided evidence that DT160 and DT56v strains were distantly related with an estimated date of common ancestor between 1769 and 1821. The strains replicated at different rates but had similar antimicrobial susceptibility profiles. Both strains were resistant to the phage expressed from the chromosome of the other strain, which may have contributed to the emergence of DT56v. DT160 contained the pSLT virulence plasmid, and the sseJ and sseK2 genes that may have contributed to the higher reported prevalence compared to DT56v. A linear pBSSB1-family plasmid was also found in one of the DT56v isolates, but there was no evidence that this plasmid affected bacterial replication or antimicrobial susceptibility. One of the DT56v isolates was also sequenced using long-read technology and found to contain an uncommon chromosome arrangement for a Typhimurium isolate. This study demonstrates how comparative genomics and phenotypic testing can help identify strain-specific elements and factors that may have influenced the emergence and supersession of bacterial strains of public health importance.

RESUMO

Extended-spectrum beta-lactamase (ESBL)-producing Enterobacteriaceae are considered a critical priority by the World Health Organization. Presented here are two genome sequences of Escherichia coli strains isolated from New Zealand freshwater. The genome sequences' mean size was 5.2 Mb, with a mean of 4,848 coding sequences. Both genomes carried the ESBL blaCTX-M gene.

Non-reactive phosphorus (nRP) contains condensed phosphates and organic phosphorus (OP) species that are recalcitrant in secondary wastewater treatment and tend to remain in final effluents. To meet ultra-low effluent P discharge limits, persistent nRP must be removed. The objective of this study was to evaluate the use of an advanced oxidation process (AOP) which couples TiO2/UV photolysis with ultrafiltration to oxidize and remove nRP species. Initial tests utilized OP model compounds, adenosine triphosphate (ATP) and aminoethylphosphonate (AEP), in a binary mixture to evaluate AOP treatment and to elucidate possible mechanisms of phosphorus removal. The results were consistent with a model of preferential ATP binding to the TiO2 surface compared to AEP. On UV light exposure, AEP was removed from solution due to the photooxidation of ATP freeing up binding sites for AEP adsorption and subsequent oxidation. Orthophosphate released during AOP treatment was retained on the TiO2 solids. The AOP was applied to three municipal wastewaters and one automotive industry effluent for P removal. In all cases, phosphorus removal was found to occur through filtration, surface complexation and UV oxidation. Total phosphorus removal efficiencies between 90 and 97% were observed for the municipal wastewater effluents and 44% removal was observed in the industrial effluent after treatment using AOP.

The purpose of this study was to evaluate the quasi-static compressive strength and the compressive fatigue limit of four different dental restorative materials, before and after aging in distilled water for 30 days. A conventional glass ionomer cement (Fuji IX GP; IG), a zinc-reinforced glass ionomer cement (Chemfil rock; CF), a light curable resin-reinforced glass ionomer cement (Fuji II LC; LC) and a resin-based composite (Quixfil; QF) were investigated. Cylindrical specimens (4mm in diameter and 6mm in height) were prepared according to the manufacturer׳s instructions. The compressive fatigue limit was obtained using the staircase method. Samples were tested in distilled water at 37°C, at a frequency of 10Hz with 10(5) cycles set as run-out. 17 fatigue samples were tested for each group. Two-way ANOVA and one-way ANOVA followed by Tukey׳s post-hoc test were used to analyze the results. Among the four types of materials, the resin-based composite exhibited the highest compressive strength (244±13.0MPa) and compressive fatigue limit (134±7.8MPa), followed by the light-cured resin reinforced glass ionomer cement (168±8.5MPa and 92±6.6MPa, respectively) after one day of storage in distilled water. After being stored for 30 days, all specimens showed an increase in compressive strength. Aging showed no effect on the compressive fatigue limit of the resin-based composite and the light-cured resin reinforced glass ionomer cement, however, the conventional glass ionomer cements showed a drastic decrease (37% for IG, 31% for CF) in compressive fatigue limit. In conclusion, in the present study, resin modified GIC and resin-based composite were found to have superior mechanical properties to conventional GIC.

Innovative interventions increasing linkage, adherence, and retention in care among HIV-infected persons in the criminal justice system are needed. The authors conducted a qualitative study to investigate technology-based tools to facilitate linkage to community-based care and viral suppression for HIV-infected jail detainees on antiretroviral medications being released to the community. The authors conducted 24 qualitative interview-12 in Rhode Island and 12 in Washington, DC-among recently incarcerated HIV-infected persons to elicit their perceptions on the use of technology tools to support linkage to HIV care among criminal justice populations. This article discusses participants' perceptions of the acceptability of technological tools such as (a) a computer-based counseling and (b) text messaging interventions. The participants reported positive experiences when previewing the technology-based tools to facilitate linkage to HIV care and adherence to HIV medications. Successful linkage to care has been shown to improve HIV-associated and non-HIV-associated health outcomes, as well as prevent criminal recidivism and facilitate reentrants' successful and meaningful transition. These findings can be used to inform the implementation of interventions aimed at promoting adherence to antiretroviral medications and linkage to care for HIV-infected persons being released from the correctional setting.

Acute copper (Cu) toxicity tests (48-h LC50) using the euryhaline rotifer Brachionus plicatilis were performed to assess the effects of salinity (3, 16, 30 ppt) and dissolved organic carbon (DOC, ∼ 1.1, ∼ 3.1, ∼ 4.9, ∼ 13.6 mg C L(-1)) on Cu bioavailability. Total Cu was measured using anodic stripping voltammetry, and free Cu(2+) was measured using ion-selective electrodes. There was a protective effect of salinity observed in all but the highest DOC concentrations; at all other DOC concentrations the LC50 value was significantly higher at 30 ppt than at 3 ppt. At all salinities, DOC complexation significantly reduced Cu toxicity. At higher concentrations of DOC the protective effect increased, but the increase was less than expected from a linear extrapolation of the trend observed at lower concentrations, and the deviation from linearity was greatest at the highest salinity. Light-scattering data indicated that salt induced colloid formation of DOC could be occurring under these conditions, thereby decreasing the number of available reactive sites to complex Cu. When measurements of free Cu across DOC concentrations at each individual salinity were compared, values were very similar, even though the total Cu LC50 values and DOC concentrations varied considerably. Furthermore, measured free Cu values and predicted model values were comparable, highlighting the important link between the concentration of bioavailable free Cu and Cu toxicity.

Laysan Albatrosses (Phoebastria immutabilis) and Black-footed Albatrosses (P. nigripes) ingest plastic debris, as evidenced by studies showing plastic in the digestive contents of their chicks, but there is little documentation of the frequency and amount of ingested plastics carried in foraging adults. In this study, we quantify plastics among the digestive contents of 18 Laysan Albatrosses and 29 Black-footed Albatrosses collected as by-catch in the North Pacific Ocean. We found ingested plastic in 30 of the 47 birds examined, with Laysan Albatrosses exhibiting a greater frequency of plastic ingestion (83.3% n=18) than Black-footed Albatrosses (51.7% n=29) (X(2)=4.8, df=1, P=0.03). Though the mass of ingested plastic in both species (mean±SD=0.463g±1.447) was lower than previously noted among albatross chicks, the high frequency of ingested plastic we found in this study suggests that long-term effects, e.g. absorption of contaminants from plastics, may be of concern throughout the population.

Dissolved organic nitrogen (DON) accounts for a large fraction of the total nitrogen discharged to surface waters by municipal wastewater treatment plants designed for biological nutrient removal (BNR). Previous research indicates that some but not all of the DON in wastewater effluent is available to bacteria and algae over time scales that are relevant to rivers and estuaries. To separate bioavailable DON from nitrate and less reactive DON species, an XAD-8 resin coupled with an anion exchange treatment was employed prior to chemical analysis and algal bioassays. Analysis of effluent samples from a range of municipal BNR plants (total DON concentrations ranging from 0.7 to 1.8 mg N/L) employing a range of technologies indicated that hydrophilic DON, which typically accounted for approximately 80% of the total DON, stimulated algal growth, whereas hydrophobic DON, which accounted for the remaining DON, remained at nearly constant concentrations and had little or no effect on algal growth during a 14-day incubation period. The hydrophobic DON exhibits characteristics of humic substances, and is likely to persist for long periods in the aquatic environment. The distinct differences between these two classes of DON may provide a basis for considering them separately in water quality models and effluent discharge regulations.

To understand the spatial variation in concentrations and compositions of organic micropollutants in marine plastic debris and their sources, we analyzed plastic fragments (∼10 mm) from the open ocean and from remote and urban beaches. Polychlorinated biphenyls (PCBs), polycyclic aromatic hydrocarbons (PAHs), dichloro-diphenyl-trichloroethane and its metabolites (DDTs), polybrominated diphenyl ethers (PBDEs), alkylphenols and bisphenol A were detected in the fragments at concentrations from 1 to 10,000 ng/g. Concentrations showed large piece-to-piece variability. Hydrophobic organic compounds such as PCBs and PAHs were sorbed from seawater to the plastic fragments. PCBs are most probably derived from legacy pollution. PAHs showed a petrogenic signature, suggesting the sorption of PAHs from oil slicks. Nonylphenol, bisphenol A, and PBDEs came mainly from additives and were detected at high concentrations in some fragments both from remote and urban beaches and the open ocean.
