RESUMO

OBJECTIVE: To study the state of the psycho-emotional sphere and the quality of life in patients with vestibular migraine. MATERIAL AND METHODS: The study included 56 patients (10 men and 46 women), aged 18 to 50 years, with vestibular migraine and a control group (patients with migraine without au-ra). The study of neurological status, features of the psychoemotional sphere, accentuations of character and temperament of the individual, quality of life was carried out. The Beck Depres-sion Inventory, the Spielberger-Khanin State-Trait Anxiety Inventory test, the K. Leonhard - H. Schmischek Inventory test and the Vestibular Rehabilitation Benefit Questionnaire were admin-istered. RESULTS: The following features between two groups were revealed: absence of significant differences in trait anxiety, statistically significant differences in state anxiety and the severity of depressive symptoms, as well as significant variations in the spectrum of personality accentuations and a lack of quality of life. CONCLUSION: The results are relevant and important in the field of management of patients with vestibular migraine and allow us to focus due attention on psychoemotional distinctiveness and lack of quality of life in this debilitating pathology, for the possibility of presenting the nec-essary individual strategy to overcome the disease.

OBJECTIVE: To study the clinical features and the possibility of objectification of vestibular dysfunction in patients with vestibular migraine. MATERIAL AND METHODS: The study included 46 patients (10 men and 36 women) aged 18 to 50 years, with vestibular migraine and control groups. A targeted collection of complaints was carried out with an assessment of vestibular symptoms; a detailed history; a study of neurological status, with an emphasis on the study of oculomotor reactions, vestibular tests. Functional computer stabilometric tests were used to objectify vestibular dysfunction. RESULTS: Clinical features, as well as features of stabilometric parameters in patients with vestibular migraine have been identified, which can be used to obtain a quantitative assessment of vestibular dysfunction and objectification of vertigo in this category of patients. CONCLUSION: A purposeful and complete collection of complaints and anamnesis detailing the features of the symptoms in patients with vestibular migraine helps to identify the clinical polymorphism of the pathology in question and contributes to the correct diagnosis. And the use of functional stabilometric tests contributes to the objectification of vestibular dysfunction in this category of patients, quantifying the distinctive features of the balance system in vestibular migraine.

RESUMO

The purpose of the study was to compare the effectiveness of non-drug treatment methods (vestibular rehabilitation, ketogenic diet) for leveling vestibular dysfunction in elderly patients with vestibular migraine. The study included elderly patients with vestibular migraine. In the first group (28 patients), vestibular gymnastics was used to correct vestibular dysfunction, in the second group (28 patients), a ketogenic diet served as a non-pharmacological method of exposure and a control group (28 patients) who did not receive non-drug correction measures. The study showed that vestibular gymnastics for the correction of vestibular dysfunction is comparable in effectiveness with a ketogenic diet in elderly patients with vestibular migraine.

RESUMO

The purpose of the study was to study the possibility of computer stabilometric diagnostics for the objectification of vertigo in elderly patients with vestibular migraine. The study included 57 elderly patients with vestibular migraine and 57 elderly patients with central vestibular vertigo, without migraine. Specialized tests of functional computer stabilometry were used to objectify vertigo. The study revealed the features of the basic stabilometric parameters in specialized tests in elderly patients, which can be used to obtain a quantitative assessment of vestibular dysfunction and objectification of vertigo.

RESUMO

OBJECTIVE: To study characteristics of migraine-associated vertigo using a specially-developed complex program of stabilometry. MATERIAL AND METHODS: We examined 56 patients with vestibular migraine and 56 healthy controls. We used a set of tests informative for verification of the pathology of the vestibular system (examination in the Romberg position with opened and closed eyes, optic/kinetic test, test with turns and inclinations of head, tandem test). RESULTS AND CONCLUSION: Based on significant differences in stabilometric parameters that are characteristic of the main group, we concluded that computer stabilometry can be used for diagnosis of migraine-associated vertigo.

RESUMO

The results of the study opening up new possibilities for differential diagnostics of vestibular and psychogenic dizziness are presented. A specially developed combined approach was used including a set of highly informative stabilometric tests for the verification of pathological changes in the vestibular analyzer, such as Romberg test with both open and closed eyes, tandem test, head turn and tilt tests. The groups of the subjected enrolled in the study were shown to possess statistically different stabilometric characteristics.

A clinical-electroneuromyographical study of 40 children (32 (80%) of them aged from 12 to 17 years, mean age 13,9+/-1,8 years, and 8 (20%) - from 1 to 8 years, mean age 4,4+/-2,1 years) were studied in the acute period of facial nerve paralysis (FNP). Six (15%) children had FNP in the anamnesis. Among precipitating factors were the cold exposure the day before disease onset (20 (50%) patients), symptoms of flu (13 (32,5%) patients) and psycho-emotional tension (3 (7,5%) patients). No precipitation was noted in 4 (10%) children. The degree of muscle paresis was 81,9+/-7% that corresponded to clinical stages III-IV according to K. Rosler. An electroneuromyographical analysis of motor ortho- and antidromic response to the facial nerve stimulation on the side of paresis and on the contralateral side in patients and controls revealed the presence of proximal axon- and myelinopathy of facial nerve with the involvement of its own motorneurons and brain stem interneurons. The maintenance of wink reflex and F-wave blocks in the period over 3 weeks are prognostically unfavorable factors for restoration of mimic muscle function in the early stage of disease.

To carry out a differential diagnosis of two facial dyskinesia (FD) models--facial hemispasm (FH) and facial paraspasm (FP), a combined program of electroneuromyographic (ENMG) examination has been created, using statistical analyses, including that for objects identification based on hybrid neural network with the application of adaptive fuzzy logic method and standard statistics programs (Wilcoxon, Student statistics). In FH, a lesion of peripheral facial neuromotor apparatus with augmentation of functions of inter-neurons in segmental and upper segmental stem levels predominated. In FP, primary afferent strengthening in mimic muscles was accompanied by increased motor neurons activity and reciprocal augmentation of inter-neurons, inhibiting motor portion of V pair. Mathematical algorithm for ENMG results recognition worked out in the study provides a precise differentiation of two FD models and opens possibilities for differential diagnosis of other facial motor disorders.

RESUMO

In the study performed the authors focused on ICR pathogenesis and symptoms specification, improvement of ICR diagnosis and pathogenetic conservative treatment. Clinical manifestations of the disease were ascertained resting on x-ray, electroneuromyography and thermotopography evidence on 77 ICR patients. A key role of dyscirculatory ischemia in the disease origin is confirmed. The patients were divided into two groups: those with initial manifestations and those with apparent cervical radiculomyelopathy. The authors introduced an original approach to the disease treatment: the exposure of the vertebral arteries to diadynamic current adjusted to the patient's heart rate. The response to the treatment was achieved in all the patients and was confirmed by instrumental findings.
