RESUMO

Objective: This study aimed to compare the audiological characteristics between children with unilateral auditory neuropathy (UAN) and single-sided deafness (SSD) to establish a valid basis for the differential diagnosis of children with UAN. Methods: A retrospective analysis was conducted on audiological and imaging evaluations of children with UAN and SSD who were treated at Beijing Children's Hospital of Capital Medical University between May 2015 and June 2023. There were 17 children with UAN, comprising 10 males and 7 females, with an average age of 4.7 years. Additionally, there were 43 children with SSD, consisting of 27 males and 16 females, with an average age of 6.5 years. Audiological assessments included Auditory brainstem response (ABR), Steady-state auditory evoked potential (ASSR), Behavioural audiometry, Cochlear microphonic potential (CM), Distortino-product otoacoustic emission (DPOAE), and acoustic immittance test. The results of the audiological assessment and imaging phenotypic between the two groups of children were compared and analyzed by applying SPSS 27.0 statistical software. Results: (1) The UAN group (77.8%) had a significantly higher rate of ABR wave IIIL than the SSD group (20.9%) (P<0.01). The PA thresholds at 500 Hz and 1 000 Hz of children with SSD were higher than those of children with UAN, while the ASSR thresholds at 500 Hz, 1000 Hz, 2 000 Hz, and 4 000 Hz of children with SSD were significantly higher than those of children with UAN (P<0.05). (2) The degree of hearing loss in both UAN and SSD children was predominantly complete hearing loss. The percentage of complete hearing loss was significantly higher (χ²=4.353, P=0.037) in the SSD group (93.0%, 40/43) than in the UAN group (63.6%, 7/11). However, the percentage of profound hearing loss was significantly higher in the UAN group (27.3%, 3/11) than in the SSD group (2.3%, 1/43) (Fisher's exact test, P=0.023). In terms of hearing curve configuration, the percentage of flat type was significantly higher in the SSD group (76.7%, 33/43) than in the UAN group (36.4%, 4/11). The proportion of the UAN group (27.3%, 3/11) was significantly higher than that in the SSD group (2.3%, 1/43) in ascending type (P<0.05). There were no statistically significant differences in the hearing curves of the declining type and other types between the two groups (P>0.05). (3) The proportion of imaging assessment without abnormality was significantly more common in the UAN group (81.8%) than in the SSD group (37.1%) (χ²=6.695, P=0.015). Conclusions: Compared to children with SSD, the occurrence of wave IIIL on the ABR test was significantly more common in children with UAN. The percentage of ascending hearing curves was significantly higher in children with UAN than in children with SSD. ASSR thresholds were significantly lower in children with UAN. The normal imaging phenotype was significantly more common in children with UAN than in children with SSD.

Precision medicine, which is based on individualized medicine, is a new medical model developed with the cross-application of genome sequencing technology, bioinformatics, and big data science. The establishment of precision medicine model is conducive to maximize the efficient utilization of medical resources and patient benefits. Bariatric surgery is an emerging branch of general surgery in the new century. Although the development of bariatric surgery still has a long way to go, precision medicine will add significant value to the rapid and positive development of the branch of bariatric surgery. In the context of precision medicine, bariatric metabolic surgery is now in its infancy. Currently, the successful implementation of precision control in bariatric surgery is initially focusing on the prediction of postoperative weight and the study of body weight differences in epigenetics. However, there are still gaps to be filled in the area of postoperative recovery, nutritional support treatment, and realization of individualized treatment based on big data. In the future, the integration of large-scale resource and data information can be realized by establishing the multi-center clinical sample library and perfecting the data collection of long-term follow-up and clinical information. Meanwhile, we can conduct deep biological data mining effectively with the big data platform, while using data reference to achieve accurate prediction and control of obesity treatment, so as to formulate detailed implementation plans and detailed procedure standard. Last but not least, we should pay attention to the patients' privacy and prevent personal health information leakage.

Psoriasis studies increasingly employ outcomes that indicate complete disease resolution, yet remission and cure are poorly defined for psoriasis. We conducted a systematic literature review to identify definitions of psoriasis remission and cure reported in the literature. Medline, EMBASE, and The Cochrane Central Register of Controlled Trials databases were searched on July 22, 2020, for full-text studies providing definitions for psoriasis remission/cure. Definitions were analysed descriptively for endpoint, time-frame, on/off treatment, patient-reported outcomes, and disease domains. We identified 106 studies that provided 41 unique remission definitions. Most definitions included endpoints based on Psoriasis Area and Severity Index (PASI), such as PASI75 (n = 16 studies), PASI90 (n = 10), PASI100 (n = 10), and PASI of 0 (n = 3), and descriptive endpoints related to 'skin clearance' (n = 18). Few definitions specified time-frame, on/off treatment or other psoriasis-related disease domains. One small consensus-initiative defined drug-free remission for plaque psoriasis by BSA of 0 without any therapy for at least 12 months. While there is no cure for psoriasis, seven studies defined psoriasis cure using similar endpoints to those used to define remission. We identified a variety of definitions of psoriasis remission. These results will inform the development of consensus-based definitions for psoriasis remission to support efforts to improve research and clinical outcomes.

Lung cancer is a leading cause of cancer death in many countries. Notch signaling has been demonstrated to frequently participate in the process of lung carcinogenesis. Delta-like ligand 4 (Dll4) is a vascular-specific ligand of Notch, and has a critical role in the angiogenesis of numerous cancers. However, the role of Dll4 in the cross-talk between endothelial cells (ECs) and tumor cells remains obscure. Herein, our study revealed that Dll4-expressing ECs (EC-Dll4) significantly suppressed the proliferation of neighboring non-small cell lung cancer (NSCLC) cells and attenuated the growth of NSCLC xenograft in nude mice. On the contrary, silencing endothelial Dll4 by its specific interference RNA reversed these effects of Dll4 on NSCLC cell proliferation and tumor formation. Furthermore, activation of Notch1, but not Notch2 or Notch3, was enhanced in NSCLC cells cultured with EC-Dll4, as well as in xenografts induced by a mixture of NSCLC cells and EC-Dll4. Interference of Notch1 significantly attenuated Dll4-mediated suppression of NSCLC cell proliferations, indicating that Dll4/Notch1 signaling negatively modulates the NSCLC growth. Moreover, PTEN expression in NSCLC cells was increased by EC-Dll4 or rhDll4 (recombinant human-Dll4 protein), and the induction was impaired by Notch1 interference suggesting that Dll4 could upregulate PTEN expression by Notch1. Taken together, we conclude that the cross-talk between ECs and NSCLC cells by Dll4/Notch1/PTEN signaling pathway inhibits the growth of NSCLC.

RESUMO

From 1982 to 1992, 9 cases of clinical stage I serous adenocarcinoma of the endometrium were diagnosed and treated surgically in PUMC Hospital. Of these 9 patients, myometrial invasion occurred in 8 patients, lymphatic invasion in the myometrium was demonstrated in 7 patients and 6 patients were found to have extrauterine spread. The tumors studied in this paper confirmed the aggressive behavior of this particular histologic type of endometrial carcinoma. The pathological characteristics and differential diagnosis are presented.

The initial and follow-up endometrial curettings from 17 patients with atypical hyperplasia (AH) and 4 patients with complex hyperplasia (CH) were reviewed. All cases were younger than 40 years and the duration of follow-up ranged from 7 months to 38 years (mean, 11.3 years). According to the ISGP classification, the histological diagnostic criteria based solely on the presence of cytologic atypia. Twenty of them received progestin treatment and the lesion regressed at some time, but relapsed eventually. Twelve of them were performed hysterectomy after repeat progestin administrations and only 2 (11.8%) of 17 patients with AH progressed to invasive carcinoma 6 to 8 years later; however, all 4 patients with CH and 2 patients with milder forms of AH became pregnant and had normal term deliveries during the follow-up period. The histological diagnosis, prognoses and the effects on fertility in patients with AH and CH are discussed.

Nonfunctioning pancreatic endocrine tumor (NFPET) is a kind of tumor with high malignancy but bland in morphological features. It is impossible to predict the malignancy solely by gross and light microscopic pictures of the tumor. For the purpose of assessing the value of flow cytometry and morphometry in predicting the prognosis of NFPET, the DNA content and several morphological parameters of 26 NFPETs were determined by appropriate techniques. The results show that abnormal DNA content has no practical value in discriminating benign from malignant NFPETs; whereas the "score" obtained by combining the morphological measurements of the number of nuclei/mm2 and N/C, is of certain referential significance in predicting the malignancy of NFPET.

RESUMO

The clinical and pathologic features of 2 malignant Brenner tumors (MBT) were compared with those of 13 primary ovarian transitional cell carcinomas (TCC). Though histologically both tumors were invasive transitional cell carcinoma, TCC differed from MBT by the absence of benign Brenner component and stronger aggressiveness in behavior. Among tumors of both types in similar clinical stage, most of TCC (6/9) died within 1 to 2 years after the operation, but one of MBT died 4 years postoperatively. The distinction between MBT and TCC bearing on therapeutic as well as prognostic implications is discussed.

In this paper the averaged cortical evoked potential (C-CEP) induced by stimulating the C-fibers of the splanchnic nerve was used as an index of visceral pain to further study the effects of acupuncture and morphine on it. Experiments were carried out on 35 cats anesthetized with chloralose and immobilized with flaxedil. The splanchnic nerve was stimulated with electric pulses of varied strength and blocked by a galvanic current so as to cause A- or C-fibers input selectively. The cortical evoked potential was recorded on the contralateral post-sigmoid gyrus and averaged by TQ-19 Medical Data Processor. The compound action potential of splanchnic nerve was monitored. "Neiguan" acupoint was stimulated with electric pulses (0.1ms, 4v and 5Hz). The results were as follows: 1) when "Neiguan" acupoint, was excited with the electric pulses, C-CEP and the late component of A-CEP which was evoked by stimulating A-fibers of splanchnic nerve were obviously reduced in amplitudes. It was suggested that electro-acupuncture (EA) of "Neiguan" acupoint could inhibit visceral pain (the slow and fast pain). 2. After intravenous injection of morphine, the amplitudes of C-CEP and the late component of A-CEP were depressed significantly, indicating that C-CEP might reflect the visceral slow pain and the late component of A-CEP reflect the fast pain. 3. Naloxone could block partially the inhibitory effect of EA of "Neiguan" acupoint on C-CEP, suggesting that the inhibition of EA might be associated with the release of endogenous opiate like substances.
