RESUMO

INTRODUCTION: Whole blood (WB) resuscitation is increasingly used at trauma centers. Prior studies investigating outcomes in WB versus component-only (CO) resuscitation have been limited by small cohorts, low volumes of WB resuscitation, and unbalanced CO resuscitation. This study aimed to address these limitations using data from a high-volume Level I trauma center, which adopted a WB-first resuscitation paradigm in 2018. We hypothesized that the resuscitation method, WB or balanced CO, would have no impact on patient mortality. METHODS: A single-center, retrospective cohort study of adults presenting as a trauma activation from July 2016 through July 2021 was performed. Receipt of 3 or more units of WB or packed red blood cells (RBC) within the first hour of resuscitation was required for inclusion. Patients were grouped into WB versus CO resuscitation and important clinical outcomes were compared. Mortality was evaluated with Kaplan-Meier analysis, log-rank testing, and multivariable Cox proportional hazards modeling. RESULTS: There were 180 patients in the WB group and 170 patients in the CO group. Of the 180 WB patients, 110 (61%) received only WB during the first 24 hours. The WB group received a median of 5.0 units (IQR 4.0-8.0) of WB and CO group received a median of 6.0 units (IQR 4.0-11.8) of RBCs during the first 24 hours of resuscitation. In the CO group, median RBC/plasma and RBC/platelet ratios approximated 1:1:1. Groups were similar in clinicopathologic characteristics including age, injury severity score, mechanism of injury, and requirement for hemorrhage control interventions (WB 55% vs CO 59%, p = 0.60). Unadjusted survival was equivalent at 24 hours (p = 0.52) and 30 days (p = 0.70) between both groups on Kaplan-Meier analysis with log-rank testing. On multivariable Cox regression, WB resuscitation was not independently associated with improved survival after accounting for age, ISS, mechanism of injury, and receipt of hemorrhage control procedure (HR 0.85, 95% CI 0.61-1.19, p = 0.34). CONCLUSIONS: Balanced CO resuscitation is associated with similar mortality outcomes to that of WB based resuscitation. LEVEL OF EVIDENCE: Level IV; Therapeutic/Care Management.

The HOPE score (https://www.hypothermiascore.org) is a validated instrument for estimating the survival probability of patients in hypothermic cardiac arrest with ECLS rewarming. It is based on six patient characteristics: sex, age, mechanism of hypothermia, duration of cardiopulmonary resuscitation, serum potassium and temperature. The HOPE score provides a reliable estimate of survival probability that can be used to decide whether to rewarm a patient. In the initial publication of the HOPE score, a cutoff of 10% was proposed, below which a patient would not be rewarmed. This choice was tentative and subject to debate. In this paper, we examine the implications of this choice on the proportions of false positives (i.e., rewarmed patient who ends up dying) and false negatives (i.e., non-rewarmed patients who would have survived if rewarmed), and we provide approximate formulas to obtain upper bounds for these proportions as a function of the cutoff chosen. In particular, the choice of a 10% cutoff will result in a proportion of FP of less than 40% and a proportion of FN of less than 0.5% in many practical situations.

Aims: Our goal was to study hypothermic cardiac arrest (CA) patients who were not rewarmed by Extracorporeal Life Support (ECLS) but were admitted to a hospital equipped for it. The focus was on whether the decisions of non-rewarming, meaning termination of resuscitation, were compliant with international guidelines based on serum potassium at hospital admission. Methods: We retrospectively included all hypothermic CA who were not rewarmed, from three Swiss centers between 1st January 2000 and 2nd May 2021. Data were extracted from medical charts and assembled into two groups for analysis according to serum potassium. We identified the criteria used to terminate resuscitation. We also retrospectively calculated the HOPE score, a multivariable tool predicting the survival probability in hypothermic CA undergoing ECLS rewarming. Results: Thirty-eight victims were included in the study. The decision of non-rewarming was compliant with international guidelines for 12 (33%) patients. Among the 36 patients for whom the serum potassium was measured at hospital admission, 24 (67%) had a value that - alone - would have indicated ECLS. For 13 of these 24 (54%) patients, the HOPE score was <10%, meaning that ECLS was not indicated. The HOPE estimation of the survival probabilities, when used with a 10% threshold, supported 23 (68%) of the non-rewarming decisions made by the clinicians. Conclusions: This study showed a low adherence to international guidelines for hypothermic CA patients. In contrast, most of these non-rewarming decisions made by clinicians would have been compliant with current guidelines based on the HOPE score.

Purpose: To investigate the emerging role of Tc-99m-labeled diphosphonate (Tc-99m-DPD) uptake quantification by SPECT/CT in fibrous dysplasia (FD) bone lesions and its correlation with biological bone turnover markers (BTMs) of disease activity. Materials and methods: Seven patients (49 ± 16 years) with a confirmed diagnosis of FD were included in this retrospective study. Bone scans with Tc-99m-DPD and quantitative SPECT/CT (xSPECT/CT) were performed. SUVmax (maximum standard unit value) and SUVmean (mean standard unit value) were measured in all FD bone lesions. The skeletal burden score (SBS) was assessed on planar scintigraphy and multiplied by mean SUV max and SUVmean to generate two new parameters, SBS_SUVmax and SBS_SUVmean, respectively. Planar and xSPECT/CT quantitative measures were correlated with biological BTMs of disease activity, including fibroblast growth factor 23 (FGF-23), alkaline phosphatase (ALP), procollagen 1 intact N-terminal propeptide (P1NP) and C-terminal telopeptide (CTX), as well as scoliosis angle measured on radiographs. Statistical significance was evaluated with Spearman's correlations. Results: A total of 76 FD bone lesions were analyzed, showing an average SUVmax and SUVmean (g/mL) of 13 ± 7.3 and 8 ± 4.5, respectively. SBS, SBS_SUVmax and SBS_SUVmean values were 30.8 ± 25.6, 358 ± 267 and 220.1 ± 164.5, respectively. Mean measured values of FGF-23 (pg/mL), ALP (U/L), P1NP (µg/L) and CTX (pg/mL) were 98.4 (22-175), 283.5 (46-735), 283.1 (31-1,161) and 494 (360-609), respectively. Mean scoliosis angle was 15.7 (7-22) degrees. We found a very strong positive correlation between planar-derived SBS and CTX (r = 0.96, p = 0.010), but no significant correlation between SUVmax or SUVmean and biological BTMs. SBS_SUVmax showed a strong to very strong positive correlation with CTX (ρ = 0.99, p = 0.002), FGF-23 (ρ = 0.91, p = 0.010), ALP (ρ = 0.82, p = 0.020), and P1NP (ρ = 0.78, p = 0.039), respectively. Conclusion: This study showed that biological BTMs are significantly correlated with diphosphonate uptake on bone scan, quantified by a new parameter combining information from both planar and quantitative SPECT/CT. Further analysis of bone scan quantitative SPECT/CT data in a larger patient population might help better characterize the skeletal disease burden in FD, and guide treatment and follow-up.

The 5A score predicts in-hospital mortality of patients suffering from accidental hypothermia, including those not in cardiac arrest. The HOPE score was specifically developed to predict survival for the subgroup of hypothermic patients in cardiac considered for extracorporeal life support rewarming. The C-statistic in the external validation study of the HOPE score was 0.825 (95% CI: 0.753-0.897), confirming its excellent discrimination. In addition, its good calibration allows for a reliable interpretation of the corresponding survival probability after rewarming. The HOPE score should be used for predicting outcome and selecting hypothermic patients in cardiac arrest for rewarming.

INTRODUCTION: Patients with traumatic intracranial hemorrhage (tICH) often require intensive care unit (ICU) admission until bleeding stability is demonstrated through interval head computed tomography (HCT). The brain injury guidelines (BIG) suggest a minimum 24-h ICU admission for severe patients (BIG 3) regardless of repeat CT stability. We sought to evaluate the rate of tICH expansion after an initial stable interval scan was obtained. METHODS: A single-center retrospective cohort study at a level 1 trauma center was performed. All adult patients with tICH evaluated using BIG criteria were included. The primary endpoint was incidence of tICH expansion after initial stability on interval HCT performed at approximately 6 h. Secondary endpoints included time to tICH stability, frequency of neurosurgical intervention, and time to surgical intervention. RESULTS: A total of 1517 patients met inclusion criteria. Of the 1121 patients with repeat imaging, 288 (25.7%) experienced progression with 94.4% detected on the initial 6-h interval scan. Of all patients with initially stable repeat imaging (n = 833), progression occurred in 16 (1.9%) patients. Of these patients, 5 required neurosurgical intervention, 4 received increased monitoring, 2 transitioned to comfort measures and 5 had no change in management. The median time from initial scan to expansion in these patients was 42.2 h. Median time to surgical intervention after post-stability expansion was 102 h. CONCLUSION: Patients who demonstrate bleeding stability on first interval HCT after tICH rarely experience expansion. Consideration should be given to discharging patients from the ICU when initial interval HCT shows no progression.

BACKGROUND: Previous studies have found that parents of children with cancer desire more prognostic information than is often given even when prognosis is poor. We explored in audio-recorded consultations the kinds of information they seek. METHODS: Ethnographic study including observation and audio recording of consultations at diagnosis. Consultations were transcribed and analyzed using an interactionist perspective including tools drawn from conversation and discourse analysis. RESULTS: Enrolled 21 parents and 12 clinicians in 13 cases of children diagnosed with a high-risk brain tumor (HRBT) over 20 months at a tertiary pediatric oncology center. Clinicians presented prognostic information in all cases. Through their questions, parents revealed what further information they desired. Clinicians made clear that no one could be absolutely certain what the future held for an individual child. Explicit communication about prognosis did not satisfy parents' desire for information about their own child. Parents tried to personalize prognostic information and to apply it to their own situation. Parents moved beyond prognostic information presented and drew conclusions, which could change over time. Parents who were present in the same consultations could form different views of their child's prognosis. CONCLUSION: Population level prognostic information left parents uncertain about their child's future. The need parents revealed was not for more such information but rather how to use the information given and how to apply it to their child in the face of such uncertainty. Further research is needed on how best to help parents deal with uncertainty and make prognostic information actionable.

This study describes the structural features of fibula cortical shell as allowed by serial pQCT scans in 10/10 healthy men and women aged 20-40years. Indicators of cortical mass (mineral content -BMC-, cross-sectional area -CSA-), mineralization (volumetric BMD, vBMD), design (perimeters, thickness, moments of inertia -MIs-) and strength (Bone Strength Indices, BSIs; polar Strength-Strain Index, pSSI) were determined. All cross-sectional shapes and geometrical or strength indicators suggested a sequence of five different regions along the bone, which would be successively adapted to 1. transmit loads from the articular surface to the cortical shell (near the proximal tibia-fibular joint), 2. favor lateral bending (central part of upper half), 3. resist lateral bending (mid-diaphysis), 4. favor lateral bending again (central part of the lower half), and 5. resist bending/torsion (distal end). Cortical BMC and the cortical/total CSA ratio were higher at the midshaft than at both bone ends (p<0.001). However, all MIs, BSIs and pSSI values and the endocortical perimeter/cortical CSA ratio (indicator of the mechanostat's ability to re-distribute the available cortical mass) showed a "W-shaped" distribution along the bone, with maximums at the mid-shaft and at both bone's ends (site effect, p<0.001). The correlation coefficient (r) of the relationship between MIs (y) and cortical vBMD (x) at each bone site ("distribution/quality" curve that describes the efficiency of distribution of the cortical tissue as a function of the local tissue stiffness) was higher at proximal than distal bone regions (p<0.001). The results from the study suggest that human fibula is primarily adapted to resist bending and torsion rather than compression stresses, and that fibula's bending strength is lower at the center of its proximal and distal halves and higher at the mid-shaft and at both bone's ends. This would favor, proximally, the elastic absorption of energy by the attached muscles that rotate or evert the foot, and distally, the widening of the heel joint and the resistance to excessive lateral bending. Results also suggest that biomechanical control of structural stiffness differs between proximal and distal fibula.
