RESUMO

The 6-hour (6-h) time to wound closure was a controversial issue as studies have shown that time was not a substantial factor. Wounds in the face are often considered to have a lower infection risk. Despite this, the cause of injury was not extensively discussed in relation to this context. The primary objective was to investigate the association between the 6-h time to wound closure and wound complications following emergency management of facial soft tissue injuries (STIs). Additionally, the secondary objective was to explore other factors contributing to wound complications. A retrospective record review was conducted in our hospital in Kuala Lumpur, Malaysia, from 1 January 2017 to 31 December 2021. Medical records of patients with facial STIs due to road traffic accidents were included. Simple random sampling was used to select records meeting inclusion criteria. Data on demographic, injury, and treatment characteristics were collected using a standardized proforma. Descriptive, univariate and multivariate analyses were performed, including chi-square tests and binary logistic regression. A total of 295 patient records were included, with most patients being males (77.3%) and of Malay ethnicity (54.9%). The median age was 31.0 years. Majority of patients were treated within 6 h of injury (93.9%). Complications were documented in 6.1% of cases, including wound dehiscence and infection. Multivariate analysis revealed a significant association between 6-h time to closure and wound complications (OR: 7.53, 95% CI: 1.90-29.81, p = 0.004). Grade of surgeon on duty (OR: 4.61, 95% CI: 1.25-16.95, p = 0.02) and diabetes mellitus (OR: 6.12, 95% CI: 1.23-30.38, p = 0.03) were also shown to have a statistically significant association with wound complications. A 6-h time to wound closure, grade of surgeon on duty and diabetes mellitus were three major factors involved in facial wound complications following road traffic accidents.

Organic loading influences the effectiveness of producing biogas through anaerobic digestion. This study set out to investigate the effect of organic loading on the anaerobic mesophilic digestion of cow dung, the parameters involved in the digestion process and to evaluate the kinetics. Anaerobic digestion of cow dung at different organic loading (gVS/L) of 14 gVS/L, 18gVS/L, 22 gVS/L, 26 gVS/L and 30 gVS/L were investigated. Increasing the organic loading increased the methane yield of the cow dung. The highest cumulative methane yield was observed at 30 gVS/L with 63.42 mL CH4/gVS while the highest biogas yield was reported at 192.53 mL/gVS with the highest methane content of 89%. In addition, the modified Gompertz model equation with an R2 of 0.9980 demonstrated strong consistency and a good fit between predicted and experimental data. The high number of substrates added to the systems when increasing the organic loading increased the λ and slow down the nutrient transport and hydrolysis. This study provides current information on the effects of organic loading on the anaerobic digestion of cow dung in batch mode, including experimental conditions and operational parameters.

Pluripotency is a crucial feature of pluripotent stem cells, which are regulated by the core pluripotency network consisting of key transcription factors and signaling molecules. However, relatively less is known about the molecular mechanisms that modify the core pluripotency network. Here we used the CAPTURE (CRISPR Affinity Purification in situ of Regulatory Elements) to unbiasedly isolate proteins assembled on the Nanog promoter in mouse embryonic stem cells (mESCs), and then tested their functional relevance to the maintenance of mESCs and reprogramming of somatic cells. Gene ontology analysis revealed that the identified proteins, including many RNA-binding proteins (RBPs), are enriched in RNA-related functions and gene expression. ChIP-qPCR experiments confirmed that BCLAF1, FUBP1, MSH6, PARK7, PSIP1, and THRAP3 occupy the Nanog promoter region in mESCs. Knockdown experiments of these factors show that they play varying roles in self-renewal, pluripotency gene expression, and differentiation of mESCs as well as in the reprogramming of somatic cells. Our results show the utility of unbiased identification of chromatin-associated proteins on a pluripotency gene in mESCs and reveal the functional relevance of RBPs in ESC differentiation and somatic cell reprogramming.

Dabai (Canarium odontophyllum) is a fruit-bearing plant native to Borneo. Its fruit is an indigenous seasonal fruit that is considered to be underutilized due to its short shelf life. However, new products have been developed to ensure a continuous supply of dabai fruit throughout the year. Hence, the exploration of dabai fruits in characterizations and utilization for food products and essential oil has expanded exponentially. This review addresses the nutritional values, health benefits, potential food products, and essential oil processing of dabai fruit. All parts of dabai fruit, such as the pulp, skin, and kernel, contain a considerable amount of bioactive compounds, dietary fiber, and nutrients. Moreover, dabai fruit has also been proven to have health benefits such as an antioxidant capacity, cholesterol reduction, diabetes type 2 prevention, and reduction in the risk of heart disease. Some potential dabai-based food products and oil processing of dabai are also highlighted. The future perspectives and challenges concerning the potential uses of dabai are critically addressed at the end of this review. Based on this review, it is proven that dabai has various health benefits and represents a potential breakthrough in the agricultural and food industries.

BACKGROUND: The uptake of human papillomavirus vaccines (HPVV) among men who have sex with men (MSM) remains unsatisfactory. Healthcare providers play a crucial role in improving HPVV acceptability and uptake among MSM. This scoping review aims to provide an overview of (1) the perceived role of healthcare providers by MSM, and (2) the knowledge, beliefs and practices of healthcare providers themselves in promoting HPVV uptake. METHODS: A literature search was performed with PubMed and Scopus databases using a specific search string. The relevant original research articles on this topic were identified, and the major findings were charted and discussed. RESULTS: The literature search identified 18 studies on the perceived role of healthcare providers by MSM, and 6 studies on the knowledge, beliefs and practices of healthcare providers in promoting HPVV uptake among MSM. Recommendations by healthcare providers and disclosure of sexual orientation were important positive predictors of higher HPVV acceptability and uptake. Sexual healthcare providers were more confident in delivering HPVV to MSM clients compared to primary practitioners. CONCLUSION: Recommendation from, and disclosure of sexual orientation to healthcare providers are important in promoting HPVV uptake among MSM. The competency of healthcare providers in delivering HPVV to MSM can be improved by having clearer guidelines, education campaigns and better incentives.

This research aims to assess the influence of locus of control on the expression of entrepreneurial competency in a small business setting. Specifically, it predicts how this can generate positive outcomes in terms of business growth, quality of life, and sustainable entrepreneurial intention. Survey responses were collected from 102 small-sized firms in Malaysia. Structural equation modeling was performed to validate a mediation model and test nine research hypotheses. The results suggested that internal locus of control indirectly affects the venturing outcomes via entrepreneurial competency, whereas external locus of control has no such consequences. Thus, it can be deduced that beliefs based on internal attributions-rather than external forces, define entrepreneurs' destiny, and their competencies serve a perpetual role in linking these beliefs to positive business performance, life satisfaction, and sustainable entrepreneurial behavior. In practical terms, policymakers may gradually shift their focus from supplying direct financial relief assistance to the owner-managers to empowering them with core competencies building programs, especially during disasters and recessions. This study unravels the complexities of the entrepreneurial psychology-competency interface and fills a gap in the literature by providing compelling evidence of the adverse consequences of relying too heavily on fate or external assistance.

This study evaluated the respiration rate of coated and uncoated (control) papayas (Carica papaya L.) with 15% of Kelulut honey (KH) nanoparticles (Nps) coating solution during cold storage at 12 ± 1 °C for 21 days. The respiration rate of the papayas significantly changed during storage, with an increase in CO2 and a decrease in O2 and C2H4, while the ascorbic acid and total phenolic content was maintained. The changes in respiration rate were rather slower for coated papayas when compared to control ones. A kinetic model was established from the experimental data to describe the changes of O2, CO2, and C2H4 production in papayas throughout the storage period. All O2, CO2, and C2H4 were experimentally retrieved from a closed system method and then represented by the Peleg model. The outcomes indicated the Peleg constant K1 and K2, which were gained from linear regression analysis and coefficients of determination (R2), seemed to fit well with the experimental data, whereby the R2 values exceeded 0.85 for both coated and control papayas. The model confirmed both the capability and predictability aspects of the respiration rate displayed by papayas coated with KH Nps throughout the cold storage period. This is supported by the differences in the stomatal aperture of coated and control papaya shown by microstructural images.

The present study synthesized nano-magnetite (Fe3O4) from milled steel chips using the high energy ball milling (HEBM) method, characterized it, and then utilized it as a sorbent to remediate boron concentration at various pH (4-9), dosages (0.1-0.5 g), contact times (20-240 min), and initial concentrations (10-100 mg/L). The nano-sorbents were characterized based on SEM structure, elemental composition (EDX), surface area analysis (BET), crystallinity (XRD), and functional group analysis (FTIR). The highest adsorption capacity of 8.44 mg/g with removal efficiency of 84% was attained at pH 8, 0.5 g dosage, contact time of 180 min, and 50 mg/L initial concentration. The experimental data fit best with the pseudo-second-order kinetic model with R2 of 0.998, while the Freundlich adsorption isotherm describes the adsorption process with an R2 value of 0.9464. A regeneration efficiency of 47% was attained even after five cycles of reusability studies. This efficiency implies that the nano-magnetite has the potential for sustainable industrial application.

High proportion of copper has become a global challenge owing to its negative impact on the environment and public health complications. The present study focuses on the fabrication of a polyvinylidene fluoride (PVDF)-polyvinyl pyrrolidone (PVP) fiber membrane incorporated with varying loading (0, 0.5, 1.0, 1.5, and 2.0 wt%) of titanium dioxide (TiO2) nanoparticles via phase inversion technique to achieve hydrophilicity along with high selectivity for copper removal. The developed fibers were characterized based on scanning electron microscopy (SEM), energy dispersive X-ray spectroscopy (EDX), permeability, porosity, zeta potential, and contact angle. The improved membrane (with 1.0 wt% TiO2) concentration recorded the maximum flux (223 L/m2·h) and copper rejection (98.18%). Similarly, 1.0 wt% concentration of TiO2 nanoparticles made the membrane matrix more hydrophilic with the least contact angle of 50.01°. The maximum copper adsorption capacity of 69.68 mg/g was attained at 1.0 wt% TiO2 concentration. The experimental data of adsorption capacity were best fitted to the Freundlich isotherm model with R2 value of 0.99573. The hybrid membrane developed in this study has considerably eliminated copper from leachate and the concentration of copper in the permeate was substantially reduced to 0.044 mg/L, which is below standard discharge threshold.

Complementary and alternative medicine (CAM) therapies have been an integral part of Malaysia culture for many centuries. In recent years influences from other parts of the world have gained a foot-hold in Malaysian popular culture. We investigated the engagement with CAM in a non-urban, Malaysian population. We investigated the association of CAM use with cultural influence, perceived health status, gender and age. We recruited 700 adult participants to this study across three sites in central Malaysia. We found massage, Vitamin supplements, Herbal medicine and traditional Chinese medicine to be the most popular CAM modalities with participation at 67.7%, 55.7%, 55.5% and 26.3% respectively. CAM use was equally high at 67% among those people who claimed a predominantly Malay or Chinese cultural influence. The use of vitamins and participation in yoga were at higher prevalence among female participants compared to males. There was no gender difference for other CAM modalities. There were differences between age groups for most CAM modalities, and those over 50 years tended to be the highest frequency users for most modalities. There is a high rate of CAM use in non-urban Malaysia with younger people being more engaged with less traditional modalities such as aromatherapy, yoga and music therapy.

The continuous increase in anthropogenic activities resulting in an increase in boron concentration in the environment is becoming a serious threat to public health and the ecosystem. In this regard, a hybrid polyvinylidene fluoride (PVDF)-polyvinyl pyrrolidone (PVP) hollow fiber was synthesized with hydrophilic nano-titanium oxide (TiO2) at varied loadings of 0, 0.5, 1.0, 1.5, and 2.0 wt% using the phase inversion technique. The resultant membranes were characterized in terms of Scanning Electron Microscopy (SEM), Energy Dispersive X-ray Spectroscopy (EDX), contact angle, porosity, and zeta potential. The permeability flux was assessed using both pure water and leachate; also, rejection performance was evaluated based on boron removal from the leachate. The results revealed that the membrane with 1.0 wt% loading had the highest flux alongside an upturn in boron rejection percentage of 223 L/m2·h and 94.39%, respectively. In addition, the lowest contact angle of 50.01° was recorded with 1.0 wt% TiO2 loading, and this implies that it is the most hydrophilic. Throughout the experiment cycles, the fiber with 1.0 wt% TiO2 loading demonstrated a high flux recovery varying between 92.82% and 76.26% after 9 h filtration time. The physicochemical analysis of the permeate revealed that the boron concentration was significantly reduced to 0.43 mg/L, which is far lower than the discharge limit of 1.0 mg/L.

INTRODUCTION: Giant bladder uric acid stone cases that have more than 4 cm in diameters and weighing more than 100 g are considered rare. PRESENTATION OF CASES: At the end of 2019, two men presented with chronic lower abdominal pain since five years ago. Abdominal radiography found a giant opaque mass inside the bladder. Open suprapubic cystolithotomy was performed by a general surgeon. Both giant bladder stones were extracted and measured approximately 11 × 7 × 6 cm and 500 g in weight. More than 80 % of stone composition was uric acid. Neither of the patients developed severe complications after the procedure. DISCUSSION: Our patients had a history of prolonged sun exposure related to occupation and high protein intake. Benign prostatic hyperplasia as a major underlying cause for stone formation was not found in the digital rectal examination. Prostatectomy was not performed. Uric acid stone is common in Southeast Asia, with a multifactorial pathogenesis. CONCLUSIONS: Open suprapubic cystolithotomy is the best method to extract giant bladder stones in limited settings in the rural areas of Indonesia. It was likely that the most important factors associated with the stone formation were the climate and dietary habits. The findings showed that pathogenesis of bladder stone formation and particularly, uric acid bladder stone is still not clearly understood.

Background: The semaphorin 3D (SEMA3D) gene has been implicated in the pathogenesis of Hirschsprung disease (HSCR), a complex genetic disorder characterized by the loss of ganglion cells in varying lengths of gastrointestinal tract. We wished to investigate the role of SEMA3D variants, both rare and common variants, as well as its mRNA expression in Indonesian HSCR patients. Methods: Sanger sequencing was performed in 54 HSCR patients to find a pathogenic variant in SEMA3D. Next, we determined SEMA3D expression in 18 HSCR patients and 13 anorectal malformation colons as controls by quantitative real-time polymerase chain reaction (qPCR). Results: No rare variant was found in the SEMA3D gene, except one common variant in exon 17, p.Lys701Gln (rs7800072). The risk allele (C) frequency at rs7800072 among HSCR patients (23%) was similar to those reported for the 1,000 Genomes (27%) and ExAC (28%) East Asian ancestry controls (p = 0.49 and 0.41, respectively). A significant difference in SEMA3D expression was observed between groups (p = 0.04). Furthermore, qPCR revealed that SEMA3D expression was strongly up-regulated (5.5-fold) in the ganglionic colon of HSCR patients compared to control colon (ΔCT 10.8 ± 2.1 vs. 13.3 ± 3.9; p = 0.025). Conclusions: We report the first study of aberrant SEMA3D expressions in HSCR patients and suggest further understanding into the contribution of aberrant SEMA3D expression in the development of HSCR. In addition, this study is the first comprehensive analysis of SEMA3D variants in the Asian ancestry.

Sigmoid vaginoplasty has been popular for neovagina reconstruction in vaginal aplasia. The most common surgical complication was vaginal stenosis caused by inadequate vascularization and tension because of graft length. Therefore, ischemia ensued and disrupted wound healing. The selection of double pedicle artery rotation sigmoid vaginoplasty is expected to reduce this problem. Five patients from April to December 2016 were diagnosed with vaginal aplasia; 4 had history of neovagina stenosis. These patients underwent sigmoid vaginoplasty with double pedicle artery rotation. No complications occurred during or after the procedure. Assessment postsurgery was conducted at 1 year. These results suggest that double pedicle artery rotation sigmoid vaginoplasty is a safe and acceptable technique for management of vaginal aplasia. The procedure decreased tension inside vascular pedicles as a result of maintaining abundant vascularization supply. Consequently, this procedure could avert graft necrosis, leakage, and severe stenosis. All of the patients exhibited regular menstrual cycle and satisfactory sexual activity. The outcomes were excellent with remarkable anatomical and functional results.

Common conventional biological treatment methods fail to decolorize palm oil mill effluent (POME). The present study focused on using the abundant palm oil mill boiler (POMB) ashes for POME decolorization. The POMB ashes were subjected to microwave irradiation and chemical treatment using H2SO4. The resultant adsorbents were characterized using scanning electron microscopy (SEM), energy-dispersive X-ray spectroscopy (EDX), and Brunauer-Emmett-Teller (BET) analyses. The adsorption efficiency was evaluated at various pH levels (2-8.5), adsorption dosages (3-15 g) in 200 mL, and contact times (1-5 h). The microwave-irradiated POMB-retained ash recorded the highest color removal of 92.31%, for which the best conditions were pH 2, 15 g adsorbent dosage in 200 mL, and 5 h of contact time. At these best treatment conditions, the color concentration of the treated effluent was analyzed using the method proposed by the American Dye Manufacturers Institute (ADMI). The color concentration was 19.20 ADMI, which complies with the Malaysia discharge standard class A. The Freundlich isotherm model better fit the experimental data and had a high R2 of 0.9740. Based on these results, it can be deduced that microwave-irradiated POMB-retained ash has potential applications for POME decolorization via a biosorption process.
