RESUMO

Background: Managing hepatocellular carcinoma (HCC) presents significant clinical challenges, often necessitating orthotopic liver transplantation (OLT). To mitigate the risk of iatrogenic metastasis during OLT and reduce posttransplantation recurrence (PTR), we introduced the "no-touch" left (NTL) approach for recipient hepatectomy in OLT. Methods: In this retrospective cohort study, our aim was to compare the safety and PTR rates in patients undergoing OLT via either the NTL technique or the conventional approach for recipient hepatectomy. We included 106 patients who met the Hangzhou criteria and exhibited a high tumor burden in the right lobe, with 50 patients assigned to the NTL group and 56 to the conventional group. The primary endpoint was the 1-y PTR rate, whereas secondary endpoints encompassed the safety of the NTL approach, PTR rates at 2 and 5 y, and overall survival. Results: Baseline demographics and clinical characteristics showed no significant differences between the groups. The NTL approach exhibited major surgical outcomes similar to those of the conventional approach. The cumulative PTR rates at 1, 2, and 5 y were 14.0% in the NTL group, compared with 24.5%, 35.8%, and 35.8% in the conventional group (P = 0.013). Cumulative overall survival rates at 1, 2, and 5 y were 94.0%, 91.9%, and 89.7% in the NTL group and 88.7%, 75.5%, and 72.5% in the conventional group (P = 0.03). Conclusions: This innovative surgical technique enhances safety and significantly reduces the risk of PTR, leading to improved long-term survival. Further prospective studies with larger cohorts and longer follow-up periods are needed to validate our findings and establish the NTL approach as a standard practice in OLT.

RESUMO

Purpose: Whether the diagnosis of non-alcoholic fatty liver disease or metabolic dysfunction-associated fatty disease has a different impact on liver transplant recipients with hepatocellular carcinoma is not yet clear. Methods: Data from a two-center retrospective cohort study were collected to compare and investigate the differences between non-alcoholic fatty liver disease and metabolic dysfunction-associated fatty liver disease in clinicopathologic parameters and prognosis among liver transplant recipients with hepatocellular carcinoma. Results: A total of 268 liver transplant recipients with hepatocellular carcinoma were included. The prevalence among pre- and post-transplant metabolic dysfunction-associated fatty liver disease was 10.82% and 30.22%, while for non-alcoholic fatty liver disease, it was 7.09% and 26.87%, respectively. The clinicopathological parameters were similar between the two pre-transplant groups. In contrast, the post-transplant group with metabolic dysfunction-associated fatty liver disease exhibited a higher prevalence of diabetes mellitus and a greater body mass index. However, the other parameters were similar between the two post-transplant groups (p > 0.05). Factors such as the largest tumor size > 4 cm, microvascular invasion, lack of tumor capsule, post-transplant metabolic dysfunction-associated fatty liver disease, and decreased post-transplant lymphocyte percentage were related to an increased risk of recurrence. Conclusion: In patients undergone liver transplantation for hepatocellular carcinoma, the diagnosis of metabolic dysfunction-associated fatty disease is more strongly associated with metabolic abnormalities than the diagnosis of non-alcoholic fatty liver disease and is an independent predictor of hepatocellular carcinoma recurrence.

RESUMO

As a member of transition metal dichalcogenides (TMDs), NbTe2 has a work function of 5.32 eV and a band gap of 0 eV at the Fermi level, which enables it to possess broadband absorption characteristics and has huge potential in optoelectronic devices. In this work, a combination of liquid phase exfoliation (LPE) and optical deposition methods (ODMs) were used to fabricate a NbTe2 saturable absorber (SA). Based on the NbTe2 SA, a ring passive mode-locked erbium-doped fiber laser (PML-EDFL) was constructed by adding NbTe2 SA into the laser cavity. A switchable single- to multiwavelength (dual/triple/quadruple) conventional soliton (CS) and a bound-state soliton (BS) were observed for the first time. The results reveal that NbTe2 SA has excellent saturable absorption characteristics (modulation depth of 2.6%, saturation intensity of 177.4 MW/cm2, and unsaturated loss of 63.8%) and can suppress mode competition and stabilize multiwavelength oscillation. This study expands the applications of NbTe2 nanosheets in ultrafast optoelectronics. The proposed switchable PML-EDFL has extensive applications in high-capacity all-optical communication, high-sensitivity optical fiber sensing, high-precision spectral measurements, and high-energy-efficiency photon neural networks.

RESUMO

B cells possess anti-tumor functions mediated by granzyme B, in addition to their role in antigen presentation and antibody production. However, the variations in granzyme B+ B cells between tumor and non-tumor tissues have been largely unexplored. Therefore, we integrated 25 samples from the Gene Expression Omnibus database and analyzed the tumor immune microenvironment. The findings uncovered significant inter- and intra-tumoral heterogeneity. Notably, single-cell data showed higher proportions of granzyme B+ B cells in tumor samples compared to control samples, and these levels were positively associated with disease-free survival. The elevated levels of granzyme B+ B cells in tumor samples resulted from tumor cell chemotaxis through the MIF- (CD74 + CXCR4) signaling pathway. Furthermore, the anti-tumor function of granzyme B+ B cells in tumor samples was adversely affected, potentially providing an explanation for tumor progression. These findings regarding granzyme B+ B cells were further validated in an independent clinic cohort of 40 liver transplant recipients with intrahepatic cholangiocarcinoma. Our study unveils an interaction between granzyme B+ B cells and intrahepatic cholangiocarcinoma, opening up potential avenues for the development of novel therapeutic strategies against this disease.

RESUMO

Microlens arrays (MLAs) with a tunable imaging ability are core components of advanced micro-optical systems. Nevertheless, tunable MLAs generally suffer from high power consumption, an undeformable rigid body, large and complex systems, or limited focal length tunability. The combination of reconfigurable smart materials with MLAs may lead to distinct advantages including programmable deformation, remote manipulation, and multimodal tunability. However, unlike photopolymers that permit flexible structuring, the fabrication of tunable MLAs and compound eyes (CEs) based on transparent smart materials is still rare. In this work, we report reconfigurable MLAs that enable tunable imaging based on shape memory polymers (SMPs). The smart MLAs with closely packed 200 × 200 microlenses (40.0 µm in size) are fabricated via a combined technology that involves wet etching-assisted femtosecond laser direct writing of MLA templates on quartz, soft lithography for MLA duplication using SMPs, and the mechanical heat setting for programmable reconfiguration. By stretching or squeezing the shape memory MLAs at the transition temperature (80 °C), the size, profiles, and spatial distributions of the microlenses can be programmed. When the MLA is stretched from 0 to 120% (area ratio), the focal length is increased from 116 to 283 µm. As a proof of concept, reconfigurable MLAs and a 3D CE with a tunable field of view (FOV, 160-0°) have been demonstrated in which the thermally triggered shape memory deformation has been employed for tunable imaging. The reconfigurable MLAs and CEs with a tunable focal length and adjustable FOV may hold great promise for developing smart micro-optical systems.

RESUMO

Efficient energy storage systems are crucial for the optimal utilization of renewable energy. Sodium-ion batteries (SIBs) are considered potential substitutes for next-generation low-cost energy storage systems due to the low cost and abundance of sodium resources. However, the industrialization of SIBs faces a great challenge in terms of the anode. Hard carbon could be a promising anode material due to its high capacity and low cost which originates from biomass. This study used pre-treatment and template carbonization methods to extract a hard carbon material from a large amount of discarded biomass in bamboo powder waste. This material has a good initial Coulombic efficiency of 78.6 % and good cycling stability when applied to sodium ion batteries.Typically, the optimal hard carbon material is used as the anode to prepare sodium ion battery prototypes to demonstrate their potential applications. The anode exhibited excellent sodium storage performance with a reversible capacity of 303 mAh ⋅ g-1 at 1 C rate and good cycling performance, retaining 92.0 % of its capacity after 100 cycles. These results demonstrate that BPPHC is a promising candidate for anode material in sodium-ion batteries. This work suggests that bamboo powder could be a low-cost anode material for SIBs.

RESUMO

Background: Hepatic epithelioid hemangioendothelioma (EHE) is an extremely rare tumor, and no standard therapy has been established yet. Objectives: The aim of this study was to investigate the short-term results of combined therapy with sirolimus and interferon-alpha 2b (IFN-a 2b) (SI therapy). Methods: From January 2022 to April 2023, 40 patients histologically diagnosed with hepatic EHE and progressive disease received SI therapy. All patients were regularly evaluated for the safety and efficacy of the SI therapy. Patients who received SI therapy for <3 months without a tumor status evaluation after treatment were excluded. Results: Twenty-nine patients with hepatic EHE were included in this study. The Eastern Cooperative Oncology Group (ECOG) performance status was 0 in 19 (65.5%) patients and 1 in 10 (34.5%) patients. The median duration of the SI therapy was 8 months (range, 3-15 months). Twenty-three (79.3%) patients showed a decrease in tumor size, including 11 (37.9%) patients who achieved a partial response and one (3.4%) who achieved a complete response; the objective response rate was 41.4%. Stable disease was observed in 13 (44.8%) patients, with a disease control rate of 86.2%. Adverse events (AES) were observed in 18 patients, including leukopenia (31.0%), oral ulcers (13.8%), and liver injury (10.3%). No severe (grade ⩾ 3) AEs were recorded, and SI therapy was not interrupted for any patient due to AEs. Conclusion: Sirolimus and IFN-a 2b may have synergistic effects in the treatment of hepatic EHE. SI therapy is a safe and effective treatment for hepatic EHE patients with good ECOG performance status.

RESUMO

BACKGROUND: Metabolic dysfunction-associated fatty liver disease was proposed by international consensus to redefine the metabolic abnormal condition. However, its impact on liver transplant recipients with hepatitis B virus-related hepatocellular carcinoma has not been explored. METHODS: A two-center retrospective cohort study on liver transplant recipients with hepatitis B virus-related hepatocellular carcinoma was performed to analyze the impact of metabolic dysfunction-associated fatty liver disease on the clinicopathologic parameters and prognosis. RESULTS: There were 201 liver transplant recipients enrolled from two hospitals in our study. The pre- and post-transplant prevalences of metabolic dysfunction-associated fatty liver disease were 9.95% and 28.86%, respectively. The clinicopathological parameters revealed a similarity between patients with and without pre-transplant metabolic dysfunction-associated fatty liver disease. In contrast, the group with post-transplant metabolic dysfunction-associated fatty liver disease was linked with older age, a higher hepatitis recurrence rate and incidence of cardiovascular disease, usage of calcineurin inhibitors, a greater body mass index and waist circumference, lower albumin and high-density lipoprotein cholesterol levels, and poorer tumor-free survival and overall survival. The multivariate analysis showed the largest tumor size >4 cm (95% confidence intervals: 0.06~0.63, p = 0.006), microvascular invasion (95% confidence intervals: 1.61~14.92, p = 0.005), post-transplant metabolic dysfunction-associated fatty liver disease (95% confidence intervals: 1.40~10.60, p = 0.009), and calcineurin inhibitors-based regimen (95% confidence intervals: 0.33~0.96, p = 0.036) were the independent risk factors for recurrent hepatocellular carcinoma. CONCLUSIONS: Our study suggests that post-transplant metabolic dysfunction-associated fatty liver disease is more closely to metabolic abnormalities and that it can help identify liver transplant recipients at high risk of recurrent hepatocellular carcinoma.

RESUMO

MXenes are a class of two-dimensional layered structure ternary metal carbide or/and nitride materials. Recently, the MXene V2CTx has demonstrated excellent long-term stability, strong saturable absorption, and fast optical-switching capability, used to generate Q-switched and ultrashort pulsed lasers. However, bound-state fiber lasers based on V2CTx have not been reported yet. In this study, V2CTx is combined with a D-shaped fiber to form a saturable absorber device, whose modulation depth is measured to be 1.6%. By inserting the saturable absorber into an Er-doped fiber laser, bound states with different soliton separation and munbers are successfully obtained. Additionally, bound states with a compound soliton structure, such as the (2 + 2)- and (2 + 1)-type, are also realized. Our findings show that V2CTx can be developed as an efficient ultrafast photonics candidate to further understand the complex nonlinear dynamics of bound-state pulses in fiber lasers.

RESUMO

Neuromorphic photonics devices based on phase change materials (PCMs) and silicon photonics technology have emerged as promising solutions for addressing the limitations of traditional spiking neural networks in terms of scalability, response delay, and energy consumption. In this review, we provide a comprehensive analysis of various PCMs used in neuromorphic devices, comparing their optical properties and discussing their applications. We explore materials such as GST (Ge2Sb2Te5), GeTe-Sb2Te3, GSST (Ge2Sb2Se4Te1), Sb2S3/Sb2Se3, Sc0.2Sb2Te3 (SST), and In2Se3, highlighting their advantages and challenges in terms of erasure power consumption, response rate, material lifetime, and on-chip insertion loss. By investigating the integration of different PCMs with silicon-based optoelectronics, this review aims to identify potential breakthroughs in computational performance and scalability of photonic spiking neural networks. Further research and development are essential to optimize these materials and overcome their limitations, paving the way for more efficient and high-performance photonic neuromorphic devices in artificial intelligence and high-performance computing applications.

RESUMO

BACKGROUND: Hepatic epithelioid hemangioendothelioma (HEH) is extremely rare, and CT features have never been analyzed in a large group of patients. METHODS: A retrospective study was designed to review the contrast-enhanced CT images of HEH patients. Intrahepatic lesions were categorized into three types: nodular, locally coalescent (coalescent lesion contained in one segment) or diffusely coalescent (coalescent lesion occupied more than one segment). CT features were compared among lesions of different sizes and patients with different lesion types. RESULTS: A total of 93 HEH patients were included in this study, and 740 lesions were analyzed. The results of per-lesion analysis showed that medium lesions (2-5 cm) had the highest rate of lollipop sign (16.8%) and target-like enhancement (43.1%), while lesions in large group (> 5 cm) had the highest rate of capsular retraction (38.8%) and vascular invasion (38.8%). The differences on enhancement pattern and the rates of lollipop sign and capsular retraction were significant among lesions of different sizes (p < 0.001, respectively). The results of per-patient analysis showed that patients in locally coalescent group had the highest rates of lollipop sign (74.3%) and target sign (94.3%). All patients in diffusely coalescent group had capsular retraction and vascular invasion. CT appearances of capsular retraction, lollipop sign, target sign and vascular invasion differed significantly among patients with different lesion types (p < 0.001, p = 0.005, p = 0.006 and p < 0.001, respectively). CONCLUSION: CT features variated among HEH patients with different lesion types, and radiological appearances of HEH should be classified into nodular type, locally coalescent type and diffusely coalescent type.

RESUMO

Flexible electronic skin is a flexible sensor system that imitates human skin. Recently, flexible sensors have been successfully developed. However, the droplet sliding sensing technology on a flexible electronic skin surface is still challenging. In this Letter, a flexible droplet sliding sensing surface is proposed and fabricated by laser-reduced graphene oxide (LRGO). The LRGO shows porous structures and low surface energy, which are beneficial for infusing lubricants and fabricating stable slippery surfaces. The slippery surface guarantees free sliding of droplets. The droplet sliding sensing mechanism is a combination of triboelectricity and electrostatic induction. After a NaCl droplet slides from lubricant-infused LRGO, a potential difference (∼0.2 mV) can be measured between two Ag electrodes. This study reveals considerable potential applications in intelligent robots and the medical field.

RESUMO

Photothermal responsive slippery surfaces with switchable superwettability are promising in the fields of biomedicine, self-cleaning, anti-corrosion, and lab-on-a-chip systems. However, the development of a light switchable slippery surface that combines high-performance photothermal materials with hierarchical microstructures of special orientation remains challenging, which limits the applications in anisotropic droplet manipulation. Herein, we demonstrate a photothermal responsive slippery surface based on laser-structured graphene and polyvinylidene difluoride composites (L-G@PVDF) for controllable droplet manipulation. The L-G@PVDF film exhibits high light absorption (∼95.4%) in the visible and NIR region. After lubricating with paraffin, the resultant surface shows excellent self-healing ability and light-responsive wettability change due to the photothermal effect of L-G@PVDF and the hot melting effect of paraffin. Additionally, by introducing anisotropic grooved structures, the paraffin-infused L-G@PVDF surface displays anisotropic wettability that further affects droplet manipulation under light irradiation. Also, the photothermal responsive slippery property endows the paraffin-infused L-G@PVDF surface with excellent anti-frosting and de-icing capability. Moreover, the smart paraffin-infused L-G@PVDF surface can be combined with a microfluidics chip for light-driven automatic sampling. This study offers insight into the rational design of photothermal responsive slippery surfaces for controllable droplet manipulation.

RESUMO

Hepatic epithelioid hemangioendothelioma (HEH) is a very rare tumor originating from vascular endothelial cells, with unpredictable malignancy. At present, there is no standard treatment protocol yet established. Both surgical resection and liver transplantation have been reported to be effective treatments for HEH; however, multiple intrahepatic lesions or extrahepatic metastasis make these procedures unsuitable to most patients. Systematic therapy has also been investigated, but the results are undetermined due to the limited cases. Interferon-alpha 2b (IFN-a 2b) has also been used for the treatment of HEH. Based on our previous study, the rate of tumor regression with IFN-a 2b monotherapy was more than 50%. Here, we reported a patient with advanced HEH, who achieved a partial response with the combined therapy of anlotinib and IFN-a 2b. The tumor stayed stable for 2 years with anlotinib monotherapy and regressed 3 months after the combined therapy of anlotinib and IFN-a 2b. The synergistic effect of combined therapy with anlotinib and IFN-a 2b provided promising guidance for future clinical study.

RESUMO

Self-healing materials (SHMs) with unique mechanical and electronic properties are promising for self-reparable electronics and robots. However, the self-healing ability of emerging two-dimensional (2D) materials, for instance, MXenes, has not been systematically investigated, which limits their applications in self-healing electronics. Herein, we report the homogeneous self-healing assembly (homo-SHA) of MXene and the heterogeneous self-healing assembly (hetero-SHA) of MXene and graphene oxide (GO) under moisture treatments. The self-healing mechanism has been attributed to the hydration induced interlayer swelling of MXene and GO and the recombination of hydrogen bond networks after water desorption. The multiform hetero-SHA of MXene and GO not only enables facile fabrication of free-standing soft electronics and robots, but also endows the resultant devices with damage-healing properties. As proof-of-concept demonstrations, free-standing soft electronic devices including a generator, a humidity sensor, a pressure sensor, and several robotic devices have been fabricated. The hetero-SHA of MXene and GO is simple yet effective, and it may pioneer a new avenue to develop miniature soft electronics and robots based on 2D materials.

RESUMO

OBJECTIVE: To compare the differences and outcomes of surgical procedures, clinical effect, complications and patients' satisfaction between disposable oval-shaped circumcision device (Modified Chinese ShangRing series, Kiddie love®) and conventional circumcision in the treatment of children with phimosis or redundant prepuce. METHODS: The clinical data were retrospectively analyzed in 114 children with phimosis or redundant foreskin undergone circumcision using a disposable oval-shaped circumcision device, a modified Chinese ShangRing series, Kiddie Love® (Kiddie Love group) in our hospital between January 2018 and February 2020, and another 114 children with similar conditions circumcised by conventional surgical procedure before January 2018 (conventional group). The two groups were compared regarding the operative time, intraoperative blood loss, postoperative pain scores, healing time, the incidence of complications and guardian's satisfaction. RESULTS: Circumcision was successfully completed in children in both groups. The operative time, intraoperative blood loss, postoperative pain scoring in 24 h by VAS, pain at the removal of the device or stitches and wound healing were (6.4 ± 1.6) min, (34.1 ± 6.4) min; (0.7 ± 0.2) ml, (2.6 ± 0.6) ml; (2.2 ± 1.0) points, (1.3 ± 0.5) points; (23.7 ± 3.9)day, (15.9 ± 2.8)day, respectively for Kiddie Love group and conventional group(either P < 0.05 or P > 0.05). The two groups were significantly different in the incidence of hematoma, edema and incision dehiscenceyet were insignificant in incision infection. Children in both groups were followed up from 6 to 31 months (mean: 23 months), and the satisfaction rate was 94.7% (108/114) in parents of the children circumcised by the ShangRing and 83.3% (95/114) in those of children treated by conventional circumcision (P < 0.05). CONCLUSION: Modified Chinese ShangRing, Kiddie Love®, has superiorities, including simpler procedure, shorter operative time, less blood loss, fewer complications, better cosmetic results and higher satisfaction of patients over conventional circumcision in the treatment of children with phimosis or redundant foreskin, and worthy of wider clinical recommendation.

RESUMO

In this paper, we report a flexible wavelength-, pulse-controlled mode-locked all-fiber laser based on a novel fiber optic Lyot filter. The wavelength, pulse duration and spectral bandwidth of passive mode-locked lasers can be tuned by controlling the polarization controller. The proposed Lyot filter was constructed by a single-mode fiber insertion between two polarization-maintaining fibers. The filter bandwidth and laser output tunability were based on the birefringence characteristics of the polarization-maintaining fibers. This all-fiber laser is simple and stable and can be used for various applications where width-tunable or wavelength-tunable pulses are necessary.

RESUMO

Based on the dual-resonance principle around the dispersion turning point, a scheme of chiral long-period fiber gratings (CLPGs) formed by twisting a high-birefringence (Hi-Bi) fiber is herein proposed to realise ultra-broadband flat-top circular polarizers. The coupling bandwidth is approximately seven times larger than that of traditional CLPGs. In addition, by introducing chirp characteristics in these CLPGs, an ultra-broadband flat-top circular polarizer with ∼200 nm@3 dB was conveniently achieved. Subsequently, by optimising the chirped CLPGs, a circular polarizer with a bandwidth extinction ratio of approximately 30 dB and a high level of ∼100 nm at 1 dB was realised. It was shown that the mode-controlling performances of the CLPGs can be remarkably improved, which has significant applications in light-field regulation. Finally, for the first time, it was proved that the CLPG cannot generate a vortex beam.

RESUMO

Graphene oxide (GO) films with natural "quantum-confined-superfluidics" (QSF) channels for moisture actuation have emerged as a smart material for actuators and soft robots. However, programming the deformation of GO by engineering QSF nanochannels around 1 nm is extremely challenging. Herein, we report the reconfigurable, reversible, and redefinable deformation of GO under moisture actuation by tailoring QSF channels via moisture-assisted strain-induced wrinkling (MSW). The shape fixity ratio of a general GO film can reach ∼84% after the MSW process, and the shape recovery ratio is ∼83% at room temperature under moisture actuation. The flexible shaping and deformation abilites, as well as the self-healing property of GO make it possible to fabricate soft robots using GO. Besides, as a proof-of-concept, passive electronics and soft robots capable of crawling, turning, switching circuit, and automatic somersault are demonstrated. With unique shaping and deformation abilities, GO may bring great implications for future soft robotics.
