[Fournier's gangrene in a 2-month-old infant]. / Fournier'sche Gangrän bei einem zwei Monate alten Säugling.

A two month old boy with Fournier's Gangrene of the scrotum as a result of unnecessary needle puncture of the hydrocele is being reported. The importance of appropriate diagnosis operation and drug-therapy is also described.

[Percutaneous demonstration of ectopic megaureters]. / Perkutane Darstellung ektoper Megaureteren.

Preoperative diagnosis of ectopic megaureter has become much simpler using percutaneous transvesical fine needle puncture of a sonographically visible retrovesical megaureter. Antegrade ureterography using watery contrast material together with indigocarmine dye was able both to demonstrate the megaureter roentgenologically and to help finding the ectopic orifice by direct vision or colposcopy in three girls.

Long-term results of prosthetic ureteral replacement in minipigs.

In a 3-year study, silicone rubber prostheses were implanted in 6 minipigs as ureteric replacement. A Dacron velour cuff was applied for the proximal anastomosis and a disc of the same material implanted into the bladder wall to anchor the prosthesis distally. The silicone tube incorporated a metal spiral to prevent kinking. On autopsy all anastomoses were firm and resistant to tearing except in 1 animal where the proximal end of the prosthesis had loosened without further complications. Incrustation or leakage never occurred. On histology dense ingrowth of collagen fibres into the meshes of Dacron velour was evident. Malignant cells could never be demonstrated. The findings are compared with a previous 12-month study. The growth of polypoid structures occluding the prosthesis was found to be a new problem with long-term alloplastic ureteric replacement. The best results were achieved with unilateral prostheses after contralateral nephrectomy.

[Limitations of tumor marker surveillance after chemotherapy in advanced non-seminomatous testicular tumors]. / Grenzen der tumormarkerorientierten Verlaufskontrolle beim chemotherapierten, metastasierenden, nicht-seminomatösen Hodentumor.

Determination of the biochemical tumor markers, alpha-fetoprotein (AFP) and beta-human chorionic gonadotropin (beta-HCG) gained much importance in respect to staging and follow-up examination of patients presenting with testicular cancer. After cytostatic chemotherapy the reliability of tumor markers is diminished. Retroperitoneal lymphadenectomy (RLA) was done in 60 patients after chemotherapy. Tumor marker elevation was found only in 52% of the patients showing retroperitoneal carcinoma (n = 25).

Ureteral replacement with a new prosthesis.

A new flexible silicone ureteral prosthesis has been developed and implanted by a new technique in 20 miniature pigs. The improved implantation technique and the results are described. All prostheses remained firmly attached in their position and drained the upper urine collecting system adequately during an observation period up to 12 mos. Leakage, incrustation, dislocation or rejection did not occur. The promising results encouraged us to continue our investigations to get long term experience for clinical trial in human patients for whom nephrostomy is the alternative.

[Urinary calculi in children (author's transl)]. / Harnsteine im Kindesalter.

From a total of 274 children with urinary calculi, 192 cases were analyzed. 63% of these were male. In 19% a familiar disposition was noticed. Phosphate containing stones were found in 81%. Most of these had an urinary tract infection caused by urea splitting organisms. There was no correlation between the chemical composition of the stones and infection with coli and enterococci. One-third of the analyzed cases exhibited malformations of the urinary tract. Of these, 92% had urinary infections due primarily to urea splitting bacteria. This infection rate was markedly higher than in the children without combined malformations of the urinary tract. The peak incidence was around 2 years of age. In the last group symptoms were atypical but more severe than in those cases without malformations of the urinary tract.
