PURPOSE: To evaluate the position stability of a phakic intraocular lens (Cachet pIOL; Alcon, Laboratories, Inc., Fort Worth, TX) in the anterior chamber (AC) during a 6-months follow-up. METHODS: Thirty eyes of 16 subjects underwent a Cachet pIOL implantation for the correction of high myopia from -6.50 to -16.00 D with plano targeted refraction. The position stability of the Cachet pIOL was evaluated using an Anterior Segment-OCT (AS-OCT, Carl Zeiss AG, Oberkochen, Germany) at 1- and 6-months postoperatively. Three measurements have been taken into account: 1) the minimum distance between the anterior surface of the Cachet pIOL optic disk and the central corneal endothelium; 2) the minimum distance between the optic disk's edges of the pIOL and the endothelium; 3) the minimum distance between the posterior surface of the pIOL optic disk and the anterior surface of the crystalline lens. The endothelial cell density (ECD) was also recorded. RESULTS: No statistically significant changes of the Cachet pIOL position in the AC were found during follow-up. The average changes were ≤0.1 mm between 1- and 6-months postoperatively (P>0.05). At the end of follow-up, The mean ECD loss was 0.7 % (from 2794 ± 337 cell/mm(2) to 2776 ± 388 cell/mm(2); P>0.05). CONCLUSION: A high position stability of the Cachet pIOL in the anterior chamber was shown during a 6-months follow-up.

RESUMO

Recent developments in noninvasive ventilation allow the intensivists to delay the time o intubation of the patient admitted to the ICU. This new possibility of a gain in time leads to the fact that those patients who are intubated in the ICLI, are patients with a very compromised hemodynamic and respiratory situation. For this reason, we developed a specific protocol regarding the intubation procedure of this kind of patients. This paper reviews our strategies for endotracheal tube positioning in critically ill patients and shows the fundamental role of the nurses during this procedure.

Intra-abdominal pressure monitoring is a very important parameter in the ICU setting, since pathological alterations of this value are significant and are responsible for functional alterations involving cardio-respiratory system, kidneys and central nervous system. Responsibility of the professional nurse in the ICU is to perform a tight and adequate monitoring of those patients who are at risk for a rise in intra-abdominal pressure.

RESUMO

Pulse-oximeter is described as the most important technological proceeding for monitoring the patients' safety during anesthesia, after surgery and in emergency. This opinion was widely confirmed in the 1990s when pulse-oximeter has been definitively introduced in the standard for base monitoring in the OR and has been proposed for routinary use also in the ICU. In this paper we consider the importance, in the cardiovascular, respiratory and brain parameters monitoring, of continuous oximetry of mixed venous blood (SVO2) and blood venous saturation in the internal jugular vein (SjvO2).

The paper highlights the importance of invasive pressure monitoring systems. The most common catheters are considered (intra-arterial, Swan/Ganz, central venous), catheter positioning (appropriate sites, risks and early and late complications of intravascular cannulation), evaluation of pressure and finally the role of nursing in the management of the intravascular catheter in situ (care of cannulation site, management of the catheter).

RESUMO

The cost-benefit analysis provided the system planners with valuable information that served many purposes. It answered the following questions: Why was the CCF undertaking this project? What were the alternatives? How much was it going to cost? And what was the expected outcome? The process of developing cost-benefit the document kept the project team focused. It also motivated them to involve additional individuals from materials management and accounts payable in its development. A byproduct of this involvement was buy-in and commitment to the project by everyone in these areas. Consequently, the project became a team effort championed by many and not just one. We were also able to introduce two new information system processes: 1) a management review process with goals and anticipated results, and 2) a quality assurance process that ensured the CCF had a better product in the end. The cost-benefit analysis provided a planning tool that assisted in successful implementation of an integrated materials management information system.

RESUMO

The results of histological examinations conducted on 2 granulomas found in teeth and treated with calcium oxide are reported. It is concluded that the treatment produces significant changes in the histological picture with the formation of scar tissue that precedes the growth of newborn in a relatively short time.

The Authors present (5) cases of gingival tattooing resulted by rapport between argentum amalgam and prosthetic reconstruction with aurum or combination. Theu concluded is opportune to substitute the stumps reconstructions in amalgam with other materials not cause of electrogalvanism. So we avaid principally the antiaesthetic aspect of pigmentation.
