INTRODUCTION: Congenital adrenal hyperplasia (CAH) is the most common cause of genital atypia in females. A dedicated multidisciplinary team (MDT) should be included for an optimal management. Here, we aimed to review our surgical experience and to assess long-term urinary, gynecological and endocrine outcomes after primary genitoplasty in this specific cohort. METHODS: Patients born with CAH and who underwent feminizing genitoplasty in our institution were retrospectively identified (2001-2021). We analyzed patients' characteristics, intraoperative details, and postoperative urinary, gynecological, and endocrine outcomes. RESULTS: Forty patients were included and followed-up for a median (IQR) time of 7 (1-19) years. Thirty-eight (95%) had 21-hydroxylase deficiency. After multidisciplinary decision and written consent from patient and/or family, a single-stage reconstructive surgery was performed at a median age of 10 (3-165) months. Median length of hospital stay was 5 (1-7) days. Procedures were: PUM (N = 35 (87.5%)), TUM (N = 3 (7.5%)), urogenital mobilization was unnecessary in 2 (5%). Reduction clitoroplasty was done in 33 (82.5%) patients. Only 3 (7.5%) experienced significant Clavien-Dindo complications requiring additional surgery during the follow-up period. Recurrent urinary tract infections (UTI) occurred in 6 (15%), one required ureteric reimplantation for symptomatic high-grade vesicoureteric reflux. All patients over 3 years were toilet-trained without incontinence. Severe vaginal stenosis occurred in 1 (2.5%) patient. In patients who achieved puberty, 6/9 had vaginal calibration at a median age of 17.3 (16-21) years without detected stenosis. One (2.5%) had major hypertrophy of the right labia minora requiring labiaplasty. Nine (22.5%) reached puberty. Two (5%) patients developed acne/hirsutism. Short stature was noted in 11 (27.5%) and obesity in 18 (45%). CONCLUSION: Based on our contemporary series, genitourinary reconstructive surgery for female patients born with CAH is technically feasible and safe with a low complication rate. A regular follow-up with a MDT to assess long-term complications is necessary, and it is vital to inform patients and families about the different management options with all the risks and benefits of surgery. Smart Grid (SG) technology utilizes advanced network communication and monitoring technologies to manage and regulate electricity generation and transport. However, this increased reliance on technology and connectivity also introduces new vulnerabilities, making SG communication networks susceptible to large-scale attacks. While previous surveys have mainly provided high-level overviews of SG architecture, our analysis goes further by presenting a comprehensive architectural diagram encompassing key SG components and communication links. This holistic view enhances understanding of potential cyber threats and enables systematic cyber risk assessment for SGs. Additionally, we propose a taxonomy of various cyberattack types based on their targets and methods, offering detailed insights into vulnerabilities. Unlike other reviews focused narrowly on protection and detection, our proposed categorization covers all five functions of the National Institute of Standards and Technology cybersecurity framework. This delivers a broad perspective to help organizations implement balanced and robust security. Consequently, we have identified critical research gaps, especially regarding response and recovery mechanisms. This underscores the need for further investigation to bolster SG cybersecurity. These research needs, among others, are highlighted as open issues in our concluding section.

This study investigates the pattern of hand injuries presenting to a UK regional tertiary centre. All referrals to our Hand Surgery Unit in 2019 were reviewed. Subgroup analyses of sex, deprivation, occupational injuries and assault cases were carried out. Over the study period, 4216 cases were referred. Most were male (70%), with a median age of 36 years and from a deprived area (75.2%). Soft tissue injury was documented in 53.6% and bone injury (fracture/dislocation) in 52.4%. Surgical management was required in 2214 (52.5%) cases. Work-related injuries accounted for 16.7% of cases and were more likely to require surgery. Assault-related injuries accounted for 8% of cases, with a male preponderance. Our findings can help facilitate service provision and guide regional and national prevention policies.Level of evidence: III.

Studying the diet and feeding behavior of primates is essential to understanding their ecology and designing effective conservation plans. Despite decades of study on the hamadryas baboon (Papio hamadryas) in lowland habitats, little is known about the feeding ecology of this species in highland ecosystems. To address this empirical gap, we tracked temporal changes in vegetation abundance and their relation to the dietary choices of hamadryas baboons in highland habitat at Borena-Sayint National Park (BSNP) in northern Ethiopia. We performed behavioral scan sampling on a focal study band of 21-37 hamadryas baboons over a 12-month period. We found that mature and young leaves were the most abundant plant parts throughout the year, while fruits and flowers were the least abundant, with significant seasonal variation that followed the bimodal pattern of rainfall characteristic of the Ethiopian highlands ecosystem. The annual diet of hamadryas baboons at BSNP consisted mostly of fruits (32.0%) and graminoid blades (21.2%), and included 52 food species across 22 families of plants and three families of animals. Food raided from nearby farms accounted for 8.8% of their diet. The availability of fruits and flowers was positively correlated with their consumption, suggesting that these are preferred foods, whereas graminoid blades, and other leaves, appeared to be less preferred foods. The feeding ecology of hamadryas baboons at BSNP differs considerably from that of lowland populations. The well-studied lowland hamadryas baboons in Awash National Park obtain much of their diet from Acacia species and palm fruit, whereas those at BSNP, where Acacia trees are rare and palms are absent, relied on Olinia rochetiana and Rosa abyssinica for a combined 27% of their annual diet. The reliance of hamadryas baboons at BSNP on cultivated crops for nearly one-tenth of their diet leads to conflict with humans and warrants more detailed study so that this issue can be addressed in conservation plans for the area.

PURPOSE: To compare the root mean square (RMS) of anterior corneal higher-order aberrations (HOAs) in ametropic and emmetropic eyes. METHODS: This retrospective observational study was conducted at the Department of Ophthalmology, Tishreen University Hospital, Latakia, Syria. Study eyes were divided into four groups based on refractive error: mild-to-moderate myopia, hypermetropia, myopic astigmatism, and emmetropic eyes as controls. The following anterior corneal HOAs were evaluated using the Scheimpflug-Placido Sirius (CSO, Italy) tomographer over 6 mm pupil: Root mean square (RMS) total corneal HOAs, RMS trefoil, RMS coma and RMS spherical aberrations. RESULTS: RMS values of total HOAs, trefoil and coma showed statistically significant differences in all four groups (P < 0.05, all). HOAs were noted to be lowest in the control group (0.18 ± 0.09, 011 ± 0.08 and 0.09 ± 0.08 µm, respectively) and highest in the myopic astigmatism group (0.31 ± 0.16, 0.15 ± 0.12, 0.17 ± 0.14 µm, respectively). RMS spherical aberration was lowest in the astigmatism group (0.00 ± 0.16 µm) with a statistically significant difference from that in the control group (0.05 ± 0.07 µm, P = 0.049). CONCLUSION: The mean RMS values of total HOAs, trefoil and coma were highest in the astigmatism group and lowest in the control group. However, spherical aberration was minimal in the astigmatism group. A better understanding and targeted treatment of higher-order aberrations in ametropic human eyes, and in particular eyes with astigmatism, may enhance visual quality and performance in the treatment of refractive errors. Recognising atypical HOAs may also assist in the early detection of pathological conditions such as keratoconus.

Machine learning (ML) has succeeded in improving our daily routines by enabling automation and improved decision making in a variety of industries such as healthcare, finance, and transportation, resulting in increased efficiency and production. However, the development and widespread use of this technology has been significantly hampered by concerns about data privacy, confidentiality, and sensitivity, particularly in healthcare and finance. The "data hunger" of ML describes how additional data can increase performance and accuracy, which is why this question arises. Federated learning (FL) has emerged as a technology that helps solve the privacy problem by eliminating the need to send data to a primary server and collect it where it is processed and the model is trained. To maintain privacy and improve model performance, FL shares parameters rather than data during training, in contrast to the typical ML practice of sending user data during model development. Although FL is still in its infancy, there are already applications in various industries such as healthcare, finance, transportation, and others. In addition, 32% of companies have implemented or plan to implement federated learning in the next 12-24 months, according to the latest figures from KPMG, which forecasts an increase in investment in this area from USD 107 million in 2020 to USD 538 million in 2025. In this context, this article reviews federated learning, describes it technically, differentiates it from other technologies, and discusses current FL aggregation algorithms. It also discusses the use of FL in the diagnosis of cardiovascular disease, diabetes, and cancer. Finally, the problems hindering progress in this area and future strategies to overcome these limitations are discussed in detail.

Background: The advancement of information and communication technologies and the growing power of artificial intelligence are successfully transforming a number of concepts that are important to our daily lives. Many sectors, including education, healthcare, industry, and others, are benefiting greatly from the use of such resources. The healthcare sector, for example, was an early adopter of smart wearables, which primarily serve as diagnostic tools. In this context, smart wearables have demonstrated their effectiveness in detecting and predicting cardiovascular diseases (CVDs), the leading cause of death worldwide. Objective: In this study, a systematic literature review of smart wearable applications for cardiovascular disease detection and prediction is presented. After conducting the required search, the documents that met the criteria were analyzed to extract key criteria such as the publication year, vital signs recorded, diseases studied, hardware used, smart models used, datasets used, and performance metrics. Methods: This study followed the PRISMA guidelines by searching IEEE, PubMed, and Scopus for publications published between 2010 and 2022. Once records were located, they were reviewed to determine which ones should be included in the analysis. Finally, the analysis was completed, and the relevant data were included in the review along with the relevant articles. Results: As a result of the comprehensive search procedures, 87 papers were deemed relevant for further review. In addition, the results are discussed to evaluate the development and use of smart wearable devices for cardiovascular disease management, and the results demonstrate the high efficiency of such wearable devices. Conclusions: The results clearly show that interest in this topic has increased. Although the results show that smart wearables are quite accurate in detecting, predicting, and even treating cardiovascular disease, further research is needed to improve their use.

Introduction: Status epilepticus (SE) is one of the most common neurologic emergencies and is associated with significant morbidity and mortality. The underlying cause of SE varies between patients with epilepsy and those presenting without. The aim of this study was to evaluate the epidemiology, risk factors and outcomes of patients presenting with convulsive SE in the emergency department (ED) of a tertiary hospital in Mogadishu. Methods: This was a cross-sectional study conducted between July 2021 and June 2022. The study included both patients with epilepsy and those without epilepsy presenting to the ED with SE. Risk factors and underlying etiologies were evaluated in the patients in both the pediatric group (0-18 years) and adult group (18 years and above). Clinical history, neurologic examinations, neuroimaging, electroencephalography findings, and laboratory investigations were all evaluated. Results: The mean age for pediatric patients was 6 (SD±4.7), while the mean age for adult patients was 38 (SD±10.4). About 33 (36%) of the subjects had previous history of epilepsy, while 59 (64%) presented to the ED with their first seizure. About 80 (87%) had generalized seizure while 12 (13%) had focal seizure. Poor antiepileptic compliance was the most common risk factor for SE 20 (21.7%), followed by CNS infections 14 (15%), and prolonged febrile seizures 7 (7.6%). Poor antiepileptic drug compliance, cerebrovascular disorders, electrolyte imbalance, metabolic conditions, and drug abuse were the most common underlying etiologies of SE in patients older than 18 years. Prolonged febrile seizure, meningitis, encephalitis, systemic infections, and structural brain abnormalities were the most common underlying etiologies of SE in patients younger than 18 years. Conclusion: CNS infections, CVDs, metabolic disturbances, electrolyte imbalances, and systemic infections are major etiological risk factors of SE in patients without prior history of epilepsy. Medication adherence was the major etiological risk factor for SE identified in patients with epilepsy.

Inspection methods, also known as non-destructive evaluation (NDE), is a process for inspecting materials, products, and facilities to identify flaws, imperfections, and malfunctions without destruction or changing the integrity of materials, structures, and mechanisms. However, detecting those defects requires test conducting and results inferring, which is highly demanding in terms of analysis, performance, and time. New technologies are therefore needed to increase the efficiency, probability of detection, and interpretability of NDE methods to establish smart inspection. In this context, Artificial intelligence (AI), as a fundamental component of the Industry 4.0, is a well-suited tool to address downsides associated with the current NDE methods for analysis and interpretation of inspection results, where methods integrating AI into their inspection process become automated and are known as smart inspection methods. This article sheds a light on the conventional methods and the smart techniques used in defects detection. Subsequently, a comparison between the two notions is presented. Furthermore, it investigates opportunities for the integration of non-destructive evaluation (NDE) methods and Industry 4.0 technologies. In addition, the challenges hindering the progress of the domain are mentioned as the potential solutions. To this end, along with Industry 4.0 technologies, a virtual inspection system has been proposed to deploy smart inspection.

Today's world is changing dramatically due to the influence of various factors. Whether due to the rapid development of technological tools, advances in telecommunication methods, global economic and social events, or other reasons, almost everything is changing. As a result, the concepts of a "job" or work have changed as well, with new work shifts being introduced and the office no longer being the only place where work is done. In addition, our non-stop active society has increased the stress and pressure at work, causing fatigue to spread worldwide and becoming a global problem. Moreover, it is medically proven that persistent fatigue is a cause of serious diseases and health problems. Therefore, monitoring and detecting fatigue in the workplace is essential to improve worker safety in the long term. In this paper, we provide an overview of the use of smart wearable devices to monitor and detect occupational physical fatigue. In addition, we present and discuss the challenges that hinder this field and highlight what can be done to advance the use of smart wearables in workplace fatigue detection.

Acute neuromuscular weakness with associated subsequent developing respiratory failure is common neurological emergency in all emergency departments worldwide. Guillain-Barré Syndrome (GBS) remains the most common cause of acute ascending quadriplegia presents with ascending muscle weakness associated with paresthesia and loss of deep tendon reflexes and usually preceeded by diarrheal illness or upper airway infection. Here we report 49-year-old female presented with rapidly progressing, ascending quadraparesis for 48 hours duration with subsequently complicated by respiratory paralysis due to severe hypokalemia.

AIM: To evaluate changes in corneal higher-order aberrations (HOAs) following epithelium-off accelerated corneal cross-linking (A-CXL) and to explore the impact on visual acuity. METHODS: In this retrospective case series, 32 eyes of 24 patients with keratoconus (KC) underwent A-CXL. Treatment was delivered at 10 mW/cm2 for 9 min with a total dose of 5.4 J/cm2. The following anterior corneal HOAs: total corneal HOAs, trefoil, secondary trefoil, coma, secondary coma, secondary astigmatism and spherical aberrations were analysed using the Scheimpflug-Placido Sirius (CSO, Italy) corneal topographer at baseline and 12 months following treatment. Multivariate analysis was used to evaluate the independent effect of HOA subtypes on changes in uncorrected distance visual acuity (UDVA) and corrected distance visual acuity (CDVA). RESULTS: At one year post CXL, UDVA and CDVA were significantly improved, -0.13 ± 0.19 LogMAR (P = 0.0005) and -0.08 ± .0.11 LogMAR (P = 0.0003), respectively. The mean preoperative trefoil, secondary trefoil, secondary coma and secondary astigmatism were 0.95 ± 0.46; µm, 0.20 ± 0.11; µm, 0.29 ± 0.19; µm and 0.42 ± 0.17 µm, respectively. At one year, the mean values decreased significantly to 0.77 ± 0.47 µm, 0.15 ± 0.11 µm, 0.25 ± 0.18 µm and 0.34 ± 0.18 µm, respectively (P < 0.05, for all). No independent relationship between any HOA changes and change in UDVA was observed. A reduction in secondary coma aberration was associated with a change in CDVA (95% CI 0.01-1.34, P = 0.048; ß = 0.67). CONCLUSION: A 9-min protocol of Accelerated corneal cross-linking is an effective treatment in improving corneal HOAs at 12 months follow up, in eyes with progressive keratoconus at one year follow-up. A change in secondary coma had a statistically significant and independent effect on CDVA.

OBJECTIVES: To identify the potential opportunities and risks around future UK regulatory reform of medical devices. DESIGN: A mixed methods approach, comprising a rapid literature review, one-to-one, semi-structured interviews with key stakeholders, a multidisciplinary stakeholder workshop, and a post-workshop survey. SETTING: United Kingdom. PARTICIPANTS: 32 key stakeholders across the medical device sector were identified both from the public and private sectors. RESULTS: Opportunities relating to regulatory independence were identified, including the potential to create and implement a regulatory framework that ensures availability of medical devices; innovation and investment potential; and safety to the citizens of the UK. The most significant risks identified included threats to the safety of individual patients and the wider health system arising from the delay in awaiting regulatory approval due to the shortage of approved bodies; and reduced competitiveness of UK market and device manufacturers. Recommendations were identified to mitigate risks, centred on harnessing broader cross-sector collaborations, promoting patient and public partnership, and maximizing international engagement. CONCLUSIONS: The UK's medical device sector is at a time-critical juncture to construct a regulatory framework to navigate its exit of Europe and respond to Europe's transition to new medical device regulations whilst also addressing the ongoing demand for rapid approval for new devices in response to the global pandemic. Investment, capacity-building, and international engagement will play a central role in mitigating risks and maximizing opportunities for medical device regulation.

(1) Background: The use of smart devices to better manage diabetes has increased significantly in recent years. These technologies have been introduced in order to make life easier for patients with diabetes by allowing better control of the stability of blood sugar levels and anticipating the occurrence of dangerous events (hypo/hyperglycemia), etc. That being said, the main objectives of the self-management of diabetes is to improve the lifestyle and life quality of patients with diabetes; (2) Methods: We performed a systematic review based on articles that focus on the use of smart devices for the monitoring and better management of diabetes. The search was focused on keywords related to the topic, such as "Diabetes", "Technology", "Self-management", "Artificial Intelligence", etc. This was performed using databases, such as Scopus, Google Scholar, and PubMed; (3) Results: A total of 89 studies, published between 2011 and 2021, were included. The majority of the selected research aims to solve a diabetes management problem (e.g., blood glucose prediction, early detection of risk events, and the automatic adjustment of insulin doses, etc.). In these studies, wearable devices were used in combination with artificial intelligence (AI) techniques; (4) Conclusions: Wearable devices have attracted a great deal of scientific interest in the field of healthcare for people with chronic conditions, such as diabetes. They are capable of assisting in the management of diabetes, as well as preventing complications associated with this condition. Furthermore, the usage of these devices has improved illness management and quality of life.

INTRODUCTION: Splenic tumours are relatively rare and include malignancies such as lymphomas, angiosarcomas, plasmacytomas, primary malignant fibrous histiocytomas, and splenic metastases. Benign tumours of the spleen such as hemangiomas, cysts, and inflammatory pseudotumours are very rare (Kaza et al., 2010, PisaniCeretti et al., 2012) [1,2]. There are fewer than 160 cases of splenic hamartoma or splenomas having been reported in the literature (Basso et al., 2012) [3]. Only 20% of the cases were detected in children (Abramowsky et al., 2004) [4]. Although multi-modality imaging findings were described preoperatively, the final diagnosis was splenic hamartoma based on histology and immunohistochemistry. CASE REPORT: Here, we report a case of a14 year old child left upper quadrant abdominal pain and worsening sickness. Multi-modality imaging detected a solid lesion of the spleen, who required splenectomy and was pathologically diagnosed as a splenic hamartoma. The postoperative course was uneventful. DISCUSSION: Splenic hamartoma is very rare. Only 20% of hamartomas occur in children. They are commonly found incidentally on imaging with no symptoms. CONCLUSION: Splenic hamartoma is a benign vascular proliferative lesion that requires a multi-modality imaging studies for diagnosis and confirmed by histopathology. It must be included in the differential diagnosis of splenic mass forming lesions.

Given the current rate of habitat degradation and loss in the tropics, data on primate population densities and habitat use are indispensable for assessing conservation status and designing feasible management plans for primates. The Omo River guereza (Colobus guereza guereza) is a subspecies of the eastern black-and-white colobus monkey endemic to the western Rift Valley forests of Ethiopia. Their restricted distribution along with habitat loss and hunting within their range render them vulnerable to local extirpation and extinction. Furthermore, there are no published data available on the population status and habitat use patterns of the Omo River guereza. We therefore aimed to assess the population size of Omo River guerezas in different habitats (Erica-Juniperus mixed forest, mixed plantation forest, undisturbed natural forest, disturbed natural forest) using transect surveys at Wof-Washa Natural State Forest (WWNSF) in central Ethiopia. Our surveys covered a cumulative distance of 88.5 km in four different habitats, during which we recorded a total of 140 Omo River guereza groups. The average group density was 14.3 groups/km2, average individual density was 94.4 individuals/km2, and we estimated the total population size within WWNSF to be 2549 individuals. The sex ratio of the population was split evenly between males and females, though the age classes skewed strongly towards adults. Of the habitats surveyed, the highest group encounter rate (1.83 groups/km) occurred in the disturbed natural forest. However, the highest individual density (110.1 individuals/km2) was recorded in undisturbed natural forest. Still, sizable densities (group and individual) were recorded in three of the disturbed habitats (disturbed natural forest, mixed plantation forest, and to a lesser extent Erica-Juniperus mixed forest). Our study offers the first baseline information with which to compare future population density estimates and habitat use in the range of Omo River guerezas.

Publicly available skin image datasets are increasingly used to develop machine learning algorithms for skin cancer diagnosis. However, the total number of datasets and their respective content is currently unclear. This systematic review aimed to identify and evaluate all publicly available skin image datasets used for skin cancer diagnosis by exploring their characteristics, data access requirements, and associated image metadata. A combined MEDLINE, Google, and Google Dataset search identified 21 open access datasets containing 106 950 skin lesion images, 17 open access atlases, eight regulated access datasets, and three regulated access atlases. Images and accompanying data from open access datasets were evaluated by two independent reviewers. Among the 14 datasets that reported country of origin, most (11 [79%]) originated from Europe, North America, and Oceania exclusively. Most datasets (19 [91%]) contained dermoscopic images or macroscopic photographs only. Clinical information was available regarding age for 81 662 images (76·4%), sex for 82 848 (77·5%), and body site for 79 561 (74·4%). Subject ethnicity data were available for 1415 images (1·3%), and Fitzpatrick skin type data for 2236 (2·1%). There was limited and variable reporting of characteristics and metadata among datasets, with substantial under-representation of darker skin types. This is the first systematic review to characterise publicly available skin image datasets, highlighting limited applicability to real-life clinical settings and restricted population representation, precluding generalisability. Quality standards for characteristics and metadata reporting for skin image datasets are needed.

In this study, Al1050 sheets were fabricated in five passes using the accumulative roll bonding (ARB) technique. For a more accurate and complete investigation, different tests were used, including a uniaxial tensile test. The results show that elongation increases about 50% for the annealed sample, which is 2.5 times that of the fifth pass (20%). A five-fold increase can be seen in tensile strength, which was 50 MPa in the annealed sample and reached 250 MPa at the end of the fifth pass. The annealed sample's yield stress was 40 MPa, 4.5 times less than 180 MPa after five passes of ARB. Then, to evaluate sample hardness, the Vickers microhardness test was conducted in the samples' depth direction, which recorded 39 HV for the annealed piece and 68 HV after the last ARB pass. These results show that the hardness increases by 1.8 times after five passes of ARB. In the next step, by conducting fractography tests after the sample fractures during the tensile test, the fracture's mechanism and type were identified and explained. Finally, X-ray diffraction (XRD) was employed to produce pole figures of sample texture, and the anisotropy phenomena of the annealed sample and ARBed samples were wholly examined. In this study, with the help of pole figures, the anisotropic behavior after ARB was investigated and analyzed. In each step of the process, observing the samples' texture states and the anisotropy magnificent was possible. According to the results, normal anisotropy of 0.6 in the annealed sample and 1.8 achieved after the fifth pass of ARB indicates that ARB leads to an increase in anisotropy.
