RESUMO

A 67-year-old man was diagnosed with non-muscle invasive bladder cancer and underwent transurethral resection of the lesions in August 2017. The pathological findings revealed high-grade urothelial carcinoma. The tumor relapsed as urothelial carcinoma with sarcomatoid/osteosarcoma variant with vascular invasion, and transurethral resection was performed in December 2017. He underwent laparoscopic radical cystoprostatectomy and orthotopic neobladder reconstruction using ileum in March 2018. The patient developed lung metastasis in July 2018. He underwent four courses of chemotherapy with doxorubicin and thoracoscopic left lower lobectomy of the lung in October 2018.

RESUMO

Myeloid sarcoma is an extramedullary tumor composed of immature myeloid cells and occurs in various extramedullary sites. We report a 48-year-old man diagnosed with myeloid sarcoma in the epididymis. He was admitted to our hospital due to a painless right intrascrotal mass. Magnetic resonance imaging showed a 30 mm tumor in the right epididymis, and we subsequently performed right high orchiectomy. The pathological diagnosis was myeloid sarcoma. Bone marrow aspiration and biopsy revealed no hematological disease, and cytogenetic analysis in the bone marrow showed normal karyotype. He was diagnosed with isolated myeloid sarcoma in the epididymis. Six months after the operation, myeloid sarcoma recurred in the para aorta and left sub-diaphragm. Bone marrow examination revealed myelodysplastic syndrome, and cytogenetic analysis showed 46, XY. We performed surgical resection of the recurrent mass, and cytogenetic analysis showed 47, XY, +21. He was diagnosed with recurrent MS with adult-onset trisomy 21. Although the effect of trisomy 21 on prognosis is unknown, the patient is currently undergoing systemic chemotherapy with maintained remission.

The Belousov-Zhabotinsky (BZ) reaction shows temporal or spatiotemporal structures such as redox oscillation of the catalyst, [ruthenium(II)tris(2,2'-bipyridine)][PF6]2 ([Ru(bpy)3][PF6]2). In this study, autonomously oscillating hydrogel microspheres (microgels) were investigated, which show swelling/deswelling oscillation induced by the redox oscillation of the BZ reaction inside the gel. Despite the periodically and autonomously induced oscillation that does not require an external stimulus, it has not been possible to perform any manipulation of the oscillatory behavior over time. The results of the present study show that it is possible to reversibly switch the microgel oscillations from an "on" active state of the BZ reaction to an "off" inactive state by changing the temperature in combination with thermoresponsive microgels. To realize on-demand switching, the construction of double-shell structures is crucial; the thermoresponsive first shell allows the microgels to modulate the diffusion of the substrates or intermediates in the BZ reaction, while the second shell maintains colloidal stability under high temperatures and high ion concentrations. The functionalized double-shell microgels were prepared via multistep seeded precipitation polymerization. The oscillatory switching behavior of the BZ reaction was observed directly and evaluated by ultraviolet-visible (UV-vis) spectroscopy. The central concept of this study, i.e., "on-off switching" can be expected to benefit the development of advanced bioinspired materials.

A 44-year-old woman visited a hospital with microscopic hematuria in June 2009. Computed tomography showed a large bladder diverticulum at right lateral wall and urachal remnant. Cystoscopic examination showed a large diverticulum at the right lateral wall that closes to the dome. She was followed up closely without therapeutic intervention; however, the residual urine increased gradually and frequent bladder diverticulitis developed. She underwent laparoscopic bladder diverticulectomy and excision of the urachal remnant simultaneously without any complications in August 2017. Laparoscopic approach for a large bladder diverticulum and urachal remnant is useful with safe, effective, and minimally invasive.

RESUMO

Caliceal diverticular calculi are rather fixed and they are rare to be delivered spontaneously. We experienced a case of spontaneous passage of left ureteral multiple calculi from caliceal diverticular calculi. A 65-year-old female suffered from left back pain and was referred to our hospital. She was found to have left ureteral multiple calculi and left caliceal diverticular calculi. After a month observation, all ureteral calculi were delivered spontaneously. Multiple ureteral calculi from caliceal diverticular calculi could be delivered spontaneously and conservative management may be an option for the treatment.

A 56-year-old man was referred to our facility after developing edema in his right leg. Enhanced computed tomography (CT) revealed a periarterial soft tissue mass surrounding the right iliac artery compressing the iliac vein. His elevated serum IgG4 level indicated IgG4-related disease (IgG4-RD). Both a percutaneous and surgical biopsy of this periarterial lesion proved challenging and were not pursued. A prostate biopsy, however, finally provided a histological diagnosis of IgG4-RD. Oral steroid therapy successfully resolved his leg edema and periarterial lesion. This case illustrates the usefulness of an alternative prostate biopsy for the histological diagnosis of IgG4-RD when approaching the primary affected lesion is difficult.

Fast swelling/deswelling and dispersing/flocculating oscillations of autonomously oscillating hydrogel microspheres (microgels) at periods of single-order seconds were accomplished. The design of the oscillating microgels is based on the incorporation of neutral and hydrophilic acrylamide monomers into conventional microgels to suppress the irreversible aggregation of the microgels during oscillation at high substrate concentrations and high temperatures in the Belousov-Zhabotinsky (BZ) reaction. In contrast to conventional microgels, the aforementioned microgels can carry out the BZ reaction under optimized conditions without irreversible aggregation at higher temperature, resulting in short oscillation periods (∼7 s) that were obtained from optical transmittance measurements. Furthermore, a macrogel composed of organized microgels was prepared, which also showed a remarkably fast swelling/deswelling oscillation (oscillation period: ∼23 s). This new type of oscillating microgel could be applied to advanced materials, e.g. to autonomously oscillating micropumps that imitate the human heartbeat.

Background: Bilateral synchronous renal pelvic carcinomas are rare diseases. Complete urinary exenteration or kidney-preserving procedures are two different options that confer different benefits to the patient depending on the clinical situation. Case Presentation: A 69-year-old woman with bilateral synchronous renal pelvic carcinomas underwent laparoscopic complete urinary tract exenteration. Although dissection was difficult because of postoperative adhesions, the surgery was completed with minimal blood loss. Conclusion: Laparoscopic complete urinary tract exenteration allows minimally invasive surgery with minimal blood loss.
