BACKGROUND: Microparasitic diseases are caused by bacteria and viruses. Genetic improvement of resistance to microparasitic diseases in breeding programs is desirable and should aim at reducing the basic reproduction ratio [Formula: see text]. Recently, we developed a method to derive the economic value of [Formula: see text] for macroparasitic diseases. In epidemiological models for microparasitic diseases, an animal's disease status is treated as infected or not infected, resulting in a definition of [Formula: see text] that differs from that for macroparasitic diseases. Here, we extend the method for the derivation of the economic value of [Formula: see text] to microparasitic diseases. METHODS: When [Formula: see text], the economic value of [Formula: see text] is zero because the disease is very rare. When [Formula: see text]. is higher than 1, genetic improvement of [Formula: see text] can reduce expenditures on vaccination if vaccination induces herd immunity, or it can reduce production losses due to disease. When vaccination is used to achieve herd immunity, expenditures are proportional to the critical vaccination coverage, which decreases with [Formula: see text]. The effect of [Formula: see text] on losses is considered separately for epidemic and endemic disease. Losses for epidemic diseases are proportional to the probability and size of major epidemics. Losses for endemic diseases are proportional to the infected fraction of the population at the endemic equilibrium. RESULTS: When genetic improvement reduces expenditures on vaccination, expenditures decrease with [Formula: see text] at an increasing rate. When genetic improvement reduces losses in epidemic or endemic diseases, losses decrease with [Formula: see text] at an increasing rate. Hence, in all cases, the economic value of [Formula: see text] increases as [Formula: see text] decreases towards 1. DISCUSSION: [Formula: see text] and its economic value are more informative for potential benefits of genetic improvement than heritability estimates for survival after a disease challenge. In livestock, the potential for genetic improvement is small for epidemic microparasitic diseases, where disease control measures limit possibilities for phenotyping. This is not an issue in aquaculture, where controlled challenge tests are performed in dedicated facilities. If genetic evaluations include infectivity, genetic gain in [Formula: see text] can be accelerated but this would require different testing designs. CONCLUSIONS: When [Formula: see text], its economic value is zero. The economic value of [Formula: see text] is highest at low values of [Formula: see text] and approaches zero at high values of [Formula: see text].

BACKGROUND: Breeding companies may want to maximize the rate of genetic gain from their breeding program within a limited budget. In salmon breeding programs, full-sibs of selection candidates are subjected to performance tests for traits that cannot be recorded on selection candidates. While marginal gains in the aggregate genotype from phenotyping and genotyping more full-sibs per candidate decrease, costs increase linearly, which suggests that there is an optimum in the allocation of the budget among these activities. Here, we studied how allocation of the fixed budget to numbers of phenotyped and genotyped test individuals in performance tests can be optimized. METHODS: Gain in the aggregate genotype was a function of the numbers of full-sibs of selection candidates that were (1) phenotyped in a challenge test for sea lice resistance (2) phenotyped in a slaughter test (3) genotyped in the challenge test, and (4) genotyped in the slaughter test. Each of these activities was subject to budget constraints. Using a grid search, we optimized allocation of the budget among activities to maximize gain in the aggregate genotype. We performed sensitivity analyses on the maximum gain in the aggregate genotype and on the relative allocation of the budget among activities at the optimum. RESULTS: Maximum gain in the aggregate genotype was €386/ton per generation. The response surface for gain in the aggregate genotype was rather flat around the optimum, but it curved strongly near the extremes. Maximum gain was sensitive to the size of the budget and the relative emphasis on breeding goal traits, but less sensitive to the accuracy of genomic prediction and costs of phenotyping and genotyping. The relative allocation of budget among activities at the optimum was sensitive to costs of phenotyping and genotyping and the relative emphasis on breeding goal traits, but was less sensitive to the accuracy of genomic prediction and the size of the budget. CONCLUSIONS: There is an optimum allocation of budget to the numbers of full-sibs of selection candidates that are phenotyped and genotyped in performance tests that maximizes gain in the aggregate genotype. Although potential gains from optimizing group sizes and genotyping effort may be small, they come at no extra cost.

BACKGROUND: Macroparasites, such as ticks, lice, and helminths, are a concern in livestock and aquaculture production, and can be controlled by genetic improvement of the host population. Genetic improvement should aim at reducing the rate at which parasites spread across the farmed population. This rate is determined by the basic reproduction ratio, i.e. [Formula: see text], which is the appropriate breeding goal trait. This study aims at providing a method to derive the economic value of [Formula: see text]. METHODS: Costs of a disease are the sum of production losses and expenditures on disease control. Genetic improvement of [Formula: see text] lowers the loss-expenditure frontier. Its economic effect depends on whether the management strategy is optimized or not. The economic value may be derived either from the reduction in losses with constant expenditures or from the reduction in expenditures with constant losses. RESULTS: When [Formula: see text] ≤ 1, the economic value of a further reduction is zero because there is no risk of a major epidemic. When [Formula: see text] > 1 and management is optimized, the economic value increases with decreasing values of [Formula: see text], because both the mean number of parasites per host and frequency of treatments decrease at an increasing rate when [Formula: see text] decreases. When [Formula: see text] > 1 and management is not optimized, the economic value depends on whether genetic improvement is used for reducing expenditures or losses. For sea lice in salmon, the economic value depends on a reduction in expenditures with constant losses, and is estimated to be 0.065€/unit [Formula: see text]/kg production. DISCUSSION: Response to selection for measures of disease prevalence cannot be predicted from quantitative genetic theory alone. Moreover, many studies fail to address the issue of whether genetic improvement results in reduced losses or expenditures. Using [Formula: see text] as the breeding goal trait, weighed by its appropriate economic value, avoids these issues. CONCLUSION: When management is optimized, the economic value increases with decreasing values of [Formula: see text] (until the threshold of [Formula: see text], where it drops to zero). When management is not optimized, the economic value depends on whether genetic improvement is used for reduced expenditures or production losses. For sea lice in salmon, the economic value is estimated to be 0.065 €/unit [Formula: see text]/kg production.

BACKGROUND: Profitability of breeding programs is a key determinant in the adoption of selective breeding, and can be evaluated using cost-benefit analysis. There are many options to design breeding programs, with or without a multiplier tier. Our objectives were to evaluate different breeding program designs for aquaculture and to optimize the number of selection candidates for these programs. METHODS: The baseline was based on an existing breeding program for gilthead seabream, where improvement of the nucleus had priority over improvement of the multiplier tier, which was partly replaced once every 3 years. Alternative breeding programs considered were annual multiplier tier replacement, annual multiplier tier replacement with priority on improvement of the multiplier tier, and a program without a multiplier tier. Cost-benefit analyses were performed to compare breeding programs. The outcomes were used to describe relationships between profitability and the number of selection candidates, length of the time horizon, and production output, and to estimate the optimum numbers of selection candidates. RESULTS: The baseline breeding program was profitable after 5 years and reached a net present value of 2.9 million euro in year 10. All alternative programs were more profitable up to year 17. The program without a multiplier tier was the most profitable one up to year 22, followed by the program with annual multiplier tier replacement and nucleus priority. The optimum number of selection candidates increased with the length of the time horizon and production output. CONCLUSIONS: The baseline breeding program was profitable after 5 years. For a short time horizon, putting priority on improvement of the multiplier tier over the nucleus is more profitable than putting priority on nucleus improvement, and vice versa for a long time horizon. Use of a multiplier tier increases the delay between costs made for selection and resulting benefits. Thus, avoiding the use of a multiplier tier will increase the profitability of the breeding program in the short term. The optimum number of selection candidates increases with the length of the time horizon and production output. Using too many selection candidates relative to the optimum leads to less reduction in profitability than using too few selection candidates.

OBJECTIVE: In this pilot study, we investigated the feasibility of a home-based, remotely guided exercise intervention for patients with gliomas. DESIGN: Pilot randomized controlled trial (RCT) with randomization (2:1) to exercise or control group. SUBJECTS: Patients with stable grade II and III gliomas. INTERVENTION: The six-month intervention included three home-based exercise sessions per week at 60%-85% of maximum heart rate. Participants wore heart rate monitors connected to an online platform to record activities that were monitored weekly by the physiotherapist. MAIN MEASURES: Accrual, attrition, adherence, safety, satisfaction, patient-reported physical activity, VO2 peak (by maximal cardiopulmonary exercise testing) and body mass index (BMI) at baseline and at six-month follow-up. RESULTS: In all, 34 of 136 eligible patients (25%) were randomized to exercise training ( N = 23) or the control group ( N = 11), of whom 19 and 9, respectively, underwent follow-up. Mean adherence to prescribed sessions was 79%. Patients' experiences were positive. There were no adverse events. Compared to the control group, the exercise group showed larger improvements in absolute VO2 peak (+158.9 mL/min; 95% CI: -44.8 to 362.5) and BMI (-0.3 kg/m²; 95% CI: -0.9 to 0.2). The median increase in physical activity was 1489 metabolic equivalent of task (MET) minutes higher in the exercise group. The most reported reasons for non-participation were lack of motivation or time. CONCLUSION: This innovative and intensive home-based exercise intervention was feasible in a small subset of patients with stable gliomas who were interested in exercising. The observed effects suggest that the programme may improve cardiorespiratory fitness. These results support the need for large-scale trials of exercise interventions in brain tumour patients.

CONTEXT: Recurrence rates for ankle sprains are high. Therefore, preventive measures such as ankle bracing during sports are recommended. OBJECTIVE: To systematically evaluate the perceived ease of use, quality, comfort, stability, and hindrance of and the overall satisfaction with 3 contemporary brace types in 3 types of sports. DESIGN: Randomized comparative user survey. SETTING: Recreational sports: soccer, volleyball, and running. PATIENTS OR OTHER PARTICIPANTS: Young adult recreational athletes (29 soccer players, 26 volleyball players, and 31 runners). INTERVENTION(S): Compression brace (CB), lace-up brace (LB), and semirigid brace (SB). MAIN OUTCOME MEASURE(S): Rating of perceived ease of use, quality, comfort, stability, and hindrance of and overall satisfaction with the brace types during sports on a 5-point Likert scale. The secondary outcome measure was participants' willingness to buy the tested brace. RESULTS: Overall, the 3 brace types received high mean scores for ease of use and quality. Soccer players preferred the CB over both alternatives, considering the higher scores for comfort (CB = 4.0, LB = 3.5, SB = 2.8), hindrance (CB = 3.7, LB = 2.9, SB = 2.8), overall satisfaction (CB = 3.6, LB = 3.0, SB = 2.5), and greatest willingness to buy this brace. Volleyball players preferred the LB over both alternatives, considering the higher scores for stability (LB = 4.2, CB = 3.2, SB = 3.3), overall satisfaction (LB = 3.8, CB = 3.0, SB = 3.0), and greatest willingness to buy this brace. Runners preferred the CB over both alternatives considering the better score for hindrance (CB = 3.6, LB = 2.8, SB = 2.9) and greatest willingness to buy this brace. CONCLUSIONS: All 3 ankle-brace types scored high on perceived ease of use and quality. Regarding the brace types, soccer players, volleyball players, and runners differed in their assessments of subjective evaluation of comfort, stability, hindrance, overall satisfaction, and willingness to buy the brace. Soccer players and runners preferred the CB, whereas volleyball players preferred the LB.

BACKGROUND: In breeding programs for aquaculture species, breeding goal traits are often weighted based on the desired gains but economic gain would be higher if economic values were used instead. The objectives of this study were: (1) to develop a bio-economic model to derive economic values for aquaculture species, (2) to apply the model to determine the economic importance and economic values of traits in a case-study on gilthead seabream, and (3) to validate the model by comparison with a profit equation for a simplified production system. METHODS: A bio-economic model was developed to simulate a grow-out farm for gilthead seabream, and then used to simulate gross margin at the current levels of the traits and after one genetic standard deviation change in each trait with the other traits remaining unchanged. Economic values were derived for the traits included in the breeding goal: thermal growth coefficient (TGC), thermal feed intake coefficient (TFC), mortality rate (M), and standard deviation of harvest weight ([Formula: see text]). For a simplified production system, improvement in TGC was assumed to affect harvest weight instead of growing period. Using the bio-economic model and a profit equation, economic values were derived for harvest weight, cumulative feed intake at harvest, and overall survival. RESULTS: Changes in gross margin showed that the order of economic importance of the traits was: TGC, TFC, M, and [Formula: see text]. Economic values in € (kg production)-1 (trait unit)-1 were: 0.40 for TGC, -0.45 for TFC, -7.7 for M, and -0.0011 to -0.0010 for [Formula: see text]. For the simplified production system, similar economic values were obtained with the bio-economic model and the profit equation. The advantage of the profit equation is its simplicity, while that of the bio-economic model is that it can be applied to any aquaculture species, because it can include any limiting factor and/or environmental condition that affects production. CONCLUSIONS: We confirmed the validity of the bio-economic model. TGC is the most important trait to improve, followed by TFC and M, and the effect of [Formula: see text] on gross margin is small.

OBJECTIVES: To describe the association between participants' person-related potential predictor variables and cumulative compliance with interventions for preventing ankle sprains: neuromuscular training, wearing an ankle brace, and a combined training and bracing. DESIGN: Secondary analysis of compliance data from a randomized controlled trial (RCT) comparing measures preventing ankle ligament injuries. METHODS: Ordinal regression with a backward selection method was used to obtain a descriptive statistical model linking participants' person-related potential predictor variables with the monthly cumulative compliance measurements for three interventions preventing ankle ligament injuries. RESULTS: Having had a previous ankle injury was significantly associated with a higher compliance with all of the preventive measures trialed. Overall compliance with bracing and the combined intervention was significantly lower than the compliance with NM training. Per group analysis found that participating in a high-risk sport, like soccer, basketball, and volleyball, was significantly associated with a higher compliance with bracing, or a combined bracing and NM training. In contrast, participating in a high-risk sport was significantly associated with a lower per group compliance with NM training. CONCLUSIONS: Future studies should include at least registration of previous ankle sprains, sport participation (high- or low-risk), experience in NM training, and hours of sport exposure as possible predictors of compliance with interventions preventing ankle sprains. Practitioners should take into account these variables when prescribing preventive neuromuscular training or bracing.

BACKGROUND: Ankle sprains are the most common sports-related injury, associated with a high rate of recurrence and societal costs. Recent studies have emphasized the effectiveness of both neuromuscular training and bracing for the secondary prevention of ankle sprains. PURPOSE: To evaluate the cost-effectiveness of the separate and combined use of bracing and neuromuscular training for the prevention of the recurrence of ankle sprains. STUDY DESIGN: Economic and decision analysis; Level of evidence, 2. METHODS: A total of 340 athletes (157 male and 183 female; aged 12-70 years) who had sustained a lateral ankle sprain up to 2 months before inclusion were randomized to a neuromuscular training group (n = 107), brace group (n = 113), and combined intervention group (n = 120). Randomization was stratified by medical treatment of the inclusion sprain. Participants in the neuromuscular training group underwent an 8-week home-based exercise program. Participants in the brace group received a semirigid ankle brace to be worn during all sports activities for a period of 12 months. Participants allocated to the combined group underwent both interventions, with the ankle brace to be worn during all sports activities for a period of 8 weeks. The recurrence of ankle sprains and associated costs were registered during the 1-year follow-up. RESULTS: There were no differences between groups at baseline with regard to age, sex, sports participation, previous injury, or knowledge of preventive measures. The incremental cost-effectiveness ratio (ICER) of the brace group in comparison with the combined group was -€2828.30 (approximately--US$3865.00), based on a difference in the mean cost of -€76.16 (approximately--US$104.00) and a difference in the mean effects of 2.68%. The ICER of the neuromuscular training group in comparison with the combined group was €310.08 (approximately US$424.00), based on a difference in the mean cost of -€28.37 (approximately--US$39.00) and a difference in the mean effects of 9.15%. CONCLUSION: Bracing was found to be the dominant secondary preventive intervention over both neuromuscular training and the combination of both measures.

BACKGROUND: Ankle sprain is the most common sports-related injury with a high rate of recurrence and associated costs. Recent studies have emphasised the effectiveness of both neuromuscular training and bracing for the secondary prevention of ankle sprains. AIM: To evaluate the effectiveness of combined bracing and neuromuscular training, or bracing alone, against the use of neuromuscular training on recurrences of ankle sprain after usual care. METHODS: 384 athletes, aged 18-70, who had sustained a lateral ankle sprain, were included (training group n=120; brace group n=126; combi group n=138). The training group received an 8-week home-based neuromuscular training programme, the brace group received a semirigid ankle brace to be worn during all sports activities for 12 months, and the combi group received both the training programme, as well as the ankle brace, to be worn during all sports activities for 8 weeks. The main outcome measure was self-reported recurrence of the ankle sprain. RESULTS: During the 1-year follow-up, 69 participants (20%) reported a recurrent ankle sprain: 29 (27%) in the training group, 17 (15%) in the brace group and 23 (19%) in the combi group. The relative risk for a recurrent ankle sprain in the brace group versus the training group was 0.53 (95% CI 0.29 to 0.97). No significant differences were found for time losses or costs due to ankle sprains between the intervention groups. CONCLUSIONS: Bracing was superior to neuromuscular training in reducing the incidence but not the severity of self-reported recurrent ankle sprains after usual care.

BACKGROUND: Ankle sprains are the most common sports and physical activity related injury. There is extensive evidence that there is a twofold increased risk for injury recurrence for at least one year post injury. In up to 50% of all cases recurrences result in disability and lead to chronic pain or instability, requiring prolonged medical care. Therefore ankle sprain recurrence prevention in athletes is essential. This RCT evaluates the effect of the combined use of braces and neuromuscular training (e.g. proprioceptive training/sensorimotor training/balance training) against the individual use of either braces or neuromuscular training alone on ankle sprain recurrences, when applied to individual athletes after usual care. METHODS/DESIGN: This study was designed as three way randomized controlled trial with one year follow-up. Healthy individuals between 12 and 70 years of age, who were actively participating in sports and who had sustained a lateral ankle sprain in the two months prior to inclusion, were eligible for inclusion. After subjects had finished ankle sprain treatment by means of usual care, they were randomised to any of the three study groups. Subjects in group 1 received an eight week neuromuscular training program, subjects in group 2 received a sports brace to be worn during all sports activities for the duration of one year, and group 3 received a combination of the neuromuscular training program and a sports brace to be worn during all sports activities for the duration of eight weeks. Outcomes were assessed at baseline and every month for 12 months therafter. The primary outcome measure was incidence of ankle sprain recurrences. Secondary outcome measures included the direct and indirect costs of recurrent injury, the severity of recurrent injury, and the residual complaints during and after the intervention. DISCUSSION: The ABrCt is the first randomized controlled trial to directly compare the secondary preventive effect of the combined use of braces and neuromuscular training, against the use of either braces or neuromuscular training as separate secondary preventive measures. This study expects to identify the most effective and cost-efficient secondary preventive measure for ankle sprains. The study results could lead to changes in the clinical guidelines on the prevention of ankle sprains, and they will become available in 2012. A study of 24 patients who sustained an extra-articular fracture of the distal third of the tibial shaft was performed to determine the effect of the type of treatment, open reduction and internal fixation (ORIF) or closed reduction and intramedullary (IM) nailing, on the occurrence of malalignment. All patients were treated in our clinic between 1993 and 2001 for a fracture in the distal third of the tibia. Twelve patients treated with ORIF were matched to 12 patients treated with IM nailing, with regard to gender, age decade, and the AO classification of the fracture. The group treated with IM nailing was assessed after a mean 6.0 years versus ORIF after a mean of 4.5 years. Two patients treated with ORIF versus six patients treated with IM nailing had a malalignment of the tibia. Furthermore, we found no difference with regard to time to union, non-union, hardware failure or deep infections between ORIF and IM nailing. Our results suggest that control of alignment is difficult with IM nailing of distal tibial fractures. For optimal alignment we advise considering the use of ORIF for closed and type I open extra-articular fractures in the distal third of the tibia.
