RESUMO

Association between metabolic syndrome (MetS) and oxidative stress has been shown in numerous studies. It has been shown that probiotics could be the effective treatment strategy in improving oxidative stress. This study aimed to determine the effects of a new developed synbiotic yogurt on oxidative stress status in adults with MetS. Forty-four individuals were assigned into two groups and given 300 g of synbiotic yogurt containing Lactobacillus plantarum, Lactobacillus pentosus, and Chloromyces marcosianos yeast or regular yogurt for 12 weeks in this randomized, placebo-controlled clinical trial. Before and after the intervention, biochemical parameters were assessed. Daily consumption of synbiotic yogurt in adults with MetS showed a statistically significant improvement in the level of glutathione peroxidase (p = 0.01) and total oxidant status (p = 0.006) compared to the regular yogurt. Total Antioxidant Capacity and superoxide dismutase levels increased significantly (p = 0.002 and p = 0.02, respectively) in the intervention group compared to the baseline levels. In adults with MetS, daily consumption of the synbiotic yogurt containing native strains of Lactobacillus plantarum, Lactobacillus pentosus, and Chloromyces marcosianos yeast for 12 weeks was associated with improvements in oxidative stress status.Trial registration number: Iranian Registry of Clinical Trials (IRCT20220426054667N1) (18/05/2022).

Several risk factors including environmental exposures, socioeconomic status, and dietary factors including dietary patterns have been considered for childhood Asthma. The present study tried to examine the association between a western-style pattern and the likelihood of asthma and its symptoms in Yazd, Iran. In the present cross-sectional study, dietary intakes of elementary and high-school children were obtained through a validated GAN questionnaire. The GAN questionnaire, derived from the ISAAC questionnaire was used to assess the symptoms of allergic diseases and their related risk factors. A western dietary pattern score considered 9 food groups including chicken eggs, margarine, butter, sugar, fast foods, soft drinks, snacks, sauce, and chocolate. In total 7667 children aged 10.9 ± 3.35 years were included in the current investigation. Boys with higher adherence to western dietary pattern had a higher risk of wheezing in the past 12 months (OR 1.37, 5% CI 1.01-1.87, P = 0.04) and this association was also observed in the whole population (OR 1.30, 5% CI 1.05-1.60, P = 0.01). However, after adjustment for confounders this relation did not remain significant in boys. Our results support the hypothesis that a western dietary pattern is associated with an increased risk of wheezing in the past 12 months in children with asthma. Future prospective studies are needed to confirm this finding.

Diet quality in childhood and adolescence can affect health outcomes such as physical and cognitive growth and preventing chronic diseases in adulthood. This study aimed to evaluate the relationship between diet quality using the modified youth healthy eating index (MYHEI) with socioeconomic factors and nutrition status in 7-12-year-old children in Iran. This descriptive-cross-sectional study was performed on 580 students in Zabol, Iran, selected through multistage cluster sampling. The diet was assessed through the 168-item food frequency questionnaire (FFQ) and eating behaviors. Then, the MYHEI scoring system was used to calculate the diet quality. In addition, we used the WHO growth indices, such as weight to age, height to age, and body mass index (BMI) to age ratios, to evaluate nutrition status. The mean total MYHEI score in children was 56.3 ± 11.2. Among children with the highest MYHEI score quartile, the number of girls was significantly higher than boys (p = 0.001). The prevalence of underweight, stunting, and wasting was 25.3%, 17.4%, and 21.7%, respectively. The prevalence of underweight (OR: 2.2; 95% CI 1.26, 3.90, p = 0.001) and stunting (OR: 3.2; 95% CI 1.65, 6.14, p = 0.006) were significantly lower in the higher MYHEI score quartile compared to the lower quartile. The overall diet quality of most children should be modified. Therefore, to improve the children's health and nutrition status, it is necessary to perform nutritional interventions such as training and promotional programs, especially in schools.

PURPOSE: This study conducted in Dena County is a population-based cohort study as part of the Prospective Epidemiological Research Studies in Iran (PERSIAN). The specific objectives of this study were to estimate the prevalence of region-specific modifiable risk factors and their associations with the incidence of major non-communicable diseases (NCDs). PARTICIPANTS: This PERSIAN Dena Cohort Study (PDCS) was conducted on 1561 men and 2069 women aged 35-70 years from October 2016 in Dena County, Kohgiluyeh and Boyer-Ahmad Province, Southwest Iran. The overall participation rate was 82.7%. FINDINGS TO DATE: Out of 3630 participants, the mean age was 50.16 years, 2069 (56.9%) were women and 2092 (57.6%) were rural residents. Females exhibited higher prevalence rates of diabetes, hypertension, fatty liver, psychiatric disorders, thyroiditis, kidney stones, gallstones, rheumatic disease, chronic lung disease, depression and osteoporosis compared with males (p<0.05). Furthermore, the urban population showed elevated rates of diabetes, thyroiditis, kidney stones and epilepsy, whereas psychiatric disorders and lupus were more prevalent in rural areas (p<0.05). According to laboratory findings, 418 (13.0%), 1536 (48.1%) and 626 (19.3%) of the participants had fasting blood sugar >126 mg/dL, low-density lipoprotein >100 mg/dL and haematuria, respectively; most of them were female and urban people (p<0.05). FUTURE PLANS: PDCS will be planned to re-evaluate NCD-related incidence, all-cause and cause-specific mortality every 5 years, along with annual follow-up for 15 years. Some examples of additional planned studies are evaluation of genetic, environmental risk, spirometry and ECG tests.

BACKGROUND: Metabolic syndrome (MetS) is one of the leading public health issues in the world with a reported prevalence of nearly 25% in the past decades in Iran. The present research aimed to identify the association between the Healthy Eating Index (HEI) and MetS components among female teachers. METHODS: In this cross-sectional study, 97 female teachers aged 31-57 years were enrolled from 2018 to 2019 in Qom, Iran. Usual dietary intakes were assessed using a validated 168-item Food Frequency Questionnaire (FFQ). HEI-2015 was calculated according to the consumption of whole fruits, vegetables, protein foods, beans, seafood, plant proteins, total and refined grain, dairy, fatty acid ratio, saturated fats, added sugars, and sodium. We also measured anthropometric and biochemical parameters. To evaluate the association between HEI-2015 and MetS, multivariate logistic regression was performed. RESULTS: MetS was found in 59.8% of participants. Total HEI-2015 scores were significantly lower in participants with MetS compared to those without MetS (59.69 ± 8.98 vs. 64.21 ± 8.71, respectively; p = 0.02). Daily energy intake, weight, body mass index, waist circumference, systolic and diastolic blood pressure, serum triglyceride, and fasting blood sugar levels were higher in women with MetS (all p < 0.05). Higher HEI-2015 total scores (OR: 0.94; 95% CI: 0.89-0.99; p = 0.02) and scores of total vegetables (OR: 0.62; 95% CI: 0.42-0.91; p = 0.02), dark green vegetables and beans (OR: 0.62; 95% CI: 0.39-0.98; p = 0.04), fatty acid ratio (OR: 0.83; 95% CI: 0.68-0.99; p = 0.04), refined grain (OR: 0.86; 95% CI: 0.75-0.99; p = 0.04), and added sugars (OR: 0.44; 95% CI: 0.26-0.75; p = 0.002) were all associated with lower odds of having MetS. CONCLUSIONS: Higher HEI-2015 scores, particularly in total vegetables, dark green vegetables, beans, and fatty acid ratio, as well as refined grain and added sugars were found to be associated with reduced odds of having MetS among Iranian female teachers. However, further prospective studies are required to confirm this finding.

This systematic review and dose-response meta-analysis examined the risk of chronic obstructive pulmonary disease (COPD) following dietary fiber intake. Relevant articles were retrieved from a variety of databases, including Scopus, Embase, and Medline, until March 2023. Spirometry was the most frequently used method for determining the presence of COPD. Based on the search, there were a total of 213,912 participants across five separate studies. Random effects model was used to combine the data and a dose-response analysis was further conducted. Five distinct cohort studies were identified. Based on highest versus lowest analysis, there was an inverse correlation between the intake of total fiber (RR, 0.72; 95%, CI: 0.64-0.80), cereal fiber (RR: 0.76, 95% CI 0.68, 0.86), and fruit fiber (RR: 0.75, 95% CI: 0.68, 0.83). Although this result was not significant for vegetable fiber (RR, 0.95; 95% CI, 0.84-1.07). Dose-response analysis revealed that daily increase of 10 g of total dietary fiber, cereal fiber, or fruit fiber reduced the risk of COPD by 26%, 21%, and 37%, respectively. The ROBINS-E tool classified all cohort studies as having a moderate risk of bias. Total fiber, cereal fiber, and fruit fiber intake were found to have low credibility using the NutriGrade tool. Additionally, there is a lack of scientific evidence supporting the use of vegetable fiber. Larger, more comprehensive studies are required to confirm these findings.

BACKGROUND: Metabolic syndrome is recognized as one of the most common global health issues, which may cause numerous side effects. Studies have shown the favorable effects of probiotic supplements on glycemic indices, lipid profiles, and oxidative stress status. However, the number of studies investigating the effects of food products containing probiotics and prebiotics on metabolic diseases is limited. Limited evidence also shows that products containing Lactobacillus plantarum could affect metabolic alterations in chronic diseases. No previous study evaluated the impact of synbiotic yogurt containing Lactobacillus plantarum on people with metabolic syndrome. Therefore, the current study aims to investigate the effect of the newly developed synbiotic yogurt containing Lactobacillus plantarum, Lactobacillus pentosus, and Chloromyces marcosianos yeast on the components of metabolic syndrome, oxidative stress status, and some other risk factors for cardiovascular diseases in adults with metabolic syndrome. METHODS: In this study, 44 patients with metabolic syndrome will be randomly assigned to intervention and control groups in a randomized, double-blind, controlled clinical trial. Participants in the intervention group will consume 300 g of synbiotic yogurt daily, while those in the control group will consume 300 g of regular yogurt daily for 12 weeks. Anthropometric measurements, blood pressure, and biochemical parameters will be evaluated before and after the intervention. DISCUSSION: The management of the metabolic syndrome presents significant clinical challenges. While probiotic supplementation for these individuals has been considered, the consumption of probiotic-rich foods has received considerably less attention. The question of whether zinc supplementation may improve cardio-metabolic health in patients with type 2 diabetes mellitus (T2DM) remains controversial and require further evaluation. This study aimed to summarize the effectiveness of oral zinc supplementation in improving cardio-metabolic risk markers in people with T2DM. We searched PubMed, Scopus, and Web of Science from inception to April 2023, for randomized controlled trials (RCTs). RCTs of type 2 diabetic adults (aged ≥18 years) comparing zinc supplementation with placebo were included. We excluded studies if not randomized, involved co-supplementation, and were conducted in children or pregnant women. Glycemic indices, lipid profiles, blood pressure, anthropometric measure, c-reactive protein (CRP), creatinine, and serum zinc were extracted. Certainty of evidence was assessed using the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) methods. We used a random-effect model to perform the dose-response analysis. Effect sizes were presented as mean difference (MD) and 95% confidence interval (CI). 22 studies (n = 1442 participants) were included. The certainty of the evidence was rated as moderate to high. Zinc supplementation significantly reduced glycemic indices: including two-hour postprandial glucose (2hpp) (mean difference (MD): -34.34 mg/dl; 95%CI: -51.61∼ -17.07), fast blood sugar (FBS) (MD: -23.32 mg/dl; 95% CI: -33.81∼ -12.83), and hemoglobin A1c (HbA1c) (MD: -0.47; 95% CI: -0.71∼ -0.23). Zinc had a favorable effect on lipid profiles low-density lipoprotein (LDL) (MD: -10.76 mg/dl; CI: -17.79∼-3.73), triglyceride (TG) (MD: -18.23 mg/dl; CI: -32.81∼-3.65), total cholesterol (TC) (MD: -12.74 mg/dl; CI: -21.68∼-3.80), VLDL (MD: -5.39 mg/dl; CI: -7.35∼-3.43) and high-density lipoprotein (HDL) (MD: 4.04 mg/dl; CI: 0.96 ∼ 7.12). Systolic blood pressure (SBP) (MD): -3.64 mmHg; 95% CI: -6.77∼ -0.52), weight (MD: 1.00 kg; 95% CI: 0.34∼1.66), CRP (MD: -3.37 mg/l, 95% CI: -4.05∼ -2.70), and serum zinc (MD: 15.38 µg/dl; 95% CI: 10.74∼ 20.02) changed to a statistically significant extent with zinc supplementation. There was also a linear association between additional 10 mg/d zinc treatment with FBS, HbA1c, HDL, LDL, TG, TC, and serum zinc. A non-linear dose-response gradient was seen for FBS, HDL, and SBP (p < 0.05). Egger's test showed no substantial publication bias. Our findings strongly suggest a potential beneficial effect of zinc supplementation on type 2 diabetic patients. Further high-quality research is needed to determine the optimal form, dosage, and duration of zinc supplementation for this population.

Phytochemicals are bioactive compounds found in plant-based foods. Consumption of phytochemical-rich foods has been associated with cardiovascular and metabolic diseases prevention in various populations. To quantify the phytochemical content of the diet, dietary phytochemical index (DPI) was established which is defined as the proportion of daily energy intake derived from foods rich in phytochemicals. The purpose of this study was to evaluate the association between the DPI and oxidative stress markers and cardiovascular risk factors in obese adults. In this cross-sectional study, a total of 140 adults aged 20-60 years and body mass index (BMI) of ≥ 30 kg/m2 were included. A validated food frequency questionnaire (FFQ) was used to collect information on dietary intakes. The DPI was calculated based on the following formula: DPI = [daily energy obtained from foods rich in phytochemicals (kcal)/total daily energy intake (kcal) × 100]. There was an inverse association between DPI and serum concentrations of Malondialdehyde (MDA) (P = 0.004), triglyceride (TG) (P-trend = 0.003), high-sensitive C-reactive protein (hs-CRP) (P = 0.017), and erythrocyte superoxide dismutase (SOD) activity (P = 0.024). Total antioxidant capacity (TAC) was positively associated with DPI score (P = 0.045). No significant relationship was found between the DPI score and fasting blood sugar (FBS), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), total oxidant status (TOS), glutathione peroxidase (GPx), catalase (CAT), and anthropometric parameters as well as systolic and diastolic blood pressure. The current study found that there was a significant inverse association between DPI and oxidative stress, inflammation, and hypertriglyceridemia as cardiovascular disease (CVD) risk factors in obese population. However, further research is needed to confirm these findings.

Introducción: Una de las etapas más importantes de la vida de las mujeres que hacen varios cambios fisiológicos y mentales en las mujeres es el período de embarazo. Con base en la importancia de la nutrición en el período de embarazo, tener una dieta saludable y adecuada puede garantizar un período de embarazo y parto seguro; Por lo tanto, este estudio se realizó para determinar el patrón nutricional de las mujeres embarazadas que se refieren a los centros de salud. Métodos: las participantes en este estudio fueron 235 mujeres embarazadas que remitieron a centros de salud urbanos y, según la edad del embarazo, estaban pasando el segundo trimestre del embarazo. La información requerida en este estudio descriptivo transversal se recolectó mediante el cuestionario de dos secciones que contiene el Cuestionario de frecuencia alimentaria (FFQ) con 48 ítems y un cuestionario de características demográficas. El análisis del Cuestionario de Frecuencia Alimentaria se realizó mediante el software NUT4 y el análisis de datos se realizó mediante el software SPSS y pruebas estadísticas descriptivas tales como (afluencia, desviación promedio y estándar). Para comparar el número permitido de unidades de grupos de alimentos de mujeres embarazadas en el segundo trimestre del embarazo con el promedio del número de unidades de grupos de alimentos por población en estudio, se utilizó la prueba estadística T. Resultados y discusión: Los resultados de este estudio mostraron que el 94.5% de las mujeres embarazadas estaban usando el grupo de alimentos de pan y cereales más del rango permitido. La cantidad de usar el grupo de alimentos de carne y proteína, el grupo de frutas y el grupo de grasas estaba en el rango permitido. La cantidad de uso del grupo de verduras y el grupo de leche y lácteos fue inferior al rango permitido. Acerca de la cantidad de consumo de energía de los alimentos, los resultados de este estudio mostraron que el 66.4% de los participantes en este estudio obtuvieron más energía que el rango permitido. Conclusiones: Las mujeres embarazadas que participan en este estudio basadas en la ingesta de macronutrientes e ingesta de energía no se apropiaron de esta situación, por lo que es necesario brindar suficiente información sobre los grupos de alimentos y la cantidad necesaria de ellas en el período de embarazo a todas las embarazadas. mujeres que remiten a centros de salud.


Introduction: One of the most important stages of women's life which make several physiological and mental changes in women is pregnancy period. Based on the importance of nutrition in pregnancy period, having a healthy and suitable diet can ensure a secure pregnancy period and child birth; therefore, this study was done to determine the nutritional pattern of pregnant women who refer to health centers. Methods: The participants in this study were 235 pregnant women who referred to urban health centers and based on the age of pregnancy, they were passing the second trimester of pregnancy. The required information in this cross-sectional descriptive study were collected by the two sectional questionnaire which containing Food Frequency Questionnaire (FFQ) with 48 items and demographic characteristics questionnaire. The analysis of Food Frequency Questionnaire was done by NUT4 software and the analysis of data was done by SPSS software and statistic descriptive tests such as (affluence, average and standard deviation). To comparison the number of allowed using units of food groups of pregnant women in the second trimester of pregnancy with the average of the number of using units of food groups by population under study, the statistic T-test was used. Results and Discussion: The results of this study showed that 94.5% of pregnant women were using bread and cereals food group more than allowed range. The amount of using meat and protein food group, fruits group and fats group was in allowed range. The amount of using vegetables group and milk and dairy group was less than allowed range. About the amount of energy intake of food, the results of this study showed that 66.4% of participants in this study got more energy than allowed range. Conclusions: Pregnant women who participate in this study based on the intake of macronutrient and energy intake were in unappropriated situation, because of this, it is needed to give enough information about food groups and the needed amount of them in pregnancy period to all the pregnant women who refer to health centers

BACKGROUND: Many therapeutic methods are recommended for the management of Type 2 diabetes. Ash-Kardeh contains several components such as flavonoids, antioxidants, and dietary fiber which can affect lipid profile and blood pressure in diabetic patients. However, no study has examined the effects of Ash-Kardeh consumption on blood glucose, blood pressure, and lipid profile in type 2 diabetic patients; therefore, the aim of this study will be to examine the effects of Ash-Kardeh consumption on blood glucose, blood pressure, and lipid profile in type 2 diabetic patients. METHODS: This study is a randomized, no-blinded, controlled clinical trial in which 44 type 2 diabetic patients will be randomly allocated to intervention and control groups. Individuals in both the intervention and control groups will receive (the usual treatment of diabetic patients) for 6 weeks, while those in the intervention group will receive (250 g of traditional fermented food daily in addition to the usual treatment) at the same time. Assessment of anthropometric measures, blood pressure, and biochemical parameters including serum concentrations of fasting blood sugar, high-density lipoproteins-cholesterol, low-density lipoproteins-cholesterol, total cholesterol, and triglyceride will be performed at the study baseline and end of the trial.
