RESUMO

Neuroleptic malignant syndrome (NMS) is an uncommon, life-threatening complication of treatment with neuroleptic drugs. Its main features are hyperthermia, extrapyramidal signs, and autonomic instability with fluctuating consciousness. It is believed that NMS is related to dopamine receptor blockade in the brain. We describe a case in a 52-year-old diabetic woman who developed NMS after taking Torecan (thiethylperazine), a phenothiazine drug, for 3 months to relieve dizziness. It is important to recognize this syndrome early and to treat immediately.

RESUMO

In up to 72% of patients with systemic vasculitis the neurological system may be involved. This may be peripheral or central (brain or spinal cord), and is usually due to infarction or a ruptured aneurysm. Diagnosis and evaluation are based peripherally on examination of a biopsy specimen or centrally on angiography. Therapy is usually with steroids and/or immunosuppressive agents. Two patients with systemic necrotizing vasculitis with unusual neurological complications are presented.

RESUMO

Paraneoplastic complications are obscure and difficult to understand. The association of motor neuron disease and carcinoma may sometimes be more than coincidental, and 2 cases are described. One patient had motor neuron disease, limbic encephalitis (a recognized paraneoplastic disorder) and carcinoma of the oesophagus; the other had motor neuron disease and adenocarcinoma of the rectum. In the elderly male with motor neuron disease simple screening tests to exclude lymphoma and carcinoma of the lung, bowel and genito-urinary tract are advocated.
