RESUMO

An 82-year-old man had his third episode of melanotic stool. Two previous workups had failed to localize the source of bleeding. A Tc-99m labeled RBC scan visualized the gallbladder early in the study. Administration of sincalide visually decreased the activity, confirming gallbladder activity. Three months later, at his second surgery, hepatic metastases were finally identified as the source of bleeding. In retrospect, the hepatic activity is inhomogeneous with at least two cold defects that could have represented hepatic metastases.

Portable cerebral perfusion studies provide a quick and accurate alternative to traditional confirmatory examinations (such as four-vessel angiography and electroencephalography) to determine brain death. The authors retrospectively analyzed 26 cerebral perfusion studies using both Tc-99m DTPA and Tc-99m HMPAO. The flow agents gave identical results. Additionally, the delayed-phase Tc-99m HMPAO images gave the same results for brain death (14 positive and 12 negative) as the flow images from either agent. Because the Tc-99m HMPAO delayed images can be used for diagnosis, many technical problems become less crucial. Unlike Tc-99m DTPA, Tc-99m HMPAO normally visualizes the gray matter of the cerebellum, midbrain, and medulla. These areas must be evaluated to ensure that they meet the strict criteria for brain death. These differences and the present study suggest that Tc-99m HMPAO is a superior agent for cerebral perfusion studies to determine brain death.

We have performed a comparative evaluation of systemic (i.v.) and intraarterial (i.a.) cisplatin by using a trace dose of the radiolabeled form of this drug [( 195mPt]cisplatin) to monitor the drug's biodistribution by dynamic scintigraphic imaging. We have analyzed the drug's metabolism using a compartmental model both following i.a. and i.v. administration in patients with gliomas. Significantly larger amounts of radioactivity (up to 10 times higher than in the uninvolved brain) were measured in tumors following i.a. administration, whereas the differential localization following i.v. drug administration was, at best, only twofold that of the uninvolved brain. On the other hand, no significant differences could be detected in the pharmacokinetics of either free cisplatin or platinated proteins in blood. The washout slope in tumors following i.a. administration may be an indicator of the higher local concentration of free cisplatin; no such washout could be observed in tumors following i.v. administration. The present noninvasive methods may help document the amount and the rate of (active) drug deposition at the desired target site. They may also assist in monitoring, prospectively and/or, on line, the probable effect of chemotherapy in an individual patient. In turn it may lead to novel methods for optimizing chemotherapeutic effectiveness at specific tumor-bearing sites and in defined treatment protocols.

RESUMO

Iohexol and iopamidol are new second-generation nonionic contrast media. We review their physical/chemical properties and describe the improvements made over the first-generation nonionic prototype, metrizamide, and conventional ionic contrast media. The significance of low osmolar solutions is discussed and the relationship between osmolality and adverse reactions is emphasized. The convenience of a ready-to-use stable solution that offers an advantage over the lyophilized metrizamide in practical application is suggested. Clinical studies using randomized, double-blind comparison techniques are cited. An attempt is made to define the exact role for iohexol and iopamidol.

RESUMO

An operational profile for nuclear pharmacy practice is presented, and the technical and professional role of nuclear pharmacists is reviewed. Key aspects of nuclear pharmacy practice in hospitals discussed are the basic facilities and equipment for the preparation, quality control, and distribution of radioactive drug products. Standards for receiving, storing, and processing radioactive material are described. The elements of a radiopharmaceutical quality assurance program, including the working procedures, documentation systems, data analysis, and specific control tests, are presented. Details of dose preparation and administration and systems of inventory control for radioactive products are outlined.
