ABSTRACT: In this study, it was aimed to contribute to the selection of the method to perform pancreatic lesion biopsies.Data of patients, who had undergone a percutaneous biopsy because of pancreatic masses in our institution in the period between January 2015 and November 2019, were evaluated retrospectively. The percutaneous biopsy method, the type of needle used in the procedure, and periprocedural complications were listed. Pathology and cytology reports in the archive were reviewed, and biopsy results were divided into 3 groups as benign, malignant, and inadequate. Of 308 patients included in the study, the diagnostic accuracy was verified in 124 patients through the assessment of surgical outcomes, results of biopsies from metastatic lesions, or follow-up findings. The verified results were classified as true-positives and true-negatives.Of a total of 308 patients included in the study, 23 underwent a fine-needle aspiration biopsy (FNAB) and 285 underwent a core needle biopsy (CNB). No statistical differences were observed in sample acquisition success and complications between the groups.Of the lesions with a confirmed pathological diagnosis, 67.74% were malignant and 32.26% were benign. The diagnosis was correct in 107 of 112 CNB patients (95.54%) and 9 of 12 FNAB patients (75.00%). When the success of the 2 methods was compared, it was found that outcomes of CNB were statistically more successful compared with those of FNAB.A transabdominal ultrasound-guided percutaneous CNB is a safe method with a high diagnostic yield to perform a biopsy of the pancreas.

Objectives: Postoperative recurrent pilonidal sinus disease is troublesome, and its treatment is a challenge. In this study, it was aimed to present the long-term efficacy of crystallized phenol treatment on postoperative recurrent pilonidal sinus disease through our results collected within the last 20 years. Material and Methods: Two hundred and twenty-seven patients who had been previously operated on and suffered from recurrent pilonidal sinus disease were enrolled. The operation was applied in our outpatient clinic under local anesthesia. Demographic data of the patients, number of crystallized phenol treatment, duration of follow-up and recurrence numbers were prospectively recorded. Treatment success and factors affecting recurrence were examined. Results: Our success rate was found as 71.5%. This success rate belongs to the group of patients who never quit treatment and complied with the treatment. The patients were followed up for a mean 45.8 months. Of the post-crystallized phenol treatment recurrences, 72.4% took place within the first five years, while 97.4% did so within the first 10 years. Mean number of crystallized phenol applications was 2.6. The longer the duration of the disease before treatment, the more recurrence was observed after treatment (p= 0.02). There was no correlation between the number of previous operations and recurrence after treatment. As the number of sinus openings increased, so did the number of applications (p= 0.001). Conclusion: Crystallized phenol treatment yields promising long-term results in recurrent pilonidal sinus disease as well and may be recommended as the first choice in recurrent pilonidal sinus disease treatment since it is an effective non-operative treatment modality.

OBJECTIVE: This study aimed to better demonstrate the long-term outcomes and effectiveness of crystallized phenol treatment (CPT) by presenting our 20 years experience. METHODS: One thousand and twenty-six patients who have primer pilonidal sinus were enrolled. The operation was applied outpatient clinic under local anesthesia. Recurrence rate, application number, healing time, and factors affecting them were analyzed. RESULTS: The success rate was 84.3%. The mean number of CPT applications was 2.1 and the mean recovery time was 8.9 weeks. The rate of recurrence was higher in patients with a long disease duration prior to initiating treatment (p = .04). The rate of recurrence was higher in patients with a higher number of opening and CPT application, those with longer application time, and those with a positive family history (respectively p = .01, p = .002, p = .008). CONCLUSIONS: Long-term outcomes are also very successful in the treatment of pilonidal disease with CPT. We believe that CPT should be the first choice in the treatment of pilonidal disease because it is inexpensive, can be applied as an outpatient treatment, is a noninvasive procedure, has results comparable to surgical procedures, and does not cause loss of work and strength.

OBJECTIVE: Multiple treatment options for hidradenitis suppurativa (HS) have been suggested; none 100% successful. We assessed crystallized phenol treatment (CPT) for sacral HS. METHODS: Twenty-five patients with sacral HS were enrolled. HS severity was assessed by Hurley Score and HS-Physician's Global Assessment (HS-PGA). All the leaking openings were dilated by a thin mosquito clamp after local anesthesia. Any hair present was removed, and phenol was applied. RESULTS: Patients were followed-up for a mean 36.8 months. The mean number of CPT application was 3.98. After CPT mean Hurley score decreased from 2.1 ± 0.68 to 1.2 ± 0.4 and HS-PGA score from 3 ± 1.22 to 0.72 ± 8.4 (p = .01). Recurrence was seen in 10 patients during follow-up period but they were re-treated with CPT. CONCLUSION: CPT may be useful as a first-line treatment of sacral HS.

OBJECTIVE: To examine the content, quality and reliability of YouTube videos related to breast self-examination. DESIGN: A search of YouTube was made on 21.06.2018 using the keyword of "breast self-examination". The videos were categorised by two doctors as useful information or misleading information. To evaluate the quality of the videos, a 5-point global quality scale was used (GQS: 1 = poor quality, 5 = excellent quality), for reliability a 5-point DISCERN scale was used, and for content an 8-point scale (higher points indicated greater reliability and better content). RESULTS: Of the 200 videos initially included in the study, 33 (37.9%) were classified as useful and 54 (62%) as misleading information. The reliability, content and quality scores of the videos in the useful information group were higher. The length (in seconds) of the videos in the useful information group (median 301, IQR 231-512) was longer than that of those in the misleading information group (median 163, IQR 94.8-231) (p = 0.003). The majority (70.6%) of the videos in the misleading information group had been uploaded by an individual user. The number of views per day of the videos in the misleading information group (median 58.6, IQR 18.5-298) was greater than that of the videos in the useful information group (median 49.7, IQR 16.3-268) (p = 0.276). The number of total views was higher for the misleading information group (median 83807 vs. 80237) but not at a level of statistical significance (p = 0.153). There were more videos explaining breast self-examination directed at women only, and there were determined to be few videos including men only or both genders. CONCLUSION: Although there are many videos in English related to breast self-examination on YouTube, a great many of these contain misleading information. Therefore, for public information, there is a need for videos with full and accurate information to be made by universities, healthcare organisations and doctors not benefitting from the outcomes, to be uploaded to YouTube, which is a currently important source of information for the general population.

BACKGROUND: Laparoscopic cholecystectomy (LC)-related bile duct injuries remains a challenging issue with major implications for patient's outcome. METHODS: Between January 2008 and December 2012, we retrospectively analyzed the management and treatment outcomes of 90 patients with bile duct injury following LC. RESULTS: Forty-seven patients (52.2%) were treated surgically while the remaining 43 patients (47.8%) underwent non-surgical intervention. Injuries of Strasberg Type A and C were significantly more frequent in the non-surgical intervention group (P = 0.016, P = 0.044) whereas Type E2 was more frequent in the definitive surgery group (P < 0.001). The success rate of non-surgical intervention decreased as the waiting time increased whereas the success of definitive surgery was not time-dependent (P = 0.048). Initial jaundice (direct biluribin >1.3 gr/dL) significantly reduced the success rate of non-surgical interventions (P = 0.017). Presence of intraabdominal abscess significantly increased the complication rate after both definitive surgery and non-surgical interventions (P = 0.04, P = 0.023). Treatment success rates were similar in both surgery and non-surgical intervention groups according to the distribution of Strasberg injury types. CONCLUSION: A multimodality approach is recommended in planning for patient-based treatment. Delayed referral reduces the success of nonsurgical interventions while it does not seem to significantly affect the success of surgical interventions when intraabdominal sepsis is under control.

BACKGROUND: In this study, scald burns in a Turkish population (0-16 years of age) were studied with reference to their occurrence and common causes. METHODS: Of the burn victims hospitalized at the Burns Unit of Ankara Numune Education and Research Hospital between April 2001 and December 2003, 354 patients were aged 3 months to 16 years and 228 (64.4%) were male. RESULTS: When burn causes were examined, scald burns were the primary type, reported in 77.7% (275) of the patients (p < 0.001). Scalds were the most frequent burn agents in the pediatric population. Tea is a traditional beverage in Turkey. It is consumed especially at breakfast and throughout the day until late at night. Tea in Turkey is steeped and brewed for consumption. As brewing and steeping is a time-consuming process and the tea is served hot, the appropriate level of care regarding the hot tea is neglected. Consequently, tea carries a serious potential to cause burns in this country. Of the scalded children, 93 scald burns (33.8%) were due to the tea itself or to the boiling water used in steeping tea, and 87.1% of these children were preschoolers. The mortality rate was 7.1% in the hospitalized children group. CONCLUSION: Traditional behaviors may sometimes have some serious unexpected consequences. In households where family members tend to ignore an ongoing problem, governmental and non-governmental organizations should consider taking preventive measures to protect the children from the devastating consequences of such negligence.
