RESUMO

Plasma and serum fibronectin levels were determined in a serial study of 24 non-septic burned patients by means of the passive haemagglutination test and electroimmunoassay. The results showed a consistent pattern of observed changes: a decrease of circulating fibronectin in the initial (first 72 hours post-burn), and during post-operatyive period (usually by the end of the first week and during the second week post-injury). Fibronectin level decreases were more remarkable than total serum protein concentration changes following injury, as reflected by decrease of fibronectin/protein ratio.

Sera from 51 melanoma patients and from 55 healthy donors were examined for the presence of immune complexes (ICs) by the EA-rosette forming cell inhibition assay. Using a cut-off point of 5% rosette inhibition, ICs were found in 144 out of 213 sera from patients with melanoma (67.6%) and in 11 out of 55 control subjects (20%, p less than 0.001). The percentage of positive results was significantly lower in patients with no evidence of disease (60%) than in patients with clinical symptoms of their tumor (localized disease -- 80%, p less than 0.02, metastatic disease -- 78%, p less than 0.05). The mean inhibitory rate in patients with metastatic disease (21%) was higher than in patients with localized tumor (15.1%, the difference not significant) and than in patients with no evidence of disease (10%, p less than 0.01). The high levels of ICs or a tendency for their increasing in most cases accompanied the dissemination of the neoplasm. The stabilization of ICs on the low levels was found to be a prognostically favorable sign.

Leukocytes from 63 melanoma patients and from 38 control donors were examined by a leukocyte migration inhibition (LMI) assay. The antigens were three soluble extracts of fresh melanomas and one of lung cancer. A total of 62 out of the 99 (62.6%) blood samples from melanoma patients reacted with allogeneic extracts of melanoma in comparison with 11 out of the 48 (22.9%) in the controls (the values significantly different; P less than 0.001). The migration inhibition of melanoma patients' leukocytes by the lung cancer extract was markedly less than inhibition by any of the melanoma extracts. LMI in patients with distant metastases before treatment (9 out of the 11--81.8% positive results) was higher than in patients with regional lymph node metastases (5 out of the 9--55.5%) and than in patients with localized disease (3 out of the 7--47.8%). The reactivity of patients after surgery was higher than in patients before surgical resection of the tumor. Positive reactions were less frequent in patients with distant metastases receiving chemotherapy or/and immunotherapy than in patients before treatment.

Cell-mediated hypersensitivity in 64 patients with melanoma was assessed by delayed-type skin reactivity to dinitrochlorobenzine (DNCB) and tubercullin as well as by lymphocyte enumerations. The immunological status of the patients was correlated with their clinical stage. The mean absolute and percentage counts of E rosette forming cells were lower in patients than in normal controls. Patients with distant metastases showed lower levels of E rosette forming cells than the patients with localized disease. No differences were found in number of EAC rosette forming cells between melanoma patients and controls. The mean absolute and percentage counts of null cells in melanoma patients in stages II and III were significantly higher than those in controls. Melanoma patients showed reduced skin reactivity to DNCB and normal reactivity to tuberculin. Patients with positive DNCB reactivity showed significantly higher total lymphocyte counts and higher percentages of E rosette forming lymphocytes than patients unreactive to DNCB.
